STATEMENT OF WORK (SOW)

FOR CONSTRUCTION CONTRACT AWARD

UNDERGROUND FUEL STORAGE TANK REPAIRS

BUILDING 228

HILL AIR FORCE BASE, UTAH

8 January 2018 Work Order: 59527 Location: Building 228 Project Title: Rpr Underground Fuel Storage Tank Project Number: KRSM162091 Project Manager: Mr. Rodney B. Sanders

1. General Requirements

1.1 The Offeror shall provide all labor, equipment, testing, transportation, and supervision to accomplish the following in accordance with requirements contained in the original contract specifications:

1.2 The Offeror shall provide a TURN KEY proposal for all areas of the project, when completed and ready for final inspection; project shall meet all pertaining codes, regulations, and governments’ statement of work (SOW).

1.3 Only items incorporated into this document from the accepted proposal shall be considered part of this SOW.

1.4 The point of contact for this project is Mr. Rodney Sanders, 75 Civil Engineer Squadron (75th CES) Project Manager (PM), 777-6782 or Mr. Cody Duncan 75th CES PM, 777-8489.

1.5 The Offeror shall comply with the most current manufactures installation recommendations and requirements, UFC 1-200-01, IBC Codes, NEC Codes, International Mechanical Codes, Hill AFB Section 1000 Standards, ASHRAE Standards, and Base Facility Design Standards.

1.6 The Offeror shall perform the service required after coordinating with PM. Normal working hours are 0700 to 1700 Monday through Friday.

1.7 The Offeror shall provide a detailed proposal on method of project execution, material submittals prior to using and/or starting the project, provide all required as-build drawings, training, O&M manuals, and manufactures installation manuals to the project manager.

1.8 The Offeror shall have three (3) days to complete any punch list items prior to 100% beneficial occupancy being granted, based upon final visual inspection/walk thru with the customer, AFMC OL H/PZIOC contracting officer (CO), PM, Offerors, and sub-contractor(s).

1.9 The Offeror shall clean up entire project area prior to final inspection and properly dispose of all construction materials off base.

1.10 The Offeror shall use qualified sub-contractors with work experience in Architectural, Electrical, Excavation, Concrete, Stainless Steel Welding, [footnoteRef:1]and Underground Storage Tanks (UST) disciplines. [1: ]

1.11 The Offeror and sub-contractors shall visit and inspect jobsite prior to bidding project, to verify locations of all the existing electrical, piping, valves, and tanks. After or upon Offerors inspection of jobsite, Offeror may notify the CO with questions of any deficiencies that may require modifications to the government SOW, prior to contract award.

1.12 The Offeror shall conform to all safety requirements contained in all applicable standards, for activities related to the accomplishment of the work performed and provide dust protection as reasonable, to protect government assets.

1.13 The Offeror shall be responsible for safeguarding all government property. At the close of each work period, government facilities, property, and materials shall be secured.

1.14 The Offeror shall provide protection to government property, to prevent damage during the period of time the property is under the control or in possession of the Offeror.

1.15 The Offeror shall record and report promptly (within one hour) to the CO or PM, all available facts relating to each instance of damage to government property or injury to either Offeror or government personnel.

1.16 In the event of an accident/mishap, the Offeror shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

1.17 If the government elects to conduct an investigation of the accident/mishap, the Offeror shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

1.18 The Offeror shall notify the PM of any environmental spills, prior to reporting the incident to the Hill AFB Fire Dept. at 911. If the Offeror spills or releases any substance as contained in 40 CFR 302 into the environment, the liability for the spill or release of such substances rests solely with the Offeror and its agent to remediate the area as required, at no additional cost to the government.

1.19 The Offeror and sub-contractors shall provide a warranty on all materials, labor and installation of equipment for a period of one year upon completion of the project and government issuing of Beneficial Occupancy Date (BOD).

1.20 The Offeror shall implement and maintain a three phase inspection and quality control system through the life of the design, construction and closeout of the delivery order.

