STATEMENT OF WORK

FOR

UPGRADE OF HOFMANN BALANCE MACHINE

FOR

76 PMXG

OC-ALC/OBCA

07 September 2018

1.0 Scope:

The Contractor shall provide all labor, tools, equipment, material, parts, quality control and transportation necessary to provide installation upgrades for the Hofmann Balance Machines in building 214 and building 3001(listed in Appendix A) on the Oklahoma City- Air Logistics

Complex (OC-ALC). The Contractor shall also provide Personal Protective Equipment (PPE) and clothing, to include reflective clothing (belt, vest, etc); hearing and eye protection, to include prescription safety glasses; safety harnesses; fall protection equipment; lifting equipment and any incidentals necessary to perform the requirements of this Statement of Work (SOW).

1.1The Contractor shall be responsible for insuring all services, parts and components provided under this contract are in IAW the terms and conditions outlined in this SOW.

1.2 Item Description:

76 PMXG is requesting the one-time upgrade of eighteen (18) Hofmann balance Machines.

The Contractor shall provide, install and develop (as applicable) the items listed on Ap-pendix A and listed in below:

Upgrade four (4), belt, two-plane Hofmann horizontal balance machines and nine (9), single-plane, Hofmann vertical balance machines. The work consists of replacing existing

Hofmann retrofit HX electronic instrument system used for analyzing the imbalance in jet engine rotors at Tinker AFB, OK, Jet Engine maintenance facility. The new retrofit electronic measuring Instrument packages shall be installed and tested to manufacturer’s specifications and this Statement of Work (SOW). Equipment to be upgraded are listed as Items 1 to 13 in attached Appendix A.

Upgrade four (4) Hofmann horizontal balance machines and one (1) Hofmann vertical balancer machine of which two (2) were previous equipped with a HX system previously retrofitted to a PCX instrument design computer system and the other three (3) are PCX machines. The work consists of updating the existing Hofmann PCX systems to establish uniformity throughout for the purpose of analyzing the imbalance in jet engine rotors at

Tinker AFB, OK, Jet Engine maintenance facility. Equipment to be upgraded are in listed as Items 14 to 18 in attached Appendix A.

1.3. Required Services:

1.3.1. The contractor shall upgrade thirteen (13) Hofmann balance machines through replace-ment of the existing Hofmann’s retrofit HX electronics measuring instrumentation for turbines.

The new retrofit electric measure module shall be the OEM’s newest PCX electronic balance measuring instrumentation retrofit package. Items 1-13 in Appendix A.

1.3.2. The contractor shall upgrade five (5) Hofmann balance machines through refurbishment of their existing Hofmann’s PCX electronics measuring instrumentation for turbines. The new ret-rofit electric measure module shall be the OEM’s newest PCX electronic balance measuring in-strumentation package. Items 14-18 in Appendix A.

1.3.2.1. The Government intention is to reuse the PCH electronic cabinetry and just refurbish the internal components per this document.

1.3.3. PCX instrumentation shall consist of an industrial machine tool computer with a separately mounted monitor. The system shall have an A800.117 measuring board in order to convert the analytic data to a digital format for analyzing. Hofmann’s software programs shall use a Win-dows 7 or newer platform. Hofmann’s measuring instrument software shall be designed for aero-space application. The following features and support equipment shall be included:

1.3.3.1. Software shall include the standard aerospace software package with electronic zeroing

(used to trouble shoot machine and rotor interactions), tooling compensation (used for removing tooling for the equation), multi-run averaging (usable in and out of tooling compensation and allowing operators the ability to eliminate runs from the average), auto ranging, component bal-ancing, rotor memories, empirical calibration capability, remote angle correction. Software shall have the ability to set up multiple levels of security.

1.3.3.2. The following software addition to the standard aerospace software package which does not presently exist on the machines shall be added by the contractor: bode plot graph display, and

SAE ARP testing software.

