STATEMENT OF WORK

FOR

PHARMACY AC SPLIT DUCTLESS SPLIT A/C SYSTEM

AT

KAYENTA HEALTH CENTER

KAYENTA, ARIZONA

MAY 13, 2019

KAYENTA SERVICE UNIT

1. Introduction:

1.1. Kayenta Health Center (KHC) is located south of Arizona State Highway 160 at milepost 364.3. The nearest town is a two hour drive. The Kayenta Service Unit (KSU) serves a population of ~20,000 people spread across a remote and sparsely populated area. Services are provided to ~200 patients each day. KHC provides walk-in clinics and a 24 hour, 7 days per week emergency room. KHC requires professional services and expertise to perform the following work:

a. Installation of multi zone ductless split supplemental air conditioning system in the Pharmacy department and CT scanner equipment room.

b. *Site visit is encouraged and welcome.

2. PROJECT BACKGROUND:

2.1. The Kayenta Health Center’s (KHC) Pharmacy department is currently experiencing temperatures that are approaching out of compliance status with The Joint Commission (TJC) requirements. It is crucial for this pharmacy room and CT scanner room to be kept at the required temperature range due to very temperature sensitive pharmaceutical items and medical equipment being stored in these rooms. The existing AC SPLIT system does not have the capacity and operation to keep the designated pharmacy room and CT scanner room at the specified temperature range continuously at all times. With this issue in mind the area calls for a supplementary A/C supply system. This project requires services from a professional and responsible contractor.

3. PROJECT DESCRIPTION:

3.1. The Contractor shall prepare a proposal and upon award, shall perform the demo/install/construction work to completion (turn-key project).

3.2. Quote to include manufacture and model of proposed AC split system.

3.3. The project includes the following work:

a. Engineer, Furnish and Install split AC system.

b. Install new split AC system in safe and efficient manner and location.

c. New split AC system shall meet or exceed KHC needs for Pharmacy department and CT scanner room’s needs.

d. See floor drawings: Pharmacy Figure 2, room 1E-020 and CT Scan Figure 1, rooms 1D-044 & 1D-045

e. Install outside condenser, run control wire, run copper tubing, and nearby electric disconnect/J-box.

Coordinate with KSU for best route.

f. Contractor to provide tools, materials, labor, equipment and project needs for the complete AC SPLIT installation and operation is the contractor’s responsibility.

g. KHC Electrician to install electrical cable from electrical breaker panel to either J-box heat exchanger and disconnect box for condenser.

h. Contractor responsible to install electric cable from J-box to heat exchanger and disconnect box to outside condenser unit. Length from boxes to equipment approximately six feet.

i. Provide a high quality factory wired thermostat that is to control temperature and shut-down.

j. Contractor is to install AC split system control system per manufacture specifications.

k. Installed plumbing needs to operate properly and drain to sewer system. Coordinate with KSU for location.

l. KSU to install concrete pad for condenser.

m. Anchoring points are expected and need to follow building codes and need facility approval.

n. Furnish complete AC split system shall meet UL safety specifications.

o. Manufacturer: YORK, TRANE, CARRIER, or Approved Equal

p. AC split system shall be sized appropriately by the contractor.

q. AC split system must be connected to fire alarm system to shut down when fire alarm is activated.

Coordinate with KHC Engineer for termination for fire shut-off control.

r. Elevation: 5,700 ft.

s. Provide onsite training session.

t. Contractor to dispose of rubbish at approve EPA disposal location.

4. CRITERIA:

4.1. Codes and Standards: the contractor shall perform the demolition and installation in accordance with the following codes and standards;

a. NFPA 70 (2012) National electric code

b. EPA, Navajo Nation EPA

c. ASHRAE

d. Performance shall be rated per ARI Std 550/590 using HFC 134a refrigerant.

e. NFPA 101 Life safety code, (2012) Section 39.5.1 and 39.5.2 (Utility & AC SPLIT Codes)

f. NFPA 99 Healthcare Facilities Code (2012)

g. Adhere to Pre-Construction Risk Assessment (PCRA), Construction Permit criteria and all other guidelines administered by KSU.

5. GENERAL REQUIREMENTS:

5.1. Contractor shall develop as-built drawing.

5.2. Submittals: contractor shall provide COR and project manager with a project proposal manual. This project manual shall provide contract terms, project description, equipment selection, and a detailed cost breakdown of the proposal.

5.3. Operating and Maintenance Manual: At completion of project, the contractor shall provide the Facility with (2) two copies of the O&M Manual for the installed equipment. The project shall be considered complete once the documents and operational and maintenance training have been completed.

5.4. The contractor shall perform work between the hours of 8:00am and 5:00pm Monday through Friday if work will not interrupt normal business. If work will interrupt normal business/patient care activities then working after hours is expected. Work shall not occur on Federal Holidays. All working hours will have to be discussed and agreed upon with facility. The contractor shall furnish a schedule of the dates agreed upon with the project officer and the facility manager at least two weeks in advance.

5.5. Weekend work from 8:00 am to 5:00 pm is acceptable.

5.6. Contractor will be responsible for AC slit system software.

5.7. Contractor will provide telephone support for all technical and operational issues that arise.

5.8. Contractor to provide full documentation of equipment installed.

5.9. All work needs to be professional, function properly, display good craftsmanship/be presentable, and meet KHC needs.

6. SPECIAL REQUIREMENTS:

6.1. A proper functional test of the AC split system installed (walk through).

a. The purpose of the AC split system test is to ensure that individual components are installed and integrated to operate each unit at stand-alone.

b. Verify all sensors, safeties, interlocks, alarms, staging and reset controls are verified.

c. All fire and smoke barriers, if modified or penetrated shall be properly sealed and certified with supporting documentation.

d. AC split systems refrigerant lines that penetrate fire/smoke barriers must meet certification and approved by KSU.

6.2. All interior work will be conducted in a manner to prevent contamination to work and adjacent areas, precautionary and best practices shall be used working in a health facility environment.

6.3. Work areas shall be kept neat, practicing good housekeeping every day.

6.4. Any change in cost to project shall be brought to the attention of the COR and project manager before proceeding. All costs to complete project from start to finish is to be included in original project bid.

7.1. The period of performance: June 1, 2019 - October 31, 2019.