1.21 The Offeror shall clean-up the exterior work area at end of each work shift.

1.22 The Offeror shall not use government furnished roll-of containers and/or dumpsters. Offeror shall provide their own containers for construction debris and provide PM with construction disposal/recyclable report and weight tickets.

1.23 The Offeror shall comply with all safety/fire requirements with any welding torch or arc, grinding and/or cutting to remove existing pipe/valves and installation of new piping and valves. Burn permits are required by the Fire Department (FD) and the Offeror and/or sub-contractor(s) must attend a burn permit class provided by the FD. The class is held the first Thursday of every month at Building 133, with start time of 8:00 a.m.

1.24 The Offeror shall provide a conceptual shop drawing with project submittals prior to starting work and must be approved by PM and CO.

2.0 Statement of Work:

2.1 This is to bring the UST’s into compliance with State regulations, by installing a line leak detector on the American pump, which is located above ground on the north tank (228.4) and bring piping on south tank (228.3) above ground and connect to fill port approximately 15’ away.

Note: Installation shall be completed by a Utah DERR Certified Installer and tested by a Certified Tester. Period of Performance (PoP) shall be 21 calendar days, to include ordering of materials, delivery of materials, labor, and material install.

2.2 The Offeror shall field verify all dimensions and quantities of required piping, conduit, electrical wire, concrete square footage, etc., to support task order proposal.

2.3 The Offeror shall post required asbestos and LBP surveys on site prior to starting the project and shall maintain on site until the project has the BOD issued as completed.

2.4 The Offeror shall provide all labor, equipment and materials to support this project.

2.5 The Offeror shall demarcate work area as a construction site with caution signs and barrier tape.

2.6 The Offeror shall provide a submittal with proposal of sub-contractor license and their S470 rider that verifies their qualifications to work on fuel systems.

2.7 The Offeror shall provide detailed narrative on stainless welding, certification and inspection.

3.0 Demolition Work:

3.1 The Offeror shall cut surrounding concrete pad as needed, to allow concrete sump to be removed and provide access to the red jacket pump.

3.2 The Offeror shall pull existing electrical wire from Building 228 pumps, back to Exterior wall mounted disconnect(s) attached to Building 227.

3.3 The Offeror shall remove old transfer line and install blind flanges where old lines entered tank 228.4.

4.0 Electrical:

4.1 The Offeror shall pull new electrical wire from Building 228 pumps, back to disconnect(s) attached to Building 228 exterior wall. Electrical wire and conduit shall meet NEC code, UFC, and manufactures recommendations to support new Line Leak Detectors (LLD).

4.2 The Offeror shall repair or add additional conduit at Building 227 to support LLD install.

4.3 The Offeror shall install new functional LLD on the American pump and pressurized piping on the north pump (228.4), which shall be a Vaporless LD99 model.

4.4 The Offeror shall provide a functional sump sensor system upon completion of this project.

4.5 The Offeror shall ensure sub-contractor is a certified Veeder-Root technician and shall program any required changes to Veeder-Root Controller settings, to provide functional LLD operations.

4.6 The Offeror shall provide as-built drawings/diagrams of new electrical controls, to include all newly installed and existing circuits that have been integrated into this LLD project.

4.7 The Offeror shall use stranded copper for all power and control wires. Note: Eight (8’ LF) of extra wire length shall be coiled in gutter below controllers on exterior wall for future upgrades as needed.

5.0 Pipe and Welding:

5.1 The Offeror shall remove existing pipe and 90° elbow, located between the north and south pumps and manufacture a new pipe section and modify existing 90° elbow in preparation for install of new LLD adaptor.

5.2 The Offeror shall install approx. 20’ LF of new 3” stainless steel pipe, from underground line (228.3) to new connection point at tank 228.4. Note: Pipe will be identified during the site walk.

5.3 The Offeror shall install, but not limited to new check valve, overfill protection, and a drop tube into the connection point.

5.4 The Offeror shall follow UFC 3-460-01 Chapter 9 for welding and inspection requirements.

Note: Offeror shall provide certifications for both welder and inspector for stainless steel work.

