Request for Proposal

To

Expand Dialysis Center

Project No. 667-23-102 10-07-2022

Overton Brooks VA Medical Center, Shreveport, LA

A. General Information A 1.0 Project Background, Description and Scope of Work Provide Architect/Engineer and Construction professional services to include design development, construction documents, specifications, cost estimates, construction period services, construction site visits and construction.

Overton Brooks VAMC has a project requirement to renovate a portion the existing Dialysis Center

(approximately 1,200 gfs, reference Attachment A, Floor Plan). This design/build shall include a renovation of surfaces and finishes in the patient and common areas. The patient areas which are currently configured as single-occupancy rooms, shall be configured into an open treatment area for multiple-occupancy, to include a nurse’s station and five bed/chair stations. Patient area renovation design requirements shall include, but not limited to the demolition of walls, reconstituting ceilings and floors, the reconfiguration of one unit of medical gasses, the addition of three eye stations at existing sinks, six potable water connections and six drains for dialysis machines. This design/build shall create more of a non-institutional setting, improve safety, infection control and reduce the cost of maintenance.

The new Dialysis center shall be in accordance with (IAW) the latest version of all applicable VA Design

Guidelines and Space Criteria. Plans should include but not limited to Life Safety, Architectural, Mechanical, Electrical, Plumbing, Structural, Controls, Communications, Asbestos Abatement and Fire

Alarms/Protection. All asbestos containing material within the space shall be abated including above the ceiling and any items determined to be abandoned or no longer required shall be identified to be removed and capped or terminated at the main source of supply. Access and continued function/operation of adjacent occupied spaces (above, below and beside) shall be considered within the construction phasing plan to minimize the overall impact and disruptions generated by the construction activities. The nature of the work and location may require after hours and/or weekend work.

This contract will provide for a complete site investigation (reference Attachment B, Existing Layout) and construction site visits.

PERIOD OF PERFORMANCE

The delivery date for this design/build contract shall be 120 days from Notice to Proceed.

TERMS OF THE SOLICITATION

A. The term “responsibility,” used in the instructions that follow, includes cost, schedule and construction responsibility for completion of this project in accordance with the documents of this RFP of OBVAMC project no. 667-23-102. (Drawings and specifications) for a fully functional facility.

B. The terms “the work and project” refers to the all-site investigation, site survey, calculations, and testing to renovate apportion of the Dialysis Center.

C. The term “VA” and the term “VAMC” refers to the Overton Brooks Veteran Affairs Medical

Center in Shreveport, LA.

D. The “VAMC Coordinator” and COR (Contracting Officer’s Representative) for this project is

E. The “CO” (Contracting Officer) for this project is to be determined (TBD).

DRAWINGS

This project will utilize AutoCAD, version 2021. All VA design guides and requirements can be found online at: https://www.cfm.va.gove/TIL/. Where conflicts occur between VA standards and applicable codes, the most stringent requirement shall govern. All drawings must be bundled using the “e-transmit” function.

SITE AND TOPOGRAPHY SURVERY

During the design phase, the A/E will coordinate with VAMC Staff to perform a site survey to facilitate the design. A topographic survey is NOT required.

GEOTECHNICAL SURVEY

A geotechnical survey of this area is NOT required.

UTILITY SURVEY

During the design phase, the designer/builder shall verify all VA information provided, the contractor will coordinate with VAMC Staff to obtain any available utility information to facilitate the design/build.

Otherwise, what was not obtained shall be surveyed to extent necessary by the designer. Designer/

Builder shall procure any service (to include, but not limited to HVAC air and hydronic pre-testing and evaluation) required to adequately test and determine the capacity of the existing utility system.

ASBESTOS ABATEMENT SURVEY

During the design, the contractor will coordinate with VAMC Staff to perform an asbestos abatement survey to facilitate construction. The designer and VA shall survey the areas of conflict/in question to incorporate abatement accordingly.

HANDICAP ACCESSIBLITY

The design/construction will be required to comply with the Uniform Federal Accessibility Standards

(UFAS), ABAAS (Architectural Barriers Act Accessibility Standards) and PG-18-13 (Barrier Free Design

Guide) in all areas.
