PERFORMANCE-BASED WORK STATEMENT (PWS)
FOR
LAUNDRY SERVICES
75TH MEDICAL GROUP
1 April 2019

1. DESCRIPTION OF SERVICES. The Contractor shall furnish all personnel, equipment, transportation, and supplies, except as specified herein as government-furnished, to provide laundry service. The contractor shall pick up and deliver laundry at the 75th Medical Group Clinic, 7321 Balmer Ave, Bldg 570, Hill AFB, Utah, for the items listed in Appendix C, “Identification, Standard Packaging and Method of Finish.” Performance shall be in accordance with the scope of this Performance-Based Work Statement and other specifications or provisions of the contract. Pick-up and delivery points are listed in Appendix D, “Pick-up and Delivery Schedule.”
1.2. Special requirements:
1.2.1. Unserviceable Articles. The Contractor shall replace all unserviceable and non-repairable articles on the rental item list in Appendix D.
1.2.2. State or Local Department of Health Requirements. Compliance with all certification or labeling requirements, arising from state or local Department of Health regulations, is the responsibility of the Contractor.
1.2.3. Vehicle Interior. Soiled items and clean items shall not be transported in the same vehicle at the same time. All clean items are to be protected from contamination and soiling during transit to the Medical Treatment Facility (MTF). After transporting dirty/soiled and contaminated laundry, the vehicle interior shall be cleaned and sanitized before loading the clean articles.
1.2.4. Workmanship and Sanitation. Delivered finished articles shall conform to the generally accepted industry standards of quality of cleanliness, finish, and appearance. All work performed shall be done under sanitary conditions. The items shall be clean, not only in the sense of being free from soil and stains, but also free from bacteria, fungi, and other microorganisms, including pathogens or disease-producing organisms.
1.2.5. Re-performance of Laundering. Any items found to not meet the requirements of 1.2.4., shall be re-cleaned at no additional cost to the Government. Such items (found by the MTF personnel as packages are opened) will be identified by the Functional Requirements Evaluator Designee (FRED) to the Contractors representative at the next scheduled pick up after the unsatisfactory condition is discovered. After they are re-cleaned, the Contractor shall pack these items separately and identify by marking “Attention FRED.”
1.2.6. Hangers. Hangers shall be of a type that does not stain garments.
1.2.7. Cart Exchange Service. Contractor must insure that there are two clean and serviceable laundry carts available for use within the facility at all times to hold laundry. Carts shall be at least four feet high.
1.2.8. Sorting of Laundry. The Contractor shall sort items removed from the impervious laundry bags prior to washing. Items shall be sorted by dark and lights or whites washed with whites or light colored items. Dark colored items washed with dark items.
1.2.9. Packaged By Size/Color. The Contractor shall package items that come in several different sizes and colors with only the same size and color of package.
1.2.10. Lost or Damaged Items. The Contractor shall reimburse Government owned items that are lost or damaged by the Contractor. Items will be determined missing or damaged by the FRED/COR and notify the Contracting Officer (CO). The CO shall then notify the contractor. Reimbursement shall be based on the cost for the 75th Medical Group to replace the item. The 75th Medical Group will provide a copy of the replacement invoice to the Contractor if requested.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The items to be furnished for this requirement are identified in Appendix D and E.
4. GENERAL INFORMATION:
4.1. Security Requirements.
4.1.1. The Contracting Officer Representative (COR) shall complete a "Request for Identification Credential (AFMC Form 496)" for each employee of the contractor requiring access to Hill AFB. The requests shall be submitted to Pass and Registration (South or West Gate Visitors Center).
4.2. Quality Control
4.2.1 The FRED is the authorized individual who will perform assessments of the contractor’s performance.
4.2.2 The FRED or alternate(s) will inform the contract manager in person when discrepancies occur and will request corrective action. The FRED or alternate(s) will make a notation of the discrepancy on their assessment checklist with the date and time the discrepancy was noted and will request the contract manager (or authorized representative) to initial the entry on the checklist.
4.2.3. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer.
4.2.4. The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the Contracting Officer and or authorized Government representative(s).
4.2.5. Performance evaluation meetings: The Contracting Officer (CO) may require the contract manager to meet with the CO, COR and FRED, and other government personnel as deemed necessary. The Contractor may request a meeting with the CO when he or she believes such a meeting is necessary. Written minutes of any such meeting shall be recorded in the contract and signed by the contract manager and the CO or contract administrator. If the Contractor does not concur with any portion of the minutes, such non-concurrence shall be provided in writing to the CO within 10 calendar days following receipt of the minutes.
4.2.6. The Government reserves the right to inspect the contractor’s facility and observe in-process work to assure performance is in accordance with contract requirements.
4.3 HOURS OF OPERATION:
4.3.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: before 3:00 p.m., Monday and Wednesday.
4.3.1.1. Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day – (observed 1 January), Martin Luther King Day – (observed the third Monday in January), Presidents Day – (observed the third Monday in February), Memorial Day – (observed the last Monday in May), Independence Day – (observed 4 July), Labor Day – (observed the first Monday in September), Columbus Day – (observed the second Monday in October), Veterans Days – (observed 11 November), Thanksgiving Day – (observed the fourth Thursday in November), and Christmas Day – (observed 25 December). If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
4.3.2. Emergency Services. On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours described above. The contractor’s responsibilities for emergency support are described in Section 1.
4.4. RECORDS. The contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in this PWS or required by the provisions of a mandatory directive listed in (LIST APPLICABLE APPENDICE), Applicable Publications and Forms. If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request.
4.5. ENVIRONMENTAL CONTROLS:
4.5.1. Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the Contracting Officer pursuant to notice requirements and negotiate a modification to the contract.
4.5.2. Notification of Environmental Spills. If the contractor spills or releases any substance contained in 40 CFR 302 into the environment on Hill AFB, the contractor or its agent shall immediately report the incident to Hill AFB Fire Department at 777-1911. The liability for the spill or release of such substances rests solely with the contractor and its agent.
4.5.3. Material Storage and Use. The contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.
4.6. GOVERNMENT OBSERVATIONS. Government personnel, other than contracting officers (COs) and COR may from time‑to‑time, with COR coordination, observe contractor operations. However, these personnel may not interfere with contractor performance or make any changes to the contract.
4.7. SAFETY REQUIREMENTS. In performing work under this contract, the contractor shall:
4.7.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.
4.7.2. Take such additional immediate precautions as the contracting officer may reasonably require for safety and mishap prevention purposes.
4.7.3. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.
4.7.4. Include a clause in all subcontracts to require sub-contractors to comply with the safety provisions of this contract as applicable.
4.7.5. Record and report promptly (within one hour) to the contracting officer or FRED, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.
4.7.6. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.
4.7.7. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.
4.7.8. Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident reporting and investigation.
4.8. PHASE OUT:
4.8.1 If there is a change in contractor or if the operation reverts to in-house, the incumbent contractor will provide familiarization, to the government or the follow-on contractor, whichever the case may be. During the phase-out familiarization period, the incumbent will be fully responsible for the operation of laundry services for the 75th Medical Group/SGSM.
4.8.2 The government reserves the right to conduct site visits in all contractor operated facilities in conjunction with the solicitation of offers for the follow-on contract. In the event the follow-on contract is awarded to other than the incumbent, the incumbent contractor will cooperate to the extent required to permit an orderly change over to the successful contractor. With regard to the successor contractor’s access to incumbent employees, a recruitment notice may be placed in each facility.
4.9. PUBLICATIONS:
4.91. Compliance with all publications, regulations and operating instructions provided by the
Government is required when:
4.9.2. They pertain to the procedures for materials expediting herein and where the contractor is authorized by the performance-based statement of work to accomplish the work specified in the publication, regulation or operating instructions.
4.9.3. The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements.
4.9.4 The contractor is required to acquire and work on the latest version of the publication.

