STATEMENT OF WORK (SOW)
FOR
ZEISS EVO MA 15 LaB6 SCANNING ELECTRON MICROSCOPE With
SmartPI System
22 March 2017

Statement of Work On
Zeiss EVO MA 15 LaB6 Scanning Electron Microscope with SmartPI System
1.0. Scope – The Contractor shall provide all labor, tools, equipment, test equipment, materials, parts, personal protective equipment (PPE), hearing and eye protection, transportation and other incidentals necessary to travel to Tinker AFB OK, for the delivery, installation, test of equipment, system certification, calibration, provide operator and maintenance (O&M) manuals, and training of personnel on the Zeiss EVO MA 15 LaB6 Scanning Electron Microscope (SEM) with SmartPI System. Any required maintenance shall be provided by Contractor as guaranteed in their one-year warranty. All services and parts are in accordance with the original equipment manufacturer (OEM) and are in accordance with (IAW) the terms and conditions outlined herein.
1.1. Item Description: The Contractor shall provide:
-One (1) Zeiss EVO MA 15 LaB6 SEM with SmartPI (see Technical Specifications Attachment for Parts to be included) -Delivery and Installation -Test and Start up for Zeiss EVO MA 15 LaB6 SEM with SmartPI -System Certification -System Calibration -Operator and Maintenance (O&M) manuals -Training of personnel on Zeiss EVO MA 15 LaB6 SEM with SmartPI
1.2. Technical Requirements: Key features and requirements of the SEM-EDS. All Technical Requirements are listed on the Technical Specifications Attachment. Acceptance will be performed during installation to guarantee all line items were delivered and the instrument performs as advertised.
1.3. Delivery –Prior to shipping, the vendor shall contact the contract monitor to provide notification of a delivery date. The supplier is responsible for coordinating with the Government Point of contact for instrument delivery and escorting to the final location. The supplier is responsible for supplying all required installation of hardware, software, and fluids (if any).
1.4. Installation – The Government is responsible for allocating space for the unit to be installed including utilities. It is the responsibility of the Contractor to connect all electrical connections along with filling all fluids necessary to operate chillers, pumps, etc. It is also the responsibility of the supplier to have analytical tools installed and functional on the instrument. All installation procedures should be completed within 20 calendar days of shipping. The Contractor shall supply any unique supplies required for the operation of the instrument.
1.5. Testing - The tests required for the start up of the Zeiss EVO MA 15 LaB6 SEM with SmartPI shall be witnessed and approved by a representative of the Chemical Analysis Section (76 MXSS/MXDTA). Calibration tools and standards shall be provided by the Contractor. The vendor shall show System Certification. The final inspection and acceptance shall be performed by the Contractor and contract monitor no later than 30 days after installation (including the EDS analytical tools attached to the SEM) or as agreed by the Contractor and Contract Monitor.
1.6. Training – The Contractor shall provide a on-site training on the operation and use of the new SEM-EDS including advanced applications. Training will be performed within 2 months after
Zeiss SEM 3 installation.
1.7. Warranty – At minimum 1 year complete package parts/labor/software warranty.
1.8. Service Report: Immediately upon completion of installation and before leaving the OC-ALC, the Contractor shall provide a written Service Report to the Government Technical POC or COR.
The Government Technical POC will forward the service report to the COR within two (2) business days of receipt. The Contractor shall provide, by fax or email, the final service report within five (5) business days to the COR. The Government will not pay an invoice submitted until an acceptable service report has been received from the Contractor. As a minimum, the final service report shall contain the following information:
Name of the company Requested service date Contract number Equipment ID# Description of services provided Additional services recommended (if any) Printed name and signature of technician who performed the services Printed name and signature of on-site Government identified Technical POC or COR Start and completion dates Materials and parts used Printed name and signature stating only new parts and materials were used Written verification statement that a performance check to verify operational condition was performed and that the equipment is in working order.
