Homestead ISWM PWS 02/05/2013
PERFORMANCE WORK STATEMENT
FOR
INTEGRATED SOLID WASTE MANAGEMENT
HOMESTEAD ARB
20 February 2013
1. DESCRIPTION OF SERVICES. The Contractor shall provide all labor, tools, transportation, material, equipment, and supervision necessary to provide containers and non-hazardous solid waste refuse collection and disposal services for Homestead Air Reserve Base, Florida in accordance with all applicable laws, regulations, standards, instructions and commercial practices as described in the Performance Work Statement (PWS) with the focus on safety and customer service. The Contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish PWS requirements in a timely manner at reduced costs, and in a way that fosters pride and ownership in the work performed. Estimated workload is listed in Appendix A, titled Workload Estimates. The estimated workload is based on historical records and may fluctuate + or - 15% of 350 gross tons. Any questions concerning the operation of refuse collection and disposal services during the life of the contract shall be addressed through the COR to the Contracting Officer.
1.1. COLLECTION REQUIREMENTS.
1.1.1 Main Base
1.1.1.1. Integrated solid waste management. Empty MSW/recycling containers/dumpsters based on historical data for the first 30 days of performance or at least weekly. The contractor shall establish initial vehicle routes and collection schedules upon contract award. They are to be submitted to the Contracting Officer (CO) five (5) calendar days prior to start of contract performance. The contractor may propose adjustments to the schedule at any time but as a minimum the Contractor Officer Representative (COR) and contractor shall monitor fill capacities during the first 30 days of performance and during one other period to be agreed upon during the same performance period. No changes are allowed to the schedule or haul route without CO or COR coordination. Historical frequency tables are located at Appendix A, Table A2. When the contractor completes service at a location there shall be no litter, waste, and recyclable material on the ground within a 10 foot radius of the container.
1.1.1.2. Food Waste Containers. Empty containers used to dispose of food waste daily (on days when the serviced facility is in operation). Food Waste containers and pickup days are identified in Appendix A. (CLIN 0001). International regulated food waste will be disposed of using a USDA approved sealed 6cy dumpster and emptied as required.

1.1.3. Over and Above Requirements. The following services shall be priced separately in accordance with the cost contained in Exhibit A of the bid schedule (CLIN 0009). The contractor shall be compensated IAW the price specified on Exhibit A of the bid schedule for that service.
1.1.3.1. Unscheduled Pickup and Special Events. In addition to the scheduled solid waste collections, the CO or COR may request the contractor make unscheduled collections and disposals. For unscheduled events, the contractor shall respond within one business day of notification from the CO or COR. For special events, the contractor shall also provide (or re-position as directed) solid waste containers. These containers shall be placed in locations for a length of time designated by the CO or COR. The CO shall notify the contractor in writing at least 7 days in advance of the special event date. See Appendix A for workload data. (CLIN 0009)
1.1.3.2. Construction and Demolition Debris (C&D). When requested by the CO or COR, contractor shall provide roll-off containers for collection of C&D. The contractor shall place containers at the designated site within one business day of notification by the CO or COR. The contractor shall recycle or dispose of all C&D at a permitted landfill or recycling facility.

1.1.3.6. Additional Pickups: In the event unusual conditions cause a container to require pickup before the scheduled time, the CO or COR may request in writing (email/ Fax sufficient) pickup within 24 hours and the contractor will be compensated at a unit rate based on the container size.

