Performance Work Statement (PWS) For
Linen Laundering, Delivery, and Pickup Services
Objective. Acquire linen laundering services for 355 Medical Group (MDG) personnel.
Contractor shall manage all off‐site aspects of the laundering services. The contractor shall ensure that the 355 MDG receives timely accurate deliveries twice a week.
1.0. Description of Services. The contractor shall provide all equipment, materials, transportation, personnel, labor, supervision, and management to perform linen laundering, delivery and pick up services, in support of patient care programs and base veterinary services for the 355 MDG, Davis‐Monthan, AFB (DMAFB), AZ. All delivery and pick‐ups shall occur at the 355 MDG Military Treatment Facility (MTF), building 400, unless otherwise indicated by the Contracting Officer (CO) or Contracting Officer Representative (COR).

1.2. Product Specifications. All contractor furnished wash formulas, chemicals, materials, equipment, and tools used shall be approved by the 355th Medical Group Infection Control Function prior to being used and contract award. The contractor shall provide literature for all supplies and products used or changes in previously approved supplies or products. The contractor may be required to provide samples of supplies or products. All materials shall comply with all applicable environmental laws outlined in the Guidelines for Environmental Infection Control in Health Care Facilities in compliance with Appendix C.
1.3. Clean Linen. All clean items are to be protected from contamination and soiling during transit. This shall be accomplished by ensuring delivery is in an enclosed truck that is kept sealed at all times during transportation. Further, laundry shall be bundled, wrapped and sealed in clear plastic wrap. Government personnel will weigh cleaned linen upon delivery, utilizing a Government scale. Contractor personnel shall physically unload clean linen and deliver it to the clean linen room after weighing.
1.4. Soiled Linen. Contractor personnel shall physically load soiled linen from the dirty linen room to the contractor's vehicle.

1.5. Infection Control. Laundering services shall be performed at the contractor‐owned and operated facility. The contractor shall comply with infection control standards as outlined in the checklist at Appendix B of this PWS and quality as outlined in PWS section 4.0. The contractor must meet certification standards required by the Healthcare Laundry Accreditation Council
(HLAC).
1.6. Ordering. Designated Government personnel will place orders, via fax, web based ordering or electronic mail, 2 days prior to delivery by the contractor, for the quantity of linen required for the MTF.
1.7. Pick‐Up and Delivery Hours. The contractor shall provide linen pickup and delivery services between the hours of 8:00 a.m. and 10:00 a.m., Tuesdays and Fridays. Deliveries shall include sufficient linens to ensure coverage in between deliveries. Piecemeal delivery of linen will not be accepted unless given prior approval by Contracting Officer.
1.8. Delivery Tickets. The contractor shall furnish a delivery ticket for each delivery. The delivery ticket shall be completed by the contractor in two copies and signed by both the contractor and a Government representative at the time of delivery. One copy of each delivery ticket shall be provided to the Government representative at the time of delivery and a copy shall accompany the contractor’s invoice. Each delivery ticket shall clearly state the following:

