52.212-1 ADDENDUM TO INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2015)

The provision at 52.212-1, incorporated by reference, applies with the following changes/additions:

(a) North American Industry Classification System (NAICS) code and small business size standard.

The NAICS code and small business size standard for this acquisition appear in Block 10 of the Request for Quotes cover sheet (SF 1449). However, the small business size standard for a concern which submits a quote in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees. Actions indicated as set-aside actions in block 10 are limited to qualifying entities per FAR part 19. Non-U.S. business concerns of any size are not qualifying entities.

(b) SUBMISSION OF QUOTES.

Submit signed and dated quotes to the office specified in this Request for Quotes (RFQ), by email using the specified contact information in section (n) below, at or before the exact time specified in block 8 of this RFQ.

Quotes may be submitted electronically (emailed); however, due to government computer firewall security and file size limitations, it is the Offeror’s responsibility to ensure that the Contracting Office receives all required documents. After submitting quote(s), it is the Offeror’s responsibility to receive acknowledgment of receipt from the Government.

(a) Offerors shall fill in prices for all Contract Line Items. Price Breakdown: Offerors shall submit a detailed pricing break down that details the calculation used for determining proposed pricing. All prices contained in the pricing schedule shall reflect total compensation for the services detailed therein and are fully inclusive of all costs including labor (workers, management, aids, assistants, and operators), overhead, general and administrative (G&A) expenses, and profit.

The Offeror shall submit past performance information (Attachment 1) no more than three summaries of contracts similar in size, scope, and complexity with performance that occurred within 36 months of the submission deadline. Past performance as a prime or as a subcontractor as well as past performance by a proposed subcontractor for this effort is acceptable for consideration.

The Offeror shall also provide each reference the attached past performance questionnaire

(Attachment 2) and ensure it is submitted to the Government no later than the closing date of this solicitation. Should any of the identified past performance efforts be registered in the

Contract Performance Assessment Reporting System (CPARS), the Offeror may notify the

Government to evaluate the CPARS report in lieu of a questionnaire. While the Government may contact past performance reference POCs in order to obtain any necessary clarification on performance feedback information that the Government has received, it will not take additional action to obtain past performance feedback information in cases where the Offeror provides no information. The Offeror maintains the burden of providing thorough and complete past performance information and feedback.

(c) Period for acceptance of quotes.

The Offeror shall agree to hold the prices in its quote firm for 60 calendar days from the date specified for receipt of quotes.

All and any communications/questions pertaining to this solicitation must be submitted via e-mail to the Point of

Addendum to 52.212-2 Evaluation -- Commercial Items (Oct 2014)

(a) This is a commercial item requirement using Simplified Acquisition Procedures under FAR part 12 and 13, respectively. Evaluation will be conducted using simplified procedures in accordance with FAR 13.5.

(b) The Government will award to the responsible offeror whose quote, conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes:

1) Technical Approach

2) Past Performance

3) Price

Technical Approach and Past Performance are more important than Price. Ratings will be determined at the factor level. A quote determined by the contracting officer or Technical Evaluation Board (TEB) to be ‘Unacceptable’ in any area, including unsatisfactory past performance, will not be considered for award.

(c) For purposes of evaluation and determination of responsibility, and in accordance with FAR 13.106-2(b)(3), this assessment will primarily be conducted through Federal Awardee Performance and Integrity Information System

(FAPIIS) / Past Performance Information Retrieval System (PPIRS) and offeror reference questionnaires received.

Past performance, for purposes of this evaluation, will be assessed as ‘satisfactory’ or ‘unsatisfactory’ at the discretion of the contracting officer based on the totality of information available. A past performance determination of ‘unsatisfactory’ will exclude the offer from consideration for award.

