Replace Electrical Panel Boards in OR with Dual Feed Project Number 546-20-103

1. General Information

The Department of Veterans Affairs, Network Contracting Office 8 (NCO-8) is seeking and intends to award a fixed-price contract for construction services for the complete project, which include construction services for project number 546-20-103, “Replace Electrical Panel Boards in OR with Dual Feed” located at the Miami VA Medical Center (MVAMC) at 1201 NW 16TH Street, Miami, FL 33125. The project requires the corrections of all electrical violations found in the most recent Condition Assessment Report (CAR) for the MVAMC as per National Electrical Code (NEC) and National Fire Protection Association (NFPA) 99. The Operating Rooms (ORs) are Considered Wet Locations and require Isolated Power Panels and receptacles shall be split between two branch power sources.

1.1 Scope

The main objective is to replace the existing standard ground panel boards with Isolated Power Systems (IPS)/Isolated Power Panels (IPP) in the 3rd floor operating rooms OR-1, OR-2, OR-3, OR-4, Special OR-1(OR-5), Special OR-2(OR-6), Special OR-3(OR-7), Cystoscopy OR-1(OR-11), Cystoscopy OR-2(OR-10) and provide redundant sources of power from two different critical branch ATS circuits in each OR to comply with current VA Criteria. Contractor shall read and analyze the complete Design Package and performed all construction activities (build, installation, replacement, wiring, control, calibration, adjustment, testing, etc) while maintaining the operation of the OR during construction as per the 100% Final Design, Narrative and Specifications provided from Toland Mizell Molnar.

After the completion of replacement of all Electrical Panels as part of the Improvement system coordination, the entire system shall be tested. The Contracting Officer Representative (COR) and an electrical representative from MVMC shall be present at the time of testing.

A complete electronic copy of the 100% Design, Narrative and Specifications from Mizell Molnar will be provided to all Bidders as part of the Request for Proposal (RFP) package.

Note: If available, Existing Facilities Drawings, Policies or Supporting Documents can be requested at Facilities Management Services. Most drawings are in AutoCAD format, but some are scanned PDF.

1.2 Submittals

The contractor shall provide the MVAMC a detailed proposal (no lump sum line items) and a detailed construction completion schedule with a detailed tasks of activities per each Phase of the project construction (10 to 15 days maximum per Phase, except Phase 9) to minimize the OR downtime and impact as per the Narrative Document provided from the AE firm of records for review and acceptance.

The construction Phase Order Schedule must be coordinated with the Project COR especially for OR 6 which depends on OR Team availability.

All Parts Submittals must be Submitted and Approved prior to start any work.

All Parts, Materials and Equipment must be on hand prior to start any work.

The Contractor shall provide to the Engineering Service one electronic copy of all As-Built

Drawings as well as equipment brochures, operating, maintenance and user manuals.

1.3 Project Justification

The ORs have Ground Electrical Panels installed that must be replaced by Isolation Power Panels and receptacles shall be split between two branch power sources as per NEC and NFPA 99 requirements for Wet Locations. Further, all the panelboards in the operating rooms are fed from a single panel with a 400A breaker; that panel is fed from another 400A breaker in a different room which is fed from yet another 400A breaker in another floor. All these circuit breakers are down-stream of the automatic transfer switch. If any (or all) of those breakers are opened (such as a fault condition), 100% of the electrical power in all OR will be lost simultaneously for however long it takes for someone to find and reset the affected circuit breakers (likely 15-30 minutes).

3 Project Classification

This project has been classified per the Construction Infection Control Risk Assessment as a Class III (Type D, High Risk Group) project.

3.1 Meetings, Conferences, and Site Visits:

Meetings at the site are anticipated however, teleconference may be request by the contractor if necessary.

3.2 Meeting Minutes. The contractor shall be responsible for generating meeting minutes for the post award meeting and any subsequent progress meetings. The meeting minutes shall document all items discussed at the meeting and shall include a list of meeting attendees. If meeting occur

4. Requirements

4.1 General

Conduct independent review of the service and production programs to identify requirements consistent with directives governing the acquisition of system and equipment.

Using acquisition plans, existing hardware contracts and inherent lead-time items, construction schedules for inclusion in documentation for system and equipment acquisitions.

