I. Agency and Contracting Activity.

II. Nature and/or description of the action being approved.
Due to the circumstances described herein, the activity requests authority to acquire Ambulance Services for
Contract FA8501-18-C-0001listed in this document using Other Than Full and Open Competition as prescribed by FAR 6.302-2, Unusual and Compelling Urgency. This action will be achieved by awarding a contract modification, resulting in the addition of a one (1) month extension and four (4) options IA W FAR
52.217-9. Each option will consist of a 1-month Period of Performance. This action will utilize a Firm Fixed
Price contract type. The period of performance is 1 April 2023 to 1 September 2023.
Ill. Description of supplies/services required to meet agency needs.
The services covered under under this effort are for all labor, management, supervision, vehicles, supplies, equipment, transportation, and reports - except as provided in section 3, government furnished property and services for 24-hour a day Paramedic emergency ambulance services for patient transport from all of Robins AFB (RAFB), GA. Contractor shall provide two(2) fully equipped ambulances with personnel on base. Their responsibilities include: (1) emergency Paramedic response and patient transport, (2) in-flight emergency response, (3) participation in RAFB and
Military Treatment Facility exercises, and (4) provide third party collections forms (DD Form
2569). Contractor care shall be commensurate to Emergency Medical Technician (EMT), Paramedic and ambulance services provided in the local community. Performance shall be according to the requirements contained in this performance work statement (PWS), and professional standards of The Joint Commission, AFI 44-119 Medical Quality Operations, the
Department of Transportation and the National Registry for Emergency Medical Technicians
(NREMT). The contractor shall participate in flight-line hazard and mishap response planning.
These services will be paid for by the government, the contractor shall not bill patients.
Emergency reporting (Fire Department) telephone numbers are 911 for emergency and (478)
222-2900/DSN: 472-2900 for non-emergency calls.

IV. Demonstration that the contractor's unique qualifications or the nature of the acquisition requires use of the authority cited above.
United Medevac Solutions Inc. provides immediate medical care and patient transport to over 24,000 contractors, civilians, military members and their dependents at Robins AFB. United Medevac Solutions Inc. has responded to an average of twenty (20) emergencies per month over the life of the current contract.
Past performance indicates that United Medevac Solutions Inc. has consistently met the PWS response time thresholds: no more than seven (7) minutes for primary emergency calls and no more than twelve (12) minutes for secondary calls. Robins AFB personnel and residents would not have on base first responder life saving emergency support.
United Medevac Solutions Inc. has been performing the work for 5 ½ years and has personnel already in place to support while the Government finishes the source selection process for the 3 year follow on contract.
United Medevac Solutions Inc. is the only firm capable of immediately providing the requirements described in Section III above without 78 MDG experiencing unacceptable delays in fulfilling its requirements.
The following data represents approximated calls by Calendar Year:
Approximately 300 calls consisting of 50% medicals with transport 30% non-transport/refusal 20% standby/in flight emergencies
Approximately 300 calls consisting of 45% medicals with transports 25% non-transport/refusal 30% standby/in flight emergencies
Approximately 350 calls consisting of 45% medical with transports 15% non-transport/refusal 45% standby/in flight emergencies
Approximately 700 calls consisting of 30% medicals with transports 15% non-transport/refusal 55% standby/in flight emergencies

Approximately 600 calls consisting of 40% medicals with transports 40% non-transport/refusal 20% standby/ in flight emergencies
V. Description of efforts made to ensure that offers are solicited from as many potential sources as practicable, including whether a notice was or will be publicized as required by subpart 5.2 and, if not, which exception under FAR 5.202 applies.
In accordance with FAR 5.202(a)(2), the contracting officer did not submit the notice required by FAR 5.201 because the contracting officer determined that the proposed contract action is made under the conditions described in FAR 6.302-2 and the Ambulance Services requirement for Robins AFB would be seriously injured if the agency complies with the time periods specified in FAR 5.203.
VI. Determination by the Contracting Officer that the anticipated cost to the Government will be fair and reasonable.
The cost of the current Ambulance Services Option V was found to be fair and reasonable when exercised on 1 October 2022. The pricing noted above in section III was derived by dividing the option V pricing by 6 months.
VII. Description of the market research conducted and the results, or a statement of the reasons market research was not conducted.
Due to the commercial nature of the services it was determined further market research was not necessary.
The Market Research conducted from the initial award was relied upon and is considered sufficient for the nature of this action.
Internet search were conducted to identify potential vendors. A search was conducted reviewing SBA NAICS Code (621910) seeking any company that could provide this service. A reviewing of Small Businesses, SBA Certified 8(a) Program, SDB Certified, HUB Zone, Veteran Owned, Women Owned, and Service-Disabled Veteran Owned under NAICS 621910. The search returned 787 total vendors. The vendors identified reasonably met the search criteria for particular needs. Please see attached vendor listing.

X. A statement of the actions, if any, the agency may take to remove or overcome any barriers to competition before any subsequent acquisitions for the supplies or services required.
No actions are planned to remove or overcome any barriers to competition for future acquisitions as this is a one time buy to cover a potential lapse in coverage of a critical support contract. There was not a prior J&A, therefore higher level approval IA W DF ARS PGI 206.304(a)(S-70)(ii) does not apply.