APPENDIX B
Definition
Definitions of special terms and phrases used in this Performance Work Statement (PWS) are as follows:
1.
Clean: Finished articles are dry and free of visible soil, lint, and objectionable odors. Spots and stains are removed without damaging the fabric and have been treated.
2.
Laundering: The process by which soiled fabric items are cleaned and treated. The term includes cleaning by mechanical action in appropriate water temperature with solutions of detergents and other additives. These additives include repellent treating, bleaching, bluing, chemical rinse, fabric softener, and any other agent for controlling the growth of microorganisms, (such as molds and bacteria), starching, ironing and pressing agents. The appropriate products and procedures are to be determined by the contractor for the various types of fabric and nature of soil.
3.
Medical Treatment Facility (MTF): Those activities providing outpatient health care services for authorized personnel.
4.
Replaceable Items: Articles that have minor tears (less than four inches), open seams, holes, missing buttons, and broken zippers shall be replaced.
5.
Unserviceable Articles: Articles that are no longer usable for the purpose intended.
6. Functional Requirements Evaluator Designee (FRED): A Government representative that is responsible for the surveillance of the contractor’s performance.

Identification, Standard Packaging, Method of Finish
1. General. The following are the routine requirements for packaging of laundry and dry-cleaning.

Packaging and Finish. The items listed below will be included in the yearly laundry. See Appendix D.
4.
Impervious Laundry Bags. Impervious laundry bags will be used by the MTF for all laundry items. Impervious laundry bags may contain any combination of items.
4. Package by Size. Items that come in several different sizes shall be packaged by size. For example, small scrub shirts shall be packaged in smalls; all shirts in one package will be size small.
5. Package by Color. Items that come in several different colors shall be packaged by color and size. For example, small purple scrub shirts will be packaged in smalls; all shirts in one package will be size small, purple color.

APPENDIX E
GOVERNMENT FURNISHED PROPERTY/SERVICES/EQUIPMENT
The Government shall provide, without cost, the items listed below:
1. Government owned linen items identified in Appendix D.
2. Authorized copies of government forms and publications specifically identified in this contract.
3. Storage of laundry carts.
appendix F Contractor furnished PROPERTY/SERVICES/EQUIPMENT
GENERAL:
1.
A pick-up and delivery vehicle and driver.
2.
All packaging and marking material.
3. Laundry carts. The carts shall have a solid bottom shelf. Carts shall also be covered unless linen is shrink wrapped.
4. Hangers and bar carts for delivery of scrubs.
5. Materials for repair of clothing (thread zippers, snaps, binding, collars, etc.).
6. Packing slip type document for each cart delivered. This document shall identify the contents on the cart by item count for each type item on the cart.
7. Any other supplies, materials, and equipment necessary to perform the services required by this contract.
8. The Contractor shall provide a facility equipped and ventilated to prevent dissemination of contaminants. The ventilation system shall include adequate intake filtration, exchange rate, and exhaust in accordance with local, State, and Federal requirements.
9. The Contractors facility shall be subject to inspection of sanitary conditions at any time by an authorized representative of the Government.
10. All commercial, industry and safety forms and publications related to the performance of laundry service.
11. Rental linen items identified in Appendix D.