1.4. Contractor shall furnish a printer package with each instrument package that is a conversion from a HX system bring the total to thirteen (13) printer packages (see Appendix A). A printer package shall consist of the required software, and a laser printer. Operators shall be able to printout unbalance, part specific, and customer information in a report or in a continuous mode.

1.4.1. One (1) printer shelf of sufficient in size and construction to support the printer shall be provide for a HMV/HXV-30.1J balancer (see Appendix A).

1.5. Contractor shall also replace all instrumentation cabling and hardware, and any pedestals’ connectors that are damage.

1.6. Certification of Repair to Balance Machine for Appendix A Items 1 through 18. Contractor shall perform the appropriate SAE ARP test for evaluating hard-bearing type of gas turbine ro-tors balance machines in the presence of Tinker PMEL certification personnel. The documents covering the evaluations shall be SAE ARP4048, 4162, and 5323.

1.7. The contractor shall be allowed to have one balance machine down at a time in a product shop and no more than two in the facility at a time.

1.8. The contractor shall provide the latest upgrades at the time of installation.

1.9. Approval by the contracting officer and the authorized Government representative is re-quired prior to the implementation of software changes, modifications, or updates when the con-tractor determines software changes, modifications or updates are required to correct defects or to ensure system performance conforms to production specifications.

1.10. The contractor shall provide two (2) USB Flash Drive (for Use on Hofmann equipment on-ly), and 2 sets of CDs mirroring the flash drives (Note: For use on government computers only!)

containing a complete set of documents. The following shall be included: Operations & Controls

Manuals, Equipment Overview, Basic Troubleshooting Guide, Preventive Maintenance with Service & Inspection Instruction, Mechanical & Electrical Drawings, Mechanical General

Arrangement Drawing, Mechanical Assembly Drawings w/ Part Numbers, Foundation Plan with

Installation Requirements, Electrical Schematic, Pneumatic Schematic (As Required), Hydraulic

Schematic (As Required), Spare Parts List & BOM, Recommended Spare Parts List, Complete

Bill of Materials Including Part Numbers.

1.11. Contractors shall provide training with each system to Original Equipment Manufacture specification.

1.12. Schedule. The contractor field service visit shall be scheduled as soon as possible after re-ceipt of this purchase order. All repairs shall be completed at the earliest possible date

1.13. Parts and Materials: The Contractor shall provide and use only new parts and material in performance of this SOW unless otherwise authorized by the CO.

1.14. Service Report: Immediately upon completion of the upgrade, the Contractor shall pro-vide a written Service Report to the Government SME. When necessary the Government SME will forward the service report to the Government POC within two (2) business days of receipt.

The Contractor shall provide, by fax or email, the final service report within five (5) business days to the Government POC. As a minimum, the final service report shall contain the following information:

name of the company requested service date contract number equipment ID# description of services provided additional services recommended (if any) printed name and signature of technician who performed the services printed name and signature of on-site Government SME or Government POC start and completion dates complete breakdown of the labor cost (hours and rate) materials and parts used details of any other charges printed name and signature stating only new parts and materials were used written verification statement that a performance check to verify operational condition was performed and that the equipment is in working order.

1.15. Corrective Action Report (CAR)/Complaint Contractor Response Time: The Contrac-tor shall provide a written response within two (2) business days which addresses the root cause, corrective action, and preventive actions in relation to the submitted CAR or Customer Com-plaint.

1.16. Maintenance Documentation: The Contractor shall notify the Government Subject Mat-ter Expert (SME) upon completion of each repair or similar requirement, IAW OC-ALC Operat-ing Instruction (OI) 21-203, Chapter 1, 2, and 3, as applicable.

1.17. Continuation of Essential Department of Defense (DoD) Contractor Services During

Crisis: This requirement has been determined not to be essential and does not require continued support during a crisis as defined in DFARS 252.237-7023.

