Translation and Interpreting Services (FEMA, DR-4277-LA)

REQUEST FOR QUOTE

A. DESCRIPTION OF REQUEST

Sign language interpretation services and communication access real-time translation (CART) services provided by a contractor knowledgeable in disability law and the accommodation therein (See Attachment 1 – Statement of Work). This requirement is in support of FEMA disaster, DR-4277-Louisiana.

B. PRICING SCHEDULE

- 0001 Interpreting Services – This CLIN includes all sign language interpreting hours in support of DR

4277 when given a minimum of 24 hours prior notice to the start of the assignment.

2. Response Date and Proposal Submission:

Questions: Questions regarding this solicitation shall be submitted to the contracting officer and contract specialist via email only, no later than 3pm central time, on Tuesday, May 16, 2017.

Questions received after this time and date may not be considered.

Proposals: Proposals are due no later than 2:00 pm central time on Monday May 22, 2017.

Questions and quotes shall be emailed to the contracting officer, Sharon Edwards, at Sharon.edwards3@fema.dhs.gov and the contract specialist, Kimberly Hodge, at kimberly.hodge@fema.dhs.gov.

Late proposals may not be accepted.

3. Submission Requirements:

Response to this solicitation is restricted to the participation of small businesses residing or primarily doing business in the disaster affected area. The disaster affected parishes are: East Baton Rouge, Livingston, St. Helena, Tangipahoa, Arcadia, Ascension, East Feliciana, Iberia, Lafayette, Point Coupee, St. Landry, Vermilion, Avoyelles, Evangeline, Iberville, Jefferson Davis, St Martin, St Tammany, Washington, West Feliciana, St James and West Baton Rouge Parish.

4. Proposal Cover Letter - Content:

All proposal submissions must include the following information:

a. Company’s Complete Name and Address

b. Tax Identification Number (TIN)

c. Dun & Bradstreet Number (DUNS)

d. Point of Contact Name

e. Contact email address

f. Contact telephone

g. Complete business mailing address

E. PROOF OF LOCAL AREA REPRESENTATION: THIS INFORMATION MUST BE

SUBMITTED WITH YOUR PROPOSAL):

This requirement is being set-aside for vendors located in the disaster affected area. The following parishes are located in the disaster affected area: Tangipahoa, Avoyelles, Jefferson Davis, St. Helena, East Baton Rouge, West Baton Rouge, Ascension, St. Martin, Iberia, East Feliciana, West Feliciana, St. Landry, Iberville, Vermillion, Livingston, Lafayette, Pointe Coupee, St. Tammany, Washington, Acadia, Evangeline, and St. James.

1. FAR Clause 52.226-3-(d), states:

(c) An offeror is considered to be residing or primarily doing business in the set-aside area if, during the last twelve months--

(1) The offeror had its main operating office in the area; and

(2) That office generated at least half of the offeror's gross revenues and employed at least half of the offeror's permanent employees.

The information listed in section (c) 1 and 2 must be submitted with your bid. The following information must be submitted.

2. Proof that the vendor’s main operating office is located within the disaster affected area. The Contracting Officer will look up the offeror SAM.gov registration, to determine the address listed in the SAM.gov. Please ensure that your registration is up to date.

3. That office generated at least half of the offeror’s gross revenue: A statement listing the total revenue generated within the last 12 months. How much of this revenue was generated in the disaster affected area.

4. The office employed at least half of the offeror’s permanent employees: A statement listing the names of the permanent employees employed by your main operating office. A statement confirming that at least half of the listed employees represent half of the offeror’s permanent employees.

Section (d) and (e) of that same clause states that:

(d) If the offeror does not meet the criteria in paragraph (c) of this provision, factors to be considered in determining whether an offeror resides or primarily does business in the set-aside area include:

(1) Physical location(s) of the offeror's permanent office(s) and date any office in the set-aside area(s) was established;

(2) Current state licenses;

(3) Record of past work in the set-aside area(s) (e.g., how much and for how long);

(4) Contractual history the offeror has had with subcontractors and/or suppliers in the set-aside area;

(5) Percentage of the offeror's gross revenues attributable to work performed in the set-aside area;

(6) Number of permanent employees the offeror employs in the set- aside area;

(7) Membership in local and state organizations in the set-aside area; and

(8) Other evidence that establishes the offeror resides or primarily does business in the set-aside area. For example, sole proprietorships may submit utility bills and bank statements.

(e) If the offeror represents it resides or primarily does business in the set-aside area, the offeror shall furnish documentation to support its representation if requested by the Contracting Officer. The solicitation may require the offeror to submit with its offer documentation to support the representation.

If the information in section cannot be provided, the offeror shall provide as much information in section (d) (1 through 8) as possible for the Contracting Officer to make a determination that the offeror primarily does business in the disaster affected area.

