PERFORMANCE WORK STATEMENT

for

MCIEAST TELEPHONE NETWORK TECHNICAL ASSISTANCE AND MAINTENANCE SUPPORT

SERVICES

1.0 BACKGROUND. The G6 Telecommunications Support Division (TSD) provides critical voice and data communications to the Camp Lejeune area, and requires technical assistance and maintenance support in order to provide reliable service.

2.0 SCOPE. This Performance Work Statement (PWS) establishes and defines the requirements for the contractor to provide technical support, repair/return of parts and on-site maintenance support as required for the

Base Telephony Network for the Greater Camp Lejeune area. Specifics for each line item are included below. This will be a Firm Fixed Price (FFP) contract.

3.0 CONTRACT OBJECTIVE. The purpose of this contract is to provide Telecommunications Support

Division, G-6, MCIEAST with Technical Assistance Service (TAS), / Extended Technical Assistance Services

(ETAS), Return & Repair Replacement Service, and Remote Diagnostics Service for Nortel/Avaya/ Genband equipment located aboard MCB Camp Lejeune and Marine Corps Air Station (MCAS) New River.

4.0 REQUIREMENTS.

4.1 This support agreement shall include the service requirements of Technical Assistance Service (TAS), /

Extended Technical Assistance Services (ETAS), Return & Repair Replacement, and Remote Diagnostics, based on the Avaya CS-2100 host (with 4 MG9000 remotes, 3 RCC2 remotes, and one CM6) and remote having 27,503 active ports, one (1) Callegra Callware Messaging system.

4.2 The support agreement package shall consist of core Nortel/Avaya/Genband TAS/ETAS support on a 24/7 basis for the term of the contract. The contractor shall take incoming service calls. The contractor shall provide initial Tier 1 support and shall open ETAS tickets with Avaya/Genband on required service issues. This core

TAS/ETAS support package shall provide the primary services required for day-to-day support of the MCB Camp

Lejeune Meridian CS-2100, remotes, voice mail system, and all power inverters, and rectifiers.

4.3 The contractor shall provide unlimited remote and on-site maintenance, diagnostics, routine technical assistance, and emergency technical assistance services as required. In addition to technical assistance, services shall include testing, diagnostic fault isolation, problem resolution, and system restoration from the Avaya/Genband TAS facilities for the Avaya CS-2100 switching system. Remote support shall include both telephonic assistance, direct remote access and on site assistance to the switching system via PSTN modem connection and/or Ethernet connection, as available.

4.4 There shall be special remote terminals to allow the Avaya/Genband TAS center to communicate with the system to diagnose fault conditions and recommend corrective actions. Emergency TAS shall be available 24 hours a day, seven days a week. For those customers requiring non-service threatening support, routine TAS shall be available from 7:00 a.m. to 4:00 p.m. EST, Monday through Friday.

4.5 In addition to core TAS/ETAS coverage, the support agreement shall allow for Repair and Replacement

(R&R) of CS-2100 hardware, including disk drives, tape drives, power rectifiers and inverter modules. Typical turn-around time for R&R service shall be 20 business days or less. During emergency situations, necessary replacements shall be shipped within 24 hours. Coverage shall include all CS-2100 equipment, rectifiers, inverters, voice mail components, and business telephone instruments (M5300 series) to be identified via a polling report. Non-

Avaya/Genband CS-2100 peripherals such as administration terminals and printers will not be covered under this contract.

4.6 DEPOT LEVEL SUPPORT.

4.6.1 Repair and Return of Failed Items. The contractor shall utilize Avaya/Genband services to support CS-2100 telecommunication systems. The contractor shall closely monitor Avaya/Genband’s performance to ensure that quality standards are being observed; thereby providing a well-coordinated effort to ensure highly responsive Depot

Level Support for all items and support equipment that comprise the CS-2100 switch system installed at MCB Camp

Lejeune, NC.

4.6.2 Depot Center Operations. The contractor shall utilize Avaya/Genband’s dedicated full-time facilities for

Depot Level Support including the repair of all CS-2100 and peripheral equipment. This support shall consist of the repair or replacement of electronic circuit packs, major subassemblies, modules, printed circuit boards, and other repairable items. The depot centers shall be fully staffed during normal duty hours and maintain on call standby coverage to handle emergency orders during all non-duty hours.

