STATEMENT OF WORK
STATEMENT OF WORK
PERFORMANCE WORK STATEMENT FOR ON-SITE DOCUMENT DESTRUCTION AND REMOVAL
1. GENERAL REQUIREMENTS
1.1
The Contractor shall plan, program, administer, execute and provide personnel, management, equipment and other items and services not Government furnished but necessary to perform On-site Document
Destruction and Removal as defined in this Performance Work Statement at Naval Hospital Bremerton
(NHB) and outlying clinics. The Contractor shall comply with applicable Federal, State, and local laws, regulations. Codes, technical manuals, manufacturer’s instructions, recommendations, and acceptable commercial practices in the delivery required services. See Appendix A for Facilities and Frequency.
1.2
The Contractor shall furnish locked security containers to NHB, as required. Government will place material with sensitive information, but no classified information, in the containers. The Contractor shall be responsible for delivery of containers, maintenance and removal of storage containers throughout the contract period. Contractor shall collect the containers, destroy the material on the grounds of the NHB facility, and provide the certificates of destruction to the Government representative.
1.3
At a minimum, the Contractor shall provide locked security containers on wheels and sufficient number of keys. Containers shall have individual, permanent/imprinted numbers to facilitate inventory and assignment accordingly. Contractors can number containers as they choose as long as the numbers are permanent and readily identifiable on each container provided.
2. CONTAINERS
2.1
At a minimum the Contractor shall provide locked, wheeled containers that an average employee can move when filled. The Contractor shall initially provide the number of containers for each location as specified in Appendix A with the ability to increase or decrease the number and size of containers for each site. If changes are required, the Government will issue a contract modification.
3. COLLECTION POINTS AND SERCURITY/DESTRUCTION STANDARD
3.1
The Government will be responsible for moving locked containers to and from the designated destruction/shredding location site. The Contractor shall ensure the security of all waste material until it is successfully destroyed. Destruction of all waste material must be in such a manner as to guarantee that the material can never be reconstructed back to its original form. The Contractor shall leave the service loading areas in a clean and orderly condition at each shredding site. The Government shall not be held responsible for damage to the Contractor’s equipment. The Government shall ensure a representative at every destruction evolution is present to ensure above parameters are met.
3.2
The Contractor shall only bill for and report actual number of containers of materials for destruction and not by the total quantity of containers assigned to each location.
4. MATERIAL TO BE DESTROYED AND RECYCLED
4.1
The Contractor shall ensure that all eligible documents and records are recycled.
4.2
Paper document-type material will include mixed size documents varying in type, weight, size and color.
Material will contain staples, paperclips, and folders with metal clips. Additionally, non-paper document-type includes, but not limited to, photo impressions, computer tape printouts, microfilm, carbon paper, carbon typewriter ribbons, CDs, DVDs, streaming tape cassettes, or any other material containing individually identifiable health information protected by the Health Insurance Portability and
Accountability Act (HIPAA), or other Government sensitive information which has served its purpose.
5. ADDITIONAL WORK – NEW CUSTOMERS AND DOCUMENT PURGES
5.1
The Contractor shall have the capability to add additional facilities. The Government may add or delete locations based upon operational requirements via a modification to the contract. The Contractor shall be able to perform unscheduled document destruction services for document purges with ample notification from the Government. If the Government requires unscheduled services, the Government representative will coordinate with the Contractor and the Contracting Officer with a minimum of 72 hours prior to the work being accomplished. Special pickups may be required during times of overflow conditions.
6. MONTHLY REPORTING
6.1
The Contractor shall provide monthly of total weight collected and total weight recycled, and email report to the Government representative and to the NHB Environmental office (email to be provided post-award).
The report may be done in a conjunction with invoice. The report shall be in electronic and readable format by Microsoft Office (spreadsheet is preferred).
7. CONTRACTOR PERSONNEL AND SECURITY
7.1
The Contractor shall screen contract employees (subcontractors are considered employees) which have access to sensitive materials in the performance of this contract, prior to performing work at NHB.
Screening shall include (1 ) third-party Criminal History Background Check for each county and/or state where the employee has resided during the previous seven years, and (2) in-house or third-party verification of all employment within the last seven years. Screening is at contractor’s expense. No contractor employee having been convicted of a felony, involving any theft of larceny that is related to any fiscal, financial matter shall perform under this contract. Employees must be U.S. citizens.
7.2
The Contractor screened employees who require access to NHB to perform work under this contract shall obtain a Government-furnished identification (ID) card via current Navy regulations which requires the ability to pass a National Crime Information Center database check. See Clause 5252.204-9400.
Contractor employees shall be in a company uniform and wear photo ID badge while at NHB locations in
Appendix A.
7.3
Contractor employees shall be able to read, write, speak, and understand English in order to effectively communicate with Government personnel.
8. SAFETY
8.1
The Contractor shall exercise all standard safety precautions (e.g., warning signs, interruption of work, phasing of work, etc.) to allow safe passage of visitors, staff, and equipment in work areas. The Contractor shall provide appropriate warning signs where operations may present a traffic obstruction or safety hazard.
No smoking is allowed on NHB installations.
9. HOURS OF OPERATION
9.1
The hours of operation for performance of this contract are Monday through Friday, 0800 hours until 1530 hours. However, established dedicated days and times of the week per location in Appendix A shall be established to maintain consistency for Government personnel thus lessening the impact to Government operations. No pickups shall be required on Saturdays, Sundays, or Federal Holidays. However, should a
Federal holiday fall on a weekday, it is understood that the Contractor will be on site the following day or
Monday, at the same established time. On approximately a quarterly basis, non-paper destruction services will occur in which the Government will have the flexibility to schedule.
10. GOVERNMENT FURNISHED PROPERTY, SERVICE AND UTILITIES
10.1
The use of Government furnished property shall not be authorized for this solicitation / award. The
Government will designate a location near the exterior of each building or complex where the Contractor shall perform destruction services.