(2) Company Phone Number
(3) Contract number
(4) Date of Delivery
(5) Description and number of items delivered by size
(6) Weight of items delivered
1.9 Weight Report. The contractor shall submit a monthly weight report summarizing the weight of cleaned linen delivered for the previous month. The report shall be submitted to the Government Contracting Officer Representative (COR), no later than the second (2nd) business day of each month. The weights on the weight report must match the weights on the delivery tickets for the month. The total weight for each month will be verified by the Government and the contractor, no later than the fifth (5th) business day of each month.
1.10. Laundry Carts. The contractor shall deliver clean linen to the MTF in serviceable, contractor‐provided sanitized laundry carts. The laundry carts shall be on casters or wheels to allow maneuverability, be labeled with the cart weight and have a minimum load capacity of 400 pounds. Soiled laundry carts and/or covers shall be steam cleaned or cleaned with a soap and water solution or a germicidal agent before they are used to transport clean linen. The contractor shall cover carts containing clean linen with plastic or a lid to protect the contents from dust and contaminants. The carts will be used by Government personnel to gather soiled linen for pick up
21 December 2021 Page | 3 by the contractor on the next scheduled day. The contractor shall ensure that there are two serviceable laundry carts available for use within the MTF at all times to hold soiled laundry.
1.11. Packaging. All cleaned linen items shall be bundled and tied together in small quantities, to be determined by the contractor, to allow easy access, inventory and handling by housekeeping personnel. All packaged linen shall be wrapped in plastic film unless otherwise specified in the contract. Laundered and clean linen shall be free of contamination.
1.12. Bulk Laundry. Bulk laundry shall be wrapped and enclosed in plastic film, in packages in accordance with (IAW) PWS Appendix A.
1.13. Bedside Screen Curtains. Bedside screen curtains shall be individually packaged and marked by size based on the number of eyelets on the top of each curtain. Curtains will be laundered at least semi‐annually or as needed.
1.14. Weight Verification. All clean linen will be weighed by Government personnel at the time of delivery. The linen weights of delivered linen will match the linen weights entered on the contractor’s delivery ticket. Soiled linen will be weighed by Government personnel prior to pick‐
up. Weights will be recorded by Government personnel and the record will be signed by the contractor and Government personnel. A copy of the weight record will be provided to the contractor.
1.15. Contaminated Laundry Pick‐up. Contaminated laundry will be picked up Tuesdays and Fridays regardless if a linen order is placed. Contaminated laundry will be packed in plastic bags by Government personnel and placed in the contractor‐provided laundry carts. After the carts are weighed, the contractor shall load the carts on the delivery vehicle and deliver the soiled linen to the laundering facility. The contractor is responsible for the proper handling of soiled linen in compliance with standard infection control procedures according to Healthcare Infection Control Practice Advisory Committee (HICPAC) “Guidelines for Environmental Infection Control in Healthcare Facilities” and HLAC’s “Accreditation Standards for Processing Reusable Textiles for Use in Healthcare Facilities.” This contract does not include services for radioactive contaminated laundry.
1.16. Operating Shirts, Trousers, and Jackets. Operating shirts, trousers, and jackets shall be packaged with like items according to color and size.
1.17. Pediatric Garments. Pediatric garments shall be free of soap, treated with a souring agent for sterilization and a sufficient quantity of softening agent.
1.18. Veterinarian Clinic. Linen shall be free of soap, treated with a souring agent for sterilization and a sufficient quantity of softening agent.
1.19. Contaminated Laundry. All soiled laundry originating from the MTF is categorized as
21 December 2021 Page | 4 contaminated. The contractor shall handle contaminated laundry in accordance with HICPAC “Guidelines for Environmental Infection Control in Health Care Facilities’. The contractor is responsible for proper training of employees and the employees’ use of standard precautions in handling of contaminated laundry.
1.20. Stain Treatment. Linen to be stain treated shall be weighed and accounted for separate from any other soiled linen. The contractor shall treat the stained laundry in a chemical solution that will remove the stain(s) from any article of fabric. After the stain treatment of the linen has been completed, the contractor shall launder, sort, package and weigh the linen for accountability and distribution.
1.21. Rejected Laundry. Linen will be rejected when it fails to meet the standards of cleanliness as defined in this PWS in 2.0. Services Summary, “#1. Clean Linen” at the time of delivery or when unpacked. Rejected laundry shall be collected by MTF personnel when discovered and returned to the contractor who shall re‐ perform and deliver re‐cleaned laundry by the next scheduled delivery day. The Government may reject by individual piece, package, or cart(s). The contractor shall re‐perform at no cost to the Government.
1.22. Re‐Performance. In accordance with the provisions of this contract covering inspection and acceptance, items found not to meet the requirements of the contract terms shall be re‐ cleaned by the contractor at no additional cost to the Government. Item(s) found by the Government of an unsatisfactory condition shall be reported to the contractor's Contract Manager at the next scheduled pick‐up day by the COR. After the items are re‐cleaned, the contractor shall pack these items separately and mark each bundle "Attention Contracting Officer Representative." If re‐performance of linen is required, the contractor shall ensure adequate quantities remain in‐ house to meet functional requirements.
