QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
1. Contract or Task Order Title; UPS and DC Power Plant Services Contract JITC Fort Huachuca, AZ
2. Work Requirements:
· Task 1 - Predictive/Preventive Maintenance
· Task 2 - Equipment Repair
· Task 3 - Work Documentation
3. Primary Method of Surveillance:
100 Percent Inspection. This is usually only the most appropriate method for infrequent tasks or tasks with stringent performance requirements, e.g., where safety or health is a concern. With this method, performance is inspected/evaluated at each occurrence. One hundred percent inspection is too expensive to be used in most cases.
Customer Input. Although usually not a primary method, this is a valuable supplement to more systematic methods. For example, in a case where random sampling indicates unsatisfactory service, customer complaints can be used as substantiating evidence. In certain situations where customers can be relied upon to complain consistently when the quality of performance is poor, e.g., dining facilities, building services, customer surveys and customer complaints may be a primary surveillance method, and customer satisfaction an appropriate performance standard. In all cases, complaints should be documented, preferably on a standard form.
4. Scope of Performance:
Performance Requirements: Perform scheduled services (preventative and routine) and unscheduled maintenance (repairs) as required on the UPS units and DC Power Plants identified in Table 1.
6.1.1 Subtask 1- Scheduled maintenance is performed every six (6) months. The contractor will perform the services listed below during normal duty hours (paragraph 9) during the months of September and March. Scheduled maintenance periods will be no less then 170 days and no more then 190 days apart.
Each scheduled service includes but is not limited to the following:
a. Check of all system components, to include batteries, to ensure they are operating to specification.
b. Clean, adjust, or replace components as required to ensure peak performance.
c. Recommend system changes/enhancements that optimize system performance to the project manager and technical coordinator in a written report.
d. The contractor shall provide a current updated detailed diagnostic/status report in softcopy (electronic) on all units within 3 working days following each scheduled service.
6.1.2 Subtask 2 - Unscheduled maintenance – The contractor will provide cost of service delivery in response to system malfunction. This requirement is 24 hours per day 7 days a week (24/7). Response to unscheduled maintenance shall be provided within a 4-hour time frame, unless otherwise agreed upon with the JITC Service Desk on a case-by-case basis. The cost shall be expressed as an hourly rate for services. See paragraph 9 for normal and non-duty hours.
Each un-scheduled service includes but is not limited to the following a.
Quick response time to system outages is critical as many of these units support sensitive facilities. The contractor must respond within four hours to any reported system outage. Contractor must keep the project manager informed of the status of all systems which are under repair. Although the terms are established on this contract, time and materials for unscheduled maintenance will be invoiced and paid as they occur via separate funding.
b.
The contractor shall provide a detailed outage/diagnostic/status report in softcopy (electronic) for the repaired unit upon completion of the repair action/s.
c.
Only contractors licensed in power systems by the Arizona Registrar of Contractors (ROC) may perform services.
5. Performance Standards:
The industry standards for UPS and DC Power Plant services and repairs shall be used to assess performance; and the National Electrical Code/National Fire Protection Association Part 70 for all matters concerning the electrical connectivity will be utilized to determine if standards have been meet or complied with.
· Response times for maintenance calls should occur within 4 hours of the contractor being contacted.
6. Acceptable Quality Level (AQL): Complete 100% of scheduled maintenance on time. Respond to un-scheduled maintenance within 4 hours 95% of the time.
7. Evaluation Method:
The JITC Service Desk will document the time of verbal notification to the contractor. The JITC Service Desk will document the official time and date of notification on the Maintenance Call Record. The COR/TM will verify and validate contractor performance. The contractor shall enter in the record the official time the system is restored to full operational status. The JITC Service Desk will confirm the date and time of problem resolution in the record.
8. Incentives (Positive and/or Negative):
Failure to meet normal industry standards may result in delayed payment, a reduction in the contract and the requirement to redo the work to meet established normal professional industry standards. Repeated problem could result in cancellation of the contract. Meeting or exceeding normal professional industry standards will be the primary consideration in determining if the options are exercised.
