1. Purpose: In accordance with ref (a) and (b), a quote is requested to provide Materials Handling Equipment (MHE) service and support to include an open/inspection, repairs, services, and/or materials. The following information and requirements are hereby provided for planning and pricing purposes. Submission of “Price & Availability Feedback” does not constitute a guarantee of purchase, service contract, or delivery order.

3. Location of Equipment/Services: SI RACHA, THAILAND – (C7F AOR)
4. Period of Performance (POP): 04 JAN 2019 to 06 FEB 2019
5. SOR: N2900383044001
6. Statement of Work (SOW): The following work items shall be accomplished by the selected service provider (contractor) in conjunction with this repair and maintenance requirement. The Government intends to award a Fixed Price Contract to the selected service provider:
a. Contractor shall conduct training and instruction for up to three (3) crewmembers in the procedures required to test and certify all onboard Material Handling Equipment (MHE) – and associated equipment – in accordance with the procedures and requirements set forth in references (a) and (b).
b. Contractor shall conduct training and instruction for up to three (3) crewmembers in the procedures associated with the successful completion of periodic maintenance and repair of all equipment listed in Tables 1 and 2, below.
c. Contractor shall perform all maintenance, repair, fork (tine) inspections, and safety certification actions on equipment identified in Tables 1 and 2 in accordance with the requirements set forth in attachments 1 through 5, vehicle technical manuals, and references (a) and (b).
d. Contractor shall conduct training for up to three (3) crewmembers in the use and maintenance of installed electric forklift battery chargers. Contractor shall oversee or perform an operational check on all battery chargers listed in Table 3 to ensure proper operation, voltage, and amperage settings. Contractor shall reset battery chargers in accordance with the specifications provided in the battery charger technical manuals, as required.
e. Contractor shall train ship’s crew in the performance of basic corrosion control on all MHE utilizing appropriate wire brush and spot painting techniques. Contractor shall perform corrosion control that is beyond the capability/available manpower of ship’s personnel. Approved spot paint includes “Rust-Oleum brand” Enamel Gray Primer (V2182838), Black Rust Reformer (215634), Equipment Yellow Paint (V2148838), and Gloss Black paint (V2179838), or equivalent.
f. Scope of repair work includes, but is not limited to: repair/replacement of brakes (including parking brakes, master cylinders, wheel cylinders, brake shoes, and brake lines); lights and light wiring; vehicle electronics and computers; encoder bearings; bushings; mechanical and hydraulic systems; hydraulic hoses; lifting components; repack lift and tilt cylinders as necessary; verification of error codes and completion of repairs required to clear the codes; re-stenciling; tire replacement utilizing vessel tire stock and onboard tire press. Items outside the scope of work include: transmission repairs, major engine repairs, differential repairs, and repairs requiring the removal of any major component (e.g. engine, mast, counterweight, axle).
g. Contractor shall perform a test of all rear brake systems on standard HYSTER forklifts listed in Table 1 by lifting the rear axle off the ground, applying the brakes and testing the brake operation. Proper operation of the rear master cylinder and wheel cylinders will be verified. Master cylinder and brake drums shall be adjusted to ensure appropriate rotational resistance of the rear wheel with brake pressure applied.
h. Contractor shall perform an operational inspection and a periodic maintenance service on the 10K ordnance handling trailers identified in Table 2 in accordance with manufacture recommendations. Contractor shall inspect 10K trailer non-skid surfaces for durability and wear, and provide condition (satisfactory/unsatisfactory) status report, as required.
i. Contractor shall remove, and inspect the counterweight, mounting bolt, and the base plate threads for wear on all shipboard diesel forklifts to include removing the bolt, and counter weights for inspection of rust, corrosion, and worn threads on the bolts, and threaded base plate surface. Contractor shall re-install the counterweight, and mounting bolts that are within SAE standards, and re-torque the counterweight bolts to 410 foot pounds, or replace the bolts, and re-thread the base plates that fail the visual inspection, as required.
j. In preparation for the onboard service, the contractor shall coordinate with the vessel’s Chief Engineer to prepare an estimate of required parts needed to complete the service. Based on this evaluation, the Contractor shall provide a quote to MSC for repair parts. Upon approval and authorization from an official DOD contracting officer, the Contractor shall procure all required parts and liaise with the MHE Program Management Office for prepositioning parts onboard the ship 30 days prior to the start of the MHE recertification project. All required parts must be prepositioned onboard prior to commencement of the service.

