Performance Work Statement (PWS)

Provide and Install Epoxy Floor Topcoat—Multiple Locations throughout GRSM

1. Background. The Great Smoky Mountains National Park (GRSM) is located in Tennessee and North Carolina and consists of nearly 521,000 acres. The park was established by Congress on June 15, 1934. The parks mission is to protect, preserve, and interpret the natural, cultural, and historical resources for the benefit and inspiration of present and future generations. There are over 9 million visitors annually.

2. Required Services. The contractor shall provide all labor, tools, equipment, materials and supervision required to install epoxy floor topcoat (BRAND NAME ONLY: Permaflex with 5 year adhesion warranty) to up to 27 public restrooms throughout different locations of GRSM.

3. Contractor Responsibilities:

a. Locations, Sizes, and Current Floor Conditions (See blue arrows on attached PDF map)

(1) Sugarlands Visitor Center Restrooms (4 Restrooms)

(a) 1 Building

(b) 2 men’s (322 SF each) & 2 women’s (384 SF (1) and 412 SF (1))

b. General Information for All Locations

(1) None of the restrooms in this scope are climate controlled

(2) Vendor shall provide smooth transitions at entrance door thresholds at all locations that will pose no tripping hazard to visitors

(3) Vendor shall remove and reinstall all floor mounted toilets using new wax seals and flange bolts; where needed. Most are wall mounted

(4) Wall mounted toilets shall remain undisturbed

(5) Partitions are in place

(a) Vendor shall loosen floor trim rings and tape up for installation

(b) Vendor shall prepare and install product around floor mounted stall partitions.

(6) Sinks and related plumbing are wall mount and will remain undisturbed

(7) Only 110v type power is available at each location

(8) Product shall be PermaFlex brand by Sani-Tred

(a) Color scheme shall be Creekbed

(b) Color flakes shall be ¼” in size type

(9) Floor Coating Application

(a) Shall be seamless

(b) Shall be Creekbed color with ¼” flakes

(c) Shall slope to drain

(d) Shall roll up wall for a 6” tall cove base seamless

(e) Texture shall be medium slip coefficient

(f) Shall be free of tripping hazards

(10) Contractor shall provide a five year adhesion warranty

(11) All work shall be coordinated and scheduled with the Contracting Officer Representative (COR)

(12) Contractor shall provide all PPE, barriers, tools, materials, labor, and equipment to complete this project

(13) Contractor shall dispose of any and all waste generated by this scope of work in compliance with EPA regulations and outside of Park boundaries

c. Scope per Location

(1) Sugarlands Visitor Center Restrooms – See Drawings

(a) NOTE: This is one of the busiest Visitor Centers in the National Park Service

(b) Open to the general public and may be viewed by interested vendors without appointment

(c) The janitor closet, foyers, nor the entry hall shall be disturbed

(d) New flooring project shall begin at the thresholds of doors 16, 17, 18, & 19

(e) One women’s room and one men’s room shall remain open to the public at all times

1. Contractor shall ensure that the public has a safe, maintained unobstructed entry path to the open facility

(f) Existing ceramic tile shall be etched and cleaned

(g) Apply tile prep to create a bond over tile and grout

(h) Provide positive slope to floor drains

(i) Apply prime coat PermaFlex

(j) Apply liquid rubber SLR to smooth surface to ensure that grout lines or defects do not telegraph though to new finish floor

(k) Apply base coat PermaFlex and color flake

(l) Apply 2 coats polyaspartic

(m) Apply top clear coat of Graniseal

(2) Metcalf Bottoms (1), Cades Cove Picnic Area (2), and Deep Creek Group Camp (2)

(a) Five Restrooms in Total

(b) Have fresh water and 110 volt type power available

(c) Open to the general public and may be viewed by interested vendors without appointment

(d) Have the same footprint as shown on drawing

(e) Have approximately 300 SF of floor to be applied per building

(f) Currently have epoxy over slab

(g) Will be closed to the public for flooring project

(h) Contractor shall schedule all work with the COR

(i) Contractor shall schedule work for two buildings at a time in the same geographic location with the exception of Metcalf Bottoms (1)

(j) Contractor shall not be responsible for providing temporary porta-johns

(k) Contractor shall remove existing epoxy floor and base to bare substrate using a 99% dust extraction grinding system

(l) Existing concrete shall be etched and cleaned

(m) Apply crack fix to correct defects in concrete

(n) Provide positive slope to floor drains

(o) Apply prime coat PermaFlex

(p) Apply base coat PermaFlex and color flake

(q) Apply 2 coats polyaspartic

(r) Apply top clear coat of Graniseal

(3) Option 1

(a) See small bathroom drawings (300 SF each bldg.)

(b) Same scope and project work contained in this scope applies

1. Prepare and apply PermaFlex topcoat over concrete slab inside restrooms as heretofore described

(c) Additional 8 bathrooms in two areas on the Tennessee side of the park

1. Additional 4 bathrooms in Elkmont Campground

2. Additional 4 bathrooms in Cades Cove Campground

(4) Option 2

(a) See small bathroom drawings (300 SF each bldg.)