4.6.3 Emergency Requests. To meet the Government's high-priority, short lead time emergency requirements, the contractor's emergency replacement service shall be available 24 hours a day, seven days per week, 365 days per year for the MCB Camp Lejeune CS-2100 system. An emergency is defined as a critical system service outage which significantly impacts customer operations. The Government is responsible for making the final determination of an emergency. The contract will identify the point of contact that has been authorized by the Government to request this emergency service.

4.6.4 Remote Diagnostics. The contractor shall provide remote diagnostic services for Avaya CS-2100 switch systems that may occasionally experience hardware or software problems. Avaya/Genband shall have the ability to dial in through a modem to investigate the CS-2100 system in order to diagnose problems, provide technical assistance, and upload software patches. The contractor shall provide MCB Camp Lejeune with the phone numbers, addresses, and POCs for the remote diagnostic facilities.

4.6.5 Scope of Remote Diagnostic Services. Remote diagnostic services shall be available for use during emergency situations when all other Government, third-party, and/or on-site contractor support have failed to correct a problem. Emergencies shall be handled immediately. With non-emergencies, the site shall be called back to work the problem with the ETAS engineer. The contractor shall immediately investigate and resolve emergency situations that occur in the operation and maintenance of the switch systems and continue to work every such situation remotely until the problem is corrected and service is restored.

4.6.6 Remote Diagnostic Service Centers. The Remote Diagnostic Service Centers for Avaya/Genband switches and switch system peripherals shall be available 24 hours a day, seven days per week, 365 days per year for the duration of the contract.

4.6.7 On Site Diagnostic Service. If trouble analysis remote diagnostic services or replacement of contractor supplied parts by local technical experts are unable to isolate and affect repairs, then onsite contractor support shall be required. The contractor shall provide for and assume all costs associated for travel, lodging and subsistence of any support staff required to travel to the MCB Camp Lejeune sites covered under this support agreement.

6.0 PLACE OF PERFORMANCE. Performance may require shipping to the following locations:

6.1 Marine Corps Base (MCB) Camp Lejeune, NC

6.2 Marine Corps Air Station (MCAS) New River, NC.

7.0 PERIOD OF PERFORMANCE. The period of performance will include a base period of 12 months, and two 12 month option periods. The base period will be 1 April 2016 to 31 March 2017.

8.0 GOVERNMENT FURNISHED INFORMATION AND FACILITIES. Government furnished information, such as building locations, required by the contractor for the performance of this contract will be provided with each request. All information received remains the property of the United States Government. The

Contractor may be in contact with data subject to the Privacy Act (Title 5 of U.S. Code, Section 552.a). Reports and data shall be identified and safeguarded accordingly. Agency procedures shall be followed. The Contractor shall ensure that contractor employees assigned to this requirement are briefed annually on properly identifying and handling privacy act data/ information.

There is no requirement for Government Furnished Facilities. If vendor provides “on-site” support, access will be granted as required.

9.0 CONTRACTOR RESPONSIBILITIES. The Contractor shall provide shipping as required. The

Government will not pay any travel expenses as part of this PWS.

10.0 CONTRACTOR UNAUTHORIZED WORK PERFORMANCE. The Contractor shall not perform work that deviates from contract requirements and specifications. If the Contractor deviates from contract requirements and specifications without approval of the CONTRACTING OFFICER, such deviation shall be at the risk of the Contractor and any cost related thereto shall be borne by the Contractor.

11.0 GOVERNMENT POINT OF CONTACT. The Government points of contact will be provided at time of contract award.

12.0 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). The QASP is developed to provide the standard of surveillance for monitoring the support services contract and provides a systematic method to evaluate the services the contractor is required to furnish.

12.1 The vendor shall include a Quality Assurance Surveillance Plan (QASP) to be considered a complete quote.

While the quality of QASP will not be evaluated as a part of the technical capability factor, the vendor shall submit an effective QASP from which the Government can predict the likelihood of receiving quality services. The

Government reserves the right to approve, alter, or replace the submitted QASP with the Government's own plan.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014) (TAILORED)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

This is a request for OPEN MARKET PRICED quotes. The proposed acquisition is UNRESTRICTED. The associated North American Industrial Classification System (NAICS) Code is 811213 Communications Equipment

Repair and Maintenance.

