Submit bid for all items; failure to do so may render the bid non-responsive. On lump-sum bid items, provide total price only; on unit price bid items, provide the unit price and the extended amount of bid. If no bid item exists for a portion of the work, include the costs in a related bid item. In case of error in extension of prices, unit price governs. In case of error in summation, the total of the corrected bid amounts governs.

Determination of the low Bidder will be made by taking each Bidder's base bid and adding as many of the additives (taken in the sequence listed) as available funds will permit. Only entire additives will be awarded; partial additives will not be considered.

Mammoth Cave National 06/27/2017 Lead, Asbestos, Mold Removal

STANDARD GENERAL CONSTRUCTION SPECIFICATIONS

Mammoth Cave National Park

MITIGATE MOLD, ASBISTIOS, AND LEAD – BUILDINGS 30, 31, 32, and 33

PART 1: GENERAL

1-1 DESCRIPTION: This project entails the mitigation of mold, asbestos, and lead in Buildings 30, 31, 32, and 33.

1-2 LOCATION: Mammoth Cave National Park is located approximately halfway between Nashville, Tennessee and Louisville, Kentucky near interstate 65 in Edmonson County, Kentucky. See attachment A.

The work site is located at buildings 29, 30, 31, 32, and 33 in the Administrative Area at Mammoth Cave National Park

3 SAFETY: The Safety of the visitors and staff at Mammoth Cave National Park is of utmost importance. This includes personnel working on contracts within the park’s boundary. The Contractor shall comply with all local, state, park, and federal regulations that apply to safety and accident prevention.

The Contractor will schedule a kick-off and then weekly tail gate safety meetings. The Contractor will notify and coordinate with the Contracting Officer the date and time of these meetings.

4 BARRICADES: The Contractor shall provide barricades as directed by the Contracting Officer to protect the work site and to keep park visitors out of the work area.

1-5 UTILITIES: Notify Contracting Officer and utility companies of proposed locations and times for excavation. The Contractor shall be responsible for locating and preventing damage to known utilities.

If damage occurs to known utility, repair the utility at no additional expense to the Government as directed by the Contracting Officer.

If damage occurs to an unknown utility, repair utility. An equitable adjustment will be made in accordance with the Changes clause of the contract.

1-6 INTERRUPTION OF EXISTING UTILITIES: The Contractor shall not interrupt existing utility service without the approval of the Contracting Officer.

1-7 UTILITIES AVAILABLE DURING CONSTRUCTION: If available in a reasonable manner and approved by the Contracting Officer, the Contractor can utilize water, and electricity provided by Mammoth Cave National Park. If not available, the Contractor shall make arrangement for his own utilities.

1-8 SANITARY FACITITIES: Unless otherwise approved by the Contracting Officer, the Contractor shall furnish sanitary facilities for the construction crew.

1-9 STORAGE OF MATERIALS: The Contractor can store materials in areas designated by the Contracting Officer. The Contractor is responsible for the security of the materials/equipment in the storage areas.

1-10 WORK AREA CLEANLINESS: Keep the storage area and work site clean.

If directed by the Contracting Officer, clear the site of all debris and litter. Dispose in a legal manner. Do not dispose in the park's garbage cans or dumpsters unless approved by the Contracting Officer.

These requirements also apply to all sub-contractors.

1-11 LITTERING: Littering is not tolerated within Mammoth Cave National Park. Any member of the Contractor’s crew caught littering will be removed from the project. The Contractor shall provide facilities at all work sites for the proper disposal of litter.

This includes inappropriate disposal of cigarette butts. Smoking is not allowed within 25 feet of any building or within 200 feet of any cave entrance.

1-12 DISPOSAL: All waste material resulting from this contract shall become the contractor's property. It shall be disposed in accordance with current laws, regulations, and policies. Disposal of materials within the park is not allowed.

1-13 Hauling Restrictions: Comply with all legal load restrictions in the hauling of materials. Load restrictions on park roads are identical to the state load restrictions with such additional regulations as may be imposed by the Park Superintendent. Information regarding rules and regulations for vehicular traffic on park roads may be obtained from the Office of the Park Superintendent. A special permit will not relieve Contractor of liability for damage that resulted from moving of equipment on park roads.

1-14 IMPACT ON PARK VISITORS: The Contractor shall at all times conduct his operations to ensure the least inconvenience to the park’s visitors and staff equipment.

1-15 PRESERVATION OF NATURAL FEATURES: Confine all operations to work limits of the project. Prevent damage to natural surroundings.

