Attachment 1: Performance Work Statement

PERFORMANCE WORK STATEMENT

Wright-Patterson AFB Medical Treatment Facility

Uninterruptible Power Source (UPS) Maintenance

24 November 2015

1.0 SCOPE: The Medical Information Systems Branch at Wright Patterson AFB, Ohio needs to obtain a one-year preventative and remedial maintenance agreement for one (1) Eaton 93E 20KVA UPS with one (1) external battery cabinet.

2.0 REQUIREMENTS:

2.1 Maintenance: The vendor shall provide preventative maintenance calls that include, but are not limited to, start/stop inspections, calibration, cleaning, aligning, resolution checks, replacement of parts, lubrication and electronics checks to ensure the system meets factory specifications.

2.2 Parts/Material: The vendor shall include in all preventative maintenance calls and emergency service: parts, labor and travel at no additional cost to the US

Government.

2.3 Schedule: The vendor shall make two (2) manufacturer recommended bi-annual preventative maintenance inspections on the UPS and batteries and one (1) manufacturer recommended preventative maintenance inspection on the PDUs.

The vendor shall make manufacturer recommended preventative maintenance calls within the service year during normal duty, (0800-1700), Monday through

Friday (except for federal holidays). Exact dates and times will be mutual agreement between the vendor and equipment user.

2.4 Emergency calls: The vendor shall respond to emergency calls within 4 hours of notification, 24 hours/day, 7 days/week with all labor, travel, and replacement parts included.

2.5 Engineering Updates: The vendor shall make recommendations and cost estimates to improve operations that include, but aren't limited to, modifications and replacements on the items that may need additional attention.

2.6 Support: The vendor shall provide unlimited telephone support for instrument, reagents, and applications at no additional charge. The vendor shall provide information as to what items are not included in a preventive maintenance agreement.

2.7 All services listed above are considered to be mission-essential.
