INSTRUCTIONS TO OFFERORS

Addendum to 52.212-1 – Instructions to Quoters – Commercial Items

The Government intends to award a single award contract as a result of this solicitation. The Quoter shall submit a quote that clearly and concisely describes their response to the requirements of this solicitation. Use of general or vague statements such as “standard procedures will be used” will not satisfy this requirement. Unnecessary elaboration or presentations beyond that sufficient to present a complete and effective quote are not desired and may be construed as an indication of the Quoter’s lack of understanding. Elaborate artwork and expensive visual or other presentation aids are discouraged.

Quotes must be prepared in accordance with these instructions. Failure to do so may result in the Quoter’s submission being determined to be unacceptable and ineligible for award. The Quoter’s submission shall be screened by the Contracting Officer or a designee upon receipt to ensure completeness and compliance with the instructions contained in the RFP. Elimination of a Quoter is at the sole discretion of the Contracting Officer.

The purpose of this procurement is to restore the UH-34D Sikorsky helicopter, which was in action on 18 August

1965 during the initial assault landings of Operation STARLITE, back to its original paint scheme and markings currently carried on the helicopter. The restored helicopter will be on display in the National Museum of the Marine

Corps, Marine Corps Base, Quantico, VA. The contract award shall be made in accordance with the following information contained within the solicitation. The Quoter must submit a quote including information pertaining to

Technical Capability, Price, and Past Performance.

ADDITIONAL INSTRUCTIONS, CONDITIONS, AND NOTICES TO QUOTERS

QUESTIONS: The Quoter may submit questions requesting clarification of solicitation requirements. All questions must be received no later than 10:00 am EST on 4 September 2015. No questions will be responded to after the cut-off date. For quote purposes the estimated date of Contract award is anticipated with a period of performance start date of 25 September 2015.

Note: Due to the firewall at Quantico, please be advised that it is the Quoter's responsibility to ensure all electronic submittals of questions are received by the Contracting Office in the timeframe given above.

INSTRUCTIONS FOR PREPARATION AND SUBMISSION OF QUOTES

Quotes must be submitted no later than 2:00 PM EST, 17 September 2015. In order to maximize efficiency and minimize the effort involved in the quote evaluation process, all Quoters must comply with the following instructions for quote format and content. Quotes that do not comply may be considered non-responsive and may render the quote ineligible for award. Quotes shall be sent via Federal Express, private courier, hand delivered or mailed, to the following address prior to the cutoff date/time to:

The Quoter’s quote shall consist of two (2) separate volumes with one (1) original and three (3) copies of Volume I and one (1) original of Volume II. The Volumes are: I - Technical and II - Business.

Electronic quotes are not authorized for this solicitation and will not be considered for evaluation.

In addition to the paper copies identified above, the Quoter shall submit all quote information in electronic format on a CD. Text and graphics portions of the electronic copies shall be in a format readable by MS Word 2003 or earlier.

Data submitted in spreadsheet format shall be readable by MS Excel 2003 or earlier. In case of conflict between the paper copy and the electronic copy of the quotes submitted, the paper copy shall take precedence. In order to maximize efficiency and minimize the time for quote evaluation, it is required that all Quoters submit their quotes in accordance with the format and content specified.

QUOTE FORMAT

The quote shall be formatted as follows:

• 8.5x11 inch paper

• 8.5x14 inch for foldouts, matrixes, WBS;

• Single-spaced typed lines;

• 1 inch margins on all sides;

• 11 point Arial font;

• Headers/footers – ensure the company name and RFP number is included on each page;

• Graphs and drawings must be clear/legible and may use a smaller font;

• No photographs or hyperlinks are permitted; and

• MS Office compatible files are required for electronic content

QUOTE CONTENT

Each quote shall contain the following volumes/sections:

• Technical Volume (I)–NTE 25-pages, including a Letter of Transmission, Table of Contents, Compliance

Matrix, Technical Capability, Performance Execution and Management and Past Performance.

• Business Volume (II)–NTE 10 pages, including a Formal Offer Letter, SF1449, and Price Quote.

Include the following and adhere to the page limitations:

The Letter of Transmission – (See Volume I – Technical Volume above) Should identify the solicitation number, corporate principal submitting the offer, key points of contact within the firm who are authorized to bind the firm, (e.g. contracts, pricing, technical), and a summary of the material content (to include identifying all subcontractors, if applicable.

A Table of Contents and Compliance Matrix (See Volume I – Technical Volume above) pertinent to the quote’s material organization and providing traceability from the Quoter’s quote, to the PWS, to the evaluation criteria, and to other solicitation sections as necessary. The Compliance Matrix may be presented as an 11 x 14 foldout.

Determination of Contractor’s Responsibility Information – The Quoter shall include the company’s most current balance sheet and Income statement. Additionally included in the Executive summary shall be the company’s mission statement.

It is an quoter's responsibility to ensure and verify its quote has been received by the proper person at the proper place at the proper time. It is the quoter's responsibility to confirm proper delivery to, and timely receipt by, the

RCO, Christa Eggleston-Scott.

Evaluation Factors

Factor 1 - Technical Capability.