(b) Same scope and project work contained in this scope applies

1. Prepare and apply PermaFlex topcoat over concrete slab inside restrooms as heretofore described

4. Government Furnished Property, Materials & Supplies. None.

5. Business Hours. The contractor shall complete on-site work during normal business hours of 7:30 am to 4:30 pm, Monday through Friday, excluding Federal holidays. The contractor shall coordinate specific on-site work schedule with the Contracting Officer and Contracting Officer Representative (COR) five days prior to performance start date. While on site, the COR may be reached by mobile phone during normal business hours. Work shall be completed between August 8, 2016 and September 23, 2016 (possible extension depending on options awarded).

Any work outside of normal business hours shall be pre-approved by the Contracting Officer and

COR.

6. Management. The contractor shall manage the total work effort in accordance with effective and efficient management principles and safe working practices to ensure on-time completion of this contract.

7. Safety & Environmental Compliance.

The contractor shall comply with pertinent provisions of the Occupational Safety and Health Act (OSHA), and state, local, safety, laws.

The contractor shall comply with pertinent Environmental Protection Agency laws and regulations.

The Park safety officer has the authority to issue restraint or cease-and-desist orders to the contractor when violations or unsafe acts are present and threaten Government and contractor employees or damage to Government property.

The contractor shall immediately report to the Contracting Officer, COR and Park safety officer all accidents resulting in employee injury or death or damage to property while performing this contract.

8. Insurance. The Contractor shall be insured in accordance with State of Tennessee and North Carolina insurance regulations. The Government is not responsible for damage to Contractor’s equipment, vehicles, tools or materials or their employees and the personal property brought on-site. The Government will not accept any liability due to contractor employee negligence.

Workers Compensation: The contractor shall furnish proof of workers compensation insurance to the Contracting Officer for those employees performing work related to this contract and shall comply with the Workman’s Compensation Laws of the State of Tennessee and North Carolina.

Liability: The Contractor shall maintain a level of insurance that will protect the contractor and the Government from any claims as a result of bodily injury (including death) and property damage which may arise from and during the performance of this contract, and shall submit proof of insurance to the Contracting Officer.

9. Performance Requirements Summary (PRS). The contractor performance requirements are summarized into required services that relates directly to successful contract completion.

The performance standard identifies the minimum acceptable levels of service allowed.

10. Motor Vehicle & Traffic Regulations. Contractor employees operating motor vehicles shall possess a valid driver's license. Driver and vehicle permits, licenses, registrations and proof of insurance shall be maintained in the vehicle at all times and made available upon request.

Contractor employees shall comply with federal vehicle motor law requiring seat belts be worn at all times. Contractor employees shall adhere to the posted speed limits.

11. Employee Badges/Parking. All contractor employees shall comply with parking procedures published for the Park. Contractor employees shall return any PARK issued personal or vehicle identification badges immediately upon termination of employment or completion of this contract.

12. Replacement of Contractor Employee for Security Reasons. The Government retains the right to request removal of the contractor’s employee(s), regardless of prior clearances.

13. Non-Payment for Additional Work. Any additional contractor performed services not specified in contract, either at its own volition or at the request of an individual other than an appointed Contracting Officer, except as may be explicitly authorized in the contract, shall be performed at the financial risk of the contractor. Only an appointed Contracting Officer is authorized to bind the Government to a change in specifications, contract terms, or conditions.

14. Conflict of Interest. The contractor shall not employ off-duty COR’s or other Government employees if such employment would create a conflict of interest or be contrary to the policies contained in conflict-of-interest directives.

15. Quality Control. The contractor shall establish and maintain a Quality Control Plan (QCP) to ensure the work performed conforms to the contract requirements. The Performance Requirements Summary establishes performance standards that are the minimum requirements for acceptable performance of required services. The contractor’s QCP shall address methods for remedying poor performance. The Contractor shall develop QCP and submit with proposal package. The Contractor’s QCP includes:

16. Quality Assurance. The COR will monitor the contractor’s performance under this contract using the quality assurance procedures specified in the Government’s Quality Assurance Surveillance Plan (QASP). The QASP may be modified after contract award upon review and acceptance of the contractor’s QCP to ensure both plans are mutually supportive. The purpose of the QASP is to ensure that the required services are performed in accordance with the contract requirements. The plan ensures reliable and continued operation by precluding unnecessary complaints and unacceptable performance. Government’s QASP includes:

Acceptance of the contractor’s quality control plan Random inspection of the contractors work Communicating customer complaints Progress meetings with the contractor

The implementation of this QASP does not relieve the contractor of its responsibility to implement and abide by the QCP incorporated into this contract, and to provide acceptable performance of all required services listed herein. At the CO’s discretion, the contractor may incur a payment deduction for lack of compliance with the QCP, PWS or PRS.

17. Contractor’s Proposal Package. The following deliverables are to be submitted with offer in response to RFQ.

18. Inspection. The Government will conduct inspection upon completion of required services.

Final payment will be processed upon confirming that all noted contractor performance deficiencies have been satisfactorily corrected.
