PERFORMANCE WORK STATEMENT

FOR

HOISTING EQUIPMENT

INSPECTION, MAINTENANCE AND REPAIR

FOR

94TH MAINTENANCE GROUP’S

DOBBINS ARB, GA

JULY 29, 2014

I. DESCRIPTION OF SERVICES. The Contractor shall inspect, maintain and repair designated hoists at

Dobbins Air Reserve Base (DARB), Georgia in accordance with standard commercial practices, manufacturer's specifications, Air Force Occupational Safety and Health Standard (AFOSH) 91-46, American National

Standards Institute (ANSI) Standards ·ANSI B30 Series for Cranes and Other Lifting and Material Handling

Related Equipment; AFOSH 91·10, Civil Engineering (reference Paragraph 2.10, Hoists and Cranes); and

American S o c i e t y of Mechanical E n g i n e e r s (ASME) B30. I I -2004, Monorails and Underhung

Cranes, and this Performance Work Statement (PWS). As such that will ensure continuous and safe operation in accordance with all federal, state, and local laws and regulations.

Services include maintenance, both scheduled and unscheduled, incidental repairs, and operations activities necessary to keep the equipment in good working order. Maintenance includes regularly scheduled annual inspections, preventative maintenance, emergency response, and service calls for incidental repairs. All work shall be executed in a consistent manner by individuals who are qualified, careful and efficient in the trade.

I.I. INSPECTION AND PREVENTATIVE MAINTENANCE SERVICES

1.1.1. Annual Inspection. The contractor shall perform inspections of all hoists as identified in Appendix B.

The contractor shall perform one (1) annual inspection of all equipment, during the month of November each year. The contractor shall prepare and submit a written report to the Contracting Officer Representative (COR) within 10 days after performing the annual inspection and preventative maintenance. The report shall identify each hoist, location, date of inspection , PM performed repairs needed, name of inspector and overall condition of the hoistn ht

1.1.2. Rated Load Tests. The contractor shall perform load tests of all equipment during annual inspections, at the completion of any extensive repairs, complete overhauls, and modifications or cable replacements. The contractor shall adjust upper and lower limit switches to perform the rated load tests.

l.l.3. Preventive Maintenance. While performing the annual inspection the contractor shall perform all required preventative maintenance in accordance with manufacturer's instructions on all equipment identified in

Appendix B. The contractor shall provide all required equipment, tools, personnel, materials and supplies necessary to accomplish the services. The contractor shall perform preventative maintenance (i.e. cleaning, adjusting, lubricating, tightening nuts/bolts, and replacing wearable parts) and make repairs as required of all government furnished equipment (GFE) (see Appendix B). The contractor is responsible for all costs (labor, equipment, tools, materials/supplies, including any part, up to $100.00/part) for accomplishing the preventative maintenance service.

1.1.4. Preventative Maintenance Parts. The contractor shall furnish a written cost estimate to the COR for any part exceeding $100.00 up to $2,500.00 to include material and labor costs. The cost of any part exceeding

$2,500.00 will be sent to the Contracting Officer (CO) for approval for processing. The contractor shall be reimbursed by the government for $1 00.00 and greater at actual cost to the contractor. The contractor shall credit all discounts and rebates to the government.

l.l.5. Annual Schedule. Upon contract award and prior to commencing services, the contractor shall submit an annual inspection schedule to the Contracting Officer and Contracting Officer for acceptance. The schedule shall cover the period of 1 November - 30 November of each year. The schedule shall include the specific dates for which the contractor will perform the annual inspection on each piece of equipment. The schedule may be affected by Aircraft hangar movement, if so, the contractor and COR will coordinate changes to the schedule. The contractor shall submit a new schedule no later than (NLT) October 1st each year or when changes to the schedule occur, for CO/COR acceptance. The contractor shall not implement changes to the schedule until the changes are accepted by the COR.

1.2. SERVICE CALLS

1.2.1. Routine Service Calls. The COR will notify the contractor of any repairs to be made in addition to the repairs identified during the annual inspection. Routine service call response shall be within 24 hours after notification during normal hours of operation. Response times for routine service calls placed on the day preceding a weekend or federal holiday shall be 7:30 a.m. on the day following the weekend or federal holiday. The contractor shall survey the repair, and provide the COR with an estimated time and cost of repairs prior to commencing work. The contractor shall notify the user of the equipment of all projected downtime.

The contractor shall be responsible for any equipment that leaves the government site for repair until it is reinstalled at the government site in proper operating condition.

l.2.1.1. The contractor shall furnish all replacement parts. The contractor shall own all parts purchased until they are installed on government-owned equipment.

1.2.1.2. If the contractor and COR determine equipment is beyond economical repair, the contractor shall recommend to the COR, in writing, that such equipment be replaced. The contractor shall include substantiating data to support the recommended replacement. The contractor shall return equipment no longer used to the government.