(d) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the

Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

PERFORMANCE WORK STATEMENT

1. GENERAL DESCRIPTION

1.0. OBJECTIVE: The key objective of this contract defines Special Missions Branch (SMB) Training Support

Services that shall result in:

a) Effectively and properly conduct classroom and practical application instruction for Close Quarters Tactics

(CQT), Urban Reconnaissance and Surveillance (R&S), Visit, Board, Search and Seizure (VBSS), and Urban Sniper

Courses.

b) Act as Range Safety Officers and/or Site Controllers when required in support of courses and training exercises.

c) Attend conferences and training exercises as required.

1.1. SCOPE OF WORK

The Contractor shall provide fully qualified personnel, administrative support, and training necessary to perform quality instruction and training support services to the Maritime Raid Force (MRF) supporting the 31st Marine

Expeditionary Unit (MEU) and additional units as directed by III Marine Expeditionary Forces (MEF). The

Contractor shall utilize their technical expertise to provide SMB exercise support for training courses and exercises.

The Contractor will be responsible for the planning, coordination, instruction and execution of the following courses: III Expeditionary Operational Training Group (EOTG) Urban Sniper, III EOTG Urban R&S, III EOTG

CQT and III EOTG VBSS in addition to any formal and informal instruction given to other units at the direction of

III MEF. This is a non-personal services contract.

The Contractor shall assist in planning, coordination, and evaluation of the following MEU Exercises: Realistic

Urban Training Exercise (RUTEX), MEU Certification Exercise (CERTEX), and MEU InterOp Exercise

(INTEROP). All training and evaluation will be in accordance with the Marine Corps Program of Instruction (POI) and Training and Requirements (T&R) training standards established in the publications and references which are listed in paragraph 4.6. The references will be provided to the Contractor upon request.

The Contractor is responsible for providing the instruction of the 31st MEU’s MRF and any additional units at the discretion of III MEF in accordance with the T&R standards in order to prepare designated Marines for deployment.

The Contractor shall provide technical expertise to assist the EOTG with the training development continuum with emphasis on continuous improvement to increase realism and to keep the training relevant, dynamic and challenging.

1.1.1. Number of Required Instructors and Performance: The Contractor shall provide instructors with the training and experience outlined in paragraph 1.5. The Contractor shall have all required personnel physically present at the work location. The contractor is responsible for all costs of performance not identified as Government provided equipment or materials.

1.1.2. Replacement of Personnel (Instructors): Contractor Instructor performance will be measured in accordance with the Performance Requirement Summary (PRS) in Section II. Failure of an instructor to meet acceptable standards after remediation will result in Contracting Officer request for a replacement instructor. Any Contractor instructor involved in a safety violation that puts a student or fellow instructor’s life in danger is also grounds for removal, and the requirement of a replacement.

1.2 PRE-REQUISITE FOR DEPLOYMENT AND/OR REPLACEMENT OF PERSONNEL:

1.2.1. Pre-Deployment: The contractor shall provide, and the government Contracting Officer or COR shall approve a statement of qualifications and experience for any personnel anticipated to work for the Contractor in support of this contract in order to verify the standards of paragraph 1.4. are met prior to deployment. No contractor personnel will be approved unless they fully meet the required qualifications as described herein.

1.2.2. Replacement Personnel: The Contractor shall provide the Government at least two weeks (or 24 hours on emergency terminations) written notice of its intent to replace any personnel; notification shall include the requirements and conditions of paragraph 1.4. The Government will have five (5) business days to review/approve the information and if approved, the approved individual(s) shall be on site and continue contract performance within fourteen (14) calendar days. Alternatively, if the Government determines proposed individuals do not meet the requirements stipulated herein, the above process shall be repeated until a suitable candidate is approved.