1.18. Hours of Operation: The Contractor shall perform the work required under this contract during the following days and hours: Monday through Friday, 7:00am to 3:45pm. If work hours are required outside the hours stated, the Contractor shall contact the GPOC for approval from the CO. Upon approval from the CO, the Government Subject Matter Expert (SME) or GPOC must be present with the Contractor.

1.18.1. These dates and hours exclude Federal Holidays, base closures due to inclement weather, and furlough days, as applicable.

1.19. Contractor Personnel: The Contractor shall provide the name of the Contract Manager

(CM) and any alternates who shall act for the Contractor when the CM is absent. They shall be designated to the CO and GPOC, in writing, within five (5) business days after the contract award date. The Contractor shall provide telephone numbers and email addresses where the CM and alternates may be contacted at any time. The CM or alternate shall have full authority to act for the Contractor on all matters relating to operation of this contract at the OC-ALC on Tinker

Air Force Base (AFB). The CM or alternate shall be available to meet on the installation with

Government personnel designated by the CO to discuss problem areas. The Contractor shall provide a CM who can read, write, speak, and understand English fluently.

1.19.1. The Contractor and Contractor personnel shall be required to identify themselves as con-tractor personnel by introducing themselves or being introduced as Contractor personnel. The

Contractor personnel shall also display distinguishing badges or other visible identification for meetings with Government personnel or while performing work for the OC-ALC on Tinker

AFB. The Contractor and Contractor personnel shall identify themselves as Contractor employ-ees in telephone conversations and in formal and informal written correspondence.

1.20. Listing of Contractor Employees: The Contractor shall provide an initial listing of names and position titles of employees who will come to Tinker Air Force Base to the GPOC within five (5) business days after the contract award date. Within five (5) business days, the Contrac-tor shall also provide an updated listing of personnel to the GPOC when employees affecting the work of this contract change status or position.

1.20.1. The Contractor shall not employ persons on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security, and/or general well-being, or operational mission of the installation and its population. The Contractor shall not employ any person who is an employee of the United States (US) Government if the employment of the per-son would create a conflict of interest. The Contractor shall not employ any person who is an employee of the Department of the Air Force (AF), either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the Contractor shall not employ any person who is an employee of the Department of the AF if such employment would be contrary to the policies contained in Air Force Instruction

(AFI) 64-106, Air Force Industrial Labor Relations Activities. The Contractor shall be prohibit-ed from employing off-duty GPOCs who are providing surveillance on any contracts/sub-contracts awarded to the Contractor. All contract personnel, depending on particular condition, occasion, or place or job assignment shall be required to wear professional and suitable attire.

All Contractor personnel shall conduct themselves in a professional and courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of contrac-tor personnel in performance of this PWS shall be the responsibility and prerogative of the Con-tractor; however, the Contractor shall comply with the requirements set forth in this PWS and

Tinker AFB regulations concerning the conduct of employees as referenced herein. The Con-tractor shall employ and maintain a technically trained and experienced work force.

1.21. Environmental Protection & Safety: The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and require-ments regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions or regulation changes during the term of this contract.

1.21.1. Contractors shall be compliant Tinker Air Force Base (TAFB) Supplement to Air Force

Instruction (AFI) 32-7086, “Hazardous Materials Management”, Tinker AFB procedures per

Tinker Air Force Base Hazardous Waste Management Plan (TAFB HWMP, and Complex pro-cedures per Oklahoma City Air Logistics Complex (OC-ALC) Sup to AFI 32-7086, “Hazardous

Material Management” and OC-ALCI 32-101 “Air Quality Record Keeping Procedure”. Publi-cations are available digitally on e-Publishing website at www.e-publishing.af.mil for download-ing or ordering.

1.22. Hazardous Material (HAZMAT): If no HAZMAT will be brought on base or used in performance on this contract, the contractor shall certify “No HAZMAT will be brought on base” or “No HAZMAT Usage” in writing on company letterhead. This certification shall be provided to the GPOC within ten (10) days after award. Upon GPOC receipt of the certification, the requirements of this section no longer apply. Appropriate penalties for the contractor, or ad-ministrative and disciplinary action for the later shall be assessed if found in violation of above certification.