1.23. Licenses and Permits. The contractor shall obtain any necessary licenses and permits for complying with all applicable Federal, State, and local laws, codes and regulations in connection with performance as stated in this PWS (see Appendix C). Personnel operating motor vehicles shall possess a valid state driver’s license. The contractor is liable for all personnel injuries and equipment damage that result from their negligence or failure to follow safety/health guidelines.
The contractor shall take proper safety and health precautions to protect work, the workers, the public, and the property of others.
1.24. On‐Site Inspections. Government representatives from the 355 MDG Infection Control team and Facility Management offices will conduct intermittent on‐site inspections of the contractor’s plant facility and the contractor shall escort the Government representatives during site visits. The site visit checklist in PWS Appendix B will be utilized for inspections.
1.25. QUALITY
1.25.1. Quality Control. The contractor shall develop and maintain a quality control program to
21 December 2021 Page | 5 ensure laundry services are preformed IAW commonly accepted commercial practices and applicable industry regulations, codes and standards (see Appendixes B & C). The contractor shall notify the Government by telephone or email when they receive other than articles for laundry services in the laundry picked up. As a minimum, the contractor shall develop and implement procedures to identify, prevent, and ensure non‐recurrence of defective services. These procedures will be included in the Contractor’s quality control plan. Upon contract award, the contractor shall submit their quality control plan to the Contracting Officer for acceptance.
1.25.2. Quality Assurance. The Government will periodically evaluate the contractor's performance by appointing a representative(s) to monitor performance to ensure services are received IAW the PWS. The Government representative will evaluate the contractor's performance through intermittent on‐site inspections of the contractor's plant facility (see Appendix B), quality control program, and receipt of customer feedback from DMAFB personnel.
The Government may inspect each task as completed or increase the number of quality assurance inspections if deemed appropriate because of repeated failures discovered during quality assurance inspections or because of repeated customer complaints. Likewise, the Government may decrease the number of quality assurance inspections if performance dictates.
The Government shall be responsible for initially validating customer complaints. The Government COR shall make final determination of the validity of customer feedback in cases of disagreement between the contractor and customer.
1.25.3. Facility Inspection. In accordance with AFI 44‐108, the MDG Infection Control team and Facility Management staff , escorted by a COR and a linen facility representative, will tour and inspect the linen facility with the linen COR, prior to contract award for locally purchased contracts and at least annually thereafter. The Government representatives will have the right to access and inspect the contractor's laundry plant to ensure compliance with industry and Air Force regulations, codes, and standards (see Appendices B and C).
1.25.4. Contract Progress Meetings. The contractor’s Contract Manager is required to meet with the Contracting Officer, Contract Administrator, and COR at least quarterly or more often, if needed, to discuss contract performance. The Contracting Officer will schedule the meetings.
1.25.5. Disruption of Services. If routine services are disrupted through fault or negligence of the contractor for more than twenty‐four (24) hours, the Government reserves the right to procure such services from another source.
2.0 Special Requirements.
2.1. Continuation of Essential Contractor Services. The Government has determined that the services in PWS paragraphs 1.0 through 1.25.5, inclusive, have been determined to be mission essential, in accordance with the Defense Federal Acquisition Regulation Supplement (DFARS), Subpart 237.76, Continuation of Essential Contractor Services. The contractor providing essential
21 December 2021 Page | 6 services shall be prepared to continue providing such services, in accordance with the terms and conditions of their contract, during periods of crisis, as determined by Government personnel.
2.1.1. Essential Contractor/Liaison Services Plan. In accordance with DFARS 252.237‐7024 provision, Notice of Continuation of Essential Contractor Services, offerors shall provide with their offer a written plan describing how it will continue to perform the essential contractor/liaison services listed in the PWS. The required contents of the mission essential plan can be found in the DFARS 252.237‐7024 provision.
2.1.2. Contractor Mission Essential Employee Identification. Upon contract award, the contractor shall provide the name, address, and telephone number of a point of contact for purposes of continuation of essential DoD contractor services during crisis. The contractor shall also provide the names of contractor employees having military mobilization recall commitments, and have adequate plans for replacing those employees in the event of mobilization, in accordance with DoDD 1200.7., Screening the Ready Reserve, and DoDD 1352.1, Management of Regular Reserve Retired Military Members, which the contractor shall provide with their offer.