Restore damaged areas, repairing or replacing damaged trees and plants, at no additional expense to the Government. Do not remove, injure, or destroy trees or other plants without prior approval of the Contracting Officer.

1-16 ARCHEOLOGICAL FINDINGS (Applies if there will be disturbance to ground surfaces.): There may be archeological remains in the work area;

archeological findings remain the property of the Government and are not to be removed from the park area. Petroglyphs, artifacts, burial grounds or remains, structural features, ceremonial, domestic, and archeological objects of like nature, historic or prehistoric, found within the construction area, will be removed by the Government. In the event the Contractor's operations uncover or his employees find any of the above-described objects, immediately suspend operations at the discovery site, and continue operations in other areas. Should the discovery site require archeological studies resulting in delays or additional work, the Contractor will be granted a time extension or compensated by an equitable adjustment under the General Provisions of the contract.

1-17 OTHER CONTRACTORS: The Contractor shall coordinate his/her activities with other Contractors working in the area.

1-8 WORK HOURS: Work on this project shall occur Monday through Friday, 8:00am to 5:00pm, unless otherwise approved/restricted by the Contracting Officer.

No work shall be undertaken on federal holidays, unless otherwise approved by the Contracting Officer.

1-18 PROJECT MEETINGS:

A. PRECONSTRUCTION CONFERENCE - Before start of construction, Contracting Officer will arrange an on site meeting with Contractor. The meeting agenda will include the following as a minimum:

Correspondence procedures Designation of responsible personnel Labor standards provisions Payroll reports (if required) Changes Payments to Contractor Subcontractors National Park Service regulations Documents required under the contract Park rules and regulations Saturday, Sunday, holiday and night work Safety program/Accident Reporting

Name of responsible supervisor to carry out the program.

Kickoff and weekly Tailgate Safety meetings.

First aid procedures.

Outline of each phase of the work, the hazards associated with each major phase, and the methods proposed to ensure property protection and safety of the public, National Park Service personnel, and Contractor's employees.

Planning for possible emergency situations, such as floods, fires cave ins, slides, explosions, power outages, and wind storms. Such planning shall take into consideration the nature of construction, site conditions, and degree of exposure of persons and property.

13. Cultural Survey and Natural Resource Compliance requirements.

B. PROGRESS MEETINGS – If the on-site work exceeds 10 working days, the Contracting Officer will schedule weekly progress meetings. The meeting agenda will include the following as a minimum:

02. Review of work progress

03. Planned progress during succeeding work period

03. Field observations, problems, and decisions

05. Review of submittals schedule and status of submittals

06. Review of off-site fabrication and delivery schedules

07. Maintenance of progress schedule - Corrective measures to regain projected schedules

08. Maintenance of quality and work standards

09. Effect of proposed changes on progress schedule and coordination

10. Other business relating to work

1-19 TEMPORARY CONTROLS: The Contractor is responsible for the installation of all barriers, as directed by the Contracting Officer.

1-20 PROJECT CLOSEOUT:

A. Notify the Contracting Officer when the project (or designated portion of project) is complete, and request final inspection.

Contracting Officer will make an inspection within 5 days of receipt of request.

B. If, following final inspection, the work is determined to be substantially complete, the Contracting Officer will prepare a list of deficiencies to be corrected before final acceptance.

PART 2: MATERIALS

2-1 GOVERNMENT FURNISHED MATERIALS: The Government will furnish no Materials

2-2 CONTRACTOR FURNISHED MATERIALS: All other materials (Not listed in paragraph 2-1), including borrow and aggregates shall be Contractor-furnished from outside the park.

2-3 SUBMITTALS: The Contractor shall provide submittals as required by the Contracting Officer.

PART 3: EXECUTION NOT USED

PART 4: MEASUREMENT AND PAYMENT: No measurement and payment will be made directly for the work in this section. It will be considered subsidiary to the bid items.

LEAD, MOLD, AND ASBESTOS REMOVAL

PART 1: GENERAL

GENERAL INFORMAITON

Project site entails removal and remediation of all lead, mold, and asbestos containing materials in 2 CCC era houses located in Mammoth Cave National Park. The building numbers are 30,31,32,and 33.

All of the houses are single story and contain approximately 1,000 square feet of floor space. Building 31 has a basement. Note: It is the contractor’s responsibility to verify these numbers.

MOLD: The Contractor will Remediate all mold. This includes the removal of all interior sheetrock/paneling walls, ceilings, insulation, appliances, cabinets and fixtures. Properly package and dispose of all universal waste.