1.22.1. The Contractor shall not store chemicals or hazardous material (HAZMAT) on Tinker

AFB. Contractors shall be compliant with HAZMAT management procedures per Oklahoma

City Air Logistics Complex (OC-ALC) Supplement to AFI 32-7086, “Hazardous Materials

Management”. The Contractor shall maintain a log of all HAZMAT that is used on base. This usage log shall be provided to the GPOC monthly, the last business day of each month, or when http://www.e-publishing.af.mil/ the required work is completed. The GPOC will forward the records to the Unit Environmental

Coordinator (UEC) no later than the fourth (4th) day of the following month or the first (1st) sub-sequent business day.

1.23. Safety Data Sheet (SDS) and Labeling: Upon GPOC receipt of the “No HAZMAT Us-age” certification, the requirements of this section no longer apply.

1.23.1. The Contractor shall not transport, ship or use any chemical, material or substance used in the performance of this PWS on Tinker AFB before written approval is provided by 72

ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). The Contractor shall submit to the GPOC, ten (10) calendar days after contract award date all Safety Data Sheets

(SDSs) and a completed OC-ALC Form 152, SDS Information Sheet. The Contractor shall re-ceive approval from the GPOC prior to shipping any chemical, material or substance to Tinker

AFB. A copy of the manufacturer SDS shall accompany the shipment. Contractor shall comply with the effective dates for SDS requirements IAW the Occupational Safety and Health Admin-istrations (OSHA) Hazardous Communication Standard, Title 29, Code of Federal Regulation

(CFR) Part 1910.1200. The Contractor shall ensure containers of hazardous chemicals are la-beled with the following information, prior to shipping to Tinker AFB IAW OSHA Hazardous

Communication Standard, Title 29, CFR Part 1910.1200 (2012): product identifier; signal word;

hazard statement(s); precautionary statement(s); pictogram(s); and name, address and telephone number of the chemical manufacturer, importer, or other responsible party. The GPOC will for-ward all SDS submittals to 72 ABW/CEIE for approval. A current manufacturer SDS shall be submitted to the GPOC if the Contractor changes manufacturer or vendor for any chemicals that were prior approved. The Contractor shall maintain copies of manufacturer SDSs for all chemi-cals and SDSs shall be readily accessible at all times by Contractor personnel as well as govern-ment employees.

1.24. Hazardous Waste: If no HAZWASTE will be generated during performance of this con-tract, the contractor shall certify “NO Hazardous Waste Generation” in writing on company let-terhead. This certification shall be provided to the GPOC within ten (10) days after award. Up-on GPOC receipt of the “No Hazardous Waste Generation” certification, the requirements of this section no longer apply. Appropriate penalties for the contractor, or administrative and discipli-nary action for the later shall be assessed if found in violation of above certification.

1.24.1. The Contractor shall dispose of contaminated solid waste, spent or processed chemical solutions, used rags and hazardous waste in government furnished waste containers located in the shop or area the work is being performed. The Contractor shall notify the GPOC if the required waste containers are not available on site.

1.25. Discharges: Any discharges into Tinker’s industrial waste treatment plant (IWTP) or sani-tary sewer shall be approved and done IAW Tinker Industrial Waste Water Discharge Permit as instructed by 72 ABW/CEIE, Natural Infrastructure Management (Environmental Compliance).

Notice of intent documentation shall be provided to the GPOC within ten (10) days after award.

The GPOC will submit the required notice of intent documentation to the UEC for 72

ABW/CEIE approval. The Contractor shall not discharge into IWTP and sanitary sewer prior to approval issued by 72 ABW/CEIE. The Contractor shall maintain records of written approval issued by 72 ABW/CEIE for discharges into IWTP and sanitary sewer.