4.0. General Information
4.1. Contract Manager. The contractor shall designate a Contract Manager, and an alternate, who will be responsible for performance of services and have full authority to act for the contractor on all matters relating to daily operation of this contract. Upon contract award and prior to the contract start date, the contractor shall submit the name and telephone number of the Contract Manager and alternate, in writing, to the Contracting Officer. The Contract Manager or alternate shall be available to meet with Government personnel within one business day of notification during normal business hours and shall be available by telephone between 7:30 a.m.
and 4:30 p.m., Monday through Friday, to resolve any issues or problems that may arise. The Contract Manager and alternate must be able to read, write, speak, and understand English fluently.
4.2. Contractor Employees. The contractor shall maintain a sufficient number of qualified personnel to perform the requirements of this PWS. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor, by the Contracting Officer, as a potential threat to the health, safety, security, general well‐being, or operational mission of the installation and its population. The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the

Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with Department of Defense (DoD) Regulation 5500.7 and the Joint Ethics Regulation.
4.3. Listing of Employees. The contractor shall maintain a current listing of employees performing services on DMAFB. The list shall include employee's name, social security number, driver’s license number and state of issue, date of birth, and country of citizenship. Foremen, supervisors, and superintendents must be identified on the list of employees. The contractor shall submit the list to the Contracting Officer and the COR, prior to the contract start date, and submit an updated list when an employee's status or information changes, within 5 working days of the change. The name of any contractor employee that is terminated during the period of the contract must be reported to the Contracting Officer in writing within one workday of the employee’s termination.
4.4. Antiterrorism Awareness Training. Level I – Antiterrorism (AT) Awareness Training is available to all contractor employees. Non‐Common Access Card (CAC) holders can accomplish Level 1 ‐ AT Awareness Training at https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf.
Additionally, contractor employees may contact the Government sponsoring agency’s Unit Antiterrorism Representative (UATR) to request Antiterrorism Awareness Training material.
4.5. Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor personnel are required to review suspicious activity reporting procedures. Contact the Government sponsoring agency’s Unit Antiterrorism Representative (UATR) to request Suspicious Activity Reporting training material.
Contact the Base Defense Operation Center (BDOC) to report actual suspicious activity at 520‐ 228‐3200.
4.6. Incident Notification. Contractor personnel with access to a government computer shall update their AtHoc profile with current contact information to ensure receipt of incident notification. Contractor personnel without computer access should be alert for notifications from the base public address system (i.e. Giant Voice). Additional notifications can be received via the Davis Monthan AFB Facebook Page. In the event of an incident, contractor personnel should adhere to the guidance provided in the Antiterrorism Awareness Training.
4.7. Pass and Identification Items. The contractor shall ensure that all necessary pass and identification items required for contract performance are obtained for contractor employees and non‐Government owned vehicles.
4.8. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from contractor employees that depart for any reason before the contract expires (e.g. terminated for cause, retirement, etc.) and turn them in to the issuing office. Once contractor employees are no longer employed, the contractor office shall notify the
21 December 2021 Page | 9 issuing office of the termination immediately.
4.9. Traffic Laws. Contractor employees shall comply with all base traffic regulations. Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of seat belts is mandatory for all vehicle occupants. The use of cell phones is prohibited while driving on base, unless the phone is a hands‐free cell phone.
4.10. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.
4.11. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis‐Monthan AFB.
4.12. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7‐R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.13. Contractor Personnel Travel on the Installation. All contractor personnel will limit their travel on the installation only to specific areas required for performance of services, specified break and meal areas, or travel directly to and from these locations.
4.14. Privacy Act. The Contractor shall ensure that its employees fully understand and comply with Title 5 of US Code, Section 552.a and DoD 5400.7‐R, DoD Freedom of Information Act Program. These procedures must be followed to identify and safeguard reports and data accordingly. The contractor will ensure that Contractor employees are briefed annually on properly identifying and handling Privacy Act data/information.
4.14.1. Protection of Personally Identifiable Information. Personally Identifiable Information (PII) refers to information that can be used to distinguish or trace an individual’s identity either alone or when combined with other information that is linked or linkable to a specific individual.
PII may range from common data elements such as names, addresses, dates of birth, and places of employment, to identity documents, Social Security numbers (SSN) or other government‐ issued identity, precise location information, medical history, and biometrics. The contractor shall protect all PII encountered in the performance of services in accordance with Department of Defense Directive 5400.11, Department of Defense Privacy Program, and DoD 5400.11‐R, Department of Defense Privacy Program. If a PII breach results from the contractor's violation of the aforementioned policies, the contractor shall bear all notification costs, call‐center support costs, and credit monitoring service costs for all individuals who's PII has been compromised.