1.3. REQUIRED COURSES

1.3.1. III MEF EOTG Close Quarters Training Course (CQT): The Contractor shall provide Special Missions

Branch (SMB) instructors focusing on close quarters tactics and assisting in reconnaissance and surveillance and sniper periods of instruction. The course includes advanced marksmanship training with the M-4 carbine and the

Close Quarters Battle Pistol (CQBP). Employment of dynamic entry techniques, securing various objectives, detailed planning and execution of movement to and from objectives, and discriminative target engagements are also learned during this course. Field firing exercises and tactical decision-making exercises with live ammunition and

Special Effects Small Arms Marking System (SESAMS) are used extensively to reinforce classroom instruction.

The instructor(s) shall provide students with detailed instructions and closely evaluate the students on an individual basis. The Contractor shall be responsible for assisting the CQT lead instructor with the planning, coordination, instruction and execution of the CQT Course. Contractors will also be responsible for providing support to the

Visit, Board, Search, and Seizure Course which is part of the CQT training continuum for the Maritime Raid Force.

1.3.2 III EOTG Visit, Board, Search, and Seizure Course. The Contractor shall provide Special Missions Branch

(SMB) instructors focusing on close quarter’s battle tactics as they apply to maritime interdiction operations (MIO).

The VBSS course consists of MIO familiarization and performance-orientated instruction that relate precisely to

VBSS operations during daylight and reduced visibility. The course builds upon the proficiency and qualifications obtained during the Close Quarters Tactics (CQT) and Security Element course, and applies the task proficiency to a confined, maritime environment. The focus of instruction and practical application includes multiple insertion methods (Helicopter Assault Force (HAF), Boat Assault Force (BAF), and Combined HAF/BAF), external entry, personnel processing, and tactical withdrawal. Realistic tactical scenarios are employed throughout the practical applications and performance evaluations to simulate situations the MRF may encounter if tasked with a VBSS operation.

1.3.3. III EOTG Urban Sniper Course. The Contractor shall provide an instructor for EOTG Urban Sniper

Courses and conduct instruction in advanced combat marksmanship at known and unknown distances during daylight, low light, and darkness with the M40 A3/A5, MK 13, M110 SASS sniper rifle, the M107 Special

Application Scoped Rifle (SASR) and the M4 and M9. Instruction in precision shooting includes basic field firing techniques, target engagement in a built up area, night fire with the aid of night optics, position shooting, ballistic classes, immediate action drills, and aerial shooting from a helicopter platform. Students receive instruction on sniping during Situational Training Exercises (STX) in support of Battalion Landing Team (BLT) and MEU training exercises. The Contractor shall be responsible for the planning, coordination, instruction and execution of the Urban

Sniper Course.

1.3.4. III EOTG Urban Reconnaissance and Surveillance (R&S) Course. The Contractor shall provide instructors to support the Reconnaissance and Surveillance (R&S) Courses with instruction in urban reconnaissance and surveillance, surveillance detection routes, fixed and mobile surveillance/counter surveillance, force protection and threat vulnerability assessments, and other phase zero joint force operations. Instructor will also be responsible to teach observation techniques, patrolling, hide site construction, field skills, mission planning, communications and Surveillance and Reconnaissance Center (SARC) operations. Specific classes include concealed carry, joint force phase zero employment, embassy orientation, surveillance detection routes, fixed and mobile surveillance/counter surveillance, force protection and threat vulnerability assessments, non-standard communications procedures, operating from a hide, stalking, preparing a field sketch, reporting information and performing reconnaissance on an objective. Marines employ field expedient antennas and operate HF, UHF, VHF and SATCOM radios as well as the Marine Air, Ground Task Force (MAGTF) Secondary Image Dissemination

System (MSIDS). The Contractor shall be responsible for assisting the R&S lead instructor with the planning, coordination, instruction and execution of the Urban R&S Course.

1.4. CONTRACTOR EXERCISE PLANNING, COORDINATION, AND EVALUATION SUPPORT. The contractor shall provide planning, coordination and evaluation support for the following MEU Exercises: Realistic

Urban Training Exercise (RUTEX), MEU Certification Exercise (CERTEX), and MEU InterOp Exercise

(INTEROP) on an as required basis. The following deliverables will be submitted in support of the Government exercise lead project officer as required:

1.4.1. Pre-exercise draft of letters of instruction, powerpoint presentations for meetings and e-mail and telephone correspondences to coordinate planning conferences and exercise conduct as necessary. Contractor will not participate in planning conferences.

