About this file

This document is a Human Trafficking Attestation form for a contract between a nongovernmental entity and the Flagler County Board of County Commissioners in Florida, required pursuant to Section 787.06, Florida Statutes. The form requires the contractor to affirm under penalty of perjury that they do not use coercion to employ persons for labor or services, with a comprehensive definition of coercion including physical force, debt bondage, document confiscation, financial threats, fraud, and other exploitative practices.

The attestation is specifically tied to a professional land management services contract, where the contractor must explicitly declare they will not engage in any form of human trafficking or labor exploitation. The document mandates that if the nongovernmental entity ever uses coercion for labor or services in the future, they must immediately notify the Flagler County governmental entity, and no further contracts may be executed, renewed, or extended between the parties. The form requires a signature from an authorized representative, along with their printed name and title, serving as a legal commitment to ethical labor practices.