4.15. Place of Performance. The contractor shall primarily make deliveries to 355 MDG facilities, on DMAFB. All laundering services shall take place at contractor owned and operated facilities.
4.16. Medical and Emergency Absences. The contractor shall have multiple drivers able to access DMAFB to ensure that no medical or emergency absence of a driver hinders services to
355 MDG.
4.17. Federal Holidays. The Federal Government observes the following holidays. (Note: If the holiday falls on a Saturday then the holiday will be observed as recognized by the Federal Government). The contractor shall not normally be required to provide routine services on holidays, or the actual day set aside for observation.

4.17.1. The contractor shall make arrangements with Government personnel at least three (3) days prior to delivering linen on a day agreed upon by the Government when a delivery day coincides with a holiday. The Government may also request additional linen on the normal delivery day prior to the holiday.
4.18. Contractor Employee Appearance. Contractor employees shall present a professional, conservative, and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting the work environment, and have complied with socially acceptable standards of personal hygiene. While performing services, contractor employees shall be neat and clean (free of visible dirt and stains), well‐groomed and appropriately dressed.
The contractor employee's clothing shall fit correctly to provide a professional, modest appearance, in keeping with normally accepted commercial standards of dress for the work being performed. Personal Protective Equipment (PPE) shall be used by the contractor employees IAW commercial, industry regulations, codes and standards for hospital linen and laundering services (see Appendix C) while performing services under this contract.
4.19. Communication. Contractor employees shall maintain open, professional, and respectful communication with Government personnel (military and civilian) and other contractor employees. The COR will report valid complaints in writing to the CO for action. Valid complaints will be considered a performance failure. The contractor shall resolve all substantiated complaints immediately upon notification of the complaint. Contractor employees are required to identify themselves as a contractor at all times while on the job, (e.g. in the workplace, when attending meetings, when answering Government telephones, in e‐mails, or when making phone calls on behalf of the Government).
4.20. Contractor Employee Conduct. The Government reserves the right to require removal
21 December 2021 Page | 11 from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.
4.21. Smoking in AF Facilities. Contractor personnel are advised that the Air Force has placed restrictions on the smoking of tobacco products in Air Force facilities. AFI 40‐102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.
4.22. Safety. The contractor shall comply with all applicable local, state and federal laws and safety regulations. Compliance with Occupational Safety & Health Administration (OSHA) and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor's compliance or non‐compliance with such responsibilities. Any fines, penalties, etc., levied for contractor caused safety violations are the contractor's responsibility. The contractor shall immediately notify the 355 MDG Facility Manager, at 228‐2557, and 355 Wing Ground Safety Office, at 228‐ 5342 when safety violations occur or are discovered. Contractor employees shall fully cooperate with Government safety investigators.
4.22.1. Personal Protective Clothing and Equipment. The contractor shall provide contractor employees with all necessary personal protective clothing and equipment.
5.0. Government Provided Items and Services
5.1. Plastic Bags. The Government will provide the plastic bags that will be used by Government personnel to collect soiled linen and given to the contractor for removal from the MTF.
5.2. Commercial Grade Scale. The Government will provide a commercial grade platform scale, of a type capable of weights up to 2,000 pounds. The scale is located at the MTF loading dock, building 400. The Government will calibrate the scales for accuracy at least annually.
5.3. Emergency Services. The Government will provide security forces services (call (520) 228‐ 4444 for emergencies only), and fire protection services (call (520) 228‐3333). There is no emergency room on DMAFB, therefore contractor personnel shall call (520) 228‐3333 for medical emergencies.

