PERFORMANCE-BASED WORK SPECIFICATION (PWS)

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements:

Voluntary Protection Program (VPP). The Air Force (AF) is an active participant in the Occupational Safety and Health (OSHA) Voluntary Protection Program (VPP) in accordance with OSHA’s Instruction, CSP 03-01-002, Voluntary Protection Programs (VPP) Policies and Procedures Manual. We recommend all contractors become familiar with VPP. Information on VPP can be accessed at: http://www.osha.gov/dcsp/vpp/index.html. The AF’s intent in this participation is to ensure the safety and health of all employees, including contractors, and communicate our safety and health expectations with contractors. The AF’s interest is to protect AF personnel working in or around contractor operations and to protect AF equipment and property. In the event an unsafe condition is discovered with AF provided facilities or equipment, the contractor shall immediately notify the Contracting Officer and Safety Office associated with that Delivery/Task Order (D/TO) for evaluation. This requirement applies to on and off base D/TOs.

The contractor will implement a safety program that ensures the protection of Government personnel and property. In addition, the contractor must comply with all the applicable Occupational Safety and Health Administration (OSHA) Standards and National Fire Protection Association (NFPA) Standards at their facilities. The contractor must have a Safety Plan and submit it to the contracting officer for approval by 78th ABW/SEG (Safety Office) prior to commencement of any work described in this contract. The contractor’s Safety program will consist of the following:

1. A Safety Plan that addresses, as a minimum, the subjects listed in Section II (Specific Requirements), which is to be used during the performance of work described in this contract.

2. Mishap reporting procedures as defined in paragraph B1 below.

3. Conduct routine and recurring surveillance to ensure the safety requirements of this contract are enforced and complied with.

4. Competent personnel shall be used to perform the surveillance.

5. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

6. All contractor personnel shall be trained and qualified to perform their job safely.

7. The contractor must identify segregated work-site locations for operations that cannot be co-mingled with general industrial operations.

B. Mishap Notification:

1.

The contractor shall incorporate the following mishap notification procedures into their safety plan prior to submitting it to the government for review:

The contractor shall notify 78th ABW/SEG (Safety Office) telephonically at (478)926-6271 during normal duty hours, (478)327-2612 after duty hours, within one (1) hour after initial notification of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract. A follow-up (written or e-mail) of this mishap/incident notification shall be sent within three calendar days to 78th ABW/SEG (Safety Office) at the address below:

The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap. Mishap notification shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of mishap/incident

(c) Brief Narrative of mishap/incident (Events leading to accident/incident)

(d) Cause of mishap/incident, if known

(e) Estimated cost of mishap/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in mishap/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information.

2. If requested by Government Personnel (PCO/ACO), the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the 78th ABW/SEG (Safety Office). The contractor and subcontractors shall cooperate fully and assist government personnel until the investigation is finalized and closed out.

3. Safety requirements listed in this package that do not relate to the contractor’s operations or services under this contract shall be considered self-deleting as mutually agreed upon by the contractor and the ACO.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall clearly define procedures, personnel qualifications, facilities, required equipment and any other items necessary to fulfill the following elements for the protection of government personnel and property. The contractor’s safety plan shall meet or exceed the referenced standards below.