5.5 The Offeror shall install a new light weight manhole lid and bring pipe up through the lid in existing sump, ensuring pipe has no contact with any soil or concrete.

6.0 Architectural:

6.1 The Offeror shall manufacture and install a 42” inch light weight manhole ring and manhole cover, which shall be higher than ground level, to prevent water infiltration into sump.

6.2 The Offeror shall paint new pipe and 90° elbow in accordance with (IAW) UFC 3-460-01, Chapters 9-10.

6.3 The Offeror shall purchase and install signage to meet JET-A requirements IAW MIL-STD-161H.

7.0 If Excavation is Required for Sump Access:

7.1 The Offeror shall cut and excavate existing concrete material to be replaced (approx. 7’ x 7’ SF work area). The excavated material shall be transported to Hill AFB concrete landfill.

7.2 The Offeror shall excavate base course and sub-base course materials encountered and haul to the Hill AFB land fill as needed.

7.3 The Offeror shall not exceed weight limit restrictions as defined by UDOT while hauling excavated materials to designated dump site(s).

7.4 The Offeror shall weigh every load in coordination with the 75th CES Pavements Shop.

7.5 The Offeror shall use caution and avoid damaging any government property.

7.6 The Offeror shall follow all UFGS specifications regarding excavation.

7.7 The Offeror shall follow all rules and regulations for trenching and excavation safety. Note: See 29 CFR 1926.651 and 1926.652 (https://www.osha.gov/)

7.8 This phase of the contract will be discussed during the site walk and determined at that time if this requirement is valid or not.

8.0 Compaction if Required:

8.1 Compaction requirements for the sub-grade and granular layers are expressed as a percent of maximum density. The Offeror shall compact the material to (95-100 percent of maximum density). Select materials and sub-grades in-fills will be placed at no less than 95 percent maximum density.

8.2 The Offeror shall ensure during construction, that excavation and backfilling shall be performed in such a manner and sequence that will provide proper drainage at all times.

8.3 The Offeror shall excavate and remove any and all soft or otherwise unsatisfactory material in the sub-grade, as well as any areas displaying the characteristics of pumping.

8.4 The Offeror shall provide soil and road base to back fill opening from excavated sump.

8.5 The Offeror shall install 12” base course. The base shall be brought up by way of three (3) - 4” lifts, with 95-100% compaction established after the addition of each lift. The final grade of the base course shall not have any abrupt deviations or grade changes of more than ¼” in 12’.

Drainage is a critical factor in aggregate surface design, construction, and maintenance. Provide adequate surface drainage to minimize moisture damage. Quick removal of surface water reduces absorption and ensures more consistent strength and reduced maintenance.

8.6 The Offeror shall ensure grade and sub-grade work is maintained at the proper moisture density relationship throughout placement.

8.7 The Offeror shall hire an independent consultant, to test and certify compaction of backfilled materials, prior to installation of concrete materials.

8.8 This phase of the contract will be discussed during the site walk and determined at that time, if this requirement is valid or not.

9.0 Portland Cement Concrete Sump:

9.1 The Offeror shall design new sump to accommodate newly installed red jacket pump, new flange and to allow personnel access to system for future inspections/repairs as needed.

9.2 The Offeror shall form, replace and match existing concrete sump.

9.3 The Offeror shall drill into existing concrete pad, epoxy, grout, and install 16 inch long by 5/8” inch diameter (ASTM #5) smooth dowel reinforcing bars 12” inches on center and 4” below finish grade in all construction joints. All dowels both smooth and deformed shall be a minimum of 16” long and shall be placed so that 8” of dowel protrudes into the concrete on each side of the construction joint or contraction joint.

9.4 The Offeror shall notify PM for visual inspection prior to pouring concrete and follow Base Facility Design Standards, UFGS-32-11-33, and UFC 3-250-04.

9.5 The Offeror shall use Type II Cement with a 2 slump mix and shall meet at a minimum 4,000 PSI compressive strength, and 650 PSI flexural strength. The concrete shall meet or exceed the UFGS specifications and shall match the material submittal and shall match, exactly, the mix design submitted with Offeror’s proposals.