This RFQ requires registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Registration must be "ACTIVE" when your bid is submitted.

F. EVALUATION FACTORS, RATINGS AND BASIS OF AWARD:

1. Award will be made to the Lowest Priced Technically Acceptable (LPTA) quote that meets or exceeds the acceptability standards for non-price factors (e.g. Technical). Technical factors must be rated as acceptable in order to evaluate the price factor. For example, if all of the technical factors are rated as acceptable, price will then be evaluated. However, if one or more of the technical factors are rated as unacceptable; the evaluation process stops and award cannot be made to that offeror. If all offerors are rated technically acceptable, award will be made to that offeror with the lowest priced technically acceptable quote. This will end the evaluation process. The Government intends to make award without discussions, however, clarifications may be necessary

3. Evaluation Factors and Submittal Requirements:

a. The Government will evaluate price to determine whether the offered price is fair and reasonable, realistic for work to be performed, and reflects a clear understanding of the solicitation requirements. Price will be the factor in making the final determination for award.

b. Analysis of price will be performed in accordance with FAR 15.404-1 to determine completeness, reasonableness, and understanding of requirement

c. Offerors must submit complete Contract Line Item Numbers (CLIN) with unit price, quantity, and total pricing on the Pricing Schedule above.

4. Technical:

a. Qualified Personnel - Offeror must submit in writing their ability to meet or exceed the required number of qualified personnel – 12 interpreters and 6 CART writers (see SOW for specific qualifications required).

b. 24/7 Response - Offeror must submit in writing their ability to respond 24 hours a day 7 days a week (24/7) when given a 24 hour notice.

c. Respond to anywhere in the declared area - Offeror must submit in writing their ability to respond anywhere within the declared area (see SOW for a list of declared parishes).

d. Past Performance - Name at three projects completed in the last three years that are similar in size, scope, and complexity in which you provided Sign language and CART services.

List three references to include contact name and phone number.

5. Ratings:

a. Price: will be evaluated for reasonableness and completeness

b. Technical: Technical Capability will be evaluated using the technical evaluation rating criteria set forth below to determine the most qualified source from a technical standpoint.

An objective technical evaluation will be performed on each technical proposal based solely on the information furnished and not on previous knowledge or associations. The technical evaluation will result in a score of Acceptable or Unacceptable:

• Acceptable: expected to meets or exceed the requirement.

• Unacceptable: is not expected to meet the requirements

c. Past Performance will be evaluated to determine if the Past Performance listed is similar to the requirement being requested in this RFQ. The following rating will be used for Past Performance:

• Pass: Based on the offeror’s past performance record, essentially no doubt exists that the Offeror will successfully perform the required effort.

• Neutral: The offeror has no relevant past performance

• Fail: Based on the offeror’s past performance record, it is not likely that the Offeror will successfully perform the required effort.

In addition to contacting references, the Government reserves the right to check outside sources and systems such as the Contractor Performance Assessment and Reporting System (CPARS) and the Past Performance Information Retrieval System (PPIRS).

G. INVOICING INSTRUCTIONS:

An invoice is a written request for payment under this contract for supplies delivered or services rendered. Payment of invoices submitted under this contract shall be handled in Office of Management and Budget (OMB) prompt payment regulations at 5 CFR Part 1315, and in accordance with the provisions of other clauses in this contract. Failure or refusal to provide the following information on all invoices submitted under this contract may result in the invoice being considered improper for payment in accordance with the Prompt Payment Act. In order to be proper, an invoice must include, as applicable, the following:

1. Name of Contractor

2. Invoice Date

3. Contract Number (including order number, is any), contract line item number, contract description of supplies or services, quantity, contract unit of measure and unit price and extended total.

4. Shipment number and date of shipment (bill-of-landing number and weight of shipment will be shown for shipments on Government bills of lading).

5. Name, title, phone number and complete mailing address of responsible Official who can be contacted in the event of an improper invoice, if there are questions or additional information is needed by this agency to process payment.

6. Any other information or documentation required by other provisions of the Contract (such as evidence of shipment).

7. Invoices shall be prepared and submitted as follows:

The most expeditious method submitting invoices is via email to the FEMA Finance Center

(FFC):

A copy of the invoice shall be sent electronically to FEMA-Finance-Vendor- Payments@fema.dhs.gov, ashley.haven@fema.dhs.gov, and sharon.edwards3@fema.dhs.gov in read-only format, and the supporting documentation should be supplied as an attachment.

H. ELECTRONIC FUNDS TRANSFER (EFT) INFORMATION:

The information contained in the System for Award Management (SAM) database will be used for payment purposes.

If at any time during the term of this contract, the vendor’s EFT information changes, (i.e. financial agent, RTN, account number, etc.) the vendor must edit their SAM information to reflect these changes.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e)(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause--
