PERFORMANCE WORK STATEMENT (PWS) FOR

LEASE OF MATERIAL HANDLING EQUIPMENT

1. SCOPE OF WORK: The contractor shall provide requested equipment to the Utah Test and Training Range (UTTR) Detachment 1 (DET 1) Dugway Operations located at 1010

Avery Road Dugway Proving Ground, Utah. Contractor shall provide all required repair, service or replacement of vehicles if required at the above address or on the UTTR.

2. GENERAL INFORMATION: Leased Material Handling Equipment is required in order to provide range support at the UTTR. During various tests during the lease period targets as well as camera poles located on the ranges will require welding, and the range area requires grooming and maintenance after tests. The government does not have the equipment or organic capability to fulfil this requirement and as such, in order to meet customer test requirements leasing these pieces of equipment is required.

3. PERIOD OF PERFORMANCE: The period of performance shall be from 3 April 2017 to

31 October 2017.

4. REQUIREMENTS: Except for those items or services specifically stated to be government-furnished the contractor shall furnish everything required to perform this contract in accordance with all of its terms and conditions. New or used equipment is acceptable. All equipment functionality must work as designed. All safety items must be installed and operable. Unlimited mileage/hour usage and normal wear and tear should be expected. The contractor shall provide the following equipment:

4.1 One (1) each Loader: 3.5 yard rubber wheel, center articulating

1. 37,000 Lb. breakout force

2. 3.5 yard Bucket capacity

3. Dump clearance 9.5’ minimum

4. Hinge pin max height 13’ minimum

5. Quick attach front end

a. Attachments to include

b. 72” forks

c. 3.5 yard bucket

6. Safety equipment

a. Horn

b. Backup alarm

c. Wipers

4.2 One (1) each 80’ Self-propelled Man Lift with Sky Welder and boom welding package.

The boom shall have a minimum 250 amp welder package positioned under the control station.

1. All terrain

2. Fuel Type Gas or Diesel

4.3 One (1) each Dozer: Size T7 Low Ground Pressure (LGP) crawler dozer

1. Width over tracks 7.5” minimum

2. 24” minimum track width

3. Minimum 90 Horsepower

4. 6 way blade

5. Optional equipment

a. Rear ripper teeth

b. Rear winch

6. Safety equipment

a. Horn

b. Backup alarm

c. Wipers

4.4 The contractor shall be responsible for normal vehicle maintenance.

4.5 The contractor shall be responsible for any repairs for damage was not caused by the government.

4.6 The government will be responsible for repairs to damage due to negligence by government personnel.

4.7 Upon award of contract, the contractor shall deliver bucket trucks to UTTR/Det 1, 1010

Avery Rd, Dugway, UT 84022-1103. Upon completion of lease, the contractor shall pick-up bucket trucks at UTTR/Det 1, 1010 Avery Rd., Dugway, UT 84022-1103.

5. GOVERNMENT PROVIDED SERVICES: The Government shall provide an escort or range safety briefing, map of the range, hand held radio, call sign and proper radio etiquette and use per Air Force instruction and Army regulations if required to access the UTTR.

6. SECURITY REQUIREMENTS: The contractor shall obtain the necessary vehicle, personnel passes and identification as required from the contracting/Security office prior to commencing performance. United States citizenship is required for access to installation/work location. Cameras, guns or weapons of any kind are not allowed on the

Dugway Proving Ground/UTTR if found shall be confiscated and NOT returned to the individual. See attachment 1 "Memorandum for DPG Visitors'', detailing visitor driving directions and Range restrictions.

7. REPORTS: The contract number and delivery date must be clearly visible on each invoice.

At the time of acceptance, the government representative shall conduct a visual inspection of each piece of equipment leased and annotate any findings on the delivery paperwork and drawing. Upon pick up of equipment the government and vendor shall perform a walk around with the original documentation noting any damage that may have occurred while the equipment was utilized by the UTTR. UTTR shall retain and file with the award all documentation from delivery and pick up of equipment.

8. HOURS OF OPERATION: Contractor shall perform all necessary deliveries, pick-ups or other services under this contract during normal duty hours, which are 7:00 A.M. to 4:00

P.M., Monday through Thursday. No services shall be performed on Friday’s, weekends, oe

U.S. Federal holidays.

9. GENERAL INFORMATION: UTTR/Det 1 is located at 1010 Avery Rd, Dugway, UT

84022-1103. The South Range is approximately 94 miles from Salt Lake City, UT (45 miles west then south 49 miles) off I-80 at Dugway/ Rowley Exit 77. Except for those items or services specifically stated to be government-furnished the contractor shall furnish everything required to perform this contract in accordance with all of its terms and conditions.

There are two main routes into DPG (South Range, UTTR):

a. Route 1 (mountain pass suitable for small vehicles): From I-80 take exit 99 southbound toward Tooele on Hwy 36. Go south for approximately 21 miles to Hwy 199. Turn right on

Hwy 199 and continue for approx. 24 miles through Johnson’s Pass to the Dugway main gate.

b. Route 2 (suitable for hauling purposes): From I-80 take exit 77 southbound. Go south on

Hwy 196 through Skull Valley for approximately 38 miles to the Dugway main gate. Be aware that you shall be traveling through open range country with horses and cattle free ranging

9.1 Contract Manager: Contract Manager or alternate shall be responsible for the execution of this contract commensurate with the requirements of this PWS. The Contract Manager or alternate must be able to read, write, speak, and understand English.

9.2 Employees: The Government reserves the right to restrict employment of any contractor employee, or prospective contractor employee, who is identified as a potential threat to the health, general well-being, safety, security, or operational mission of the installation and its population. A police check shall be performed prior to access.

9.3 Driving on Post: Employees must possess a valid civilian driver’s license to drive on post.

9.4 Security Requirements: Obtain the necessary vehicle, personnel passes and identification as required from the contracting/Security office prior to commencing performance. United States citizenship is required for access to installation/work location. Cameras, guns or weapons of any kind are not allowed on the Dugway Proving

Ground/UTTR if found shall be confiscated and NOT returned to the individual.

9.5 Government Responsibilites: The government is not responsible for payment of leased items beyond the negotiated pick-up date if the contractor fails to pick-up the equipment.
