PERFORMANCE WORK STATEMENT (PWS)

1SFG (A) Tactical Human Optimization Rapid Rehabilitation Reconditioning (THOR3) Facility Equipment Life Cycle

General Information

1. GENERAL: This is a non-personal services contract to provide fitness facility related equipment and rubber floor coverings, transportation of equipment and rubber floor coverings, removal of existing rubber floor coverings, installation of new rubber floor coverings, disassembly, moving and reassembly and installation of fitness equipment under which the personnel rendering the services are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the government. The Contractor shall ensure that all contractor employees and subcontractor employees are medically and physically fit for performance of their duties.

1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform current rubber and wood floor covering removal and moving, new rubber and wood floor covering professional installation, current weight room equipment disassembly, moving and reassembly, and new weight room equipment set-up and installation defined in this PWS except for those items specified as government furnished property (GFP) and government furnished services (GFS). The contractor shall perform to the standards in this contract.

1.2 Background: Due to the nature of Special Forces specialized physical fitness and operational readiness requirements, and the need to increase physical fitness and injury healing through physical therapy of Soldier’s assigned to 1st SFG (A), we request to establish this contract to update physical fitness equipment and rubber floor coverings to better serve our Soldiers.

1.3 Objectives: The basic service objectives are to disassemble eighteen (18) Sorinex Base Camp racking systems currently within the THOR3 facility, move and reassemble six (6) in the same facility, and move the remaining twelve (12) racking systems into a storage container approximately 100’ from the facility.

Additional service objectives are to deliver all physical fitness equipment, rubber floor coverings, and supplies, professionally remove current 2x2x1 ECORE Ultra Tiles, install new Ultra Tiles and 4x4x1 wood inserts (to create in-laid platforms), and install new fitness equipment to meet the needs of the Soldiers at 1SFG (A).

1.4 Scope of work:

i. Contractor shall comply with applicable Federal, State, and local laws, regulations, codes, technical manuals, manufacturer’s instructions and recommendations, and acceptable commercial practices in the delivery of required services.

ii. Contractor shall furnish all tools necessary to complete the work stated in this PWS. This includes, but is not limited to, the removal and installation of rubber floor coverings, assembly and installation of new fitness equipment, and disassembly, moving and reassembly of fitness equipment currently in the facility.

iii. The contractor shall provide the delivery of all fitness equipment, rubber floor coverings and supplies during the overall period of performance for this contract. All fitness equipment, rubber floor coverings, and supplies will be delivered at one time, in order to reduce the overall impact to Soldiers during the period of performance when the facility will be operating at a limited capacity to allow for work.

iv. The contractor shall disassemble six (6) 8’ Sorinex racking systems

(including racks, platforms, plate storage and rack connectors), move into an outdoor 20’ container approximately 100’ from the facility while not disrupting Soldier operations and/or other contractor work.

v. The contractor shall disassemble twelve (12) 9’ Sorinex racking systems (including racks, platforms, plate storage and rack connectors), move six (6) within the current facility (approximately 200’) while not disrupting Soldier operations and/or other contractor work, and reassemble the same six (6) 9’ racking systems within the same facility. The additional disassembled six (6) 9’ racking systems will be moved into an outdoor 20’ container approximately 100’ from the facility while not disrupting Soldier operations and/or other contractor work.

vi. The contractor shall remove approximately 144 pieces of existing

ECORE 2x2x1 Ultra Tile and move into a 20’ container approximately 100’ from the facility while not disrupting Soldier operations and/or other contractor work.

vii. The contractor will professionally install approximately 72 new ECORE

2x2x1 Ultra Tiles and 12 new 4x4x1 wood center platform inserts. The contractor shall install all new rubber floor coverings and wood inserts within the THOR3 facility.

viii. The contractor shall provide necessary equipment and transportation to haul and dispose of flooring determined to be in disrepair.

ix. The contractor shall set-up, assemble and install all new fitness equipment in the THOR3 facility.

x. The contractor shall provide the necessary number of personnel required for transportation, delivery, removal, disassembly, moving, assembly, and installation of rubber floor coverings and fitness equipment.

1.5 Period of Performance: The period of Performance shall be from 13 June 2019 – 15 June 2019 and is estimated to take no more than three (3) business days to complete.

i. General Information

1. Recognized Holidays: The contractor is not required to perform services on the below recognized Federal holidays.

ii. Hours of Operation: The contractor is responsible for conducting business between the hours of 0600-1500 Monday through Saturday, except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons.

The Contractor must, at all times, maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons.

iii. Place of Performance: The work to be performed under this contract will be performed at the 1st SFG (A) THOR3 Facility, located at Building 9655 Madigan By Pass, JBLM, WA 98433.

iv. Emergency Services: The contractor shall have competent personnel, trained and physically capable of performing the tasks outlined within the Statement of Work. The contractor or their employees shall immediately notify their supervisor of any accident requiring emergency medical treatment. The contractor will, in turn, notify the KO or COR within thirty (30) minutes of the incident.

v. Contractor furnished items and services:

1. The contractor furnish all supplies, flooring, equipment, tools, transportation, and services required to perform work under this contract that are not listed under this PWS.

2. The contractor shall furnish the personnel to perform work within the timeline to meet the requirements under this PWS.

vi. Government furnished items and services:

1. The Government will provide clear parking lot space for new flooring and equipment to be unloaded.

2. The Government will provide electricity for power tools for equipment installation. The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the faucets, turning off lights, and powering off equipment at the end of the work day.

1.6 Access and general protection policy and procedures: All contractor employees, including subcontractor employees, requiring access to an Army or USSOCOM controlled installation, facility, or area shall comply with applicable security policies and procedures (provided by the government representative).

This includes policies pertaining to the use or prohibition of electronic recorders, devices, cameras, etc. If the Contractor is required to take photographs or videos on a Government Installation, the Contractor must obtain written permission from the Senior Commander. The contractor shall also provide all information required for background checks to meet installation and facility access requirements to be completed by the installation Provost Marshal Office, Director of Emergency Services, or Security Office. The contractor workforce must comply with all personal identity verification requirements as directed by DoD, HQDA, USSOCOM, USASOC and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should Force Protection Condition (FPCON) at any individual facility or installation change, the government may require changes in contractor security matters or processes.

1.7. For contractors that do not require CAC, but require a federal ID access to a DoD facility or installation: Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations. When issued by local authority the Contractor shall report departed employees and the dates their local identification cards were returned to the COR or the Installation issuing facility as of the last day of the month on a monthly basis. The report will include the names and circumstances of those departed employees whose local identification card was not retrieved. Negative reports are required.

1.8 Will be escorted in areas where they may be exposed to classified and/or sensitive materials: All contractor employees, including subcontractor employees who are not in possession of the appropriate security clearance, will be escorted in areas where they may be exposed to classified and/or sensitive materials. The Vendor will coordinate with the COR and/or the facility security office for access when required. (Use when security clearances are not required, i.e. facility repair or construction)

1.9 Pre-screen applicants using E-Verify Program: The Vendor must pre-screen applicants using the E-verify Program (http://www.dhs.gov/E-Verify) website to meet the established employment eligibility requirements. The vendor must ensure that the applicant has two valid forms of government issued identification.
