Combined Synopsis/Solicitation For AFDW Safety Directorate (AFDW/SE) – Line Operations Safety Audit (LOSA)

(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) This solicitation/synopsis reference number is FA7014-19-R-LOSA and is being issued as a Request for Quotes (RFQ).

(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-03, 12 July 2019.

(iv) This solicitation is 100% Set Aside for Small Businesses. The North American Industry Classification System (NAICS) code is 541690 - Other Scientific and Technical Consulting Services with a Small Business Size Standard of $15M.

(v) The purpose of this combined synopsis and solicitation is to advertise the requirement to have Offeror perform a Line Operation Safety Audit (LOSA) that provides a rigorous and methodical assessment of observed operations during regularly scheduled UH-1N flights, collecting and analyzing safety-related data on environmental conditions, operational complexity, and crew/team performance. A LOSA provides a diagnostic snapshot of strengths and weaknesses that AFDW and other major Commands (MAJCOMs) can use to bolster the “Health” of its safety margins and prevent mission degradation. Confidential data collection and non-jeopardy assurance for crew/team members are fundamental to the LOSA process. See attached Performance Work Statement (Attachment 1) for a detailed description of requirements for the service being acquired.

Line Operations Safety Audit (LOSA) Observations to include:

1) Threat and Error (T&E) Matrix Development:

a. Meet with UH-1N operational SMEs and develop UH-1N Specific T&E.

b. Prepare and develop software, functionality and Quality Assurance.

2) Observer Training:

a. Develop Observer Curriculum.

b. Conduct 5 day training event.

3) LOSA data creation and analysis:

a. Oversight of Database supported by SME research.

b. Database manipulation to ensure statistical significance.

c. Process each observation received and prepared for data analysis.

d. Provide data validation in conjunction with government representatives prior to final report.

4) Written Report and Final Presentation

a. Utilize data collected from LOSA observations to prepare a comprehensive written report.

b. Brief the findings to USAF Leaders identifying key Threats, Errors facing the UH-1H community during flight operations.

c. Support USAF Safety Investigation Board, which will follow senior leader briefing.

Travel Expense from contractor location to Joint Base Andrews, MD or other government-identified location (includes lodging, meals, rental car, and airfare).

The Government directed travel IAW PWS reference 7.0 through 7.3. The Government will only reimburse allowable costs IAW FAR 31.205-46. All travel shall be pre-approved by the COR IAW Joint Federal Travel Regulations (JFTR).

Not-To-Exceed (NTE) amount $30,000.

(vi) Refer to the attached PWS for a detailed description of requirements for the service being acquired.

(vii) Performance period is anticipated to be 16 Sep 2019 – 15 Sep 2020.

(viii) 52.212-1, Instruction to Offerors – Commercial, applies to this acquisition. With the following addendum:

52.212-1 Addendum

The Offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipt of offers.

PROPOSAL PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of the proposal, the Offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of three (3) separate parts: Part I – Price Proposal, Part II – Past Performance Information and Part III – Technical Proposal.

B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and, if in the contracting officer’s opinion adequate price competition exists, no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists Offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness.

C. Specific Instructions:

Documents submitted in response to this solicitation must be fully responsive to and consistent with the following:

1. Requirements of the solicitation (Item Numbers) and Performance Work Statement (PWS), and government standards and regulations pertaining to the PWS.

2. Evaluation Factors for Award.

1. PART I – PRICE PROPOSAL - Submit one (1) copy.

(a) Complete the Solicitation Bid Schedule in paragraph “(v)” above. The completed schedule must be returned with the solicitation as part of the Price Proposal. Any offer received without the completed schedule will be determined non-responsive and will not be considered for award.

(b) Insert proposed unit and extended prices in the Bid Schedule. Unit prices must be whole dollar values (e.g. $10.00). The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal(s) must be submitted for a 1 year period of performance.

(c) Complete the necessary fill-ins and certifications in provisions. The provisions 52.212-3 shall be returned along with the proposal.

(d) Include the total dollar value of your proposal at the bottom of the bid schedule.