BASIS OF AWARD. The Government will solicit and award this contract using simplified acquisition procedures, in accordance with FAR part 12 and 13. It is the Government's intention to issue a single firm-fixed-price contract on an "all or none" basis. ). Award will be made on the basis of the lowest price technically acceptable (LPTA) of quotes meeting or exceeding the acceptability standards for non-cost factors.

The Government intends to award to the quoter who represents the Lowest Price Technically Acceptable (LPTA).

Quotes will be evaluated and ranked according to price, from lowest to highest. The Government reserves the right to evaluate only the technical quote of the lowest price quoter. Other technical quotes may not be evaluated unless the Government determines the lowest price quoter to be unacceptable or non-responsive. In the event the

Government determines the lowest price quoter to be unacceptable, the technical quote of the next lowest price quoter will be evaluated. This process will continue until a technical proposal is determined to be acceptable.

EVALUATION. Vendors responding to this announcement shall submit their quote in accordance with FAR

52.212-1. Submission of quote shall include the following:

(1) Technical Acceptability: Detailed technical narrative that shall include the contractor’s technical approach for accomplishment of work that explains how the requirements described in the Performance Work Statement (PWS) will be accomplished from beginning to end. A vendor’s quote shall be written in a logical, practical, clear, concise, and logically assembled manner, containing all pertinent information in sufficient detail to provide the evaluators with a clear understanding of the vendor’s approach and to permit evaluation of the proposed program. It is the responsibility of the vendor to present enough information to allow the various work efforts, support, and management approaches to be meaningfully evaluated without discussions. Quote shall include confirmation of

Nortel/Avaya/Genband Partnership;

(2) Delivery: Clearly state ability to meet the Period of Performance start date of 1 April 2016;

(3) Past Performance: A review of the vendor’s record in the Past Performance Information Retrieval System

(www.ppirs.gov) will be conducted. To be considered acceptable the vendor must have ratings of Exceptional, Very

Good, Satisfactory, or Marginal. An Unsatisfactory rating will be considered unacceptable. In the case of an offeror without a record, or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance;

(4) Price: All quotes submitted shall be the Contractor’s Best Quote as it is Government’s intent to make award based on the original quotes received.

Request quotes be prepared as FOB Destination (shipping included in price). Award will be made on the basis of the lowest price technically acceptable (LPTA) of quotes meeting or exceeding the acceptability standards for non-cost factors. The Government intends to evaluate quotes and award a contract without discussions with vendors.

Vendors should not expect requests for clarification or additional information from the Government. Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. However, the

Government reserves the right to conduct discussions with vendors if later determined by the Contracting Officer to be necessary and in the best interest to the Government. The Government may reject any or all quotes if such action is in the public interest; and may waive informalities and minor irregularities in quotes received.

SUBMISSION REQUIREMENTS. Quoters shall include:

1) Completed FAR provision 52.204-17 - Check appropriate box. No signature required but please put company name on document.

2) Completed FAR provision 52.212-3 - Check appropriate boxes at 52.212-3(b)(2) and (3) and put company name on document. If no changes are necessary, clearly state so in submission package.

3) Completed DFARS provisions 252.209-7991 and 252.239-7009 - Check appropriate boxes. No signature required but please put company name on document.

4) Clearly state ability to meet specified delivery time after receipt of contract.

5) Technical narrative (see EVALUATION paragraph above) and confirmation of Nortel/Avaya/Genband

Partnership.

6) Vendor's quote shall include a Quality Assurance Surveillance Plan (QASP) to be considered a complete quote.

While the quality of QASP will not be evaluated as a part of the technical capability factor, the vendor shall submit an effective QASP from which the Government can predict the likelihood of receiving quality services. The

Government reserves the right to approve, alter, or replace the submitted QASP with the Government's own plan.

Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the

Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment

Act of 2007) conducting the business can demonstrate--

(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;

Service-disabled veteran-owned small business concern--

(1) Means a small business concern--
