1.0 Background cover sheet or appendix
The 75th MDG is in the process of replacing old worn lobby seating in buildings 570, 249, 545,546, and 401. It was decided to replace all patient waiting area furniture at the same time in order to standardize them and bring them up to current infection control standards.
2.0 Scope
For this project we require the following:
75th MEDICAL GROUP
· One hundred and thirty (130) Foster Guest 1 Seater G4, SIL Frame – F1010-OA-MRM-SVR-G4-G4 or equivalent:
· Removable Antimicrobal Fabric Covers (Seat & Back) (Color Blue), Moisture Barrier (Seat), Clean Out Areas,16 Gauge Tubular Steel, Steel Seat Frame with Active Seat Suspension, High Density Seat Foam, Latex/ Lead (metal)/ and Mercury-Free, Flex Back, Wallsaver Frame Design, and Non-maring glides, 500lbs weight capacity, 10 year warranty on parts, labor and fabric. Dimensions: (Overall 34"H, 26"W, 24.75"D) (Seat: 18"H, 21"W, 19"D) (Back: 16"H, 21"W) (Arm: 26.25"H) (tolerance +/- 1”)
· Sixtyfive (65) Foster Guest 2 Seater G4, SIL Frame – F1020-OA-MRM-SVR-G4-G4-G4 or equivalent:
· Removable Antimicrobal Fabric Covers (Seat & Back) (Color Blue), Moisture Barrier (Seat), Clean Out Areas,16 Gauge Tubular Steel, Steel Seat Frame with Active Seat Suspension, High Density Seat Foam, Latex/ Lead (metal)/ and Mercury-Free, Flex Back, Wallsaver Frame Design, and Non-maring glides, 500lbs weight capacity, 10 year warranty on parts, labor and fabric. · Four (4) Foster Coffee Table – F4X02-SVR-HRM or equivalent:
· Dimensions: 16"H, 40"W, 22"D (tolerance +/- 1”)
Description of Services
2.1 Product Markings/Documentation/Tools
Items used after product acceptance used to identify or modify a particular product.
2.1.1 All documentation (Owner’s manuals, warranty information, assembly instructions, maintenance and care instructions, operating instructions, etc.) shall be provided to the 75th Medical Group.
2.2 Delivery Services
Transportation of all new products, loading, unloading, storage and all necessary movement from factory to final onsite set up; expected delivery 30 Days ADC.
2.2.1 All chairs will be delivered to the following buildings and installed in the specified locations. Locations of the tables will be determined prior to installation, they will be delivered to the medical logistics wharehouse in Bldg 570 until installed. The entire job must be completed in one duty day.
· Bldg 570 basement:
· Corridor LC04 (14 single seat)
· Corridor LC09 (2 single seat, 2 double seat)
· Waiting room L0623 (2 double seat)
· Bldg 570 first floor:
· Corridor MC32 (1 double seat)
· Waiting room MC17A (20 single seat, 10 double seat)
· Waiting room MC05 (3 single seat, 8 double seat)
· Waiting room MC01 (10 single seat, 10 double seat)
· Waiting room MC20 (10 single seat, 20 double seat)
· Bldg 570 second floor:
· Corridor UC19 (10 single seat)
· Corridor UC20 (1 double seat)
· Corridor UC21 (2 single seat)
· Bldg 570 Annex waiting room 105 (9 single seat, 10 double seat)
· Bldg 249 lobby C113 (18 single seat)
· Bldg 401 waiting room V1 (9 single seat).
· Bldg 545 waiting room R102 (9 single seat, 1 double seat)
· Bldg 546 waiting room R102 (8 single seat)
2.2.2 The contractor Project Manager shall be responsible for on-site coordination of all deliveries of their product, to include access to and operation of any equipment needed for unloading and movement of materials or product.
2.2.3 Contractor shall be responsible for determination of the appropriate delivery method and handling, based on site conditions.
2.2.4 Delivery shall be in accordance with the terms of the contractor's agreement. If the ordering activity designates an accelerated delivery schedule it will be identified in the contract.
2.2.5 The contractor shall be responsible for following all safety and security guidelines within the area in which work is being performed.
2.2.6 The contractor shall prevent its personnel from entering any area other than the designated work area.
2.2.7 Contractor shall be liable for any damage to government property caused by the contractor or their representatives.
2.2.8 Services shall include inspection of product for shipping damage. If damage is found it shall be the responsibility of the contractor to notify the authorized government representative and make all arrangements for replacement or repair of the item(s) damaged. Damaged items are not to be installed unless otherwise directed by the Government Acceptor.
2.2.9 The contractor shall maintain environmental controls when working in any Air Force facility, to include but not limited to turning off vehicle engines when parked.
2.2.10 The contractor shall remove all trash generated from the installation and dispose of it off base.
3.0 Base/Installation Specifics
3.1 All commercial trucks must enter through the Roy gate which is exit 338 off Interstate 15, for inspection. In accordance with the Installation 75th SFS base entry procedures must adhere to delivery vehicle process requirements mandated by the Wing. Once at the Roy gate the driver must have a visit request submitted through the Visitor Control Center and be able to show:
· A current Vehicle Registration
· A current Proof of Insurance
· A valid and current Driver's License for the driver
· A valid and current Driver's License or other State Identification for any passenger(s) in the vehicle.
3.2 Gate Procedures:
· Once at the Roy gate be prepared to exit the vehicle and allow for vehicle inspection. At this time you may ask for directions to your final destination.
· If the final destination requires the driver to leave the Roy gate and re-enter the base through an alternate gate, the pass received will be sufficient documentation for entry into both gates, within a pre-established time directed by the Security Forces representative.
· Arrival at the appropriate gate after the designated allotted time will result in non-admittance to the installation.
· This procedure will occur each time a commercial truck requests access to the base.
3.3 Current Bill of Lading (BOL) to get delivery driver and the vehicle on base. BOL must include the AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).
3.4 Personnel should have the following items when entering the installation:
· Current Registration for all vehicles
· Current Proof of Insurance for all vehicles
· Valid driver’s license for each driver
· Valid driver’s license or other state identification for each passenger
· Depending on the current Force Protection Condition, a government representative may be required to sponsor those people in person at Pass & ID, (building 408)
· The sponsor must state the guest's purpose and destination. It is the responsibility of the contractor to validate this need with their government POC prior to their arrival so as to coordinate the arrival time accordingly. Sponsors may request that a pass be applicable for multiple days if needed.
3.5 To sponsor 10 or more people, a typed list of those people must be submitted by the Government Acceptor in Word or Excel format, stating LAST NAME then FIRST NAME of each person. The sponsor must submit that list to Pass & ID building 408 no fewer than three days prior to the anticipated arrival. Names listed must be complete names, (no nicknames), and correspond to the names on the state ID for each person.
3.6 Mission Essential Services: Contractors are not considered mission essential.
3.7 Prohibitions: Firearms may not be brought on base under any circumstances. Individuals with criminal records are not allowed on base. Contractor personnel are prohibited from sponsoring guests onto the installation.
