STATEMENT OF WORK

LIGHT PRODUCTION MACHINE:

1. REQUIREMENT: Marine Corps Engineer School, TCom, TECOM has a requirement to lease two(2) high-speed high-volume production level, black and white (B&W) copier/printer/scanners that operate from a standalone computer.

2. BACKGROUND/PURPOSE: The purpose of these machines is to print student outlines and course curriculum for MOS specific training courses at Marine Corps Engineer School.

SPECS:

-Printing:- Black and White -Print Speeds-minimum 105/ (8.5 inch x 11 inch/ A4) -Automatic duplex feeder -Simplex and duplex printing:

1200 x 1200 dpi RIP resolution and up to 2400 x 2400 dpi print resolution with halinch one screen up to 150 lpi -Scanning – save documents to a standalone computer.

-Dual Head Scanner rated at 100 images per minute for single and double sided original -Resolution: optical 600 x 600 dpi 8-bit Gray (256 shades) -Scans in multipage PDF, JPEG, TIFF -Scan to network file server -Throughput sizes: 5.5 inch x 8.5 inch to 11 inch x 17 inch (AS to A3) -Throughput weights: Simplex or Duplex 16 lb. to 110 lb. bond -Paper Handling:

3. GENERAL REQUIREMENTS: The Contractor shall provide professional, administrative, certified technical services to support maintenance requirements - preventative and on-call maintenance services on both government leased machines for a one year base period, with two (2) option years. The work effort is to provide support required to achieve the desired degree of responsiveness essential to accommodate workload demands.

4. SPECIFIC TASK DESCRIPTION:

a. Contractor shall provide all toner, parts, travel, labor, soinch ware, network connectivity and preventative maintenance. Paper is not included.

b. Machines leased by the Government shall be installed by the Contractor as part of basic contract. Contractor shall provide experienced personnel/technicians to train government personnel on the proper operation and full capabilities of the machines as part of the basic contract and once a year for each option period exercised.

c. Contractor shall respond to repair calls within 2 business hours and repair non-functioning equipment within 8 business hours.

d. Preventative maintenance shall be performed once every three months, for the base year and any Option Period if exercised.

5. PERIOD OF PERFORMANCE:

The period of performance is a one year base period, with two (2) one year option periods to be exercised at the discretion of the Government.

a. Base Year, 1 April 2016 – 31 March 2017

b. First Option, 1 April 2017 – 31 March 2018

c. Second Option, 1 April 2018 – 31 March 2019

6. GOVERNEMNT POINT OF CONTACT

The point of contact is Mary Simpson, 910-440-7440, BB49 (Supply).

7. CONTRACTOR RESPONSIBILITIES:

Upon receipt of required repair calls from the Government POC, the Contractor shall accomplish all repairs within 8 business hours ainch er receipt. If repairs cannot be accomplished within 8 business hours, the Contractor shall furnish the Government POC written documentation within 8 working hours showing the cause for delay of repairs. If machines are down for more than 24 hours, the Contractor shall credit the government lease for each 24 hours period the equipment is non-operational.

8. ON-CALL MAINTENANCE SERVICE

The Contractor shall provide on-call maintenance service and repairs on all items listed in this contract. The Contractor shall furnish all repair parts, labor, and transportation required to accomplish item repairs. The Contractor shall maintain all equipment in this contract at levels necessary to provide the specified functions to meet the manufacturerinch s current equipment specifications.

9. PREVENTIVE MAINTENANCE SERVICE

The Contractor shall provide preventative maintenance service on equipment listed in this contract. The Contractor shall furnish all repair parts, labor, and transportation required to accomplish inspection, cleaning, lubrication, adjusting, calibrating, vacuuming, (if required), and repairing of items. Contractor shall maintain all equipment in this contract at levels necessary to provide the specified functions to meet the manufacturerinch s current equipment specifications and applicable Government regulations and directives.

All services defined in this contract shall be accomplished during regular work hours of 8:00 a.m. – 4:00 p.m., Monday- Friday, except for federal holidays.