1.9. Environmental Protection & Safety: The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, instructions, and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions, or regulation changes during the term of this contract. Contractors shall be compliant with the Tinker AFB Supplement to Air Force Instruction (AFI) 32-7086, “Hazardous Materials Management”, Tinker AFB procedures per Tinker AFB Instruction (TAFBI) 32-7004, “Hazardous Waste Management”, and Complex procedures per OC-ALC Sup to AFI 32-7086, “Hazardous Material Management” and OC-ALC Instruction (OC-ALCI) 32-101 “Air Quality Record Keeping Procedure”. Publications are available digitally on the e-Publishing website at www.e-publishing.af.mil for downloading or ordering. Housekeeping: The Contractor shall keep the work areas clean and neat IAW Industry Standards, OSHA, Fire and Safety Standards and this
SOW.
1.10. Spills: Upon occurrence of any spill that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident. When possible, the Contractor shall use a base telephone to receive help from Tinker AFB 911. If using a cell phone, the Contractor shall request Tinker AFB 911. The Contractor shall immediately report all environmental violations to the contract monitor and UEC. The Contractor shall provide five (5) copies of a typed or written report about the incident within four (4) clock hours as follows:
One (1) copy to the CO, two (2) copies to the Government POC, a copy for UEC and one (1) copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information:
Name and contact number of person reporting the spill
Zeiss SEM 4
Date and time of incident Location and source of spill Substance or pollutant spilled Amount spilled and rate of discharge Any damages or injuries involved Extent of area impacted Potential hazards Actions taken
Organizations contacted Remarks Section - that addresses the cause of the spill (e.g. human error, equipment, etc.)
and whether 911 was called and which agencies responded.
1.11. Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Tinker AFB per AFMAN 31-116, in its entirety, “Motor Vehicle Traffic Supervision”. The Contractor shall comply with Tinker AFBI 24-302, in its entirety for the operation of POVs within Tinker AFB.
1.12. Visitors Pass: The Contractor shall contact the Government POC seven (7) business days before arriving on Tinker AFB in order to make arrangements for Visitors Passes. The Government POC will assign a sponsor for the Contractor from within the organization. The Contractor and the sponsor shall arrange to meet at Pass and Registration, Building 6611 at the Tinker Gate which is South of I-40 off of Air Depot. The Contractor shall provide two (2) forms of identification, one of which shall be a picture ID, to the 72 SFS. The 72 SFS will provide an AF Form 75, Visitors Pass, which will be valid for up to thirty (30) calendar days.
1.13. Foreign Nationals: The Contractor shall notify the Government POC before sending a Foreign National representative to perform services. Special rules apply for foreign nationals visiting a US Government facility.
1.14. Safety and Health: While performing work under this contract the contractor shall comply with all applicable federal, state and local regulations regarding occupational safety and health. The contractor shall notify the Contracting Officer (CO), within eight (8) hours of any damage to government property where the dollar value exceeds $500,000.00 and within two workdays, for any damage to government property less than $500,000.00 during the execution of the contract.
Mishap notifications shall contain, as a minimum, the following information:
a. Contract, Contract Number, Name and Title of Person(s) Reporting
b. Date, Time and exact location of accident/incident
c. Brief Narrative of accident/incident (Events leading to accident/incident)
d. Cause of accident/incident, if known
e. Estimated cost of accident/incident (material and labor to repair/replace)
f. Nomenclature of equipment and personnel involved in accident/incident
g. Corrective actions (taken or proposed)
h. Other pertinent information If requested by the designated CO, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the Procuring Safety Office. Contractors will not dispose of contract data related to a mishap until notified to do so by the CO.
1.15. Emergency Procedures: The Contractor personnel shall follow the direction of the Government personnel with regard to emergency procedures, such as fire, tornado, active shooter, bomb threats, or other emergency. It is the responsibility of the Contractor to provide guidance and
Zeiss SEM 5 to establish procedures for its employees to respond to an emergency. It is the Contractor’s responsibility to account for their employees following an emergency evacuation. The Contractor personnel shall participate with the Government personnel in all fire and tornado drills at no additional cost to the Government. No hazmat shall be used on Tinker AFB for this SOW.
