1. Blocks 7, 8, 14:
a. The contractor shall report End Item transactional data through electronic submission in CAV AF using the CAV AF website or CICA XML.
b. The contractor shall submit a DD Form 250 (Material Inspection and Receiving Report) electronically via the Internet to iRAPT (WAWF) website address: https://wawf.eb.mil, using DODAAC FA8212. MARK FOR: Address in block 14A.
c. The contractor shall submit a Letter of Transmittal (LT) in the month the delivery order is completed, covering all reports transmitted for that order. The Letter of Transmittal (LT) shall be submitted via e-mail to the following e-mail address: 4165CM5.CDRL.Workflow@us.af.mil
2. Block 10: The Contractor reporting requirements to CAY AF shall be accomplished daily and/or within one (1) business day of a maintenance action JAW Contractor Users Guide.
3. Block 11: Data shall be current within one (1) business day of the last End Item maintenance occurrence and reflect an accurate representation/status of all assets at a contractor for repair.
4. Block 12: Date of first submission shall be within one (1) business day following a maintenance transaction after the contract award date and Government training as specified JAW CAY AF Users Guide.
5. Block 13: Date of subsequent submittal(s) shall be within one (1) business day of each End Item maintenance action/occurrence.

DCMA will review and provide comments in coordination with the A-i 0 CCTVS Program Office NLT seven (7) calendar days following receipt. Final report is due NLT seven (7) business days after receipt and incorporation ofDCMA comments.
4. Block 9: The Distribution Statement, Export Control Warning and Destruction Notice shall be placed on the first sheet of each document and be located in a conspicuous location.