8. Deliverables:
a. Contractor shall provide a condition found report (CFR) for each unit that identifies the current operational condition of onboard MHE, status of safety devices (e.g. backup-alarms), and all repairs completed.
b. Contractor shall complete and sign an MHE Safety Certification form for each forklift asset listed in Table 1 in accordance with NAVSEA Instruction SW023-AH-WHM-010 [reference (a)]. The crew member performing/assisting in the certification shall also sign each document.
c. Contractor shall complete and sign a Fork Inspection form for each unit listed in Table 1 in accordance with NAVSUP P-538 guidance.
d. Contractor shall complete and sign a Safety Certification form on all Aerial Work Platform (AWP) asset(s), ordnance pallet mules (Trucks/Jacks) and 10K ordnance handling trailers listed in Table 2 in accordance with references (a) and (b), associated equipment technical manuals and attached technical exhibits.
e. Contractor shall out-brief the ship’s Master and Chief Engineer on the completed project prior to departing the vessel. A written report shall accompany this out-brief.
f. Contractor shall provide all reports and signed forms to the ship’s Master and Chief Engineer. A copy shall be provided to the MHE Program Management Office at MSC Norfolk (N44).
9. Qualifications: The selected service provider/contractor must meet the following qualifications.
a. All management and technical personnel performing above listed services must be able to demonstrate, in the form of written documentation, the following experience and skills:
- Three years’ experience performing safety certifications on shipboard-approved electric forklifts, diesel forklifts, and aerial work platforms (AWP) in accordance with the requirements set forth in the NAVSEA SW023-AH-WHM-010, NAVSUP PUB 538, and equipment technical manuals.
- Experience performing independent services to shipboard MHE while the equipment is located onboard a vessel. This shall include the maintenance, repair, and safety certification of all MHE onboard multiple vessels configured with electric forklifts, diesel forklifts, manual pallet jacks, aerial work platforms (AWP) and 36V-72V battery chargers.
- Independent services shall be defined as performing the service without direct supervision from shipboard personnel. Services performed with the "assistance" of junior personnel, or personnel in training, qualify as independent.
- Three years’ experience interpreting, trouble-shooting, and resetting HYSTER MHE on-board dash displays.
b. Contractor shall staff and equip the service with the capability to perform all actions without assistance from ship’s personnel.
c. Contractor must have access to technical manuals and diagnostic manuals applicable to the equipment being serviced.
d. Contractor shall have the ability to change/press tires using the existing onboard tire press.
e. Contractor must be able to perform the required services within the period of performance identified in the Statement of Work. If additional work is identified, and approved by the Government, the Contractor must have the ability to ride the vessel underway to complete the additional service work.
f. Contractor shall complete a security form for each employee that will be servicing MHE onboard government installations and submit the information (including, but not limited to name, social security number, and birth date) to Military Sealift Command (MSC)/MSC Security Officer, as requested. The Government will provide approval for personnel to access installations for a pre-determined timeframe (up to one year), as appropriate.
g. Contractor personnel performing repairs and maintenance onboard MSC vessels or activities shall abide by all DOD, Navy, MSC security requirements and clearance procedures required to access U.S. Government facilities and/or MSC vessels.
h. Proof of U.S. citizenship shall be provided in order to enter a U.S. military facility, if requested by the Government. When accessing a U.S. Government installation, proposed contractor personnel shall be vetted by the Government through the Government's El Paso Intelligence Center (EPIC) before accessing the Government installation.

a. Access paperwork: Will be provided by the regional Ship Support Unit (SSU), the ship’s Principal Port Engineer (PPE) or vessel’s designated representative, as appropriate.
b. Travel expenses: Will be in accordance with Department of Defense JOINT TRAVEL REGULATIONS, (JTR) VOL. 2 Change 561 dated 1 JULY 2012.