1.1.4. Inclement Weather Schedule. The contractor shall collect MSW and recyclable material during periods of inclement weather. In cases of severe weather, the CO or COR may authorize exceptions. When exceptions are granted all missed collections shall be performed within 24 hours after the severe weather has terminated, unless the CO authorizes additional time.
1.1.5. Route Parameters. The contractor shall propose routes to the base through the main gate to the CO for acceptance. Daily and weekly collection shall be made between the hours of 7:00AM to 4:00PM, Monday through Saturday. Collection outside these hours shall require prior approval of the CO or COR
1.1.6. Points of Collection. Collection stations for MSW and recyclable materials are shown in Appendix B. The contractor shall position bulk containers for customer ease in depositing MSW. The contractor shall also position containers to minimize interference with adjacent parking areas, sidewalks, roadways, overhead utilities, trees, and other potential obstructions.
1.1.7. Cubic Yard Capacity. Proposed capacity of containers is indicated in Appendix A, Table A2. The contractor is encouraged to identify changes to the capacity of containers and/or pickup frequency to improve the overall efficiency of MSW and recycling removal, and to submit these changes to the CO or COR. No changes shall be made until the proposed changes are coordinated with the CO or COR.
1.1.8. Government-Approved Containers. The Contractor shall provide all refuse containers necessary to perform this contract. The number, size, and location of the containers listed in Appendix B are estimates only and changes may occur. In addition, the Government reserves the right to change sizes, locations and quantities. All refuse containers shall be clean and serviceable at the beginning of the contract. All refuse containers shall have sliding side doors.
The Contractor shall permanently affix a luminous sign; approximately 9 X 6 inches; with distinctive letters to the lower center of three outside walls of the containers. The sign shall read
“WARNING – STAND CLEAR AT ALL TIMES WHEN CONTAINER IS OFF
GROUND”. Another sign shall be permanently affixed stating DO NOT PARK WITHIN 15
FEET”.
1.1.8.1 Refuse Container Painting. Containers initially furnished by the Contractor shall be painted brown in color (color number 20100, MFR Sherwin Williams or equal). The Government will approve the color of the containers prior to placing or painting containers.
Containers will be re-painted as required. Additional re-painting of the containers shall match the approved color scheme with the Government’s approval.
1.1.8.2. Roll Off Containers. The Contractor shall furnish 20 cubic yard roll off containers constructed for commercial use and meets all Occupational Safety and Health Administration (OSHA) safety standards. Containers shall be in good working order and must not contain any graffiti. Rolls off container locations are identified in Table A2 and the Appendix B map.
1.1.9. Maintaining Containers and Collection Area. The contractor shall return all MSW and recycling containers to their original location after servicing, in an upright position with the lids securely in place (closed). The contractor is responsible for cleaning up any spills, debris, etc., on the ground within ten feet of the container when servicing a location. The contractor shall collect debris placed at collection stations; for example, tree branches, sacks, cartons, boxes, cans, tied bundles, or other containers. It is intended that the majority of debris will be placed in containers. The contractor shall notify the COR of locations where debris is frequently placed outside of containers so the COR may notify facility managers and, if necessary, the contractor can adjust the container size or pickup frequency with CO approval. The contractor shall be responsible for keeping collection areas free of solid waste and debris.
1.1.10. Weighing of Vehicles. All vehicles used in the collection of MSW, C&D and regulated food waste materials shall be weighed on state certified vehicle scales located at building 164 prior to any base collections and before departing the base. Incoming and outgoing vehicle weights shall be recorded on weigh tickets provided by the operator at the weigh station The contractor shall submit a monthly report (para 1.5) that includes weight tickets that reflect the disposed tonnage figures, date of disposal, and type of vehicle and cost of landfill tipping fees.
The contractor shall be reimbursed for tipping fees according to submitted invoices. In the event the base vehicle scales are inoperative the contractor will use historical weight data from the month prior to calculate weights.

1.3. DISPOSAL.
1.3.1. Off-Base Disposal. The contractor shall transport and dispose of all municipal solid waste at a permitted off-base disposal site. A copy of the landfill(s) operating permit that material is transported to shall be provided to the CO. The contractor shall transport recyclable commodities to an off-base material recovery facility. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice to CLIN 0014 for reimbursement.
1.3.2. RESERVED
1.4. EQUIPMENT MAINTENANCE. The contractor shall furnish containers that match the color scheme of the base. Additional re-painting must match the approved color scheme. The contractor shall maintain all contractor and government provided containers ensuring they are free of unpleasant odors, dirt, debris, and pests. All containers must remain in good, workable condition, with no leaks. They must remain easily accessible to customers. The contractor shall perform all cleaning, painting, repair and other maintenance tasks off-base. Trucks and trailers used for hauling and collecting shall be free of odor, dirt, debris, and pests. Contractor vehicles and equipment shall not have any waste or equipment leaks such as oil or hydraulic fluid. Should any contractor vehicles or equipment have any leaks, the contractor is responsible for cleanup.
1.5. REPORTS AND RECORDS. The contractor shall provide a monthly report detailing total tonnage of solid waste (MSW, and C&D) collected, landfill tipping fees and weight tickets.
Format for these monthly reports shall be coordinated with the CO or COR
1.6. CONTRACT MANPOWER REPORTING: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Homestead Air Reserve Base via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
1.6.1. Reporting inputs: Will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
1.6.2. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
1.6.3. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
2. SERVICE SUMMARY. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
3.1. SECURITY POLICE AND FIRE PROTECTION. The Government will provide general on base Security Police and Fire Protection service. Security police phone number is (305)224-7114.
The Fire Department phone number is (305) 224-7117. There is no 911 service on base.
4. GENERAL INFORMATION.
4.1. QUALITY CONTROL. The contractor shall develop and maintain a quality program to ensure integrated solid waste management are performed in accordance with commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum the contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Service Summary.
4.2. HOURS OF OPERATION. 7AM – 4PM
Holiday Collections. The Contractor shall perform contract services on the days listed in the Frequency column of Appendix B, Task and Frequency Table of Scheduled Collections, except on days when Federal holidays are observed.