All services defined in this contract shall be performed at, Marine Corps Engineer School, Marine Corps Base Camp Lejeune (Courthouse Bay), NC. Machine 1: Bldg BB14, Academics Office (2nd Floor). Machine 2: Bldg BB13, Academics Office (2nd Floor).

10. WORK TICKET

A work ticket for each repair call shall be submitted to the Government POC upon completion of the repair call. The work ticket shall have clearly marked spaces for, and include:

a. Contract/Order Number

b. Date/Time Notified

c. Manufacturer/Model/Serial Number(s) of Equipment

d. Unit/Section/Equipment Operator/Bldg. No. /Phone No.

e. Description of Problem (Unit)

f. Parts Used (Description and Quantity)

11. ANNUAL TRAINING

The Contractor shall perform on-site training for the provided machines once per year. For the base year, training shall occur within a week of delivery of the machines. For the option years, training shall occur in the month of August during a work day at a time mutually agreeable to both parties. Training shall be for up to 10 personnel for up to 2 hours. Subjects covered shall be a functional overview of the equipment and basic functions, customized defaults, settings, supplies, and how to set up for tasks specific to Marine Corps Engineer School.

12. CONTRACTOR UNAUTHORIZED WORK PERFORMANCE

The Contractor shall not perform work that deviates from contract requirements and specifications. If the Contractor deviates from contract requirements and specifications, such deviation shall be at the risk of the Contractor and any cost related thereto shall be borne by the Contractor.

BASE ACCESS

1. Marine Corps Installations East - Marine Corps Base Camp Lejeune and Marine Corps Air Station New River have implemented the new RAPIDGate program intended to enhance installation access control. However, RAPIDGate contains significant changes that may affect contractor’s and their employee’s access to these installations. In order to gain access to Marine Corps Base (MCB), Camp Lejeune and/or Marine Corps Air Station (MCAS), New River, non-Federal Government and non-DoD issued ID cardholders require identity proofing and vetting to determine fitness and eligibility for access. Specifically, a check of records through the National Crime Information Center (NCIC) Interstate Identification Index (III) is the installation's minimum background check for access to MCB, Camp Lejeune and/or MCAS, New River for non-Common Access Card holders, to include entrance of visitors.

2. MCB, Camp Lejeune and MCAS, New River applies the following non-Federal Government cardholder criteria to determine the authorized type of access control credentials:

a. Contractors: RAPIDGate or 30-Day Pass followed by 4-DayPass*

b. Sub-Contractors: RAPIDGate or 30-Day Pass followed by 4-Day Pass*

c. Vendors: RAPIDGate or 30-Day Pass followed by 4-Day Pass*

d. Suppliers: RAPIDGate or 30-Day Pass followed by 4-Day Pass*

e. Service Providers: RAPIDGate or 30-Day Pass followed by 4-Day Pass*

f. Delivery Personnel: RAPIDGate or 30-Day Pass followed by 4-Day Pass*

g. Public-Private Venture (PPV) Housing and Installation Volunteers: Local Population Credential (The following are examples of Installation Volunteers: (Red Cross, Community College Instructors and Students, Commissary Baggers, Naval Hospital Volunteers, Family Readiness Volunteers, Wounded Warrior Volunteers, and any other organization approved by the Commanding General, Marine Corps Installation East-Marine Corps Base Camp Lejeune and Commanding Officer, MCAS, New River)

3. Contractor Common Access Card (CAC) Eligibility: Contractor personnel authorized CAC’s are exempt from RAPIDGate enrollment.

4. Contractor personnel who are ineligible for a CAC will be required to either obtain a 30-Day pass followed by a 4-Day pass* or enroll in the RAPIDGate Program. RAPIDGate is a voluntary program due to associated costs. For further information concerning contractor access, please contact the Contractor Vetting Office at 910-450-5978 or visit Marine Corps Base Camp Lejeune (http://www.lejeune.marines.mil/) or MCAS New River (http://www.newriver.marines.mil/) websites. For information concerning enrolling in RAPIDGate please visit their website at www.rapidgate.com.