Based on production program schedules as well as system configurations, formulate technical documentation for these programs itemizing all supplies, data and services to be obtained.

Provide impact statements when deviations or changes occur and alternative recommendations when required to maintain individual program production integrity.

Identify, compile, and utilize available information, update and input data for manual or automated production scheduling information systems, prepare government production reports germane to maintaining system production status and inventory.

Prepare recommendation for identifying project data to be entered existing Automatic Data Processing (ADP) programs. When data system deficiencies are discovered, provide recommendations for solutions.

Have a minimum of ten years of specialized continuous experience in Installation of Electrical Switchgear, Substations, Panelboards, Arc Flash Analysis, Circuit Coordination, Short Circuit and knowledge of the VA working conditions as required by this Statement of Work (SOW).

Document all work performed. Material installations performed shall follow manufacturers’ recommendations and applicable NEMA, NEC, ANSI, ASA and VA standards and VA Electrical/Mechanical Design Manual.

Comply with all current codes and standards as they relate to that facility.

Be responsible for all required licenses and permits.

Furnish enough technical, supervisory, and administrative personnel to ensure the expeditious accomplishment of the work specified in this SOW.

Adapt as necessary any Government drawings, specifications, or standards that may be provided for areas and other structures to meet the requirements of the project.

Be responsible for providing all labor, materials, equipment, and the necessary manpower.

Follow all applicable VA Directives and Specifications, including GEMS, Safety, Fire Protection, and Infection Control.

4.2 Detail Tasks

The Contractor shall accordance with the 100% Design, Narrative and Specs Submittal from Toland Mizell Molnar, prepare site for building operations, including demolition and removal of existing structures, furnish labor, materials and perform work as required.

Contractor Shall coordinate all work with project COR, provide a cost loaded schedule after issuance of NTP and before commencement of work.

Total Project Period of Performance is 550 Calendar Days from the Notice to Proceed (NTP).

4.3 Additional Responsibilities

Contractor shall use environmentally friendly products whenever possible.

Contractor is responsible for relocation and/or covering any furnishings in the work area.

Contractor shall ensure all products/equipment used for this project are stored, handled, applied, and disposed of following the manufacturers’ instructions.

Contractor shall only use products suitable for the job conditions. Manufacturer’s instructions and recommendations shall be followed for all products used.

Contractor shall replace any damaged grass, sprinkler heads, or piping caused by or during the work performed.

Contractor shall adhere to all safety and infection control measures while performing work at the facility.

Contractor shall provide negative air pressure machines as required to maintain a pressure of 0.01-inch WC.

Contractor shall be responsible to maintain and upkeep their staging area which will be provided by Police Services.

Contractor shall provide their own dumpster and properly dispose of all items and debris created from the construction and demolition in every aspect of this project and provide Construction Waste and Recycling Documents to the Project COR for VA GEMS Records.

Contractor shall Daily dispose of waste and debris from Construction Site.

Contractor shall follow all additional requirements as specified in the VA Construction Policy.

COR will provide permits necessary to complete the project.

Contractor will submit before and after pictures of the project.

Contractor will ensure value engineering is included in this project to maintain approved budget.

Contractor shall develop a plan detailing project specific fire-safety measure.

Contractor shall maintain site and building access free and unobstructed to facility emergency services. Means of egress shall not be blocked.

All temporary construction partitions must meet fire-safety requirements.

Temporary heating and electrical installations must comply with OSHA 1926, NFPA 241 and NFPA 70.

Contractor furnished fire extinguishers must be provided and maintained in construction areas and in temporary storage areas.

Flammable and combustible liquids must be stored, disposed, and used in accordance with OSHA 1926, NFPA 241 and NFPA 30.

Existing fire protection must not be impaired. Provide fire watch for impairments more than four (4) hours in a 24-hour period.

Contractor shall perform Hot Work in accordance with NFPA 241 and NFPA 51B.

Contractor shall maintain all construction permits visible and updated as required by the VA Safety Office and Infection Control Department.

4.4 Expectations

Contractors will make site visits as necessary to survey existing conditions and to determine existing as-built conditions affecting this project.

Contractor shall provide any structural, architectural, electrical, and mechanical investigation work of existing and designed systems necessary to ensure the accuracy of the project.