The quote should clearly address the Quoter’s understanding of the technical and programmatic requirements outlined in the Performance Work Statement (PWS); ensuring their discussion articulates their capability so as to provide continuous quality support. The discussion must demonstrate the Quoter’s capacity to support the identified demonstrated mission areas, understands the priorities and emerging issues within those areas, and can execute a comprehensive approach that ensures quality Performance and use of staff resources to meet objectives. The quote should clearly demonstrate the Quoter’s ability to adequately identify a suitable level of technical understanding of the issues. This includes, at a minimum, describing contract level performance objectives that can be accomplished by their technical approach; describe the benefits and advantages of that approach; and effectively illustrate the roles of staff and team resources to execute the intended strategy.

Factor 2 - Performance Execution and Management.

The quote shall clearly demonstrate the Quoter’s management approach suitable to respond to the types of requirements delineated in the PWS, consistent with their technical approach. The discussion must provide a comprehensive approach to managing task order, performance-based work requirements; assurance of consistent quality of performance; capacity for handling matters of staffing and resource balancing; and ensuring complementary roles between the contractor’s managers and government staff. The evaluation will be based on the

Quoter's understanding of the critical support areas, emerging issues, and risks associated with the support goals set forth in the PWS, and have the management techniques, controls, subcontractor management processes, and quality provisions necessary to execute their approach.

The quote shall demonstrate their ability to adequately define an effective management approach complementary to the proposed technical solution set forth in response to the criteria in the PWS. At a minimum, this includes identifying responsibilities and roles of team members in attaining performance objectives; describing the processes and quality measures associated with executing the strategy; and illustrating the roles of staff and team resources relevant to the approach so as to ensure suitable oversight and control of performance execution. Collectively, the integrated approach must demonstrate to the evaluator that the Quoter possesses the capabilities and resources to establish and operate an efficient organization that is both responsive and flexible to meet the requirements. Roles of management and other key staff are annotated and provisions for independent, objective quality assurance (e.g., customer surveys, etc.) are incorporated. Measures for immediate action and remediation are noted in the discussion so as to ensure the quality of contract deliverables and timeliness of service delivery. Key considerations include process maturity, roles of responsible staff, degree of WBS integration, and controls at both the corporate and contract level under performance-based operating parameters.

Factor 3. Past Performance.

The quote shall address the quality of an Quoter’s relevant and recent (within the previous 5 years) past performance based on the input received from the Quoter, its references via the Past Performance Questionaire and other reliable sources contacted by the Government. Quoter’s that have no record of past performance (i.e. new businesses) must submit a signed and dated statement to that effect. If an Quoter submits a certification statement and the Government has no information available regarding the Quoter’s past performance, that Quoter will receive an Unknown Confidence (Neutral) rating (i.e. the Quoter is evaluated neither favorably nor unfavorably) for past performance. Quoters are advised that any contract reference submission that is not relevant will receive a neutral rating for that contract reference in Past Performance. The Quoter will provide at least (3) references demonstrating relevant past performance involving the quality of performing the same or similar services within the last five (5) years.

Include the following fields:

• Customer;

• Contract/Task Order Number;

• Program name;

• Period of performance

• Contract type;

• Total contract value;

• Identification of any Key Staff involved who will be assigned to this contract;

• Names/Telephone numbers/e-mail for the PCO and COR/TOM.

In addition, Quoters should provide the enclosed Past Performance Questionnaire to the same three (3) previous references. The references should email the form directly to the RCO-NCR at the address provided on the form on or before 4:00PM EST, 15 September 2015.

The Government may request additional past performance information from others using the Past Performance

Questionnaire. The Government may also use other information such as CPARS/PPIRS/FAPIIS data available from

Government sources to evaluate an Quoter’s past performance. The Government reserves the right to limit or expand the number of references it decides to contact and to contact references other than those provided by the

Quoter.

Volume II – Business Volume: The Quoters shall provide a Formal Offer letter, to include the total contract dollar value. Provide prices corresponding to the CLIN’s in the solicitation, to include Labor categories, fully loaded rates, and number of hours (to include supporting material documentation) for your firm fixed pricing. There is a 10 page limit for Business volume II. Quoters are required only to price the one-year base and one-year option periods.

Quoters shall not submit a price for the potential six month extension of services period.

Volume II - Business Volume

Factor 4. Price.

The fully developed technical solution and corresponding grand total price for each function area established in the

CLIN structure of the solicitation should be clearly annotated in the quote and include the information requested in the Instructions to Quoter and Evaluation.

Completeness. The Quoter must sign the SF1449, acknowledge all amendments, and include a compliance matrix and Table of Contents corresponding to its quote submission.

At a minimum, the following areas must be addressed:

-Quote is valid for 90 days from (Date of Submission);

-No exception to any Terms and Conditions has been taken;

-No assumptions or constraints included in Quoter’s quote;

-Acknowledge any amendments to the RFP;

-SAM validated / NAICS 488190 compliant;

-Signed Quote of $XXXXXXX

-CLIN pricing and supporting documentation is provided;

-Staff positions based upon contingency hires must include a signed letter of quote/acceptance in the Business Volume
