DATA ITEM DESCRIPTION
Title: TEST INFORMATION SHEET (TIS)
Number: DI-QCIC-81536A Approved Date: 20180227 AMSC Number: F9890 Limitation: N/A DTIC Applicable: No GIDEP Applicable: No Preparing Activity: 11 (AFLCMC/WWM) Project Number: QCIC-2015-012 Applicable Forms: N/A Use/Relationship: The Test Information Sheet (TIS) is used to provide detailed planning information necessary to review the test program specifics, conduct the tests, record test data, and evaluate the test results.
a. This Data Item Description (DID) contains the format and content preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract.
b. This DID is applicable to engineering (developmental), preliminary qualification, qualification, and acceptance testing.
c. This DID interrelates with DI-FACR-80810, Test Facility Requirements Document (TFRD).
d. This DID supersedes DI-QCIC-81536.

Requirements:
1. Reference documents. The applicable issue of the documents cited herein, including their approval dates and dates of any applicable amendments, notices, and revisions, shall be as specified in the contract.
2. Format. Contractor format is acceptable.
3. Content. The TIS shall include the title and number of the specification paragraph (if applicable), title of test, functional category, and whether flight or ground test. Each TIS shall include the following information, to the extent applicable on the particular test:
a. The prioritized objectives, test conditions, success and failure criteria for each objective, and the step-by-step procedures to be followed. These procedures will be to the level of detail to allow reproducibility of testing.
b. A description of each test item configuration as it relates to the system requirements, specification, and to the particular test. Examples of information include:
(1) The number of items to be tested and the number of times each item is to be tested.
(2) Description of all systems, subsystems, components, and their performance features having a direct bearing on the purpose of the test.
(3) Test parameters (e.g., velocity, altitude, flight maneuvers, power settings, weight changes, and center of gravity travel, etc.).

(4) Test profiles, performance predictions, anticipated system restrictions or limitations (redlines), with justification for each. Include specific high-risks to personnel and material or tests outside of normal parameters.
(5) Identification and location of all non-production equipment – data pickups, console controls, and recording equipment. For all non-production equipment used to simulate or substitute for production equipment (e.g., cables, etc.), identify the difference in salient characteristics (e.g., shielding, size, etc.).
(6) Test apparatus descriptions and method of installation.
(7) Safety considerations and hazardous operating procedures (e.g., propellant, electrical, high pressure, etc.).
(8) Security, including special access considerations.
c. Test location(s), schedule, phasing, and duration of tests.
d. Description of support equipment required from other sources, such as government furnished equipment or associated contractor equipment.
e. Extent of government, prime contractor, and associated contractor participation.
f. Type, number and description of data recording devices to be used.
g. Data to be recorded, data reduction and analysis procedures, and data uncertainty criteria.
Include test results presented in terms which can be correlated with specification requirements.
h. Toxic materials used in or resulting from the test and their respective handling and disposal requirements and procedures.
i. Proper location of warnings, cautions, and notes in the test procedures.
j. Certification(s) for personnel operating facility equipment.
k. Recovery methodology in the event of test failure, including restart criteria (e.g., start over, start at last step successfully completed, etc.).
