AMENDMENT

The purpose of this amendment is to extend the solicitation, answer questions submitted by interested offerors, and schedule a site visit.

First: The solicitation closing date is hereby extended as follows:

Second: The following questions were submitted:

1. Which floor is autoclave currently on? I presume Air Force desires for a contractor who can lift it out of the building where it is currently located. How high is the ceiling? Who is responsible for the roof disassemble?

What type of roofing material currently used?

Government Response: The autoclave currently sits on a slab that is approximately 8 inches higher than ground level. The autoclave is in an enclosure attached to the south side of B2211. The height of the lowest part of the enclosure is around 11 feet. The ceiling will not have to be removed. The Government will be responsible for removing the wall that the autoclave will be moved through.

2. What are the dimensions, size, level of the building where Autoclave will be moved into? I presume it is a single story building, what are capabilities of that structure?

Government Response: Drawing 14D008 Relocate Composite Lab Autoclave2.pdf has the building diagram along with aisle widths.

3. 1.1.2. The Autoclave shall be set up and bolted to the floor IAW Appendix D. The Contractor shall connect all accessory equipment/interconnections associated wiring and piping: to include power panel, vacuum system, vacuum station, control panel, cooling plumbing, ventilation. Contractor shall connect control wiring (analog signals and digital signals) from the Autoclave control panel to the data acquisition and control systems.

I would like to verify that the items to be moved are (as show in the Appendix D):

- "Power Panel 480V, 3PH, 400Amps OC10522"

- "Lab Autoclave OC#10522"

- "Upgraded Control Panel"

- "Vacuum Sub-System"

Government Response: There is control cabinet, server cabinet, autoclave power cabinet, vacuum pump with receiver, autoclave mechanical (annotated as a square box attached to the autoclave) and the water softener and brine tank that will be moved with the autoclave.

4. In particular I want to verify (because asked by sub-contractors) items such as FUME HOODS Explosion proof oven, etc., are NOT part of this bid. In other words items on the other side of the wall from the Autoclave.

Government Response: This contract is only for the autoclave and the items necessary to operate it, including the water softener. The fume hoods, explosion proof ovens etc. will not be part of the autoclave relocation contract.

5. Appendix A – Do we need to fill any information in or return this document with the proposal?

Government Response: It is not required to send the appendix back with the proposal.

6. Safety Appendix C – Do we need to do anything with this document or return it with the proposal? We understand we need to follow these guide lines, but we are not sure if we need to sign anything and return it.

Government Response: It is not required to send the appendix back with the proposal.

7. Wage Determination – Do we need to do anything with this document or return it with the proposal?

Government Response: The wage determination is to show the minimum wage that employees get paid for specific work.

8. Does the Government have pictures and/or prints of the building the autoclave is currently in and the building the autoclave will be moving to that shows clear dimensions of aisle ways and overhead doors to move the autoclave outside to be loaded and inside the new building? We need to see how we will be removing the autoclave and how the autoclave will go into the new building.

Government Response: There is an attachment named Autoclave pictures that can be downloaded off FBO with this amendment.

9. Can we get pictures and estimated overall dimensions of all the equipment that we are expected to move? The print, Appendix D, shows several pieces of equipment. We are assuming that we are moving the lab autoclave, work table with PC, upgraded control panel, power panel, vacuum sub-system, and two water softener tanks.

Please let us know if there are other items we need to move.

Government Response: The autoclave is approximately 20 to 24 feet long and 9 feet wide and around 20,000 lbs. The “AUTOCALVE MECHANICAL” will have to be disconnected from the autoclave.

10. When reconnecting the autoclave typically the customer brings power, water, nitrogen, etc. within 10’ to 20’ of the connecting point on the autoclave. Are we to assume this will be correct?

Government Response: The contract specifies that Tinker will be responsible for bringing power, nitrogen and other utilities within 25 feet of the autoclave location.

11. Combined Synopsis Solicitation FA8125-15-R-0011 – I see where at the beginning of the document it says to sign and return with the proposal, however, do we need to fill in all the other blanks throughout the document such as tax identification number?

Government Response: Yes, all blanks throughout the document need be filled out; clauses and provisions.

12. Do you have electrical prints of the autoclave and are the wires clearly marked?

Government Response: We do not have electrical prints of the autoclave and the peripheral equipment. It’s the contractor’s responsibility to mark the wires and do what is necessary to reconfigure the autoclave and the equipment associated with it.

13. Existing autoclave mechanical, please provide description/detail of items to include (e.g. steam separators, pumps, etc.)

Government Response: Shown in Autoclave pictures.

14. Vacuum sub system, please provide description/detail of items to include?

Government Response: The vacuum system consists of two sixty gallon ASME rated Tanks and one Busch vacuum pump (#RC0025-A005-1001) along with the associated plumbing and electrical components necessary to operate with the autoclave.

15. Water tanks: please provide description/detail of items to include?

Government Response: The water softener tank is a North Star PA322S. Includes the resin tank and associated plumbing and electrical components necessary to operate with the autoclave.