1.26. Off-Site Discharges: For discharges outside of Tinker AFB, the Contractor shall ensure waste water discharges, that may contain pollutants resulting from any contracted service, go to a state or federal permitted treatment facility such as a municipal Publicly Owned Treatment

Works (POTW). The Contractor shall provide a letter ten (10) calendar days after contract award date to the GPOC from the POTW that states the POTW is permitted for all pollutants that the Contractor may discharge during the contracted service.

1.27. Spills: Upon occurrence of any spill that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident. When pos-sible, the Contractor shall use a base telephone to receive help from Tinker AFB 911. If using a cell phone, the Contractor shall request Tinker AFB 911. The Contractor shall immediately re-port all environmental violations to the GPOC and UEC. The Contractor shall provide five (5) copies of a typed or written report about the incident within four (4) clock hours as follows:

1.27.1. One (1) copy to CO, two (2) copies to the GPOC, a copy for UEC and one (1) copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information:

1.) Name and contact number of person reporting the spill

2.) Date and time of incident

3.) Location and source of spill

4.) Substance or pollutant spilled

5.) Amount spilled and rate of discharge

6.) Any damages or injuries involved

7.) Extent of area impacted

8.) Potential hazards

9.) Actions taken

10.) Organizations contacted

11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equip-ment, etc.) and whether 911 were called and which agencies responded.

1.28. Environmental Training: Upon GPOC receipt of the “No Hazardous Waste Generation” certification, the requirements of this section no longer apply.

1.28.1. Contractor personnel performing duties in the service of this PWS shall have training that acquaints the person with the details of Resource Conservation Recovery Act (RCRA) regu-lations in 40 CFR 260 through 279; applicable State Laws and regulations; DoD, Air Force and local requirements. This training can be obtained through creditable training institutions or com-panies. All training shall be approved by 72 ABW/CEIE, Natural Infrastructure Management

(Environmental Compliance). RCRA training is available at http://www.natlenvtrainers.com/RCRA-hazardous-waste.htm;

Prior to obtaining training, the Contractor shall provide the name of the training institution or company and course description to the GPOC for approval within ten (10) calendar days after contract award date. Once written approval is received from the GPOC, the Contractor shall provide the environmental training certificates for employees performing duties in the service of this PWS to the GPOC, within thirty (30) calendar days after contract award date. In addition to the required environmental training, Contractors shall be compliant with Tinker AFB procedures per TAFB

HWMP, “Hazardous Waste Management”.

1.29. Forms, Technical Orders (T.O.), AFI, Air Force Material Command Instructions

(AFMCI), and Publications: The Contractor shall obtain the forms and publications expressly required to perform the work in this contract via the internet at www.e-publishing.af.mil. In the event a form or publication is not available online, the GPOC will provide the form, instruction, or publication to the Contractor upon the Contractor’s written request (e-mail is acceptable). In the event, laws, regulations, T.O.’s, AFI’s, or AFMCI’s change during the term of this contract, the Contractor shall be required to comply as the changes come into effect. Contractors deliver-ing or handling official United States Air Force (USAF) records shall comply with the applicable records management regulations in AFI 33-322, Chapter 3, in its entirety.

1.30. Housekeeping: The Contractor shall keep the work areas clean and neat IAW industry standards, OSHA, fire and safety standards, and this PWS.

1.31. Foreign Object Damage (FOD) and Dropped Object Prevention: Contractor personnel who work in or travel though areas near operational and production aircraft, engines, munitions, missiles, drones, space systems, support equipment, Aerospace Ground Equipment (AGE), train-ers, and the Contactor personnel operating vehicles and equipment on flight lines, runways, tax-iways, parking ramps, and in aircraft hangers and maintenance areas are required to receive the

FOD/DOP training and to comply with AFI 36-2232, AFMC Supplement 1, paragraph 9.2., and

A4.3.4. (Initial)/A4.3.5. (Refresher). The Contractor and Contractor personnel shall acknowledge the FOD/DOP training either by viewing the Digital Versatile Disc (DVD) provid-ed by the GPOC or the You Tube site, http://www.youtube.com/watch?v=tYcbodjFPnM. The