2. PART II – PAST PERFORMANCE INFORMATION - Only references for same or similar type contracts are desired. Submit one (1) copy.

Offerors shall provide three (3) references of “recent” and “relevant” projects that have occurred. “Recent” is defined as; projects within the past three years. “Relevant” is defined as performance effort involved essentially the same magnitude of effort and complexity this RFQ requires.

(a) Provide information currently available which demonstrates customer satisfaction with overall job performance and quality of completed product for same or similar type contract. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, etc.

(b) Provide a list of no more than three (3) of the most relevant contracts performed for Federal agencies and commercial customers within the last three years. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Experience with rotary wing LOSAs should be noted. Furnish the following information for each contract referenced:

(i) Company/Division name

(ii) Product/Service

(iii) Contracting Agency/Customer

(iv) Contract Number

(v) Contract Dollar Value

(vi) Period of Performance

(vii) Verified, up-to-date name, address, FAX and telephone number of the contracting officer

(viii) Comments regarding compliance with contract terms and conditions

(ix) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.

If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past/present performance information on previous teaming arrangement with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts.

3. PART III – TECHNICAL PROPOSAL – Submit one (1) copy.

The Technical Proposal should be specific and complete. Legibility, clarity, and coherence are very important. Provide as specifically as possible the actual methodology you would use for accomplishing/satisfying the requirement. Address your technical solution for meeting or exceeding the Government’s minimum performance or capability requirements of the PWS. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.

The Offeror shall provide a methodology and complete approach, to include quality control IAW the PWS. This approach will include accomplishing all Tasks, Services, Deliverables, Quality Control Measures, and other requirements outlined in the PWS to include but not limited to:

1) Threat and Error Matrix (TEM) Development, Methodology, and Analysis: The offeror’s proposal shall demonstrate ability to develop customized threat and error methodology based on the UH-1N operations IAW PWS Tasks 1-2 (PWS Section 2.1, and 2.2).

2) Observer Training Plan, Curriculum and Oversight: The offeror’s proposal shall describe the offeror’s proposed LOSA observer training curriculum, training plan and Observer Oversight Management IAW PWS Tasks 3, 6, and 7 (PWS Sections 2.3, 2.6 and 2.7).

3) LOSA Data Collection and Analysis Solution: The offeror’s proposal shall demonstrate ability to create an end-to-end solution (hardware, software, network-connectivity) for LOSA data collection and analysis, and a Subject Matter Expert Roundtable IAW PWS Tasks 4, 5, 8, 9, and 10 (PWS Section 2.4, 2.5, 2.8, 2.9 and 2.10).

4) Written Report and Final Presentation: The offeror’s proposal shall demonstrate ability to develop a final written report and presentation to selected USAF leaders, as well as support to the USAF Safety Investigation Board which will follow the presentation IAW PWS Tasks 6 and 11 (PWS Section 2.6, and 2.11).

52.212-2 -- Evaluation -- Commercial Items (Jan 1999)

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers:

1) Technical Acceptability - Technical capability of the service offered to meet the Government requirement. The technical proposal should demonstrate the offeror’s ability to perform the technical requirements IAW the PWS.

2) Past Performance - The government will seek recent and relevant performance information on all Offerors based on (1) the references provided by the Offeror and (2) data independently obtained from other government and commercial sources. Relevant present/past performance is defined as effort involving a similar scope and similar magnitude of effort and complexity as this solicitation requires. See attachment 3 and 4 for Past Performance Questionnaire and Past Performance Information Format.

I. If the lowest priced evaluated responsible offer is judged to have a “Substantial Confidence” Performance Assessment Rating that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.

II. If the lowest priced Offeror is not judged to have a “Substantial Confidence” performance assessment, the next lowest priced Offeror will be evaluated and the process will continue (in order by price) until an Offeror is judged to have a “Substantial Confidence” performance assessment or until all Offerors are evaluated. The Contracting Officer will then make an integrated assessment best value award decision.

III. Offerors may be asked to clarify certain aspects of their proposal or respond to adverse past performance information to which the Offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.

3) Price - The Government will evaluate the total price of the offer for award purposes.

A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