16. Cooling water source system, please provide description/detail of items to include? (e.g. cooling tower)

Government Response: The cooling water is potable water that is cycled through a water softener and discharged into IW drains.

17. How far is autoclave move?

Government Response: Approx. 3 miles.

18. Is floor more/less level (or step up (e.g. dock, curb, split level)?

Government Response: Level.

19. Current location termination (hanging) and site remediation (e.g. fill holes in floor) requirements

Government Response: Clear, no repairs necessary.

20. New location - floor loading, anchor bolts scope/requirements

Government Response: Government is responsible to ensure the floor will support the load. The contractor is responsible to anchored/bolted to the floor (PWS 1.1.2)

21. Distance needed to run from customer supplied connection to autoclave (Vent, Nitrogen, Water supply, Water return, Compressed air, electrical 480V)?

Government Response: 25 feet.

22. What are the crane requirements for Tinker AFB.

Govrnment Response: Tinker AFB does not have any crane requirements.

Third: A site vist for the Autoclave Relocation will be conducted on 19 March 2015 at 9:00am, CST. Please see the site visit agenda below for additional details.

9:00am – All attendees shall meet at the Midwest City Welcome Center located North of Tinker AFB on the Southside of 29th Street between Air Depot and Midwest Boulevard.

9:00am - 9:15am- Attendees sign-in and obtain visitor passes.

9:15am - Provided transportation will depart Midwest City Welcome Center and go to Tinker AFB building 2211 10:15am We will depart from building 2211 and head to building 9001.

11:00pm – Conclude site visit and return attendees to Midwest City Welcome Center

252.232-7003 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS AND RECEIVING REPORTS

(JUNE 2012)

(a) Definitions. As used in this clause–

(1) Contract financing payment and invoice payment have the meanings given in section 32.001 of the Federal Acquisition Regulation.

(2) Electronic form means any automated system that transmits information electronically from the initiating system to all affected systems. Facsimile, e-mail, and scanned documents are not acceptable electronic forms for submission of payment requests. However, scanned documents are acceptable when they are part of a submission of a payment request made using Wide Area WorkFlow (WAWF) or another electronic form authorized by the Contracting Officer.

(3) Payment request means any request for contract financing payment or invoice payment submitted by the Contractor under this contract.

(4) Receiving report means the data required by the clause at 252.246-7000, Material Inspection and Receiving Report.

(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests and receiving reports using WAWF, in one of the following electronic formats that WAWF accepts: Electronic Data Interchange, Secure File Transfer Protocol, or World Wide Web input. Information regarding WAWF is available on the Internet at https://wawf.eb.mil/.

(c) The Contractor may submit a payment request and receiving report using other than WAWF only when–

(1) The Contracting Officer administering the contract for payment has determined, in writing, that electronic submission would be unduly burdensome to the Contractor. In such cases, the Contractor shall include a copy of the Contracting Officer's determination with each request for payment;

(2) DoD makes payment for commercial transportation services provided under a Government rate tender or a contract for transportation services using a DoD-approved electronic third party payment system or other exempted vendor payment/invoicing system (e.g., PowerTrack, Transportation Financial Management System, and Cargo and Billing System);

(3) DoD makes payment for rendered health care services using the TRICARE Encounter Data System (TEDS) as the electronic format; or

(4) When the Governmentwide commercial purchase card is used as the method of payment, only submission of the receiving report in electronic form is required.

(d) The Contractor shall submit any non-electronic payment requests using the method or methods specified in Section G of the contract.

(e) In addition to the requirements of this clause, the Contractor shall meet the requirements of the appropriate payment clauses in this contract when submitting payments requests.

NOTE : CONTRACT HOLIDAYS

(a) The prices/costs in Section B of the contract holiday include observances; accordingly, the Government will not be billed for such holidays, except when services are required by the Government and are actually performed on a holiday. Holidays in addition to those reflected in this contract, which are designated by the Government, will be billable provided the assigned Contractor employee was available for performance and was precluded from such performance.

(b) The following days are contract holidays: New Years Day, Martin Luther King, Jr. Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day.

NOTE : CONTRACTOR ID BADGE REQUIREMENTS

Contractors working on Tinker AFB shall comply with all procedures outlined in TAFBI 31-101, AFI 10-245 and TAFBP 31-101. OF SPECIAL NOTE, in accordance with TAFBI 31-101 para 5.3, all contractors/contractor personnel will be assessed a charge of $50.00 for each badge lost, willfully damaged, or not surrendered to Pass and Registration. No charge will be made on a replacement required due to normal wear. Personnel will report to Accounting and Finance, Accounts Receivable Unit and a DD Form 1131, Cash Collection Voucher, will be prepared and presented to Pass and Registration prior to issuing a replacement identification. Identification will be surrendered to Security Forces personnel upon demand for the purpose of identification, investigation, abuse, misuse, or for any other reason necessary. Civilian and military retirees who conduct commercial business on base must obtain a badge through their place of employment. ALSO OF SPECIAL NOTE, in accordance with TAFBI 31-101 para 7, all contractors/contractor personnel shall be subject to background checks as deemed necessary.

