Attachment 3

1. GENERAL INSTRUCTIONS

1. Your attention is directed to the requirements for technical and business quotes to be submitted in accordance with the following instructions establishing the minimum requirements for the format and content of quotes.

2. The proposal must be prepared in two parts: Volume I – Technical Quote and Volume II

– Price Quote. Each of these parts shall be separate and complete in itself so that evaluation of one may be accomplished independently of evaluation of the other.

Volume I – Technical Quote must not contain references to cost; however, resource information such as data concerning labor hours and categories, materials, subcontracts, etc., must be contained in the technical quote so that your understanding of the scope of work may be evaluated. It must disclose your technical approach in sufficient detail to provide a clear and concise presentation that includes, but is not limited to, the requirements of the technical quote instructions.

The quote must be signed by an official authorized to bind your organization.

3. It is understood that your quote shall become part of the official contract file.

4. This RFQ does not commit the Government to pay any cost for preparation and submission of a quote. In addition, the Contracting Officer is the only individual who can legally commit the Government to the expenditure of public funds in connection with this proposed acquisition.

5. Any resultant award shall include all applicable clauses to the type of task order awarded.

Copies of clauses may be obtained by contacting the Contracting Officer. Any additional clauses required by public law, executive order, or acquisition regulations, in effect at the time of execution of the proposed contract, will be included.

2. VOLUME I - TECHNICAL QUOTE INSTRUCTIONS

Quoter shall submit quotes and data comprehensive enough to provide the basis for a sound evaluation. The information should be precise, factual and responsive.

The quoter is expected to show a thorough understanding of the proposed tasks as specified in the Statement of Work (SOW). The quote should address the specifications in the SOW, not simply repeat the language. All information necessary to judge the technical soundness and management capabilities of the Contractor will be contained in the technical quote.

The technical quote shall not contain price information; however, resource information such as data concerning labor hours and categories, materials, subcontractors, etc., must be detailed in the technical quote so that the Quoter’s understanding of the detail and complexity of the requirements may be evaluated.

The technical Quote must contain:

1. Cover page – Including the name of the proposing organization, author(s) of the technical proposal, the solicitation number, and the title of the solicitation should appear on the cover.

2. A resume or curriculum vitae detailing the Quoter’s expertise and experience in serving as a subject matter expert in the field of clinical consulting.

3. Information relative to experience in clinical consulting services.

4. Technical quote must meet the core requirements listed below:

a. Hold a degree in medicine, a State license to practice medicine, and a satisfactory record in practicing medicine for a minimum of 20 years;

b. Demonstrate knowledge and expertise in hospital patient safety and infectious disease issues, with a minimum of 20 years’ experience working in a hospital setting;

c. Demonstrate national leadership in assessing hospital patient safety and infectious disease control;

d. Demonstrate ability to develop evaluation protocols for data collection, analysis, and aggregation of data from healthcare sources;

e. Demonstrate experience working in research projects regarding management and clinical approach to infectious disease control;

f. Demonstrate ability and experience in using software to enter and analyze data, including Microsoft Excel and Microsoft Access;

g. Demonstrate knowledge and experience in working with large projects with limited capacity as well as those with larger capacity (i.e. one or more FTE’s);

h. Demonstrate satisfactory record of 5 years’ experience as trainer and technical assistance provider regarding patient safety and quality of care issues;

i. Demonstrate a satisfactory record of integrity, business ethics and provide a list of prior contracted labor for past 5 years related to review and evaluation projects targeted at hospital safety and infection control.

j. Demonstrate a satisfactory record of integrity, business ethics and provide a list of prior contracted labor for past 5 years related to review and evaluation projects targeted at hospital safety and infection control.

3. VOLUME II- PRICE QUOTE INSTRUCTIONS

The quoter must stipulate that it is predicated upon all the terms and conditions of this RFQ. In addition, it must contain a statement to the effect that the offer is firm for a period of at least 90 days from the date of receipt by the Government. The Quoter shall include a completed copy of the following charts in its quote:

Base Period: 12 months Labor Category Fully Loaded Hourly

Rate Estimated Hours Total Price

Total Direct Labor

Total Ceiling Price

The Quoter shall submit a quote and data comprehensive enough to provide the basis for a sound evaluation. The information should be precise, factual and responsive.

4. EVALUATION FACTORS FOR AWARD

4.1 GENERAL

In order for quotes to be considered acceptable and eligible for evaluation, they must be prepared in accordance with and in compliance with the instructions given in this RFQ and address all of the requirements set forth in Attachment 1. A quote which merely offers to conduct a program in accordance with the requirements of the Government’s scope of work will not be eligible for award.

4.2 BASIS FOR AWARD

The following factors will be considered in evaluating proposals: Technical and Price

The Government will award a contract resulting from this solicitation to the responsible quoter whose quote conforming to the RFQ will be most advantageous to the Government, price and other factors considered. The initial quote should contain the Quoter’s best terms from a cost and technical standpoint.

The offeror’s technical quote will be evaluated and determined technically acceptable or unacceptable. Discussions will be held with the quoter if the Contracting Officer determines that it is necessary, however the Government may award the contract without discussions. Therefore, the quoter’s initial proposal should contain the quoter’s best terms from a cost and technical standpoint.

Award will only be made if the quoter’s quote is technically acceptable and costs are considered to be fair and reasonable.

4.3 TECHNICAL EVALUATION

Technical proposals will be evaluated by the Government to determine the degree to which the quote satisfies the requirement, i.e. technically acceptable.

4.4 PRICE EVALUATION

The proposed price will be evaluated on the basis of price reasonableness. For a price to be reasonable, it must represent a price to the Government that a prudent person would pay when consideration is given to prices in the market. The total evaluated price for each quoter will be for the entire Period of Performance.
