The solicitation number is FA9301-14-R-0023. This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this requirement. This is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76 and DFARs Change Notice (DCN) DPN 20140828. Federal Supply Class/Service (FSC) code is G002. NAICS is 813110 and the size standard is $7,500,000.00. This acquisition is a total small business set aside.

The Air Force Test Center (AFTC) Contracting Directorate, Edwards Air Force Base, CA is seeking to purchase the following service via an Indefinite Delivery / Indefinite Quantity contract:

The Wing Chaplain’s Office has a requirement to provide Religious Services for the Edwards Community. The services include providing Protestant and Catholic Religious Education and youth programs, Musicians and Music Directors for scheduled and special Chapel Services conducted on Edwards at the two chapels. The mission and ministry of the chaplaincy is supported, extended and enhanced by other religious ministry personnel. These include Department of the Air Force Civilians (DAFCs), civilian contractors (clergy, music directors youth workers, etc.), distinctive faith group leaders, and volunteers. These personnel support the chaplaincy in various professional and technical ways in bringing quality ministry and spiritual support to the base populace. This provides quality of life resources to the military communities.

Read attached Performance-Based Work Statement (PWS) Dated 25 August 2014 for further detailed requirements (Attachment 1).

The anticipated contract period of performance is 29 September 2014 -30 September 2019.

Task Orders will be issued on a monthly basis. The anticipated contract period of performance will contain 12 months. Additional task orders are TBD on the amount of months they will contain.

The minimum is the guaranteed quantity to be ordered, the maximum quantity is the most that could be requested throughout the entire period of performance.

The minimum guarantee is for $2,000.00 The maximum quantity to be ordered is $997,750.00

The first task order is anticipated to have a period of performance of 29 September 2014 – 30 September 2015, which will also meet the minimum guarantee.

There will be one CLIN

CLIN 0001 Chapel Ministry Support Director Services, unit of issue will be months.

The proposal shall be submitted following the attached bidding structure. If it does not follow the above CLIN Structure, it will be deemed as an incomplete proposal.

Award will be made to the Lowest Priced, Technically Acceptable offer that is most advantageous to the Government. Technical acceptability will be based on the offeror’s responsiveness in accordance with Exhibit A (attached) Technical Evaluation Sheet as well as Attachment 1, Performance Work Statement.

Offerors should pay close attention to the instructions in Addendum to 52.212-1, Instructions to Offerors - Commercial Items, which is incorporated in the solicitation document. The required contents of the proposal package are described in detail in the Addendum to 52.212-1.

METHOD OF EVALUATION: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and technical factors considered. The following factors shall be used to evaluate offers:

Price Prior Experience

a. Managing Military Chapel Contracts

b. Protestant and Catholic Religious Education and Youth Programs, Musicians, and Music Directors

c. Interacting with a variety of denominations and religious perspectives Process

a. Process in place that provides a suitable subcontractor/employee substitute of equal or greater qualifications, experience and skills when unable to perform duties.

b. Process in place and be familiar with hot to perform local law enforcement background checks on any employees that work directly with children and youth before and unit of services are performed.

c. Contractor shall be familiar with the Catechetical Certification Program through the Archdiocese for Military Services.

All factors are equal.

TYPE OF CONTRACT CONTEMPLATED: The Government anticipates award of an Indefinite Delivery/Indefinite Quantity--Firm-Fixed-price contract to the responsible offeror whose proposal is most advantageous to the Government. However, the Government reserves the right to make no award at all depending on the quality of the proposals received, prices submitted, and the availability of funds. Contract financing will not be provided for this acquisition.

Addendum to FAR 52.212-1, Instructions to Offerors

The following is to be added to FAR 52.212-1 paragraph (b):

1. Please provide your price quote on the attached bid schedule.

2. Please provide the following doc. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional price information will be necessary. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information other than pricing data to support a determination of price reasonableness.

A. Specific Instructions: Documents submitted in response to this must be fully responsive to and consistent with the following:

1. Requirements of the (CLINSs & Performance Work Statement (PWS)) and government standards and regulations pertaining to the PWS.

