PERFORMANCE WORK STATEMENT (PWS)

FOR

Aircrew Alerting System/Klaxon Alarms Expansion

Building 989

Version 1 24 July 2017

1. DESCRIPTION OF SERVICES:

The work shall consist of furnishing all plant, labor, equipment, instruments, materials, transportation, and incidentals required to expand the capabilities of the Aircrew Alerting System (AAS) on Tinker Air Force Base (TAFB), Oklahoma. The system is located in Buildings (B) 820, B989, and B1066 and provide both audible/visual notifications 24 hours/day, 7 days/week at fixed locations within the buildings. These audible and visual devices serve as the primary means of alerting aircrew members in the event of air crew activation. These buildings are located in a restricted area on TAFB.

2. BASIC SERVICES:

2.1 Upon completion of the service, the AAS shall be a fully operational and functional system that is complete, finished, and ready for use by the Government and meets all of the following PWS requirements.

2.2 The Contractor shall install additional notification devices to match existing equipment in the sleeping quarters in B989.

2.3 The Contractor shall install 4 Local Operating Console (LOCs). One (1) adjacent to the main panel in B989, one (1) in the control tower, one (1) in B1066, and one (1) in B820.

2.4. The Contractor shall utilize existing (GFE) fiber to connect new system module between all buildings.

2.5 The Contractor shall install NAC boosters throughout each building to accommodate notifications.

2.6. The Contractor shall establish a fully operational and audible AAS with the following requirements and or specifications:

1. Disconnect the present AAS from the AT&T Merlin Legend phone system;

2. Loud speaker(s) and all components required for operation to include activation points from various locations inside the Navy alert facility (B989). Control of these components shall be individual and as specified by the Navy at time of site visit. The speakers shall provide sufficient indoor and outdoor audible alert tone (no less than (80 decibels (dB) and no more than 120dB);

3. Visual alert (70K peak candle power) with siren to ensure alert crews in B989 are immediately aware of system activation. Ex: a white LED strobe with siren at minimum 90 decibels in B989 in each berthing room and aircrew areas as determined by the Navy.

The exterior strobe and spot lights shall allow for 360 degree coverage;

4. Sirens in the watch tower shall be capable of broadcasting at adjustable level from 80- 120 dB throughout the Navy alert facility and exterior of B989. The sirens shall be tied into the system with the capability to control each one individually;

5. Microphone installed at the system panel located in B989 for testing and backup;

6. Telephone for use as pager of load speakers in berthing rooms and hallways in B989 to access individual speakers;

7. Two handsets each at all B989, B820,and B1066 providing watch standers a direct line of communication that is on battery backup power;.

8. Provide an instruction and maintenance manual on a CD and in a limited paper copy that describes instructions on how to operate system, system components and part numbers, battery backup system maintenance, and how to isolate and operate the system in the event of a power outage;

9. Annual Maintenance/Service proposal to ensure equipment remains operational; and

10. Provide Warranty information and response time to maintenance/service requests.

2.7 The Contractor shall provide the parts (or equivalent) for this requirement as identified in Appendix A. Should any changes be necessary to the parts listed, the Contractor shall notify the CO immediately.

2.8 The Contractor shall conduct an operational demonstration of all system components/ functions to test and verify the system is fully operational at conclusion of the work.

3. SPECIAL REQUIREMENTS:

3.1 The Contractor shall install necessary cabling for the AAS.

3.2 The Contractor shall provide design and prints for review before/after the work.

3.3 Warranty: The contractor shall warranty the AAS In Accordance With (IAW) commercial standards but no less than one (1) year.

3.4 Training: The Contractor shall provide training on the operation and control of the AAS for no more than twelve Government employees once the equipment has been installed and is fully operational. Training shall also include routine maintenance and troubleshooting of the system.

3.5 Housekeeping Requirements: The Contractor shall ensure upon completion of work that the area is free of debris/waste and daily to the maximum extent practicable.

4. SERVICE SUMMARY (SS)

The Contractor shall have established a complete Quality Control Program (QCP) to ensure the requirements of this contract are provided as specified.

8.1 The Contractor's quality management program

shall comply with the minimum quality management systems elements specified in the American National Standards Institute (ANSI), American Standards Organization (ASO), and International Standard Organization (ISO) family of standards.

As part of the Safety Program the contractor shall establish a Safety Plan and checklist IAW Appendix B of this PWS.