9.6 The Offeror shall hire independent consultant to test compaction and provide results prior to concrete phase of this project.

9.7 The Offeror shall ensure each sump pad surface has a non-skid finish.

9.8 The Offeror shall seal concrete with curing compound, immediately after placement and finish.

9.9 The Offeror shall guarantee that installed concrete will pass a 90-day 650 psi flexural strength test and a 28-day 4,000 psi compressive strength test.

9.10 The Offeror shall repair or replace any and all concrete that cracks during the construction process.

9.11 The Offeror shall Sikaflex all joints where existing materials meet new concrete sump.

9.12 The Offeror shall be responsible to keep frost out of the ground by covering with heat blankets.

9.13 The Offeror shall be responsible to protect all concrete from freezing as per American Concrete Institute (ACI) ACI 306.1-90 cold weather concreting.

9.14 The Offeror shall assure that concrete develops the required strength for the safe removal of forms.

9.15 The Offeror shall maintain curing conditions that foster normal strength development without using excessive heat.

9.16 The Offeror shall limit rapid temperature changes in the concrete to prevent thermal cracking. Concrete should be properly air entrained if it will be saturated and exposed to freezing and thawing cycles during construction.

9.17 This phase of the contract will be discussed during the site walk and determined at that time, if this requirement is valid or not.

10.0 Submittals:

10.1 The Offeror shall supply submittals with their proposals for sump, LLD, manhole ring and cover, drawing and design for new system, and concrete mix.

10.2 The Offeror shall provide a Gant Chart/construction schedule at the time of NTP. Schedule shall be provided to PM and must indicate any day or days in which Offeror requires after hour work.

10.3 The Offeror shall provide a completed Utah DEQ Underground Storage Tank Installation Notification Form (Available at https://documents.deq.utah.gov/environmental-response-and-remediation/ust-lust/underground-storage-tanks/ust-installation-notification-form.pdf).

10.4 The Offeror shall provide welding certification and inspection certification for stainless steel welding work.

11.0 Miscellaneous:

11.1 The Offeror shall provide manufactures recommended preventative maintenance schedule.

11.2 Any changes, addendums, or modifications to the original statement of work must be approved by the Contracting Officer. No other parties will have that approving authority.

11.3 The Offeror shall provide a thorough and complete cleanup of the exterior work area prior to pre-final and final inspection and before final progress report and BOD is issued by PM.

11.4 The Offeror shall schedule the downtime with the users, to minimize impact to their war Fighter mission.

11.5 The Offeror shall provide cost line items and quantities for AF Form 1354 to the PM, 7 days prior to final inspection and close-out/BOD of project task order.

11.6 The Offeror shall be a Utah DEQ Division of Environmental Response and Remediation (DERR) Certified Installer and provide a fully functional and operational leak detection system upon completion of this task order.

11.7 The Offeror shall be responsible for scheduling and requesting all utility outages through proper coordination, 21 days prior to needed outage date.

11.8 The Offeror shall provide protection consistent with the intended serviceability of the structure.

11.9 The Offeror shall provide passing LLD results performed by a certified tester to 75th CES/CEOIU.

11.10 The Offeror shall provide a complete and functional line leak detection system upon completion of this contract award.

12.0 Construction/Recyclable Debris:

12.1 The Offeror shall provide weigh tickets to the PM at the conclusion of this project, for disposal of construction debris and/or recycled materials (i.e. metal doors, aluminum frames, wood doors, etc.). Requirements can be found in the Base Design Standard, Section 5.5 and under the Unified Facility Guide Specs (UFGS) specifically for C&D Waste Management in Section 01 74 19 on the Whole Bldg Design Guide: http://www.wbdg.org/ Click on the UFGS tab on the right side for requirements.

13.0 Warranty:

13.1 The Offeror shall provide a functional system installed as per manufactures specification and shall warranty all labor, materials, and installation for a period of one year upon completion of this task order project and acceptance of Beneficial Occupancy Date (BOD).

Any changes/addendums, modifications, etc…, to the original SOW must be approved by the Contract Officer; no other parties have approval authority.
