Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C261-20-AP-4805

1. Contracting Activity: Department of Veterans Affairs, VISN 21, VA Northern California

Healthcare System, RPO-West, Network Contracting Office, NCO 21, Martinez, CA for the VA Palo Alto Health Care System in Palo Alto, CA. 2237 PR. NO. 640-20-3-163-0132.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The VA

Palo Alto Health Care System (VAPAHCS) is seeking to purchase a name brand only OPTOS Silverstone Retina Imaging Device for the Ophthalmology Clinic to replace an older model that are at end of life.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The OPTOS Silverstone P200TxE with DICOM will replace the current OPTOS TX200 located at the VA Palo Alto Ophthalmology Clinic. The new OPTOS Silverstone is currently the only machine on the market that can take 200-degree pictures of the back of the eye as well as Optical Coherence Tomography OCT imaging of the peripheral retina. OCT uses light waves to take cross-section pictures of the retina. With OCT, the ophthalmologist can see each of the retina’s distinctive layers which allows them to map and measure their thickness. This is a very important feature in making a precise and accurate diagnosis.

The new equipment must be able to integrate with the current network system and must be compatible with the OPTOS TX 200. The current OPTOS TX200 cannot be networked and all images that are taken on it cannot be printed or sent/uploaded to the patient’s chart. If anything happens to the current OPTOS TX200 these important images will be lost, and it would be damaging to the continuity of care for the eye patients. Since the new OPTOS Silverstone is compatible with the OPTOX TX200 the clinicians can easily and safely transfer the current data off the older model onto the new OPTOS Silverstone without losing any existing data. The OPTOS Silverstone with DICOM functionality also enables imaging (patient records) to be sent electronically to CPRS and the AXIS software. This means that images can be viewed on any computer with access to CPRS and Vista Imaging. The compatibility with the Axis Software is a huge benefit for both the doctors and patients. Also, the old OPTOS TX200 machine will be traded in for a credit when the new machine is purchased.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was conducted by searching and reviewing VIPBIZ, FPDS, GSA Advantage, and eCMS. It was determined that OPTOS, Inc. has one verified SDVOSB and one small business distributor that appear qualified to fulfill this requirement.

Since the OPTOS Silverstone is proprietary to OPTOS, Inc, I recommend issue a 100% small business set-aside name band only combined synopsis solicitation on Contract Opportunities as the method of procurement.