12.1 The contractor shall submit the Safety Plan upon

request by the Government.

In the event of an accident/mishap 12.2 The contractor shall adhere to the guidelines in Part B of Appendix C for notification procedures.

5. GOVERNMENT FURNISHED PROPERTY AND SERVICES

5.1 The Government will not provide any parts for this requirement.

5.2 Government Furnished Property (GFP). GFP is not applicable to the performance of this contract. Shared Property of the Government is defined as adequate storage space for supplies and equipment through the completion of the work applicable to the performance of this contract.

The Contractor shall be responsible for safeguarding all Government property provided for Contractor use.

5.3 Government Furnished Services (GFS) are applicable during the performance of this contract. The Government will furnish the following services at Tinker AFB (TAFB), OK:

5.3.1. Security Forces. The Government will provide general on-base security forces’ service.

The security forces’ telephone numbers are 911 for emergencies, 405-734-2000 for crimes in progress and 405-734-3737 for non-emergency calls. The off-base 911 center receives 911 cell phone calls. Cell phone callers need to advise the 911 off-base center they are on TAFB in order to be connected with the on-base 911 center.

5.3.2. Fire Emergency and Routine Services. The Government will provide general on-base fire protection services. The Contractor shall comply with fire prevention rules and regulations enforced by the TAFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters call 405-734-3981 or 405-734-3982 and 405-734-7964 for after hours and weekends. For routine calls to dispatch call 405-734-7964.

5.3.3. Utilities. The Government will provide utility services. These utilities include natural gas, electricity, water, and sewer for the sole purpose of accomplishing this contract. If the contractor requires additional utility services, it shall be the contractor’s responsibility to provide. Contractor shall use due care and diligence in efforts to conserve utilities to reduce utility costs.

5.3.4. Telephone. The Government will provide telephone service consisting of the following Classes of Service. The Contractor shall ensure that the Government provided telephone usage is limited to only official Government business related to the performance of this contract. The Contractor shall furnish their own telephone service provided by local commercial carrier for their use in matters other than official Government business. NOTE: Only list applicable telephone service(s).

CLASS "A"/ CLASS OF SERVICE "1"

- Dial "99" for local off-base and toll free calls (1-800, etc.) -- NOTE: Do not use this option for long distance calls

- Dial "98" for long distance calls

- Dial "94" Defense Switched Network (DSN) calls

- On-base calls

5.3.5. Refuse Collection. The Government will provide refuse collection services limited to the extent provided in the Base Refuse Collection Contract(s) for the Government provided facilities.

The Government will only allow non-hazardous refuse to be emptied into the nearest authorized refuse dumpster for trash collection. Do not use Government dumpsters to dispose of construction debris.

5.3.6. Real Property Maintenance/Repair. The Government will provide maintenance and repair of Real Property in accordance with and compliance with Government procedures. The Contractor shall submit written requests to the Government Contracting Officer Representative (COR) or Government Point of Contract (GPOC) for Real Property maintenance and repairs.

5.3.7. Workspace. The Government will provide access to the equipment and adequate workspace around the equipment.

6. GENERAL INFORMATION:

6.1 SPECIAL QUALIFICATIONS:

6.1.1 The contractor shall ensure site supervisory personnel are responsive to emergencies, problems (personnel, workmanship, etc.), or to answer questions, within 24 hours, between the hours of 0600 and 1530 CST. A 24-hour response is required on all other shifts, weekends, and holidays, unless otherwise specified. These individuals will be identified by the contractor upon award and as needed should personnel changes occur.

The Contractor shall require all motorized vehicle operators to possess and maintain current certifications and state driver’s licenses and comply with base operating regulations (i.e., hands free cell phone usage while driving, no picture taking in restricted areas, no texting while driving, and seat belt law enforced on the base).

7. SECURITY REQUIREMENTS:

7.1 The Contractor shall be responsible to gain access to Tinker Air Force Base through the contract. Once at the restricted area sites the Contractor shall be escorted at all times. The contractor shall coordinate with the CO and NAVY security manager to gain access to the restricted area and Tinker AFB.

8. QUALITY CONTROL:

8.1 The Contractor shall have established a complete Quality Control Program (QCP) to ensure the requirements of this contract are provided as specified. The Contractor's quality management program shall comply with the minimum quality management systems elements specified in the

American National Standards Institute (ANSI), American Standards Organization (ASO), and International Standard Organization (ISO) family of standards.