1.2.2. Emergency Service Calls. Calls that require the contractor to respond when the problem endangers life or property are deemed an emergency. Emergency service calls require response within two (2) hours after notification during normal hours of operation and within four (4) hours after notification at times other than normal hours of operation.

1.2.3. Reports. The contractor shall prepare and submit a written report to the COR, within ten (10) calendar days after service call repairs are completed. The report shall include the date and time of the service call, the location of the equipment serviced, the repairs performed, the designation (Routine/Emergency) and the name of the technician performing the repairs.

1.2.4. Service Call Parts. The contractor shall find sources for parts that will result in the most economical cost to the government. Parts purchased for repairs and maintenance shall be of equal or greater quality to the parts replaced. The contractor shall purchase replacement parts (when repair of the part is uneconomical) at the lowest price obtainable after securing reasonable competition for such purchases. Contractor shall provide a manufacturer warranty for any replacement part that exceeds $100.00. The contractor shall make repairs as required of all government furnished equipment (see Appendix B). The contractor shall be reimbursed by the government at actual invoice cost to the contractor. Repairs will be reimbursed under the service call/parts reimbursement CUN. The contractor shall credit all discounts and rebates to the government.

1.3. REIMBURSABLE PARTS

1.3.1. Repairs that are determined not to be preventative maintenance will be reimbursed under the service call/parts reimbursement CLIN. The contractor shall submit an invoice to the government for the price(s) of replacement part(s). Parts will be provided by the contractor to the government at the contractor's cost. The contractor shall find sources for parts that will result in the most economical cost to the government.

1.4. EXCLUDED SERVICES

1.4.1. Excluded Services. Modifications and/or additional attachments to the present hoists are not covered in this contract. All modifications or additions to equipment shall be approved by the COR.

2.0. SERVICES SUMMARY

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES

3.1. Utilities. The government will furnish electricity to operate tools at no cost to the contractor in performance of this contract.

3.2. Emergency Medical Service. The contractor is responsible for emergency treatment and emergency patient transportation service for contractor personnel. For medical emergencies dial 911.

3.3. Security. The government will provide general on-base Security services. For on-base emergencies dial 911.

4. GENERAL INFORMATION

4.1. Hours of Operation. Normal hours of operation for DARB is, 7:30 a.m. - 4:30 p.m., Monday through

Friday, excluding federal holidays. Services performed outside of normal duty hours must be coordinated with the COR in advance, at no additional cost to the Government and prior to any schedule changes

4.2. Federal Holidays. The contractor shall not perform routine maintenance and routine repair services on the following Federal holidays, or the actual day set aside for observation.

4.3. Quality Control Plan. The contractor shall develop and maintain a quality control program to ensure annual inspections, load tests, maintenance, and repairs are performed in accordance with commonly accepted commercial practices and this PWS. Through implementation of this program the contractor shall identify, prevent, and ensure non-recurrence of defective services. The Quality Control Plan shall be submitted to the

COR for acceptance within 30 days after award.

4.4. Contract Manager. The contractor shall designate a primary contract manager (CM), and at least one alternate CM, who will be responsible for performance of services. Upon contract award, the contractor shall submit the name and telephone number of the primary and alternate CM in writing to the COR. The CM shall have full authority to act for the contractor on all matters relating to daily operation of the contract. The contractor point of contact shall be available by telephone to receive service calls and to discuss maintenance, repairs, or any other issue or problems with Government personnel. The contract manager shall also be available during normal duty hours to meet on the installation with Government personnel (designated by the

COR) to discuss any issues or problem areas.

4.5. Continuation of Essential DoD Contractor Services During Crises. This requirement has been determined to be mission essential for performance during crises as defined in DoD Instruction 3020.37 and Air

Force implementation thereof, it is determined that the contractor will be required to perform during a crisis, as stated in paragraph above, unless otherwise directed by an authorized government representative.

4.6. Special Qualifications. The contractor and any subcontractor employees shall possess all training, certifications, licenses, insurance, and permits as required by State and local authorities for the maintenance and repair of hoists. Contractor shall ensure all certificates, licenses; permits and bonds are kept current and valid.

Associated expenses shall be borne by the Contractor and be available upon request by the government.

4.7. Work Clearance and Welding Requests. The contractor is responsible for obtaining a welding permit prior to welding being accomplished outside of an approved welding shop on DARB. Permits are issued by a fire inspector at the DARB Base Fire Prevention Office, building #745, or at the job site. Normal duty hours are Mon-Fri, 6:30 a.m. - 2:00 p.m. The telephone number of the fire prevention office is 678-655-4840.

Permits may be issued for a maximum of thirty (30) days at a time. After completion of the welding operation, the permits must be turned in to the Base Fire Prevention Office.