A.1.2. The following are the routine weekly estimates for this contract. A.1.3.1. Scrubs. The above quantities of scrubs will serve as a beginning count. The Government may need to adjust levels of sizes periodically to accommodate personnel rotation. Total quantity would stay the same, however levels of sizes may need to be adjusted. Contaminated scrubs will be picked up, laundered, and inventoried. All soiled scrubs will be returned on the following delivery date.
A.1.3.1.1. Reimbursable. At the end of each quarter, the contractor shall report inventory totals to the Government and at the discretion of the Government, replenish any missing quantities.
The Government will negotiate the cost of any lost inventory each quarter. If the contractor caused damage to, or lost an item(s), damaged/destroyed and/or lost item(s) (to include government owned curtains and heat pad covers) shall be replaced with an identical or like item at the contractor’s expense within 10 business days of discovery. These replacement items shall be bundled separately and clearly marked “Replacement Items”. If any damaged/destroyed or lost item(s) are not replaced within 10 business days, the contractor shall reimburse the Government in accordance with DFARS Clause 252.237‐7015, Loss or Damage (Weight of Articles). If lost scrubs are found and returned to the inventory, the contractor shall credit the monthly invoice on the next billing cycle for the returned items.

A.1.4.1. Curtain Replenishment. Curtains shall be laundered at least semi‐annually or as needed. Curtains shall be individually packaged and marked by size.
A.1.5. Heat Packs Terry Cloth Covers. Heat pack covers shall be placed in the soiled linens bag by the Physical Therapy team and collected by the contractor weekly. The following list indicates the different types of terry cloth covers and the quantity of each to be stored by Housekeeping.

APPENDIX B – INFECTION CONTROL
B.1.1. Background. Although soiled linen has been shown to be a source of large numbers of pathogenic organisms, the risk of actual disease transmission is negligible. Infection control standards must be maintained to decrease risk of exposure and infection to patients and workers.
Efforts to reduce occupational risk of infection transmission and exposure to laundry workers primarily focuses on appropriate use of hand washing, protective barriers, removal of foreign objects from soiled linen stream, and reduction of the volume of blood, body fluids, excretions, and secretions in patient linens.
B.1.2. Site Visit Checklist. The following checklist will be used to evaluate compliance during site visits to the laundry plant IAW AFI 44‐108, guidelines and industry standards and regulations (See Appendix B). Failure of the any checklist item will require the contractor to submit a plan for improvement and the Government inspection team members to conduct a follow‐up

21 December 2021 Page | 15 inspection in 30 days. If facility fails the 30‐day follow‐up inspection, then further action may be taken, to include possible termination of the contract for failure to comply.
B.1.2.1. Laundry Facility Inspection Checklist
B.1.2.1.1. Plant Facilities
B.1.2.1.1.1. The laundry facility should be planned, equipped and ventilated as to minimize the dissemination of contaminants. The ventilation system should include adequate intake, filtration, exchange rate, and exhaust.
B.1.2.1.1.2. Soiled linen will be separated from clean linen processing area by physical design or barrier, negative air pressure in the soiled linen area, and positive airflow from the clean to soiled area.
B.1.2.1.2. Soiled Linen
B.1.2.1.2.1. Gloves and appropriate personal protective equipment (PPE) worn based on likelihood of contact of exposed skin and clothing with soiled linen. IAW OSHA;
1910.1030(d)(3)(x) and 1910.1030(d)(3)(i)
B.1.2.1.2.2. Soiled linen handled as little as possible with minimum of agitation.
B.1.2.1.2.3. Hand washing facilities are in soiled linen area, to include running water, approved hand washing soap, and either a drying machine or paper towels. IAW OSHA; Paragraph 1910.1030(d)(2)(iii)
B.1.2.1.2.4. Sharps disposal container is in area. IAW OSHA Paragraph:

B.1.2.1.3. Washing Area
B.1.2.1.3.1. Temperature of water must be compatible with the detergent.
B.1.2.1.3.2. Temperature gauge on the washer must register temperature of water accurately.
B.1.2.1.3.3. Washer must be in good mechanical working order and maintained well.
Maintenance documentation needs to be ready available during inspections.
B.1.2.1.3.4. Chemicals used to launder linen must be identified in writing. Safety data sheets must be made available for all chemicals in the facility upon request. Chemicals must be Environmental Protection Agency (EPA) according to the following link:

B.1.2.1.3.5. Personnel working in sorting and soiled areas area must change all PPE, wash hands, and put on new PPE before and after shifts, breaks, and upon exiting the soiled linen area.
B.1.2.1.4. Drying Area
B.1.2.1.4.1. Dryers will be adjacent to washers.
B.1.2.1.4.2. Temperatures and time settings on dryers will be specific and standardized for each load. Documentation must be made readily available upon request.
B.1.2.1.4.3. Lint vents must be cleaned prior to each load.
B.1.2.1.4.4. A clean cart must be used to remove dry, clean linen from each dryer.
B.1.2.1.5. Sorting Area
B.1.2.1.5.1. A clean table or area on which to fold linen will be available to fold linen. The surface will be made of non‐porous, wipeable material.
B.1.2.1.5.2. The sorting area will be kept clean and free from food, soiled items, and foreign debris. IAW OSHA Paragraph; 1910.1030(d)(2)(ix)
B.1.2.1.5.3. Linen will be packaged in similar plastic packaging prior to shipment.
B.1.2.1.5.4. Linen not acceptable due to contamination will be sent back to washers for a secondary washing process.
B.1.2.1.6. Ironing Area
B.1.2.1.6.1. Linen will be sent through presses in similar processes.
B.1.2.1.6.2. Presses will be dusted at the beginning of each day.
B.1.2.1.7. Wrapping Area
B.1.2.1.7.1. A moisture repellent wrap will cover the entire bundle of similar assembled linen.
B.1.2.1.7.2. Wrapped bundles of linen will be placed in linen carts that are specifically designated only for clean linen transport to the medical treatment facility.
B.1.2.1.7.3. Clean linen transport carts will be cleaned with a disinfectant or steam on a monthly basis or sooner if cart becomes soiled.

B.1.2.1.8. Transportation
B.1.2.1.8.1. Transportation truck will be cleaned on a monthly basis or sooner if truck cargo area becomes soiled, contaminated, or dirty. The truck cargo area must be cleaned before and after each clean and soiled linen transport.
B.1.2.1.8.2. Clean wrapped linen will be transported in clean carts in the transport truck. Clean wrapped linen will not be transported with bio hazardous waste or soiled linen.
B.1.2.1.9. Policies and Procedures
B.1.2.1.9.1. Written policies and procedures to cover all areas of responsibility related to service provided for this medical treatment facility will be provided and adhered to.
B.1.2.1.10. Training/Education
B.1.2.1.10.1. Appropriate annual and new employee training covering personal hygiene and hand washing technique will be provided to workers. OSHA 29 CFR 1910.
B.1.2.1.10.2. Appropriate annual and new employee training covering use and application of personal protective equipment will be provided to workers. OSHA 29 CFR 1910.
B.1.2.1.10.3. Appropriate annual and new employee training covering blood borne pathogen organisms and transmission will be provided to workers. OSHA 29 CFR 1910.
B.1.2.1.10.4. Employees will have received hepatitis B vaccine series or have a signed denial waiver on file with employer. OSHA 29 CFR 1910.
B.1.2.1.10.5. A needle stick protocol/follow‐up will be available for employees. OSHA 29 CFR 1910.
B.1.2.1.10.6. All of the above training and education will be in accordance with OSHA According to OSHA; 1910.1030(g)(2), Information and Training, 1910.1030(g)(2)(i).
B.1.2.1.10.7. The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program. IAW OSHA 1910.1030(g)(2)(ii).
B.1.2.1.10.8. Training shall be provided as follows: 1910.1030(g)(2)(ii)(A).
B.1.2.1.10.8.1. At the time of initial assignment to tasks where occupational exposure may take place. IAW OSHA 1910.1030(g)(2)(ii)(B).
