PROPOSAL PREPARATION INSTRUCTIONS
PERFORMANCE PRICE TRADEOFF WITH TECHNICAL PROPOSAL
Section L
INSTRUCTIONS TO OFFERORS
PROPOSAL PREPARATION INSTRUCTIONS
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representation and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts;
Part I – Price Proposal, Part II – Compliance with Specification, and Part III – Past Performance Information.
B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
C. Specific Instructions
1. Part I—PRICE PROPOSAL – Submit one (1) copy.
(a) Complete blocks 13, 15, 16, 17, and 18 of the RFP Section A, SF1442. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K, with amendments. These sections constitute the model contract.
(b) Insert proposed unit prices in Section B for each Contract Line Item Number (CLIN) or Sub CLIN (SCLIN), including all options. The extended amount must equal the whole dollar unit multiplied by the number of units.
(c) In accordance with 52.204-7, System for Award Management Registration online representation and certification. SAM must be completed as of the date of the offer. SAM can be accessed through https://www.sam.gov.
2. Part II – COMPLIANCE WITH SPECIFICATION – Technical proposals are not required. Technically acceptability will be determined based on the offerors' compliance with the requirement as stated in Attachment 1, Specification number 13019-11-001A, Construction Specification for VKF Revitalization (HVAC-MT&L).

3. Part III—PAST PERFORMANCE INFORMATION – Limited to no more than three pages per contract listed. Only references for relevant past efforts/contracts are desired. Submit one (1) copy.
(a) Quality and Satisfaction Rating for Contracts Completed in the Past Five Years: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with the overall job performance and quality of completed product for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires.) In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.
(b) Performance Surveys: The Government will evaluate the quality and extent of the offeror’s performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance. Provide a list of no more than five (five), of the most relevant contracts performed for Federal agencies and commercial customers within the last five (5) years.
Relevant contracts include the following: Office renovations including asbestos removal, lead removal, mold remediation, and interior finishes; HVAC construction including water piping, air handlers, heat exchangers, and pumps;
Electrical construction including mechanical equipment and facility power, panel board, interior lighting, and network communication wiring; Fire system construction including sprinkler systems, fire detection systems, and mass notification systems. In addition, contracts shall include construction project management experience with $1,000,000 to $2,000,000 plus dollar projects including installation design and job planning and scheduling of all subcontracted work. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed:
(i) Company/Division Name
(ii) Product/Service
(iii) Contracting Agency/ Customer
(iv) Contract Number
(v) Contract Dollar Value
(vi) Period of Performance
(vii) Verified, up-to-date name, address, FAX & telephone number of the contracting officer or point of contact
(viii) Comments regarding compliance with contract terms and conditions
(ix) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.
Submit Past Performance Questionnaire (PPQ) to references such that three (3) to five (5) of the most relevant contracts for Federal agencies and commercial customers for projects occurring within the past five (5) years are received. References are to submit PPQs directly to the Contracting Officer identified on the PPQ; any PPQs received directly from the offeror will not be evaluated. Failure of a past performance reference to submit a completed questionnaire will be treated as a nonresponse.
If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past/present performance information on previous teaming arrangements with the same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts.
(c) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
D. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:
1. Requirements of the RFP (CLINs & Specification/SOW) and Government standards and regulations pertaining to the Specification/SOW.
2. Evaluation Factors for Award.
3. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in this Section L will not be read or evaluated, and will be removed from the proposal.
4. Format for proposals Parts II and III shall be as follows:
(a) The proposals will be 8 ½” x 11” paper except for fold-outs used for charts, tables or diagrams, which may not exceed 11” x 17”.
(b) A page is defined as one face of a sheet of paper containing information.
(c) The typing shall not be less than 12 pitch
(d) Elaborate formats, bindings or color presentations are not desired or required.
(e) Electronic submission is authorized and should be sent to christopher.fanning.1@us.af.mil no later than the 2:00PM CST on 2 September 2014.

(f) Hard copies must be received at the designated location no later than the due date and time listed in the RFP.
E. Proposal Submission Instructions:
1. Offerors are responsible for submitting proposals, and any revisions, and modifications, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. Proposals will be accepted electronically, mailed, or hand-delivered in accordance with FAR Part 15.208, Submission, Modification, Revision, and Withdrawal of Proposals. Proposals may be emailed to Mr. Christopher Fanning at christopher.fanning.1@us.af.mil, or delivered to the address listed in block 8 of the SF 1442. Proposal receipt time shall be no later than 2:00 p.m. Central Standard Time on 2 September 2014. Due to equipment concerns, facsimiles will not be accepted.
2. Hand carried proposals must be deposited at AFTC/PZI (Arnold), Suite A-332, Building 100, Arnold AFB, TN.
(a) Individuals wishing to hand deliver proposal packages must contact the Contract Specialist at christopher.fanning.1@us.af.mil and state name of the person delivering the package, company name, the date they wish to deliver the package and whether or not they are a U.S. Citizen.
(b) Offerors are cautioned that Arnold AFB, TN has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at the designated location PRIOR to the time specified for receipt. Late proposals will be processed in accordance with FAR
15.208 (b).
3. Mailed and hand carried proposals are to be in a sealed envelope or package with the following information noted on the exterior of the package: Solicitation Number, time and date specified for receipt of proposal, and the name and address of the offeror.
4. E-mailed proposals are subject to electronic firewall issues. Proposal packages greater than 8 MG and certain file formats have the potential of not passing through the firewall. If mailing a proposal, please call 931-454-4675 or separately e-mail christopher.fanning.1@us.af.mil to verify that your proposal was received. E-mailed proposals will not be reviewed for solicitation conformance or evaluation criteria prior to the due date and time listed on the solicitation. Incomplete packages have the potential of being considered non-responsive. Please review FAR 15.208(b)(1)(i) for additional guidance.
5. Federal Holidays --The following Federal Legal Holidays are observed by this base:
New Year’s Day, 1 January Martin Luther King’s Birthday, Third Monday in January Presidents Day, Third Monday in February Memorial Day, Last Monday in May Independence Day, 4 July Labor Day, First Monday in September Columbus Day, Second Monday in October Veterans Day, 11 November Thanksgiving Day, Fourth Thursday in November Christmas Day, 25 December
FAR 15.208, Submission, Modification, Revision, and Withdrawal of Proposals:
(b) (1) Any proposal, modification, or revision, that is received at the designated Government office after the exact time specified for receipt of proposals is "late" and will not be considered;
unless the contracting officer determines that accepting the late proposal would not unduly delay the acquisition; and
(i) If it was transmitted through an electronic commerce method authorized, it was received at the initial point of entry to the Government infrastructure not later than 4:30 p.m. CST one working day prior to the date specified for receipt of proposals; or
(ii) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of proposals and was under the Government's control prior to the time set for receipt of proposals; or
(iii) It was the only proposal received.
(2) However, a late modification or an otherwise successful proposal, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