3. Offers are due at 4:00 PM PST 22 Sep 2014 emailed to monika.masei@us.af.mil and anthony.trevino.2@us.af.mil.

4. Vendors shall include a completed copy of the provision at FAR 52.212-3 Alt 1, Offeror Representations and Certifications -- Commercial Items. A vendor shall complete only paragraph (b) of this provision if the vendor has completed the annual representations and certificates electronically at http://www.sam.gov . If a vendor has not completed the annual representations and certifications electronically at the SAM website, the vendor shall complete only paragraphs (c) through (o) of this provision.

52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)

(a) Subject to the provisions contained herein, award will be made to one (1) Offeror who is deemed responsible in accordance with the Federal Acquisition Regulation who conforms to the solicitation requirements; and whose proposal, judged by an overall assessment of the evaluation criteria and other considerations specified in this solicitation meets the technically acceptable standard for the non-cost factors and provides the lowest evaluated price. The following factors shall be used to evaluate offers:

Price-- Offers will be ranked according to the most favorable quoted firm fixed price with the lowest priced offer ranked first. Price will be evaluated based on the price per unit (month).

Technical Factors—to be technically acceptable the Offeror must:

a. Submit acceptable prior experience

b. Submit acceptable process to provide …

(b) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

BASIS FOR CONTRACT AWARD: The resulting award will be a Firm-Fixed Price (FFP) IDIQ contract to a single contractor. By submission of its offer, the Offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub factors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.

A. Initially offers shall be ranked according to price. An offeror's proposed prices will be determined by the price per unit (hour). Submit copies of the Pricing Schedule with proposal. Price proposals should be reasonable and competitive. A price may be considered reasonable if the amount does not exceed that which would be paid by a prudent person in the conduct of competitive business. Prices may be considered competitive if they are comparable to other prices that are determined to be reasonable. Offers should be sufficiently detailed to demonstrate their reasonableness. The burden of proof for credibility of proposed prices rests with the offeror.

B. TECHNICAL FACTORS EVALUATION. For an offer/quote to be determined ―Technically Acceptable” the offer/quote must receive a rating of ―Pass for all Technical Factors.

1. Describe specific resources that will be used to perform the work described in the Performance Work Statement, to include type of experience, length of experience, certifications and training.

2. Describe same or similar service performed as describe in the Performance Work Statement within the last 5 years.

These are the definitions for the Technical Evaluations Ratings:

Technically Acceptable/Pass Acceptable quality and meets minimum PWS requirements necessary for satisfactory contract performance. Proposal contains no deficiencies or significant weaknesses, and any identified weaknesses do not affect the probability of meeting minimum requirements. Fair probability of success with low to moderate degree of risk.

Technically Unacceptable A proposal that fails to meet minimum PWS requirements necessary for acceptable performance. Where discussions are contemplated, the proposal contains weaknesses, significant weaknesses, and deficiencies that cannot be corrected without a major rewrite of the proposal. The degree of proposal risk is high to very high.

C. If the lowest priced evaluated offer is judged to be ―Technically Acceptable on all Technical Factors and the offeror is determined to be responsible, that offer is deemed the most advantageous to the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.

D. If the lowest priced offeror is not judged to be ―Technically Acceptable on all Technical Factors, the next lowest priced offeror will be evaluated and the process will continue (in order by price) until an offeror is judged to be ―Technically Acceptable on all Technical Factors or until all offerors are evaluated.

E. . Offerors are cautioned to submit sufficient information and in the format specified in the proposal preparation instructions to permit a meaningful assessment of technical capability. Offerors may be asked to clarify certain aspects of their proposal Communication conducted to resolve minor or clerical errors will not constitute discussions.

(d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e)

(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years and 6 months.

5352.201-9101 Ombudsman

OMBUDSMAN (NOV 2012)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (NOV 2012)

(a) In performing work under this contract on a Government installation, the contractor shall:

(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and

(2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.

(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (AUGUST

2007)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and a valid vehicle insurance certification obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply citing the appropriate paragraphs as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment.