1.4.2. Observation of exercise performance with evaluation reports of exercise observations as required.

1.5. REPORTING AND DOCUMENTATION REQUIREMENTS FOR COURSE INSTRUCTION. The

Contractor shall prepare and submit the required reports with the instructions and schedules specified below:

a) Edit, proofread and prepare correspondence and letters of instruction in accordance with the Navy

Correspondence Manual (SECNAV M-5216.5).

b) Assemble reports required for briefings and presentations related to instructing the courses.

c) Prepare weekly training highlight schedule which contains such information as graduations, field training, and live fire evolutions no later than 1500 Friday, local time, the week prior to the event.

d) Receive, acknowledge and transmit electronic mail and messages.

e) Maintain organized electronic files for later retrieval as appropriate.

1.6. QUALIFICATIONS/EXPERIENCE. General qualifications for instructors are delineated below.

1.6.1. Operations and Technical Qualifications of Instructors.

a) Former Marine Force Reconnaissance /Special Forces Advanced Reconnaissance Training Course (SFARTC) trained Army Special Forces Staff Noncommissioned Officer (SNCO)/US Navy Sea, Air and Land (SEAL) Chief, with experience as a tactical operator.

b) Graduate of Marine Corps Advanced Technical Surveillance Course, Physical Surveillance Course, Surveillance

Operators Course, or service equivalent and have experience conducting operational preparation of the environment.

c) Graduate from Medical Education and Training Campus at the DOD Healthcare Education Facility, Fort Sam

Houston, Texas, or a civilian possessing a current Emergency Medical Education Standards Curricula, which is current and recognized in the state of residence. Previously qualified with the military occupational specialty of

USMC Special Amphibious Reconnaissance Corpsman or Special Forces Medic (18D) or other military service equivalent.

d) Served in a communications team member billet and/or served within the joint communications unit.

e) Former participant in EOTG Urban R&S, EOTG Sniper or CQT courses or branch equivalent (SFARTEC).

f) Graduate of Special Operations Combative Program (SOCP)

g) Combat Veteran awarded at least one combat action ribbon with U.S. Amred Forces.

h) Extensive personal and instructional experience in helicopter rope suspension techniques (HRST) and able to recertify upon arrival.

i) Familiar with Marine Corps infantry tactics, the organization of Infantry Battalions and Rifle Companies, the characteristics and employment of Marine Corps aviation assets, Amphibious Assault Vehicles (AAVs), Light

Armored Vehicles (LAVs), High Mobility Multi-Wheeled Vehicles (HMMWVs), and weapons and fire support assets to include standard issue small arms, machine guns, rockets and mortars.

j) Experience in conducting personal security detachment operations.

1.6.2. Instructor Qualifications. The Contractor shall provide fully qualified instructors with the skills listed below

(with the exception of the MCB Camp Butler RSO course, which instructors must successfully complete prior to instructing a course):

a) Graduate of USMC Basic Instructor Course (BIC) or equivalent.

b) Graduate of Marine Corps Formal Schools Instructors Course or equivalent.

c) Ability to communicate effectively orally and in writing.

d) High level familiarity with task information in the respective T&R manuals.

e) Knowledgeable and proficient in the preparation of Naval Correspondence IAW the Naval Correspondence

Manual.

f) Recent experience in coordinating, scheduling, using any military base or facility (RSO Qualifications).

g) Review of all course POIs and lesson materials. (Due to the sensitive nature of the course material, it will not be disseminated until the contract has been awarded.)

h) Lead instructor curriculum review and teach backs. Instructors will be certified for each individual period of instruction by the lead instructor. (upon arrival)

i) MCB Butler RSO Course. (upon arrival)

j) All instructors must be eligible for (and submit upon arrival) an arms, ammunition, and explosive handling form.