NOTE : TRUCK ENTRY GATE NOTICE

IMPORTANT DELIVERY INFORMATION

Tinker Air Force Base has constructed a “Truck Entry Gate” (Gate 33), which is located at the corner of 59th Street and Air Depot Blvd (southwest corner of the base). The gate will be opened beginning 15 September 2004.

Gate 33 shall be used for all deliveries by truck to this base.

(Trucks will not be allowed to gain access through any other gate.)

Since your firm’s delivery on this order is affected, please advise your delivery carrier to use the new gate. This should minimize the impact of your delivery.

DELIVERIES TO BUILDING 3001 (B / 3001)

Be advised that delivery vehicles destined for B / 3001 are prohibited from using “Staff Drive”.

Delivery vehicles may make deliveries only in designated unloading areas.

Since your firm’s delivery may be affected, ensure that your delivery carrier receives proper instructions on delivery / unloading requirements for B / 3001. Unloading in designated unloading areas will minimize any inconvenience, and, attendant ramifications, which may accrue, to your delivery / delivery carrier for failure to adhere to this requirement.

NOTE : REIMBURSABLE COSTS

Reimbursable costs:

a. Costs specified as reimbursable under this contract shall be paid in accordance with the payments clause.

Requests for payment of reimbursable costs shall be supported by copies of paid invoices. The contractor shall be paid the amount of the invoice less any discounts offered to the contractor, whether taken or not. Requests for reimbursement shall be made only after those reimbursable items have been incorporated into the work and shall not include additional charges or fees.

b. Purchases of services, parts or materials subject to reimbursement under this contract shall utilize competition to the maximum practical extent and shall be made at prices as low or lower than those prices available to any other contractor. The contractor shall maintain during performance of this contract and for one year thereafter, records of competition and/or subcontractor commercial price lists, available to the general public, for all costs reimbursed under this contract.

c. Reimbursement of items under the contract for which the contractor is a regular dealer or manufacturer of those items shall be at prices offered to the contractor's most favored customer. Invoices for payment shall be supported by commercial price lists available to the general public, covering each item invoiced.

d. The contractor shall immediately notify the contracting officer if, during performance of the contract. The remaining funds for reimbursable line items are not sufficient to complete the project. The contractor shall not incur any costs in performance of the contract that would also cause the contractor to exceed the funds remaining for any reimbursable contract line items until the government has made funds available to increase the reimbursable line items.

ALTERNATE I (OCT 2014)

An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (b) through (i) of this provision.

(a) Definitions. As used in this provision:

"Emerging small business" means a small business concern whose size is no greater than 50 percent of the numerical size standard for the NAICS code designated.

"Forced or indentured child labor" means all work or service-

(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.

Inverted domestic corporation means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).

“Manufactured end product” means any end product in Federal Supply Classes (FSC) 1000-9999, except—

(1) FSC 5510, Lumber and Related Basic Wood Materials;

(2) Federal Supply Group (FSG) 87, Agricultural Supplies;

(3) FSG 88, Live Animals;

(4) FSG 89, Food and Related Consumables;

(5) FSC 9410, Crude Grades of Plant Materials;

(6) FSC 9430, Miscellaneous Crude Animal Products, Inedible;

(7) FSC 9440, Miscellaneous Crude Agricultural and Forestry Products;

(8) FSC 9610, Ores;

“Place of manufacture” means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—

(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;

(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;

(3) Consist of providing goods or services to marginalized populations of Sudan;

(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;

(5) Consist of providing goods or services that are used only to promote health or education; or

(6) Have been voluntarily suspended.

Sensitive technology—

(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically--

(i) To restrict the free flow of unbiased information in Iran; or

(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and

(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).

Service-disabled veteran-owned small business concern—

(1) Means a small business concern--

(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and

(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.

(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).

"Small business concern" means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation.

Small disadvantaged business concern, consistent with 13 CFR 124.1002, means a small business concern under the size standard applicable to the acquisition, that--

(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by--

(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and

(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2); and

(2) The management and daily business operations of which are controlled (as defined at 13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.

Subsidiary means an entity in which more than 50 percent of the entity is owned--

(1) Directly by a parent corporation; or

(2) Through another subsidiary of a parent corporation.

Veteran-owned small business concern means a small business concern—

(B) The taxpayer is delinquent in making payment. A taxpayer is delinquent if the taxpayer has failed to pay the tax liability when full payment was due and required. A taxpayer is not delinquent in cases where enforced collection action is precluded.

(ii) Examples.

(A) The taxpayer has received a statutory notice of deficiency, under I.R.C. Sec. 6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(B) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. Sec. 6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(C) The taxpayer has entered into an installment agreement pursuant to I.R.C. Sec. 6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.

(D) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. 362 (the Bankruptcy Code).

(i) Certification Regarding Knowledge of Child Labor for Listed End Products (Executive Order 13126).
