Statement of Work

Wide field Imaging / OCT machine

1.0 BACKGROUND

The Ophthalmology Department requires 1 OCT machine for the Main Hospital at Decatur Eye Clinic. The requirement is due to end of life of the current Cirrus 4000 OCT machines used during eye exams.

2.0 SCOPE

The vendor/contractor will provide for this purchase.

1 (one) OCT machine to be used in patient eye exams. Installation and assembly of the OCT machine with training on how to use its features for the medical and biomedical staff.

3.0 VENDOR/CONTRACTOR REQUIREMENTS

The vendor/contractor shall ensure equipment meets the following specifications.

Machines software must be able to transmit images and reports the forum server.

Machine must be on the VISTA imaging DICOM approved devices list:

VistA Imaging DICOM Modality Interfaces (va.gov) Machine must have wheelchair accessible stand

4.0 TASKS

The contractor shall complete the following tasks in support of this delivery:

Delivery and installation provided by vendor.

Medical and Biomedical staff training with all OCT features and required maintenance.

5.0 DELIVERABLES

The vendor/contractor agrees to the following deliverable dates:

installation provided by vendor within 120 days of purchase

a. Vendor will deliver the equipment within (30) days after the award to the Atlanta VA Health Care System Warehouse (no installation).

6.0 PERIOD OF PERFORMANCE $ COSTING

See Table Below 9.0.

7.0 PLACE OF PERFORMANCE

Main hospital in Decatur, GA Eye Clinic

8.0 CLASSIFICATION/SECURITY

Security clearance is not required. Vendors will not install equipment/software to the IT or Biomed networks or need system access.