1.6.3. CQT Specific Qualifications. In addition to the skills listed in Paragraph 1.5.1,and 1.5.2 CQT instructors shall have the following qualifications:

a) Breacher training through MOE Quantico, MOE III EOTG, or Special Forces Advanced Reconnaissance and

Target Exploitation Course (SFARTEC), or service level equivalent.

1.6.4. Sniper Specific Qualifications. In addition to the skills listed in Paragraph 1.5.1,and 1.5.2 Sniper instructors shall have the following qualifications:

a) Formerly qualified in Marine Corps Occupational Specialty 0317, 8541, or other branch equivalent (Level 1

SOTIC Sniper).

b) Three years of deployed experience as a armed forces sniper conducting combat operations or military exercises.

c) Prior Basic Sniper Course instructor with primary billet of marksmanship instructor.

d) Background knowledge in ATRAG Ballistic Computers or commercial equivalent.

e) Experience with aerial shooting

1.6.5 Urban Reconnaissance and Surveillance. In addition to the skills listed in Paragraph 1.5.1,and 1.5.2 Urban

Reconnaissance and Surveillance instructors shall have the following qualifications:

a) Graduate of Basic Reconnaissance Course or other military service branch equivalent course.

b) Graduate of Technical/Physical Surveillance Operators Course or other military service branch equivalent course.

c)Have experience conducting steady state, phase zero operations in urban, littoral, and jungle environments.

d) Experience coordinating with US Embassy in support of mission requirements.

1.6.6. Arms Proficiency. The Contractor –provided personnel (or just assigned instructor personnel?) will be required to qualify with weapon systems taught IAW prescribed POI standards. All assigned weapons based instructors will re-qualify / conduct proficiency training on the courses of fire and with the type of weapons they instruct. Assigned instructor personnel will be provided the opportunity to sustain weapons qualification at the same time as military and Government civilian instructors. For these weapon qualifications, the Government will provide weapons, ammunition, and range facilities.Documentation of this qualification shall be included in the contractor’s

Instructor Training Jackets (ITJs).

1.6.7. General Qualifications.

a) Security Clearance: The Contractor on-site personnel directly supporting this contract are required to have and maintain a Secret Clearance. In addition, all Contractor personnel working on this contract shall be United States citizens or shall carry a valid U.S. Immigration Alien/Registration card. All Contractor employees shall be able to communicate in English, both orally and in writing. The Contractor shall be responsible for obtaining any certifications, licensing (unless otherwise noted in paragraphs 1.5.1 through 1.5.5) , and proof of citizenship/alien status that may be required by Contractor personnel to support tasks performed under this contract.

b) The Contractor on-site personnel directly supporting this contract shall provide the required services in a professional and unbiased manner, and shall maintain high personal standards of technical knowledge and demonstrate professional competence in delivering training material. In addition to this technical background and skills, the Contract Instructor personnel shall be able to effectively perform in a military environment, and possess a familiarity with the organization and hierarchy of the U. S. Marine Corps, military rank/grade structure, knowledge of Marine Corps and military terminology, and shipboard/ installation environments and configurations.

c) The Contractor on-site personnel directly supporting this contract shall have knowledge of commercially procured computer applications (e.g., Outlook, Microsoft Word, Power Point, Excel, etc) and peripheral equipment to produce a variety of documents, charts, graphs, and briefing materials. The Contractor shall create, review and edit class instruction materials and perform other functions with their computer as required in their normal work day.

1.7. TRAVEL. The Contractor on-site personnel directly supporting this contract may be required to travel three times a year to the locations specified below. Travel will consist of exercise support for RUTEX and the III EOTG

Urban R&S Course.