Contractor shall provide a contractor devised certificate to the GPOC for each employee within five (5) business days after the contract award date. The certificates shall have the title of the training, the name of the person who received the training, and the date of the training. The Con-tractor shall also ensure all new employees view the FOD/DOP training and provide a contractor devised certificate of training to the GPOC prior to the employees starting work in the OC-ALC shops. On an annual basis, the Contractor and Contractor personnel shall be required to review the FOD/DOP training and to provide an updated contractor devised certificate of training to the

GPOC within five (5) business days of the new period of performance (PoP) start date.

1.32. Tool Control and Accountability: While performing contract services in or around

Maintenance Production Shops, which includes ramp areas, the Contractor shall comply with

AFI 21-101_AFMC Supplement 1, 21-101, Chapter 10 (all paragraphs) and OC-ALCI 21-112, paragraph 14.1. and 14.5.3. The Contractor shall provide the completed MXRIWRS Form 032 prior to performing work in the OC-ALC industrial areas. The MXRIWRS Form 032 is availa-ble from the GPOC. The Contractor shall provide a Contractor Tool Listing, MXRIWRS Form http://www.e-publishing.af.mil/ http://www.youtube.com/watch?v=tYcbodjFPnM

032, to the GPOC within five (5) business days after Upgrade of Hofmann Balance Machine. The

Contractor or Contractor’s personnel shall inventory the container or bag of tools at the begin-ning of each work day and at the end of each work day. The Contractor personnel shall immedi-ately notify the GPOC of any tools that are missing or lost from the work area or container (e.g.

tool or job box or tool bag). The GPOC will report any missing Contractor’s tools which have not been located within one (1) hour after notification to the appropriate Government personnel.

The Contractor shall obtain the Form 32 from the GPOC. The CM shall ensure each employee on the job site receives a briefing from the GPOC about Housekeeping, FOD/DOP, and Tool

Control prior to starting work in OC-ALC Industrial Areas.

1.33. Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Tinker AFB per AFMAN 31-116, in its entirety, “Motor Vehicle Traffic Supervision”. The Contractor shall comply with Tinker AFBI 24-302, in its entirety for the operation of POVs within Tinker AFB.

1.34. Visitors Pass: The Contractor shall contact the GPOC seven (7) business days before ar-riving on Tinker AFB in order to make arrangements for Visitors Passes. The GPOC will assign a sponsor for the Contractor from within the organization. The Contractor and the sponsor shall arrange to meet at Pass and Registration, Building 6611 at the Tinker Gate which is South of I-40 off of Air Depot. The Contractor shall provide two (2) forms of identification, one of which shall be a picture ID, to the 72 SFS. The 72 SFS will provide an AF Form 75, Visitors Pass, which will be valid for up to thirty (30) calendar days.

1.35. Foreign Nationals: The Contractor shall notify the Government POC before sending a

Foreign National representative to perform services. Special rules apply for foreign nationals visiting a US Government facility.

1.36. Safety and Health on Government Installations: While performing work under this contract on a Government installation, the Contractor shall comply with federal, state and local regulations, and AFI 91-203, “Air Force Consolidated Occupational Safety Instruction”, as es-tablished by this contract and Appendix C. The Contractor shall notify the CO, GPOC, and

Government Contract Administrator (CA) within one (1) business day of any injuries that occur to contractor personnel while they are on the installation. The Contractor shall notify the CO, GPOC, and Government CA within one (1) business day of damage to Government property or equipment during the execution of the contract.

1.37. Emergency Procedures: The Contractor personnel shall follow the direction of the Gov-ernment personnel with regard to emergency procedures, such as fire, tornado, active shooter, bomb threats, or other emergency. It is the responsibility of the Contractor to provide guidance and to establish procedures for its employees to respond to an emergency. It is the Contractor’s responsibility to account for their employees following an emergency evacuation.