13. RFQ Submittal: In response to the above information and statement of work or work item, the contractor is afforded the opportunity to submit a quote (consistent with applicable contract rates) and total pricing to provide the aforementioned service(s) or materials.
a. Contractor shall ensure their designated representative(s) can access the specified Government activity, shipyard, and/or vessel prior to submitting a quotation.
b. Quotes shall be provided on company letterhead and reference the applicable RFQ number (provided above).

6. AERIAL WORK PLATFORMS (AWP)
Complete all safety inspection and testing requirements – including markings - in accordance with NAVSUP Pub 538 and the Manufacture’s Technical Manual.
PERIODIC MAINTENANCE INSPECTION AND SERVICE
AERIAL WORK PLATFORMS (AWP)
TECHNICAL EXHIBIT
1. Structure:
The Contractor shall inspect the platform's structural components for obvious physical damage, such as cracking, bending and deformation of members or welds. The Contractor shall also inspect the paint for cracking or flaking.
2. Hardware:
The Contractor shall inspect the entire platform for missing or loose bolts, nuts, brackets or supports.
3. Tires:
The Contractor shall inspect the condition of the tires. The Contractor shall ensure proper inflation and verify tread is not damaged or cut.
4. Hydraulic System:
The Contractor shall inspect all hydraulic lines for any possible loose connections and/or damaged fittings or hoses.
5. Hydraulic Leaks:
The Contractor shall inspect all hydraulic lines, reservoir, fuel tank, engine and gearbox for leaks.
6. Hydraulic Tank:
The Contractor shall inspect the tank for any possible damage and verify the level of hydraulic fluid. Level should be midway between 3/4 and full with the boom in the fully lowered and retracted position.
7. Gauges & Indicators:
The Contractor shall inspect for broken, missing, inoperable or non-calibrated gauges, indicators and warning lights.
8. Basket:
The Contractor shall rotate basket to the left and to the right, in order to verify the freedom of movement of the Basket.
9. Steering:
The Contractor shall rotate the steering wheel to the left and to the right. Move aerial platform forward and aft a few feet. Ensure movements are smooth and properly operational.
10. Start-up:
The Contractor shall at the ground control console, position selector switch to "Ground". Turn ignition switch and start engine. Test all functions and verify proper operation.
11. Unusual Noises:
The Contractor shall after starting engine, listen and note any unusual noises, check for any fluid leaks, loss of power, or poor response to controls. Verify the proper operation and correct readings of all gauges and indicators. Verify that the battery is charging.
12. Stop Circuit:
The Contractor shall after the initial warm-up (5 minutes), perform engine turn off at the ground control console.
13. Aerial Control:
The Contractor shall Reposition the Aerial/Ground selector switch to "Aerial". Place ignition switches to ON position and start engine from aerial platform. After Starting the engine the Contractor shall verify proper operation of the following controls: Boom Up/Down, Rotation CW/CCW, Platform Rotation, Boom Out/In, Drive FWD/ Reverse, Throttle High/Low and Platform Tilt.
14. Boom Operation:
The Contractor shall raise and lower boom checking for freedom of movement. Extend and retract boom, and check freedom of travel. Swing boom to left and right, and check for freedom of movement.
15. Emergency Stop Switches:
The Contractor shall shutdown the engine using each of the emergency stop switches. Verify entire system shuts down.
16. Emergency System Checkout:
The Contractor shall after shutting engine off using one of the emergency stop switches, start emergency motor driven hydraulic pump and operate at least one function to verify emergency hydraulic pump is working.
17. Off-Level Warning:
The Contractor shall manually operate the off level warning device so the audible alarm can be heard, indicating the level system is functional.
18. Perform Weight Test:
The Contractor shall place the rated load on platform, lift the platform to full extension, hold for two minutes, and verify that no drifting or leaking occurs. Contractor shall then fully extend the boom to each side to test vehicle stability.
19. Maintenance:
The Contractor shall perform period maintenance requirements identified in the applicable manufacturer Technical Manual.
20. Forms:
The Contractor shall fill out and sign the attached Aerial Work Platform (AWP) Safety Inspection and Weight Test Certification. The original signed form shall be provided to the ship's Master or his designated representative. A copy of the signed form shall be provided to the MSC Norfolk MHE Program Management office.