4.3. SECURITY REQUIREMENTS.
CONTRACTOR INFORMATION PAMPHLET
The 482d Security Forces welcomes you and your employees to Homestead Air Reserve Base. As a contractor with this installation, all Federal and State laws along with base instructions will apply to you as well as your employees. The Security Forces at Homestead Air Reserve Base are here to enforce all Federal and State laws and base instructions. This pamphlet covers speed limits, use of gates, some basic rules of conduct, a few prevention tips, and passes and vehicle registration procedures for you and your employees. Terrorism is a major concern to our installation and if you or any of your personnel observed any suspicious activity it should be reported immediately to our security forces.
RULES OF THE ROAD
Use and Applicability: It is unlawful for any persons to perform any act forbidden or fail to perform any act required under these rules and said rules shall apply to all personnel while upon Homestead Air Reserve Base. As the installation commander determines, persons not subject to the Uniform Code of Military Justice (UCMJ), could be barred from the installation.
Florida Crime Information Center (FCIC)/National Crime Information Center (NCIC): Contractor personnel should be aware that criminal background checks will be conducted on them prior to being granted entry to the installation. All contractor personnel will be screened through the FCIC/NCIC terminal for outstanding wants and warrants, criminal histories, and Drivers license status. They may also be screened through the National Terrorist Screening Center. Anyone found to have an outstanding warrant, to be an illegal alien, or on the terrorist watch list, etc., will be detained and turned over to local law enforcement authorities.
Florida Uniform Traffic Control Law: The laws of the State of Florida, pertaining to motor vehicle operation and registration, are applicable upon Homestead Air Reserve Base by virtue of the Assimilative Crimes Act or 1948, 18 U.S.C., Section 13. These laws are primarily found in Florida State Statute, Chapters 316, 322 and 325.
Obedience to Personnel Involved in Traffic Enforcement: No person shall willfully fail or refuse to comply with any lawful order or direction of any DoD Police official, safety representative or other personnel designated by the installation commander to supervise and/or enforce compliance with traffic rules.
Obedience to Traffic Signs and Markings: The operator of any motor vehicle shall obey all traffic signs, markings or other traffic control devices unless otherwise directed by a DoD Police official or other persons performing traffic control duties.
Speed Limits: The speed limits on Homestead Air Reserve Base are strictly enforced, particularly in the flightline areas. Speed measuring devices are utilized. As on all Federal property, the use of radar/infrared detectors is illegal. Such devices must be removed and unplugged before entering the installation. Speed limits for the installation are as follows:
Main Base: 25 Miles per hour Adjacent to the Flightline: 25 Miles per hour Actual flightline ramp: 15 Miles per hour Around Aircraft: 5 Miles per hour Parking lots: 5 Miles per hour
Violation and Citations: For expired tags, DUI or excessive number of traffic violations, civilian violators driving privileges could be suspended or revoked.