The Government will not be responsible for any access delays. Delays that may result from inadequate planning are contractor responsibility. Any and all costs associated with RAPIDGate are the sole responsibility of the contractor.

* Contractors will be issued a one-time pass for 30 days. Upon expiration of the 30 day pass, they will only be issued a 4 day pass every 4 days thereafter, for the duration of the contract.

CLAUSES INCORPORATED BY FULL TEXT

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014) (TAILORED)

(a) The Government will award a contract resulting from this solicitation to the responsible quoter whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Lowest Price Technically Acceptable (LPTA)

This is a request for OPEN MARKET PRICED quotes for a lease. The manufacturer name and part number are provided only to assist potential quoters in understanding the government's requirement. The Government is not soliciting or suggesting a particular name brand, and will consider quotes for any brand that fulfills the government's need. Technical Acceptability - The manufacturer’s name and manufacturer’s complete part number of each item shall be submitted in the quote. A mere statement that the quoter will meet the Government’s requirement is NOT sufficient. Quotes shall clearly state all product specifications for quoted items.

The associated North American Industrial Classification System (NAICS) code for this requirement is 333913.

BASIS OF AWARD The Government will solicit and award this contract using simplified acquisition procedures, in accordance with FAR Subpart 13.0. It is the Government's intention to issue a single firm-fixed price purchase order on an "all or none" basis for all items. The Government intends to award to the quoter who represents the Lowest Price Technically Acceptable (LPTA) quote. Quotes will be evaluated and ranked according to price, from lowest to highest. The Government reserves the right to evaluate only the technical quote of the lowest price quoter. Other technical quotes may not be evaluated unless the Government determines the lowest price quoter to be unacceptable or non-responsive. In the event the Government determines the lowest price quoter to be unacceptable, the technical quote of the next lowest price quoter will be evaluated. This process will continue until a technical proposal is determined to be acceptable.

EVALUATION. Vendors responding to this announcement shall submit their quote in accordance with FAR 52.212-1. Submission of quote shall include the following: (1) Technical acceptability: (See Line Item for details); (2) Price: Award will be made on the basis of the lowest price technically acceptable (LPTA) of quotes meeting or exceeding the acceptability standards for non-cost factors; (3) Delivery: 1 April 2016, period of performance beginning on date of delivery. The Government intends to evaluate quotes and award a contract without discussions with vendors. Vendors should not expect requests for clarification or additional information from the Government. Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions with vendors if later determined by the Contracting Officer to be necessary and in the best interest to the Government. The Government may reject any or all quotes if such action is in the public interest; and may waive informalities and minor irregularities in quotes received.

SUBMISSION REQUIREMENTS. Quoters shall include:

1) Completed FAR provision 52.204-17 - Check appropriate box. No signature required but please put company name on document.

2) Completed FAR provision 52.212-3 - Check appropriate boxes at 52.212-3(b)(2) and (3) and put company name on document. If no changes are necessary, clearly state so in submission package.

3) Completed DFARS provision 252.209-7991 - Check appropriate boxes. No signature required but please put company name on document.

4) Completed DFARS provision 252.239-7009 - Check appropriate boxes. No signature required but please put company name on document.

5) Clearly state ability to meet specified delivery time after receipt of contract.

6) Clearly identify manufacturer and part number for each item quoted. Descriptive literature outlining the technical specifications of the items quoted to include drawings, photographs, and sepecifications shall be included.

7) Confirm that your price is “Open Market” and not GSA pricing.

8) Provide the following information: CAGE Code and DUNS Number.