FUNGI IS PRESENT THROUGOUT THESE BUILDINGS, MO D REMEDIATION GUIDELINES

FOR LEVEL IV EXTENSIVE CONTAMINATION SH BE FOLLOWED.

ASBESTOS: The existing tile flooring and mastic are Asbestos Containing Materials and will be remov and disposed of in accordance with laws governing this type of work.

ASBESTOS CONTAINING MATERIALS NEED TO BE ABATED BY A KY CERTIFIED

ASBESTOS ABATEMENT CONTRACTTOR. ALL ASBESTOS WASTE SHOULD BE DISPOSED OF

AT A CERFIFIED LANDFILL AND MANIFEST DELIEVERED TO MAMMOTH CAVE NATIONAL

PARK.

LEAD PAINNT. Lead based paint and lead containing materials are located on all painted surfaces.

Demolition of these surfaces will be performed by a properly trained person in accordance with federal and state regulations. The waste stream should be characterized by TCLP Lead testing to determine hazardous or non-hazardous classification for the waste stream.

All windows and doors (door and frame) will be carefully removed and will become the property of Mammoth Cave National Park. Mammoth Cave intends to strip and rehabilitate the windows and doors.

Contractor is responsible for legal disposal, project completion, documentation, project notification, associated fees and final visual clearance testing.

EXISTING CONDITIONS

Mammoth Cave National Park has completed an “Environmental Assessment Report” for all buildings. A copy of these reposts is attached to these specifications.

It is the Contractor’s responsibility to review these documents and ascertain the conditions with these buildings.

PART 2: PRODUCTS AND MATERIALS

2.1 The Contractor will provide all equipment and materials.

PART 3: EXECUTION

3.1 Basements

Mammoth Cave National Park will pump the water from basements, and will install ventilation fans to expedite removal water and moisture prior to the start of contract work.

When work begins on this contract, the Contractor will remove and discard all contaminated materials, equipment, and garbage from the basement areas. The intent is to remove all materials down to the concrete block walls.

The Contractor will floor joist, wall cavities, wall studs and wall plates with antimicrobial solution; scrub.

3.2 Interior Buildings 28 and 29 (The work includes – but is not limited to the following items:

Remove/discard all kitchen cabinets.

Remove/discard all kitchen appliances.

Remove/discard bathroom sink and vanity.

Remove/discard toilet and shower units

Remove/discard all woodwork, door casings, door slabs, door trim, and baseboard trim.

Remove and dispose of all doors. Carefully remove all windows (including the frame). The windows and doors will remain the property of Mammoth Cave National Park.

Remove/discard all ceiling fixtures.

Cut/remove and dispose of all sheetrock ceilings.

Cut/remove and dispose of all sheetrock/wood panel walls prior to disposal.

Remove conventional from ceiling cavities and wall cavities prior to disposal.

HEPA vacuum exposed ceiling cavities, ceiling stringers, wall cavities, wall studs, wall plates, exposed wire runs, PVC, electrical fixtures and floors.

Treat exposed ceiling cavities, ceiling stringers, wall cavities, wall studs and wall plates with antimicrobial solution scrub.

Repeat the antimicrobial solution treatment process.

Treat exposed wire runs, PVC, electrical fixtures and floors with wiped antimicrobial solution.

Apply one coat of mold-resistant/fungi-resistant clear coat sealant on exposed ceiling cavities, ceiling stringers, exposed wall cavities, wall studs and wall plates.

3.3 GENERAL: ASBESTOS, MOLD, LEAD REMOVAL

Undertake and complete all required notifications. Supply proper documents

Provide properly trained and licensed supervisor and workers. Supply proper PPE for all employees work on this project.

Remove asbestos, lead and mold containing materials as defined in the statement of work. Dispose of this material as required by laws and regulations

Asbestos containing materials shall be abated by a KY Certified Asbestos Abatement Contractor. All asbestos wasted shall be disposed of a a certified landfill and manifest deleivered to the owner.

Demolition/removal of lead painted surfaces shall be performed by properly trained personnel in accordance with federal and state requirements. The waste stream shall be characterized by TCLP lead testing in order to determine hazardous or non-hazardous classification for the waste stream.

Fungi are present throughout the buildings. OSHA mold remediation guidelines for Level IV Extensive contamination shall be followed. These guidelines are included as Appendix A to the attached mold remediation report.

PART 4: MEASUREMENT AND PAYMENT

PAYMENT: Payment will be at the bid item to which this work relates.