Contractor shall provide any measured drawings of existing systems of facility required to ensure the accuracy of the project.

4.5 Master Specification Index

Whenever is possible, Contractor and Subcontractor shall follow the specifications on The VA Master Construction Specification which contains work procedures and materials designed to meet regulatory compliance requirements. The Master Construction Specification must be used as the basis for the project designs.

5 Environmental/Occupational Safety and Health (EOSH)

All projects and project designs shall contain language that addresses all applicable EOSH requirements.

6 Location/Hours/Deliverables

6.1 Location

Construction of the “Replace Electrical Panel Boards in OR with Dual Feed” is required at the Miami VA located at 1201 NW 16th St, Miami, FL 33125.

6.2 Hours

Work is to be performed Continuously on a 24 Hrs. Shifts, 7 Days a Week except work in the corridors or other areas that cannot be performed during normal business. Any work performed in an area necessary for the operating suite operations will be performed outside of normal business hours. Contractor access will be provided during this time. Contractor shall not have any personnel on site without the proper approval from the COR. Contractor and subcontractors are required to have a form of physical identifier such as a uniform or arm band reviewed and approved by the COR.

All work in areas involved shall be scheduled with the VA Engineering COR. Work shall be scheduled for proper execution to completion of this contract.

All shutdowns involving temporary impairment of utilities shall be requested in writing to the VA COR three (3) weeks in advance and scheduled to minimize the impact (Off Shifts, Nights, Weekends, or Holidays). Shutdowns shall be considered as an activity in the contractor schedule. The following list addresses the kind of shutdowns or service interruptions that shall require advanced notice:

· Electrical Systems shutdown including Normal or emergency Power.

· Fire Suppression Systems: Sprinkler shutdowns lasting over four (4) hours require notification and concurrence from the VISN Safety office. Fire Alarm work shall be conducted by Simplex.

· Parking.

· Cranes: Prior to crane arriving on site contractor shall submit: Crane maintenance logs, crane operator’s license, drawings showing crane’s operational and exclusion zone.

All documents shall be revised and approved by the MVAMC Safety Office prior to scheduling the work.

6.3 Deliverables

The Contractor will submit a daily log, sign-in sheet, pictures, work performed each time services is rendered.

The Contractor shall submit a report for each shipment of construction debris, the weight by type, shipment date, destination, and whether the debris is to be recycled/reclaimed or disposed to the COR. Types include concrete, lumber, metal, dry wall, etc.

7 Contractor Personnel and Management

7.1 Contractor Personnel

The Contractor shall be responsible for employing qualified personnel to perform the work specified in this SOW. The Contractor is responsible that all subcontractors and employees have the proper documentation to authorize personnel to work in the United States of America. The Contractor shall maintain the personnel, organization, and administrative control necessary to ensure that the work delivered meets the contract specifications and requirements.

Contractor and all subcontractor employees are subject to screen/background check by the Miami VA Healthcare System. Contractor and subcontractor employees are required to wear a government VA furnished badge and company furnish badge while performing services under this contract. The badge will contain a personal picture, name of employee, and Contractor's name. Badges shall be worn on the outer garment attached to the outer shirt or jacket pocket by a button or clip or worn around the neck secured by an appropriate identification card lanyard.

Government issued identification badge remains the property of the Government and shall be returned to the issuing office or other designated Government personnel upon transfer of Contractor employees from the contract.

Contractor is responsible that all subcontractors and employees have a valid driver license when driving a vehicle on property.

7.2 Management

All information generated in performance of this contract becomes property of the Government and must be surrendered upon request and/or completion of the contract. The Contractor must protect against the destruction and/or misplacement of all information produced under this contract, unless directed under local policy to destroy. This includes both hard and soft copies of data.

Contractor shall not release any information (including photographs, files, public announcements, statements, denials, or confirmations) on any part of the subject matter of this contract or any phase of any program hereunder without the prior written approval of the COR.

Contractor and subcontractor employees shall conduct themselves in a professional manner (i.e., Timeliness, Communication; Spoken and Written etc). The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of suspected misconduct, a suspected security breach, or suspected to be under the influence of alcohol, drugs, or any other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interests of the Government.

