SCOPE OF WORK
U. S. COAST GUARD
SECTION 1 – GENERAL INFORMATION
1.1 Scope of Work
The contractor shall provide all personnel, transportation, equipment, tools, supervision, and other items and services necessary to complete the required work. Kaehler Memorial Clinic will provide the parts necessary to do the job. The work of this service contract consists of completing quarterly preventative maintenance to 05 dental treatment operatories, including the patient chairs, sterilizer station, air compressor and suction pump. Yearly calibration will be needed on the dental X-Ray unit.

1.2 Contractor Personnel
1.2.1 Contract Manager
The contractor must provide a contract manager who must be responsible for the performance of the work.
The name of this person, and an alternate or alternates, who must act for the contractor when the manager is absent, must be designated in writing to the contracting officer.

1.2.1.1 The contract manager or alternate must have full authority to act for the contractor on all contract matters relating to daily operation of this contract
1.2.1.2 The contact manager and alternate or alternates must be able to read, write, speak, and understand the English language.
1.2.2 Contractor employees
The contractor must not employ persons for work on this contract if such employee is considered by the contracting officer to be a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
1.2.2.1 Contactor personnel must present a neat appearance and be easily recognized as contractor employees.
1.2.2.2 The contractor must not employ any person who is an employee of the U. S. Government if employing that person would create a conflict of interest. Additionally, the contractor must not employ any person who is an employee of the U. S. Coast Guard unless such person seeks and receives approval according to U. S. Coast Guard regulations.
1.2.3 Security requirements
Due to heightened Security at USCG Facilities, all contractor employees performing tasks related to this
SOW, while onboard a Coast Guard facility, shall have proper valid identification. After completing Joint
Base Cape Cod Security background check. Time spent undergoing entrance to USCG facilities will not be considered a government delay of work or differing site condition.
1.3 Hours of Operation
1.3.1 The contractor must perform the tasks as required by the SOW under this contract during the following hours: Monday through Friday between 8:00am and 4:00pm unless other time is specified.
1.3.2 Holidays: N/A
1.3.3 Special event, or adjusted work hours: If circumstances should arise wherein the contractor desires a different work schedule as required in section 1.3.1 above, the contractor must request same in writing.
Submit written request to the government Contracting Officers Representative (COR) a minimum of 24 hours before the requested adjusted work schedule. If the desired schedule change is to be effective on a
Monday, the request must be submitted to the COR prior to 9:00am on the Friday prior to commencement of work.
1.4 Award
It’s the governments’ intent to make an award to the Lowest Price Technically Acceptable (LPTA) offered through issuance of a firm fixed price contract.
1.4.1. Under FAR 4.1103 prospective contractor shall be registered in System Award Management (SAM) prior to award. Failure for contractor to be registered in (SAM) shall result in Contracting Officer to proceed with next successful registered offeror . Website: www.sam.gov

Invoices shall be sent to address as per the contract. To insure proper payment, all invoices shall have requisition number/order number, company name and address, DUNS number, invoice number and date clearly indicated on your invoice. It is recommended that the offeror attach a copy of the Purchase Order /
Contract to their invoice(s).
SECTION 2 – CONTRACTOR FURNISHED ITEMS AND SERVICES
Except for those items, services, or commodities specifically stated in section 3 as government furnished, the contractor must furnish everything needed to perform this contract according to all its terms.
SECTION 3 – SPECIFIC TASKS:
Detailed specification requirements that is performed by contractor as follows:
- Timeline being requested for performance: This is for a Base year plus 4 option years o Between the months of OCTOBER - SEPTEMBER each year if exercised by the government o Quarterly Maintenance of Dental Equipment in 05 operatories, autoclave, and amalgam separator o Annual Calibration of intraoral and panoramic x-ray units o Annual Inspection of functionality of Schick Sensors
- Work that is being requested:
o Contractor shall do the following quarterly in the 05 dental treatment rooms
Dental Treatment Units check the air and water regulators check the air and water syringe for leaks check the unit mounts proper movement and wear check the cuspidor bowl rinse time (8-12 Sec run time) check the cup fill (Preference) check of the HVE and saliva ejector valves (Staff lubricate and cleans as reg) check headpiece tubing for cracks, tears, etc check headpiece couplers (o-rings, gaskets, etc)
Test the foot control for air leaks
Replace the air and water filters
Inspect, adjust, and monitor the condition of vac assist units
Adjust and lubricate all-unit arm pivot pins and joints (may require two techs)
Patient Chair
Check the operation of all chair motors and hydraulic pumps
Check for hydraulic leaks
Tighten the arms and headrests
Check and adjust the chair rotation brake
Check the function of all switches and foot controls
Adjust the pre-position or auto-exit (if requested)
Check condition of chair switches, wiring, tubing, mechanical joints and components
Dental Light
Check the light beam, check lens cover, ensure lever arm is tight
Adjust the arc and drift
Tighten the handles and head pivot points
Check wiring at joints for wear
Other Operatory Equipment
Check cabinetry
Check drawers, doors, and hinges
Check x-ray arms and condition
Equipment
Check operating temperature
Check for proper operation and condition
Calibrate time cycle
Check serviceability of door gaskets and door lock, run test cycle
Check pressure release valve
Air Compressor
Check operating pressure switch
Check oil level-change if necessary
Check function of purge/unloader valve
Check pressure relief valve
Check condition of desiccant drying system
Tighten any leaking connections
Drain tank of water/oil buildup
Change air filters
Check required pump up times record times
Suction Pump
Check vacuum flow gauge record reading
Check filter screen assembly
Check vacuum breaker valve and general condition of unit
Check water flow to vacuum pumps
Amalgam
Check proper function o Contractor shall Perform the following Annually
Calibration of intraoral and panoramic x-ray units
Inspection of functionality of Schick Sensors
Inspection of 03 autoclave and change filters

o Vendor will provide quarterly maintenance of dental equipment October -September each year if exercised by the government
- Miscellaneous:
o Once the contract is awarded the first maintenance will be scheduled with the COR to ensure the proper placement of all bins.