Asset Forfeiture: All work places on Homestead Air Reserve Base are drug free. Your contract contains a section which deals with a drug-free work environment. Should illegal substances/drugs be located in your vehicle and/or equipment, it will be seized and forfeited to law enforcement agencies and/or the government. Additionally, your contract may be terminated at no expense to the government.
FLIGHTLINE PROCEDURES
If your contract does not require you to be in the flightline area, do not be around the area. The entire Restricted Area is surrounded by a red line painted on the ground and cement barriers. In addition, signs are prominently displayed indicating the restricted area and that use of deadly force is authorized.
DO NOT CROSS THE RED LINE!! If you need to operate motor vehicles in the flightline area, contact Base Operations at 7516 for the proper procedures. Photography is not permitted on the installation unless coordinated through the base Public Affairs Office.
CRIME PREVENTION TIPS
Secure and lock up tools and other valuable/pilferable items or equipment. Remember, if you can carry it in, someone can carry it out. Permanently mark your equipment by engraving or other specialized methods of identification.
IDENTIFICATION AND VEHICLE PASSES
Identification Pass: Contractors and their employees will be issued an AF Form 75, Visitor/Vehicle Pass for identification. We request 72 hours notice be given to process AF Form 75 (passes). This identification will be issued for the length of the contract and shall remain with the individual while working on the base. Contractors and employees must present picture identification for validation before issuance of the pass. Individuals who are selected to escort deliveries or other persons shall be designated by the company by assigning them the title of “Supervisor” on the Sponsorship Letter for Civilian Contractors. This in turn shall be placed on their AF Form 75 for escorting privileges. The escorts are responsible for those individual(s) they escort until such individual(s) exits the installation.
Vehicle Registration: The individual will be issued an AF Form 75 Visitor/Vehicle Pass for the duration of the contract. To receive the AF Form 75 for vehicle registration, the individual must present and maintain a current and valid drivers license, vehicle insurance, vehicle registration for the vehicle they will be driving. In addition, the driver must have a clean driving record as required by the State of Florida. Failure to produce and/or maintain the previous documents shall be cause for non-issue or confiscation of the AF Form 75 and their driving privileges shall be suspended or revoked until these documents are presented valid and current.
Use of AF Form 75 by Another Individual: At no time shall an individual permit another individual to use or borrow his/her AF Form 75, Visitor/Vehicle Pass for their own use or to operate a motor vehicle to which a pass has not been issued. AF Forms 75 are issued to one individual and one vehicle only. If individuals are found to have violated this instruction, then both subjects driving and/or base entrance privileges may be suspended or revoked.
Contractor Vehicle Registration: The company owner, partner or site supervisor can register company owned/leased vehicle(s). , They must present a current drivers license, vehicle registration/lease agreement and proof of insurance for the company vehicle(s). Once the vehicle(s) has been registered, then the individual who registered the vehicle(s) shall be responsible for any individual who operates such motor vehicle(s).
BASE ENTRY
The West Gate (Westover at Bougainville) is open 24 hours a day.
The North Gate (Coral Sea at Bougainville) is only open when the west gate is closed for maintenance or as needed. The North Gate is open Saturday and Sunday for Unit Training Assemblies (UTA’s) on the primary reserve drill weekend or as determined by operational commitments.
If employees are required to work during non-duty hours (1630-0730) or on weekends, please contact the Police Services office during normal duty hours (0700-1600) at 305 224 7537 so that proper arrangements and notifications can be made. Emergency notification can be arranged through the on-duty desk sergeant at extension 7115/7116.
All deliveries shall be met at the base gate (by the contractor) for escort to the final delivery location and those individuals escorting shall be responsible for the individual(s) that they are escorting until such individual(s) exits the installation.
BASE ENTRY CHECKPOINTS
Driving on Homestead Air Reserve Base is a privilege. You and your employees are subject to random inspections of your vehicles. These inspections are done by the direction of the installation commander. During these inspections, Security Forces look for all unauthorized items i.e., guns, knives, contraband, stolen property, etc. These items can and will be seized as evidence. If any of these items are found, individuals will be suspended or barred from the base.

AUTHORITY: 5 U.S.C., 10 U.S.C., and the Internal Security Act of 1950 PURPOSE: To identify employees of Commercial Contractors that requires access to Homestead ARB, FL.
PRINCIPLE PURPOSES: The 482 Security Forces will forward the above information on each contractor employee to the U.S.
Immigration and Naturalization Service to determine employment eligibility in the U.S.
ROUTINE USES: None. Providing information is voluntary. Failure to provide the above information will cause the employee to be denied access to Homestead ARB, FL.
4.3.1. Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes. The contractor shall comply with all requirements and procedures IAW FAR 5352.242- 9000, Contractor Access to Air Force Installations, and this PWS. All documentation shall be submitted at the pre-performance conference.
4.4. CONTINUATION OF ESSENTIAL DEPARTMENT OF DEFENSE (DOD)
CONTRACTOR SERVICES DURING CRISIS. The Functional Commander has determined that this requirement is not Mission Essential (ME) IAW DOD Instruction 1100.22, Policies & Procedures for Determining Workforce Mix, Enclosure 4. Limited services may be required and may include collections from “all” or a “portion” of the containers on base.
4.5. SPECIAL QUALIFICATIONS. None.
4.6. MULTI-FUNCTIONAL TEAM. The Senior Leadership may require a multi-functional team be established between the government and the contractor to ensure joint cooperation and a sound customer focus of all parties involved in the execution of this contract.
4.7. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a complete copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.
4.8. NOTIFICATION OF ENVIRONMENTAL SPILLS. In the event that the Contractor spills, discharges or releases any pollutant into the environment, the Contractor shall immediately report the incident to the Fire Department at (786) 415-7117. This phone number is available seven days a week. The Contractor shall be liable for the costs of clean up and remediation of any spills, discharges or the releases of such substance into the environment while performing work on Homestead Air Reserve Base.
4.8.1. Environmental Spill Plan. The Contractor’s method of clean up shall be submitted, in writing, to the CO and approved 14 days prior to start of the contract performance.
4.8.1.1. The spill shall be cleaned up within three hours from the time of the spill. The methods the Contractor uses for cleanup and disposal must comply with all Federal, State, and local regulations and protocols.
4.8.1.2. A completed written state notification and/or manifest, if required, shall be provided to the CO within 60 days.
