Statement of Work For

Purchase and Installation of Gym Equipment for 320 STS Building 3522

Kadena AB, Okinawa, Japan

320 STS, Kadena AB

1. Description of services - The contractor must provide all personnel, equipment, tools, materials, supervision, transportation, safety equipment, any other items and services necessary to perform installation of Government purchased gym equipment as defined in this Statement of Work (SOW) in the 320 Special Tactics Squadron, Building 3522 on Kadena AB, Japan.

1.1 Installation of Gym Equipment

1.1.1 The contractor must provide, properly assemble equipment, (to include tuning, calibration, straps and cable inspection) in the government facility and room IAW Government approved installation floor plan. Contractor will ensure equipment fits through all doorways and stairwells.

1.1.2 Liability for damages – The contractor will be held responsible for any damages that occur by the contractor and/or any personnel that work under this contract to Government facilities, Government owned property, and Government purchased equipment.

1.1.3 Contractor is responsible for door to door delivery, unloading, assembly in the correct room, and removal of rubbish.

1.1.4 Contractor shall closely coordinate schedule with the 320 Special Tactic Squadron to avoid or minimize downtime and interruption of the mission and operation.

Estimated delivery date and installation is NLT 90 days after date of award.

1.2 CLEANING AND PROTECTION

1.2.1 Maintain Walk Through - The contractor must maintain common walk through areas to allow proper evacuation of the building in the case of emergencies.

1.2.2 Remove Rubbish - The contractor must remove rubbish, wrapping paper, salvages and scrap. The contractor must not leave waste material inside or outside of the facility overnight, or for a prolonged period of time, when work is not done. Remove and dispose of waste materials only at authorized off base locations, daily.

1.2.3 Vacuum Carpets - The contractor will vacuum the government area that they were working at the end of each day.

1.2.4 Final Cleaning - The contractor must remove packing material, boxes, broken items, spots, smears, and other foreign matter.

2. GOVERNMENT-FURNISHED PROPERTY AND SERVICES

2.1.1 Utilities. The government will provide water and electricity, when available.

2.1.2 Conservation of Utilities. The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions, which prelude the waste of utilities, which shall include:

2.1.3 Lights shall be used only in areas where and when work is actually being performed.

2.1.4 Water faucets or valves must be turned off after the required usage has been accomplished.

2.1.5 Government or private telephones shall not be used for personal reasons or for any toll or long distance calls.

3. GENERAL INFORMATION.

3.1 HOURS OF OPERATION. The contractor must perform the on- site services required under this contract during the following hours: 08:00 am- 16:30 pm, Monday through Friday, except on U.S. holidays specified in section 3.1.1 of this SOW and PACAF Family days. No on site work will be scheduled or performed on holidays unless previously approved or directed by the Contracting Officer. If the contractor desires to work on non-scheduled workdays, he must submit a written request to the POC for approval.

3.1.1 Holidays - The contractor is not required to provide service, except as noted on the following days:

If holiday fall on a Saturday, it will be observed on the preceding Friday. If it falls on a Sunday it will be observed on the following Monday.

3.1.2 SECURITY REQUIREMENTS - The contractor must be responsible for safeguarding all contractor controlled government property at the close of each work period.

3.1.3 Pass and ID - The contractor must obtain and ensure employees have the necessary identification and gate passes in accordance with U.S. military installation regulations prior to the start of this contract. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass.

3.1.4 Safety and Fire - The contractor must comply with applicable government published safety requirements and regulations which will be available upon request by the contractor.

3.2 CONTRACTOR PERSONNEL REQUIREMENTS.

3.2.1 Contract Manager - The contractor must provide a contract manager who will be responsible for the performance of the work. The name of this person, and an alternate(s), who will act for the contractor when the contract manager is absent, must be designated in writing to the Contracting Officer, 5 working days prior to start of projects. The contract manager or alternate will have full authority to act for the contractor on all contract matter relating to the daily operation of this contract.

The contract manager or alternate will be available during normal duty hours with sixty (60) minutes to meet on the installation with the government personnel (designated by the Contracting Officer) to discuss problem areas. After normal duty hours, the manager or alternate must be available within two (2) hours. The contract manager or alternate(s) must be able to read, write, speak, and understand English.

3.2.2 Contractor Personnel - Contractor personnel must present a neat appearance and be easily recognized as contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contains the company and employee name in English. Contractor personnel must use protective clothing whenever they are performing tasks that could cause harm to the employees or damage to property.

3.2.3 The contractor must not employ persons to work on this contract if such person is identified to the contractor by the Government as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population.

3.2.4 Smoking – Smoking is not permitted in any government building or work area.
