ATTACHMENT 1
ADDENDA TO FAR 52.212‐01 INSTRUCTIONS TO OFFERORS – COMMERCIAL ITEMS (NOV 2010)
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
Proposals must be complete, self‐sufficient, and respond directly to the requirements of this solicitation.
The response shall consist of three (3) parts; Part I – Price Proposal and Part II – Technical Proposal and Part III ‐ Past Performance Information.
B. The Contracting Officer has determined there is a high probability of adequate price competition in the acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the Contracting Officer’s opinion, adequate price competition exists, no additional cost information will be requested and certification under FAR 15.406‐2 will not be required.
However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists; offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness and affordability of the price.
C. Specific Instructions:
1. PART I ‐ PRICE PROPOSAL – Submit original and 0 copies
a) Complete blocks 17a, and 30a, 30b, and 30c on the SF1449. In doing so, the offeror agrees to the contract terms and conditions as written in the solicitation with attachments. The solicitation will constitute the model contract.
b) Insert proposed unit and extended prices on the pricing schedule on page 2‐3 of the SF 1449 for CLINs 0001‐0004. Contract is to provide all parts, equipment, and labor necessary to clean, inspect, report condition, and refurbish rotor assembly in accordance with Statement of Work No. 12964‐11‐001, dated 20 April 2011.
c) Complete the necessary fill‐ins and certifications for the solicitation provisions. The solicitation provisions shall be returned in its entirety. For the solicitation clauses, the offeror shall submit only those pages that require a fill‐in.
2. PART II ‐ TECHNICAL PROPOSAL – Submit original plus (1) copy
a) The technical proposal shall demonstrate compliance with Attachment 4, Statement of
Work No. 12964‐11‐01, dated 20 April 2010.
b) Subfactor – ISO Certification: The offeror’s proposal shall include evidence of ISO 9001:2008 certification for the repair of turbo‐machinery and associated equipment.
c) Subfactor – Level II NDT Certification: The offeror’s proposal shall include evidence of Level II Non‐destructive Testing (NDT) certification of one or more employee(s) to perform magnetic particle (MT) and Ultra Sonic (UT) testing as required by the statement of work.
3. PART III ‐ PAST PERFORMANCE INFORMATION – Limited to no more than 2 pages per contract listed. Only references for same or similar type contract desired.
a) Quality and Satisfaction Rating for Contracts Completed in the past two (2) years for fabrications and refurbishments similar in scope, magnitude, and complexity as the effort being proposed: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for same or similar type contract. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.
b) Performance Surveys: The Government will evaluate the quality and extent of offeror’s performance deemed relevant to the requirements of this solicitation. The Government will use information submitted by the offeror and other sources such as other Federal Government offices and commercial sources, to assess performance. Provide a list of no more than five (5), of the most relevant contracts performed for Federal agencies and commercial customers within the last two (2) years. Relevant contracts are defined as contracts involving fabrication, refurbishment, and balancing of 9‐stage axial compressor rotors, with volumetric flowrate of 275,000 CFM (minimum) and discharge temperatures of 450 degrees F (minimum), that are similar or greater in scope, magnitude, and complexity than the effort described in this solicitation. Relevant contracts performed within the past two years are considered more relevant.
The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the acquisition. Furnish the following information for each contract in accordance with Attachment 3, Past Performance Information.
If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts.
c) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
4. Documents submitted in response to this solicitation must be fully responsive to and consistent with the following:
a) Requirements of the solicitation for CLINs 0001 – 0004 and Statement of Work for Allis Chalmers VA‐1409 Axial Compressor Rotor Refurbishment, and Government standards and regulations pertaining to the Statement of Work.
b) Evaluation Factors for Award (Attachment 2).
c) Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in these instructions will not be read or evaluated, and will be removed from the proposal.
d) Format for proposal Parts II and III shall be as follows:
1. A page is defined as one face of an 8 ½” by 11” sheet of paper containing information.
2. Typing shall not be less than 12 pitch.
3. Elaborate formats, bindings or color presentations are not desired or required.