Travel Locations: Guam, Palau, Hawaii, Thailand, or the Philippines (any other location within PACOM AO as directed by the III MEF)

Duration of Travel: Up to approximately 30 days, or as required to support training and exercises (each travel period)

Frequency of Travel: Three times a year

Temporary Additional Duty costs are in accordance with the Federal Travel Regulations (FTR) for the location in which the work is performed, at cost and all expense receipts will be required for re-imbursement. Travel, Lodging

SECTION III

GOVERNMENT FURNISHED PROPERTY AND SERVICES

3.1. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government will provide a furnished workspace, furnishings, office machines and supplies, and reference materials for onsite work requirements.

3.1.1. Government Furnished Equipment (GFE).

3.1.1.1. The Contractor will be authorized to use all administrative support available. This will include, but is not limited to, copy machines, fax machines, scanners, installation distribution channels, scanner, fax and telephone services, computer, printer, software, and network resources, access and use of Class “A” telephone lines and

Defense Switching Network (DSN) lines. (A computer will be assigned to the Contractor.) All GFE shall be used solely for official purposes related to the performance of this contract. The same restrictions that limit use of these items as official Government business shall apply.

3.1.1.2. The contractor will be prepared to utilize and maintain proficiency on the following military gear: M4 carbine service weapon, M45 MEU(SOC) service pistol to be maintained in the EOTG armory and issued on as needed basis for training.

3.1.2. Common Access Card (CAC). The Contractor may apply for a Common Access Card (CAC), United

States Department of Defense (DoD) integrated circuit card issued as standard identification for access to the MCB

Butler LAN providing Internet connection and an e-mail account (s) for business purposes as an unclassified user.

The contractor personnel shall be able to pass security requirements in order to gain access to Government Intranet.

3.1.2.1. Requirements for Issuance of a CAC The following conditions must be met prior to issuance of a CAC to any contractor employees for limited privilege/non privileged access:

The SOFA status of the employee shall be verified, and approved by the office of Staff Judge Advocate

(SJA).

The Contractor shall be registered in the Contractor Verification System (CVS) with a valid record of security clearance (background check) on Joint Personnel Adjudication System (JPAS). The CVS is conducted at MCB

Installation Personnel Administration Center (IPAC) USMC S.D. Butler.

The Contractor shall obtain proof of security clearance (background check) on JPAS at Security

Management Office, USMC S.D. Butler.

When applying for a CAC at MCB IPAC, the Contractor shall show two original forms of Identification and submit 1) contract document, 2) passport [reviewed by SJA office and SOFA stamp required], 3) letter of employment, 4) letter of authorization from the Contracting Officer [as applicable], and 5) a valid record of JPAS

(proof of background check).

All Contractor employees who will be assigned to work in Okinawa must have an active clearance/background check or must have initiated a background check before coming to Okinawa.

NOTE: Prior to applying for CVS, the Contractor’s personnel must initiate a background check on JPAS at the

Security Management Office and a record of JPAS enrollment verification (e-mail) must be submitted to a Trusted

Agent (TA) of IPAC.

3.1.2.2. FEDERAL REGULATIONS/DOD INFORMATION ASSURANCE (IA) MANDATES. The

Contractor shall adhere to and have knowledge of all applicable documents/regulations pertaining all DoD IA mandates, directives, and policies including and not limited to, the followings:

Commercial off-the-shelf (COTS);

Government off-the-shelf (GOTS) applications;

Physical and wireless infrastructure;

Non-Classified Internet Protocol Router Network (NIPRNet)

Storage Area Networks (SAN).

3.1.3. Utilities. The Government will provide utility services while at work which include water, sewage service, electricity, heating and air conditioning as needed in performance of this contract. This does not cover the contracted instructor’s cost incurred at their personal residence.

