NEUROLIGN USA, LLC. FULL COMPREHENSIVE MAINTENANCE SERVICE CONTRACT

21 July 2023

Audiology Department, NMRTC Camp Pendleton, Camp Pendleton, CA

1.0 GENERAL INFORMATION

1.1 This is a non-personal services contract to provide a full comprehensive maintenance and repair contract to for:

· Neuro Kinetics Serial Number: 801065

· Neuro Kinetics Serial Number: 890814

1.1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform the necessary maintenances, repairs or replacements of any parts as defined in this Statement of Work (SOW).

1.1.2 Background: NEUROLIGN USA, LLC. has a service contract, N68094-19-P-6063, through 30 September 2023. Thus, in need of a new service contract. Previous contract covered two systems.

1.1.3 Scope: The contractor, shall provide 1 PM services annually, repairs as necessary when the instrument does not function at its optimum, technical support when needed. Services include telephonic support (0800 – 1700 local time), annual PM, onsite repairs (as required), replacements of parts, case support, and clinical training The contractor shall respond by telephone to service calls within 1 hour of notification and provide on-site support within 24 hours of notification.

1.1.4 Period of Performance (PoP):

Base Period POP: 01 OCT 2023 – 30 SEP 2024 Option Period 1 POP: 01 OCT 2024 – 30 SEP 2025 Option Period 2 POP: 01 OCT 2025 – 30 SEP 2026 Option Period 3 POP: 01 OCT 2026 – 30 SEP 2027

1.2 Administrative Specifications

1.2.1 Place of Performance: The work shall be performed at the Audiology Department, NMRTC Camp Pendleton, Camp Pendleton, CA 92055.

1.2.2 Recognized Federal holidays: The contractor is not required to perform services on any (observed) Federal Holiday.

1.2.3 Hours of operation: The contractor is responsible for conducting business Monday through Friday between 0730 and 1600 except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The contractor shall at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this SOW when the Government facility is not closed for the above reasons.

1.3 Contractor Identification

1.3.1 Contractor personnel performing services in a contractor capacity in a Government facility are required to possess and wear an identification badge that displays his or her name and the name of their company. Ensure all those with whom the person interacts in any face-to-face dealings while supporting the Government understands that the person is contractor support personnel.

2.0 SPECIFIC TASKS

2.1 Preventive Maintenance: PMs shall be performed one time annually. This service shall include but is not limited to travel cost, non-consumable parts and labor. Availability of service technician/engineer shall be priority as to not miss due dates. PMs shall be performed within a twelve month period from the previous PM. Service technician/engineer shall contact the NMRTC Camp Pendleton Biomedical Repair Division via its customer service email to schedule service or arrange site visits at usn.pendleton.navhospcampenca.list.biomed-techs@health.mil. A report shall be provided once PM services are completed with detailed information or work performed. This can be sent via email to the department via email address stated above. Report shall be made available within a week of service.

· Perform service repair/PM to industry standard.

· Ensure that only FULLY QUALIFIED FIELD ENGINEERS and TECHNICIANS who have gone through original equipment manufacturer (OEM) shall be employed in the performance of any and all work performed under this contract. Upon request, contractor shall provide training certificates to the medical repair branch for verification.

· Make repairs to the extent necessary to ensure a functional system that will efficiently nerve its intended purpose and in compliance with federal, state and local regulations.

· Installation of system update changes to resolve specific product reliability problems. Hardware and software upgrades, defined as those changes that enhance or add product features, are not included.

· Contractor shall perform all scheduled preventive maintenance an agreed upon during initial contact with the technical or contract administrator.

2.2 Repairs: Repairs within the PoP must be prioritize by the contractor and shall be arranged in a timely manner as noted above. Telephonic technical support shall be provided immediately as to assess or troubleshoot matter or issue and coordinate repair services, when necessary. Repair services, when necessary, shall be scheduled in a timely manner within a 24 hour period from first notification made to the contractor. Report of repairs and services shall be made available upon completion of work performed prior to checking out.

· NMRTC Camp Pendleton Biomedical Engineering will provide the initial assessment.

· Provide only the work necessary to restore the equipment to a serviceable/operating condition by adjustments, replacement parts, or minor repairs when it in determined that extensive repairs and parts replacement are not necessary.

· Equipment improvements/modifications shall be made only upon Medical Repair Branch written approval and direction.

· Notify the medical repair branch immediately upon receipt of OEM or Replacement

· partn/equipment safety recalls notices.

· Ensure that original design and functional capabilities will not be changed, modified, or altered unless the medical repair branch authorizes such changes.

· Provide suitable OEM recommended repair equipment/tools required for the satisfactory execution of all repairs made.

· Furnish manufacturer OEM approved lubricants and lubricate wear points within the equipment.

· Extend to the Government all commercial warranties on replacement parts, consistent with standard industry practices.

2.3 Replacement Parts: All replacement parts needed during PM service or repair services must be provided by contractor. Consumables needed shall be noted before scheduling service to ensure these are available for contractor upon arrival to perform service(s).

2.4 Software Updates: Any software updates necessary for the operation of the NEUROLIGN USA, LLC. equipment shall be provided and updated at a minimum during the PM service.

2.5 Travel and Related Expenses:

· Contractor shall be responsible for its service-related trip expenses including round trip travel, mileage, and overnight living expenses.

2.6 Response Time: Contractor shall use commercially reasonable efforts to:

· Respond by telephone to any report of a malfunction requiring repair within one hour of notification by NMRTC Camp Pendleton.

· Provide on-site support within 24 hours of notification by NMRTC Camp Pendleton personnel.

2.7 Access To Equipment: Contractor shall be provided reasonable access to all equipment that in to be serviced and utility outlets required to do the service. The contractor shall be free to start and stop all primary equipment incidentals to the operation of the maintained equipment after permission in received from on duty personnel responsible for such equipment.

2.8 Government Personnel: NMRTC Camp Pendleton employees will not perform maintenance or attempt repair to equipment while such equipment in under the purview of thin contract unless agreed to in writing by the contractor.

2.9 Parts Availability: To ensure minimal equipment downtime, Contractor shall maintain replacement repair parts and materials necessary to perform each repair or supply said part and materials within 1 day.

2.10 Labor: All compensation for labor 7:30 am - 5:00 pm local PST time in included in the contract price. Any labor performed outside of this timeframe in not included in the contract pricing. If services become necessary to be performed beyond 5:00 pm, NHCP Biomedical Repair Divinion shall be notified prior to performance. Parts and materials: All compensation for parts and materials in included in contract price.

2.11 Contractor Check-In/Check-Out: Contractor in required to check in and out at the NMRTC Camp Pendleton Quarterdeck, Bldg H-200 during the hours of 7:30 am - 4:00 pm, Monday through Friday, prior to and upon completion of any service/repair performed.

2.12 Field Service Reports: Contractor sends a completed field service report upon completion of work performed prior to checking out to usn.pendleton.navhospcampenca.list.biomed-techs@health.mil.

3.0 QUALIFICATIONS/CONSIDERATIONS

3.1 Special Qualifications/Considerations: The contractor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.
