MEMORANDUM FOR RECORD
SUMMARY OF CHANGES: FA8053-13-R-0001-0001, AMENDMENT II
1. Change 1: The definition of Fully Burdened Hourly Rate in Section C, PWS paragraph 11.21 is being revised to clarify that travel, per diem, lodging, and rental car expenses shall all be included in the fully burdened hourly rate proposed by offerors.
Section C, PWS paragraph 11.21 changed from:
“11.21 Fully Burdened Hourly Rate.
The bill rate covering direct labor, indirect costs (e.g., overhead, General and Administrative expenses, and fringe benefits), and profit.”
To:
“11.21 Fully Burdened Hourly Rate.
The bill rate covering direct labor, indirect costs (e.g., overhead, travel, per diem, lodging, rental car, General and Administrative expenses, and fringe benefits), and profit.”
2. Change 2: The reference to “Proposal Summaries” in Section L 2.2.1 is meant to refer to section L 3.1, “Narrative Summary.”
Section L, 2.1.1 changed from:
“2.2.1 Page Limitations Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and excess paper copies will be returned to the offeror as soon as practicable. Page limitations may be placed on responses to Evaluation Notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the offerors. Each page shall be counted except the following:

3. Change 3: Based on the vendor question “Regarding Attachment L-1: Section 8 includes Attachment L-1 in the list to be provided as a separate file. Section 4.2.2 Technical Factor, Criteria 9, states that the L-1 should be submitted in Tab 2 of Volume II. Please clarify where and how we should submit Attachment L-1,”
Section L, 4.2.2, Criteria 9 changed has been changed from:
“Criteria 9: Contractor Team Organization The offeror shall submit an organizational structure in the technical proposal in Tab 2 using Attachment L-1 Contractor Team Organization for all teaming partners and subcontractors that demonstrates the service categories the prime Contractor and each teaming partner and subcontractor will provide under this contract.”

“Criteria 9: Contractor Team Organization Within the business plan, the offeror shall submit an organizational structure in the technical proposal in Tab 1 using Attachment L-1 Contractor Team Organization for all teaming partners and subcontractors that demonstrates the service categories the prime Contractor and each teaming partner and subcontractor will provide under this contract.”
4. Change 4: Section L 6.2.1 is being revised to incorporate the updated definition of Fully Burdened Hourly Rate discussed above.
Section L 6.2.1 changed from:
“6.2.1 Table B-1, offerors are to complete the following in the “STO Requirements” Tab of Table B-1:
Labor category pricing: The STO will be used to evaluate pricing for a 520 hour (13 week) requirement. Review each labor category, by base, in Table B-1 and provide the fully burdened hourly rate in Column E for the number of billable duty hours in Column D; provide the fully burdened hourly overage rate in Column G for the number of billable overage hours in Column F; and provide the fully burdened hourly on-call rate in Column I for the number of billable on-call hours in Column H. As a reminder, the fully burdened hourly rate, as defined in Section C 11.21, is the bill rate covering direct labor, indirect costs (e.g., overhead, General and Administrative expenses, and fringe benefits), and profit. Offerors should also pay close attention to column L. In instances where positions may require fills in less than 30 days, vendors may receive a separate fixed price lump sum for expedited submission of credential documentation that will be required 10 days after task order award. Offerors should input this separate fixed price lump sum in the yellow highlighted cells in column L. This separate fixed price lump sum will be used on all future task orders awards that require expedited submission of credentials documentation. The offerors are cautioned to follow the instructions listed in the instruction worksheet to complete this portion of Table B-1 correctly.”
To:

5. Change 5: Section M 1.2 is being revised to clarify that government estimates, other proposals, and salary surveys are all optional tools that may be used by the Government for comparison purposes when rejecting unrealistically low priced offers.
Section M 1.2 will be changed from:
“1.2 Rejection of Unrealistically Low Priced Offers The Government may reject any proposal that is evaluated to be unrealistic in terms of program commitments, contract terms and conditions, or unrealistically low in price when compared to Government estimates, other proposals, and salary surveys such that the proposal is deemed to reflect an inherent lack of competence or failure to comprehend the complexity and risks of the RFP.”
To:
“1.2 Rejection of Unrealistically Low Priced Offers The Government may reject any proposal that is evaluated to be unrealistic in terms of program commitments, contract terms and conditions, or unrealistically low in price when compared to Government estimates, other proposals, and/or salary surveys such that the proposal is deemed to reflect an inherent lack of competence or failure to comprehend the complexity and risks of the RFP.”
6. Change 6: Section M 5.0 is being revised to incorporate the updated definition of Fully Burdened Hourly Rate discussed above.
Section M 5.0 changed from:
“5.0 Price Price evaluation of all offerors will be accomplished with evaluation of a Sample Task Order (STO) representing the most anticipated requirements as identified by a random sampling by the Chief of Hospital Services at select Air Force MTFs.
Section J, Attachment 8, Table B-1 represents the anticipated requirements from a sampling of MTFs. The Government will evaluate offers for award purposes by adding the total price of all Government requirements identified in the Sample Task Order. The Government may award all, some, or none of the requirements using the firm fixed prices provided in Table B-1. Therefore, Table B-1 will form the offeror’s firm fixed price fully burdened hourly rates for the labor categories at each of the identified bases for the STO requirements. After contract award, the fully burdened hourly rates on Table B-1 will only be used to award the requirements listed on the STO for a period of 90 days after contract award. For instances where positions may require fills in less than 30 days, offerors may receive a separate fixed price lump sum for expedited submission of credential documentation that will be required 10 days after task order award. This separate fixed price lump sum provided on the STO will be used on all future task order awards that require expedited submission of credentials documentation.
The offeror’s submission for Volume IV - Price will be evaluated in terms of completeness, price reasonableness and balance and may be evaluated for price realism using one or more of the techniques defined in FAR 15.404 in order to insure pricing that is fair and reasonable and realistic. As a reminder, the fully burdened hourly rate, as defined in Section C 11.21, is the bill rate covering direct labor, indirect costs (e.g., overhead, General and Administrative expenses, and fringe benefits), and profit.”
To:
