Attachment I

Statement of Work

Program/Project Title: FA5587-18-Q-0109, 48 MDSS/SGSM, RAF Feltwell Warehouse Shelving

Location of Work: Bldg 70, Harvard Road, RAF Feltwell, Thetford, IP26 4HR

1. General Description:

1.1. The 48th MDSS/SGSM at RAF Feltwell, United Kingdom is requesting quotes for the removal & disposal of existing shelving and purchase & installation of new shelving.

1.2. The offeror shall provide all professional services, facilities, labor, equipment, tools, materials, travel, installation services, reports, specifications, and drawings associated with the project. Any changes suggested by the offeror to the Statement of Work (SOW) require written approval from the Contracting Officer (CO) before acceptance is given for a recommended change. Multiple awards will not be made for this solicitation.

2. Design, Delivery and Installation Plan:

2.1. Offeror shall provide a Removal, Design, Delivery and Installation Plan. The Plan shall describe how the offeror will meet the requirements of the SOW. The Plan shall include at a minimum, but is not limited to:

2.1.1. Details of electrical design and components

2.1.2. Risk assessment addressing methods and procedures of how the work will be completed to include health and safety concerns.

2.1.3. A schedule with milestones to complete work outlined in the SOW within the required project completion installation timeline.

3. Product Technical Specifications: Below are the minimum technical specifications requirements for this solicitation.

3.1. Adjustable pallet racking for guide railed system consists of:

3.1.1. Run A

3.1.1.1. 1 single face run comprising of 13 bays (type Bay 1 dimensions) and 1 bay (type Bay 2 dimensions). Total of 14 bays per run.

3.1.2. Run B-M

3.1.1.2. 6 double faced runs comprising of 13 bays (type Bay 1 dimensions) and 1 bay (type Bay 2 dimensions). Total of 28 bays per run.

4. Disassembly & Removal of Old Shelving:

4.1. Offeror shall be responsible for removal of existing shelving.

4.2. Run A – B

4.2.1. 1 double faced run comprising of 10 bays (type Bay 1 dimensions). Total of 20 bays per run.

4.3. Run C-D

4.3.1. 1 double faced run comprising of 10 bays type Bay 5 dimensions). Total of 20 bays per run.

4.4. Run E-F

4.4.1. 1 double faced run comprising of 10 bays type Bay 2 dimensions). Total of 20 bays per run.

4.5. Run G

4.5.1. 1 single faced run comprising of 12 bays (type Bay 4 dimensions). Total of 12 bays per run.

4.6. H-I

4.6.1. 1 double faced run comprising of 10 bays (type Bay 5 dimensions). Total of 20 bays per run.

5. Warranty and Support:

5.1. Offeror shall provide commercially acceptable warranties on all products, parts, accessories, services and maintenance support for a minimum of 12 months from the date of final product Acceptance. The warranty shall include at a minimum, but not limited to, parts, accessories, service and maintenance, etc. All products and accessories shall be free from manufacturing defects.

5.2. All products shall be installed per the manufacturer’s instructions, as well as be in compliance with all applicable laws, regulations, ordinances, and standards related to environmental matters.

5.3. The Offeror shall provide information on the life expectance of the structure which should be in accordance with commercially acceptable standards.

5.4. Offeror shall provide all products, parts, and accessory manuals.

6. Delivery:

6.1 All parts, materials, transportation and labor of delivery or ship (direct shipping only, not through consolidation point) all aforementioned items, fully assembled and in full working order to: Bldg 70, Harvard Road, RAF Feltwell, Thetford, IP26 4HR

6.2 Offeror shall make use of commercial best practices in the packing and shipment of products, unless otherwise stated in this order. Offers need to be submitted based on FOB destination. Inspection for final Acceptance shall be made by the requesting agency on behalf of the U.S. Government, FOB Destination.

6.3 Offeror shall coordinate all deliveries with the Primary or alternate Point of Contact

(POC).

7. Installation:

7.1. All installation requirements shall be completed by no later than 31 Dec 2018.

7.2. Offeror shall partner closely with 48th Medical Group, 48th Civil Engineering Squadron

(48th CES) and/or Ministry of Defense (MOD) Defense Infrastructure Organization (DIO) personnel.

7.3. The Offeror shall coordinate all installation efforts (times, dates, etc.) with Primary or alternate POC. The Government will provide the facility to the Offeror for install, measurements required for pre-installation and installation.

7.4. The Offeror shall perform all work in accordance with UK environmental laws, regulations and operating standards, including but not limited to the Final Governing Standards (FGS) for UK. The Offeror shall have an understanding and address environmental laws and regulations as they pertain to work performed under this contract.

7.5. Offeror shall be responsible for safekeeping of all material to be used in the work including the shelving, materials, and installation equipment. Prior to installation, the Offeror shall coordinate storage requirements with the Primary or alternate POC. The Government will make efforts to provide storage facilities. However, the Government is not obligated to provide storage facility.

7.6. Work shall be completed as expeditiously as possible. Note, the structure shall be occupied during the execution of this contract and during the installation phase.

8. Final Inspection and Product Acceptance:

8.1. The offeror shall work the Primary or alternate POC on final inspection and product acceptance. The offeror shall document all inspection results in an “Inspection Report(s)” and submit to Primary or alternate POC within three calendar days of completion of final inspection. During any inspection phase, the Government reserves the right to perform any of the offeror performed inspections to assure solutions conform to prescribed requirements. The offeror shall provide on-site support during the Final acceptance testing.

9. Site Restoration/Debris Removal:

9.1. Offeror shall protect all government and non-government property and personnel on base from inconvenience, damage and/or injury. All damaged areas during the working process should be restored to its original conditions.

9.2. Prior to project completion, the offeror shall remove all debris and surplus materials from the work place. Equipment and materials required to complete the work effort may remain on site during the project as long as equipment and materials are organized/stored in a manner that does not cause a safety or security hazard. The offeror shall return all disturbed areas to their original condition upon completion of the installation.

9.3. All equipment and residuals removed from this project including electronics, conduits and cabling, etc., shall be disposed by the Offeror in accordance with UK waste disposal rules.

9.4. If applicable, copies of any waste transfer notes shall be sent to Defense Infrastructure Organization (DIO) point of contact and to the Point of Contact.

10. Base Access:

10.1. Offeror shall ensure that sufficient personnel can access RAF Feltwell to complete the assembly/installation. Offeror shall also ensure sufficient personnel are cleared to sign-in onto the base.

10.2. In order to obtain base access, the offeror shall submit a written request on their company letterhead to the Primary or alternate POC at least 5 days prior to installation listing the following information: contract number, start and stop dates, names of employees and/or subofferor employees needing access to the base with a copy of the employees and/or subofferor employees valid US, UK or EU picture ID (example - Passport, Driver License, etc.). Upon entering the Base employees and/or subofferor employees shall possess and show valid US, UK or EU picture ID (example - Passport, Driver License, etc.).