8.1.1 The Government will verify a successful installation and operation of the system. Payment shall not be processed to the Contractor until a fully operational system is installed.

9. HOURS OF OPERATION:

9.1 The Contractor shall have access to perform services during normal working hours up to Force Protection Condition (FPCON) Charlie. Normal working hours at Tinker AFB are 6:00AM – 4:00 PM CST Monday through Friday.

9.2 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities on the following federal holidays: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day.

9.3 If the holiday falls on Sunday, it is observed on Monday. If the holiday falls on Saturday, it is observed on Friday.

9.4 All site visits shall be scheduled by the Navy POC. Schedules may have to be modified in the case of exercise, incidents or emergencies (to include natural hazards), furloughs or similar circumstances.

10. GOVERNMENT OBSERVATION:

10.1 Government personnel other than CO and COR(s) may from time to time, with CO coordination, observe contractor operations. However, these shall not interfere with Contractor performance or make any changes to the contract.

10.2 The Contractor shall not be allowed to have food, drink, cell phones, jewelry, or camera devices in the work area.

11. ENVIRONMENTAL:

11.1 Environmental Controls: The contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas. The industrial complex at TAFB has the potential to expose workers to hazardous materials, which may include, but are not limited to, hexavalent chrome, cadmium, beryllium, lead, etc. While the Contractor will not perform duties in a marked and signed regulated area (where exposures are expected to exceed the permissible exposure limits set by 29 CFR 1910.1001, there may be contact hazards with these materials during cleanup of dust, metal shavings, etc. The Contractor shall ensure proper protective measures and training is taken to ensure contracted employees are protected from these hazards.

11.2 Compliance with Laws and Regulations.The Contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the Contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

11.3 Notification of Environmental Spills. If the Contractor spills or releases any substance contained in 40 CFR 302, including hazardous materials, into the environment, the contractor or its agent shall immediately call 911. If the call is made from an on base phone it will go directly to Tinker AFB dispatch. If using a cell phone it will go off base, ask the 911 dispatcher to be transferred to Tinker AFB. The liability for the spill or release of such substances rests solely with the contractor and its agent.

11.4 Material Storage and Use. The Contractor shall follow manufacturer’s guidelines and industry standards for control of humidity, temperature, cleanliness, and materials handling to include the handling of hazardous materials such as the internal unit that contains both oil and refrigerant.

12. SAFETY PROGRAM

12.1 The Contractor shall implement a safety program that ensures protection of Government personnel and property. (See Appendix C)

12.1.2 As part of the Safety Program the Contractor shall establish a Safety Plan IAW Appendix B of this PWS. The Contractor shall submit the Safety Plan at the Government’s request.

12.2 In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence.A written or email copy of the mishap / incident notification shall be sent within three calendar days to the Government Representative, who will forward it to OC-ALC safety.

12.3 Occupational Noise Exposure: It is necessary to control the occupational noise exposure to personnel from the HPSA to comply with Federal regulations (Title 29, Code of Federal Regulations, Parts 1910.95 and 1926.52). Sound levels at any location where personnel may be located, including directly underneath the HPSA, shall not exceed 120 decibels (adjusted) (dBA) when measured on the A-scale of a standard sound level meter at slow response. Do not exceed 85 dBA at the location of the individual HPSA equipment cabinet for those HPSAs designated by the DOD installation to be furnished with a local microphone.

13. MANPOWER REPORTING:

13.1 The Contractor shall report ALL Contractor labor hours (including sub-Contractor labor hours) required for performance of services provided under this contract. The contractor is required to completely fill in all required data fields using the following web address:

13.1.2 Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. 13.2 Uses and Safeguarding of Information: Information is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.

14. FOREIGN OBJECT DAMAGE (FOD) AND TOOL CONTROL

14.1 The contractor shall follow the applicable guidance in chapters/paragraphs/sections of AFI 21-101 and HQ AFMC, Tinker AFB, supplements to the AFI 21-101 “Aircraft and Equipment Maintenance Management” concerning base tool (chapter 10 section B), and FOD control policies (chapter 14, and sub paragraphs designated as 41), daily (website: http://www.e-publishing.af.mil/index.asp). Cloth rags are considered tools and will follow the same regulations and procedures as tools. 14.2 If tools, and/or equipment are lost, the Contractor will report the loss to the Government representative no later than two hours after discovery. If tools are lost or not accounted for, then the Contractor shall implement the lost tool procedures of AFMC supplement to AFI 21-101, chapter 10, section B.