The hospital director or designated representative has the authority to bar individuals from the Government facility. The removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide enough personnel to perform the services as required by this SOW. The Government shall not reimburse the Contractor for travel and other expenses associated with the removal of personnel.

Contractor personnel are not authorized to carry or possess personal weapons to include, but not limited to, firearms and knives with a blade length more than three inches (3 Inches), while assigned under this contract.

8 Safety/Security Requirements

Contractor shall comply with all applicable federal, state, and local laws and ensure performance is secure while protecting material, equipment, and supplies from damage and loss. Contractor is responsible for proper use and wear of the Personal Protection Equipment (PPE) for subcontractor and employees during the entire project. Government security personnel shall have the express right to inspect for security violations at any time during the term of the contract.

All work performed under this contract will be in accordance with Safety and Health During Construction Activities Policy 138-80-16. Contractor shall provide a written certified statement indicating that the construction project meets the life safety code requirements. For those maintenance and repair projects where the life safety code requirements are not applicable, a written statement shall also be provided accordingly.

The Miami VA Safety Officer and COR shall have full authority to check that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract.

Contractor shall provide all necessary barricades / signage and temporary escape route alternative if any.

Contractor shall isolate the area to prevent the spread of debris into adjacent space and maintain the working area negatively pressurized when required as per the Infection Control Risk Assessment.

9 Work Attire and Appearance

The contractor and subcontractor employees shall present a professional appearance commensurate with standards delineated for Government civilian personnel acting in similar capacities.

10 Personnel Qualification

Contractor shall be responsible for employing certified license technical qualified personnel to perform the work specified in this SOW. Contractor shall maintain the personnel, organization, and administrative control necessary to ensure that the work delivered meets the contract specifications and requirements. The work history of each contractor employee must contain experience directly related to the task and functions he/she is intended to perform under this contract. The Government reserves the right, during the life of the resulting contract, to request work histories on any contractor employee for the purposes of verifying compliance with the above requirements; additionally, the Government reserves the right to review resumes of contractor personnel proposed to be assigned. Personnel assigned to, or utilized by, the Contractor in performance of work shall be fully capable of performing the requirements contained in the SOW in an efficient, reliable, and professional manner.

Contractor shall provide a supervisor on site at all times who can fluently communicate in English (speak, read and write).

Contractor is responsible for subcontractor and employees training for safety and performance of their respective duties.

Contractor shall furnish appropriate safety equipment to subcontractor and employees as prescribed by OSHA Standards.

The Government reserves the right to review Contractor personnel qualifications. Upon request, resume shall be provided to the Contracting Office prior to clearance approval, for review and acceptance by the COR. The intent is verification purpose and not for the Government to become the hiring authority.

Contractor/Subcontractor Superintendent, Project Manager, Supervisor, QC shall be OSHA 30 Certified and employees OSHA 10 Certified. Occupational Safety and Health Administration (OSHA) 30/10-hour construction safety course proof is required. The contractor shall designate a “competent person” in accordance with 29 CFR 1926. 32(f) which states, “One who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous or dangerous to employees and who has authorization to take prompt corrective measures to eliminate them”. The contractor shall instruct each of his employees in the recognition and avoidance of unsafe conditions and the regulations applicable to this work environment to control or eliminate any hazards or other exposure to illness or injury.

11 Non-Personal Service Statement

The contractor shall always supervise and control subcontractor and employees performing services under this SOW. Contractor management will ensure that subcontractor and employees comply with the performance work standards outlined on this SOW. Contractor and subcontractor employees will perform their duties independent and without the supervision of any Government Official. The tasks, duties, and responsibilities set forth in the contract may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal Policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work.

12 Damages/Insurance

The contractor and subcontractor employees are liable for any damage caused to Government property, equipment, utilities, etc., located at the site (i.e., sprinkler heads, ceiling tiles, manholes, fire hydrants, vehicles, windows, etc). The contractor shall immediately report any damages to the COR and shall provide a written report, within 24 hrs. Report shall include: What occurred, who was involved, when it occurred (Time/Date), where did it occurred, why it occurred, how it occurred, pictures, any supporting document and the proposed means to repair or replace.

The contractor shall maintain free and unobstructed access to facility emergency services and for fire, police, and other emergency response forces in accordance with NFPA 241.