3.1.4. Exercise Related Life Support. Contractor personnel will be required to participate in field training and live with the Marines in a field environment. The contractor will be responsible for providing their own sleeping system and other appropriate gear for field conditions up to 30 days. Contractor personnel will subsist in the same manner as EOTG Marines when on field duty (i.e. if EOTG Marines are on per diem, contractors will be on per diem, If

EOTG Marines are on field rations, contractor personnel will be on field rations.) Contractors may be required to travel via military transportation including ground, air, and amphibious vehicles during or in support of exercises and training.

3.2. CONTRACTOR EQUIPMENT. It is the responsibility of the Contractor to ensure it has the necessary equipment needed for the successful completion of all work performed under this contract. The Contractor is responsible for its own transportation/vehicle for camp to camp travel in the performance of services under this contract. There is no reimbursement for local training on this contract.

3.2.1 Contractor Personal Protective Equipment. The Contractor shall provide their contracted instructors personal protective equipment to include ballistic eye protection, hearing protection, ballistic body armor, ballistic helmet, and flame retardant clothing for use in a field environment when working with explosives.

3.2.2. Contractor Furnished Personal Property. The Contractor shall ensure that personal property is clearly marked and/or tagged to preclude misidentification as Government property. The Government will not be liable for loss, damage, theft, or maintenance of personal property.

3.2.3. Rental Equipment. Any rental equipment required to perform services shall be the responsibility of the

Contractor. Operation of the equipment shall be in accordance with Marine Corps Base Safety Regulations.

3.2.4. Supplies. Supplies required by the Contractor for administration and/or day-to-day operations shall be provided by the Government (i.e. computers, printers, fax machines, and office supplies such as paper, pens, pencils, staplers, etc.) in order to support work described in this PWS. The Government will provide the necessary property, equipment, supplies and facility support services (i.e. LAN infrastructure, copiers, office spaces, chairs, book cases, file cabinets, cleaning supplies, phones, shredders, and toilet paper.) The Government will also provide:

a) When required, access to classified IT equipment (computers, shredders, copiers, phones.)

b) Work spaces equivalent to spaces occupied by Government personnel.

c) Government forms, publications, documents, except for those offered for sale to the public. Items for sale to the public shall be purchased by the contractor.

SECTION IV

CONTRACT ADMINISTRATION INFORMATION

4.1. DRESS CODE AND GROOMING STANDARDS. Marine Corps EOTG training attracts a significant amount of public attention, and frequently, these training sites are visited by VIPs and foreign dignitaries.

Additionally, the EOTG instructors provide technical skill role models to the young military men and women who attend training. Therefore, it is essential that all instructors present themselves in a highly professional manner in appearance and dress (commensurate with the type of training).

4.1.1. Dress Code.

a) The Contractor shall establish and maintain specific guidelines regarding the dress code for their employees. At a minimum, these guidelines shall include prohibitions of eccentricities or extremes in dress and hairstyles, apparel in a ragged and frayed condition; tank tops; halter tops; crop tops; sleeveless shirts; mesh and see-thru garments;

exposed clothing with obscene or advertising logos or undergarment type shirts worn as exterior clothing.

Additionally, these guidelines shall provide for standardization in appearance by their Contractor personnel.

b) During normal day to day operations (all seasons) the Contractor shall be dressed uniformly in accordance with the Government’s requirement. Contractors will not wear Marine Corps or other military service uniforms. Uniform shall include standardized appearance boots, khaki trousers/pants, belt, polo style unit logo shirt, jacket/coat/sweater, and cold/inclement weather equipment.

c) Field duty will require the contracted instructors to wear a flame retardant long sleeve shirt and pants. All uniforms shall be standardized in appearance, but differentiated from USMC camouflage patterns. Contractor shall institute a uniform replenishment plan ensuring a neat professional appearance is maintained at all times.

4.1.2. Grooming Standards. The Contractor shall establish and maintain specific guidelines regarding grooming standards for their Instructors. These standards should be similar to those contained in the Marine Corps Uniform

Regulations, with the exception of the prohibition of facial hair. Beards and mustaches are acceptable, provided they are neatly trimmed.