14.3 Contractors performing functions in industrial areas on the center shall have a method for controlling and accounting for all tools used. All Contractors working on the center shall inventory their tools at the beginning and end of each shift and at the end of each task to check for any lost or missing tools.

SAFETY, FIRE PROTECTION AND HEALTH

SPECIFICATION

INDUSTRIAL SAFETY REQUIREMENTS

TINKER AIR LOGISTICS COMPLEX

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

NOTE: The contractor will implement a safety program plan that ensures protection of Government personnel and property. The program will consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II – Specific Requirements, and will be used during the performance of the work described in the contract. The Safety Plan will be accepted by OC-ALC/SE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All contractor personnel shall be trained and qualified to perform their duties safely.

7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

(1) The contractor shall notify OC-ALC/72ABW (405-734-3301) or the Tinker AFB Security Forces (405-734-3737) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA COR (Contracting Officer's Representative) within one hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the Government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OC-ALC/SE. For information not available at the time of initial notification, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) ) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

1. The contractor shall cooperate with any and all Government mishap investigations.

Additionally if requested by Government personnel or designated Government representative (GR), i.e., the ACO, PCO, or DCMA COR, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

2. The contractor shall provide copies of contractor data related to mishaps, such as contractor analyses, test reports, summaries of investigations, etc. as necessary to support the Government investigation.

3. The contractor shall support and comply with the safety investigation and reporting requirements of AFI 91-204, Chapters 1 – 5.

C. General Safety Requirements:

NOTE: If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative.

The contractor is solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, Air Force Occupational Safety and Health Standards (AFOSH STD), to include AFI 91-203, Air Force Consolidated Occupational Safety Instruction, AFI 91-204, Safety investigations http://www.osha.gov/pls/oshaweb/owasrch.search_form?p_doc_type=STANDARDS&amp%3Bp_toc_level=0

Reports, AFPAM 91-210, Contract Safety, and the protection of their employees.

Additionally, the contractor is responsible for the safety and health of all subcontractor employees. All documents required as a result of OSHA 29 CFR 1910 and 1926 regulations, (i.e. certifications, training, respiratory protection program, workplace hazard assessments), shall be made available to the COR upon request.

The contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken.

Hazard Assessment. In accordance with 1910.132(d)(2), the contractor shall conduct a certified Hazard Assessment on all work areas. A follow-on survey shall be conducted by the contractor when the workload or environment conditions change. If respirators are necessary to protect the health of the employee or whenever respirators are required by the contractor, the contractor employer shall establish and implement a written respiratory protection program according to 29CFR 1910.134.

A “Risk Assessment” will be required prior to any work being accomplished, if munitions will remain in the facility.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note: AFOSH STDs are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The Government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. The contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the contractor is required to flow down the safety requirements/specification to all subcontractors. This applies to Federal Acquisition Regulation (FAR) 12 commercial acquisitions as well. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of Government employees or cause damage to Government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall:

• Demonstrate a management commitment to employee safety and health

• Identify applicable rules and regulations

• Identify the roles and responsibilities of Management, Supervisors, Employees and Safety Coordinator

• Identify work to be performed and location of expected operations

• Provide a description of safety program, safety monitoring responsibilities, organizational structure, and contact information for on-site personnel

• Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures

• Identify employee safety and health training requirements and the documentation process

• Include emergency response plans and procedures that relate to protection of

Government personnel and property

• Include hazard reporting procedures and identify individual(s) responsible for the correcting identified hazards

• Identify procedures for accident reporting and investigation

• Identify the process for tracking controlled hazards in contractors work area

The contractor shall ensure that each element identified below is adequately addressed in detail in the safety and health plan:

PEDESTRIAN CROSSWALKS: All contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road.

Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.

MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3- 4; DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07; and AFI 91-207 AFMC SUP1, The US Air Force Traffic Safety Program, 1 Oct 07. Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition: AFI 13-213, Airfield Management, 29 Jan 08, Paragraphs 1.3.6 and

4.4.2.1 applies to all contractors, sub-contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Tinker Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.

HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.

The authorized Government Representative (OC-ALC Safety Office) shall approve the contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification / Reporting Procedures, which are outlined in Section I, paragraph B of this Appendix C.
