Continuation of block 14. DESCRIPTION OF AMENDMENT/MODIFICATION
The purpose of this amendment is to make adjustments based on questions received.
1. Proposal Adequacy Checklist has been removed from the attachments and is not required to be submitted.
2. The Cross Reference Matrix has been added to the attachments and must be submitted with the proposal.
3. The date due for proposals in Section L has been updated.
4. The PWS has been updated from 8 Jun 16 to 28 Jun 16.
5. CDRL A004 has been updated.
6. The PWS date in CLINS X001 extended description has been updated from 1 Feb 16 to 28 Jun 16.
7. Sub CLIN X002AF & X002AG have been added to the Schedule B.
8. The proposal due date will be extended to 15 Jul 16 at 2:30 PM EST.
All other terms and conditions remain the same.

SF 1034-Public Voucher for Purchases and Services Other Than Personal, shall be used for reimbursement under this CLIN. The contractor shall forward the approved SF 1034 to the DFAS office for provisional payment approval with copies also sent to DCAA, WR-AFLCMC/WWQEB Program Manager, and the WR-AFLCMC/WWQK Contracting Officer. The contractor shall be paid actual documented costs with no additional fee.

Offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined to be unacceptable/noncompliant and therefore ineligible for award. If a joint venture arrangement exists for this acquisition, the offeror shall provide a copy of the joint venture agreement that is signed and dated by all joint venture members as part of the proposal submission. Proposals shall be prepared and submitted to the Government in three (3) separate volumes as set forth below:

**Technical proposal information shall be submitted in written form (2 copies) and CD-R or DVD-R (write once/read only) (1 copy) that is “closed” so that no further writes can be made to the media. CD- R/W or DVD-R/W media types are not acceptable. Media must be virus scanned by the offeror prior to submitting to the Government. Electronic copies shall be submitted in accordance with RFP Attachment 6 Instructions to Offerors, Electronic Format for All Proposal Volumes.
In addition to the page limitations set forth above, proposals (both electronic and written) should comply with the following format:
(1) The sealed package used to submit the proposal package must show the time and date specified for receipt. Each volume shall be marked with the RFP number and the offeror’s name and address, and the number of the Volume, i.e. I and II. Proposals shall be in the English language and all monies shall be proposed in United States dollars.
(2) Print shall be no smaller than a font size of 12 and should be on 8 1/2 x 11-inch paper, excluding charts, graphs, drawings, diagrams, supporting illustrations, spreadsheets, etc. which may be on 8 1/2 x 17-inch paper. Each volume within the proposal shall be separately tabbed and identified to distinguish the volumes. Type of font shall be Times New Roman. Margins shall be no smaller than 1 inch and each page within a volume shall be numbered consecutively. Elaborate brochures, formats, bindings, or color presentations are not desired or required.
(3) The page limitation for Volume II, Written Technical Proposal, is 25 pages. The page limitation includes any charts, graphs, drawings, diagrams, supporting illustrations, spreadsheets, etc., but excludes any cover page, table of contents, or list of acronyms (if utilized). A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Offerors are cautioned that any pages that exceed the

1079195649C Highlight page limitations shall not be read but shall be removed and either retained in the contract file without being considered in the evaluation, or, sent back to the offeror.
(4) Page limitations may be placed on responses to Evaluation Notices (ENs), if issued. The specified page limits for each EN response will be identified in the correspondence forwarding the ENs to offrors or on the EN form itself.
(5) Each page containing proprietary information should be so marked.
(6) Each page should contain the following legend at the bottom of each sheet:

(7) The electronic copy of volumes shall be Microsoft Windows 7 compatible and submitted in Microsoft Office Suite 2007 (Word, Excel, Project, PowerPoint, etc.) formats.
(8) Submission of the initial proposal packages vis electronic mail will not be accepted. Initial proposal packages shall either be mailed or hand-delivered.
Offerors are cautioned that Robins Air Force Base has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. Some delay should be anticipated when hand-delivering proposal packages. Offerors should allow for sufficient time to obtain a visitor pass and arrive at the specified office PRIOR to the time specified for receipt of proposals.
2. Communications:
Exchanges of source selection information between Government and offerors will be controlled by the Contracting Officer. Exchanges of source selection information after submission of the initial proposal packages between the Government and offerors will be controlled by the Contracting Officer. Email may be used to transmit such information, but not the proposal, preferably with the email being sent encrypted, and must include “Source Selection Information – See FAR 2.101 and 3.104” in the subject line of the email. Otherwise, source selection information will be transmitted via direct mailing or facsimile.