SUBMISSION OF QUOTES. Quotes related to this RFQ shall be received by the Contracting Department by the date and time indicated in block 8 on the front page of the solicitation to be considered for award. The information submitted shall be clear and legible (Note: repetitively copied data loses its clarity and legibility.) Quotes may be submitted via email to eric.vonreichenbach@usmc.mil (PREFERRED METHOD); via USPS to Commanding General, Attn: Contracting Department, P.O. Box 8368, Camp Lejeune, NC 28547-8368; or via hand delivery/courier (UPS, FEDEX, Etc.) to Contracting Department, Bldg 1116, Camp Lejeune, NC 28542. It is solely the Contractor's responsibility to ensure quotes are received within the required timeframe.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

52.204-17 OWNERSHIP OF CONTROL OF OFFEROR (NOV 2014)

(a) Definitions. As used in this provision--

Commercial and Government Entity (CAGE) code means—

(1) An identifier assigned to entities located in the United States or its outlying areas by the Defense Logistics Agency (DLA) Contractor and Government Entity (CAGE) Branch to identify a commercial or government entity, or

(2) An identifier assigned by a member of the North Atlantic Treaty Organization (NATO) or by the NATO Support Agency (NSPA) to entities located outside the United States and its outlying areas that the DLA Contractor and Government Entity (CAGE) Branch records and maintains in the CAGE master file. This type of code is known as an NCAGE code.

Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.

Immediate owner means an entity, other than the offeror, that has direct control of the offeror. Indicators of control include, but are not limited to, one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.

52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS--COMMERCIAL ITEMS (NOV 2015)

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (p) of this provision.

(a) Definitions. As used in this provision --

“Economically disadvantaged women-owned small business (EDWOSB) Concern” means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program.

"Forced or indentured child labor" means all work or service-

(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.

Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.

Immediate owner means an entity, other than the offeror, that has direct control of the offeror. Inverted domestic corporation means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).

Manufactured end product means any end product in product and service codes (PSCs) 1000-9999, except--

(1) PSC 5510, Lumber and Related Basic Wood Materials;

(2) Product or Service Group (PSG) 87, Agricultural Supplies;

(3) PSG 88, Live Animals;

(4) PSG 89, Subsistence;

(5) PSC 9410, Crude Grades of Plant Materials;

(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;

(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;

(8) PSC 9610, Ores;

(9) PSC 9620, Minerals, Natural and Synthetic; and

(10) PSC 9630, Additive Metal Materials.

Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate--

(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;

(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;

(3) Consist of providing goods or services to marginalized populations of Sudan;

(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;

(5) Consist of providing goods or services that are used only to promote health or education; or

(6) Have been voluntarily suspended.

Sensitive technology--

(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically--

(i) To restrict the free flow of unbiased information in Iran; or

(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and

(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).

Service-disabled veteran-owned small business concern--

(1) Means a small business concern--

(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and

(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.

(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).

"Small business concern" means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation.

Small disadvantaged business concern, consistent with 13 CFR 124.1002, means a small business concern under the size standard applicable to the acquisition, that--

(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by--

(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and

(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2); and

(2) The management and daily business operations of which are controlled (as defined at 13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.

Subsidiary means an entity in which more than 50 percent of the entity is owned--

(1) Directly by a parent corporation; or

(2) Through another subsidiary of a parent corporation.

Veteran-owned small business concern means a small business concern--

(1) Not less than 51 percent of which is owned by one or more veterans (as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and

(2) The management and daily business operations of which are controlled by one or more veterans.

"Women-owned business concern" means a concern which is at least 51 percent owned by one or more women; or in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and whose management and daily business operations are controlled by one or more women.

"Women-owned small business concern" means a small business concern--

(1) That is at least 51 percent owned by one or more women or, in the case of any publicly owned business, at least 51 percent of its stock is owned by one or more women; or

(2) Whose management and daily business operations are controlled by one or more women.

Women-owned small business (WOSB) concern eligible under the WOSB Program (in accordance with 13 CFR part 127)”, means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States.

(B) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.

(ii) Examples.

(A) The taxpayer has received a statutory notice of deficiency, under I.R.C. Sec. 6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(B) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. Sec. 6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(C) The taxpayer has entered into an installment agreement pursuant to I.R.C. Sec. 6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.

(D) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. 362 (the Bankruptcy Code).

(i) Certification Regarding Knowledge of Child Labor for Listed End Products (Executive Order 13126).