4.2. LOCATION AND HOURS OF WORK. The Contractor will be required to work at Camp Hansen, Okinawa, Japan. Additional training sites may be designated by the Government and, include all major Marine

Corps installations and other DoD/military training facilities/areas. Normal hours of operation are based on the training cycles. Contractor will plan days off in sequence with training phases and events. There will be occasions when contractor personnel shall be required to exceed a normal 40 hour work week. In order to support a high operation tempo, the Government reserves the right to schedule training and contractor support on weekends, federal holidays, and at night.

4.3. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP): The Government will monitor the

Contractor's performance in accordance with the Quality Assurance Surveillance Plan (QASP), Appendix B. The

QASP defines the methodologies used to monitor and evaluate the Contractor’s performance, describes quality assurance documentation requirements, and describes the analysis of quality assurance monitoring results. The

QASP also includes “Instructor Evaluation Checklist,” “Lesson Observation Checklist” and “Lesson Evaluation

Checklist” which will be used to evaluate whether the Contractor personnel is meeting the performance standards/quality levels identified in the PWS. The Government may increase or decrease the number of quality assurance inspections as performance dictates. The QASP defines how the Government will inspect the Contractor's performance to ensure services are received and in compliance with the terms and conditions of the contract.

4.4. QUALITY CONTROL. The Contractor shall develop, and maintain a Quality Control Plan (QCP) to ensure required services are performed in accordance with commonly accepted commercial practices. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The

Contractor shall provide a copy of the QCP to the Contracting Officer no later than the pre-performance conference, and updated copies as changes occur. Records of all inspections conducted by the Contractor and corrective action taken shall be made available to the Government upon request.

4.5 GENERAL DEFINITIONS

4.5.1. Contracting Officer (CO). US Government official authorized to enter into, make changes to, or terminate contracts on behalf of the US Government. Contracting officers are responsible for ensuring performance of all necessary actions for effective contracting, ensuring compliance with the terms of the contract, and safeguarding the interests of the United States in its contractual relationships.

4.5.2. Contracting Officer Representative (COR). Government liaison appointed in writing by the Contracting

Officer and designated in the contract, who functions as the requirements representative monitoring cost/price, delivery and performance of the Contractor. The COR is not authorized to enter into any agreements, obligate the

Government or direct any changes to the contract.

4.5.3 Defective Service. A service output that does not meet the standard of performance specified in the contract for that particular service.

4.5.4 Government Furnished Equipment (GFE). Equipment in the possession of, or directly acquired by, the

Government and subsequently made available to the Contractor.

4.5.5 Quality Assurance (QA). Various functions, including inspection, performed by the Government to determine whether a Contractor has fulfilled the contract obligations pertaining to quality and quantity.

4.5.6. Quality Assurance Representative (QAR). Government liaison appointed in writing by the Contracting

Officer and designated in the contract, who functions as the representative monitoring cost/price, delivery and performance of the Contractor. The QAR is not authorized to enter into any agreements, obligate the Government or direct any changes to the contract.

4.5.7. Quality Control (QC). A complete Quality Control outline that meets the requirements of the contract shall be provided by the Contractor to illustrate how the company will ensure they conform to the contract requirements.

4.5.8. Performance Requirement Summary (PRS): A listing of the service outputs under the contract that are to be evaluated by the COR on a regular basis, the surveillance methods to be used for these outputs, and the standards of the listed outputs.

4.5.9. Standard. The point that divides acceptable and unacceptable performance of a task according to the Service

Delivery Summary. It is the number of defectives or maximum percent defective in the lot that is deemed acceptable.

4.6. APPLICABLE DIRECTIVES/PUBLICATION. The Contractor shall comply with all documents listed below as mandatory and referenced under paragraphs 2 and 3, General and Specific Requirements.
