Statement of Work For

Removal and Installation of Furniture in a Military Facility Kadena AB, Okinawa, Japan

718 CES/CEHU, Kadena AB

1. Description of services - The contractor must provide all personnel, equipment, tools, materials, supervision, transportation, safety equipment, any other items and services necessary to perform installation of Government purchased furniture as defined in this Performance Work Statement (PWS) in a military dormitory facility located on Kadena Air Base, Okinawa, Japan.

1.1 Government Responsibilities

1.1.1 The Government will ensure dormitory resident’s personal belongs are secured and are not impeding the contractor from removing the old existing furniture from individual living quarters.

1.1.2 The contractor must have an escort during removal and installation of the furniture.

- Airman Dorm Leader (ADL)

- Dorm Representative (assigned by ADL)

1.1.3 The Government will provide the contractor with DLA Disposition Services paperwork for the items that must be turned into DLA Disposition Services.

1.2 Existing Furniture

1.2.1 The contractor must obtain their own DLA Disposition Services appointments for the furniture items that must be turned into DLA Disposition Services. All other furniture items (such as mattresses and box springs) that aren’t accepted at DLA Disposition Services must be destroyed and disposed of properly at an off base location. NOTE: The Government must not be held liable for any expenses that the contractor may incur due to disposing items rejected by DLA Disposition Services at an off-base location.

1.2.2 Existing Furniture in Rooms: 114, 115, 117, 118, 221, 222, 223, 224, 323, 324, 423, & 424 must remain in in each room.

1.3 New Furniture

1.3.1 The contractor must properly assemble and install all furniture into the government facility and rooms with the exception of those listed below.

1.3.2 New furniture will not be installed in Rooms: 114, 115, 117, 118, 221, 222, 223, 224, 323, 324, 423, & 424.

1.4 CLEANING AND PROTECTION

1.4.1 Maintain Walk Through - The contractor must maintain common walk through areas to allow proper evacuation of the building in the case of emergencies.

1.4.2 Remove Rubbish - The contractor must remove rubbish, wrapping paper, salvages and scrap. The contractor must not leave waste material inside or outside of the facility overnight, or for a prolonged period of time, when work is not done. Remove and dispose of waste materials only at authorized off base locations, daily.

1.4.3 Vacuum Carpets - The contractor will vacuum the government area that they were working at the end of each day.

1.4.4 Final Cleaning - The contractor must remove packing material, boxes, broken items, spots, smears, and other foreign matter.

1.5. CONTRACTOR-FURNISHED ITEMS AND SERVICES

1.5.1 The contractor must obtain and maintain the necessary equipment and personnel to safely remove and install furniture.

2. GOVERNMENT-FURNISHED PROPERTY AND SERVICES

2.1 Utilities. The government will provide water and electricity, when available.

2.2 Conservation of Utilities. The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions, which prelude the waste of utilities, which shall include:

2.2.1 Lights shall be used only in areas where and when work is actually being performed.

2.2.2 Water faucets or valves must be turned off after the required usage has been accomplished.

2.2.3 Government or private telephones shall not be used for personal reasons or for any toll or long distance calls.

3. GENERAL INFORMATION.

3.1 QUALITY ASSURANCE - The government will periodically evaluate the contractor’s performance by appointing a Government representative(s) to monitor performance to ensure services are received. The government representative will receive complaints from base personnel. The government representative shall perform inspection and acceptance of items/services. Likewise, the government will also receive and investigate complaints from various customers located on the installation. The contractor shall be responsible for initially validating customer complaints. However, the government representative will make the final determination of the validity of the customer complaint(s) in cases of disagreement with the customer(s).

3.1.1 The below items will be sent to DLA Disposition Services (this is an estimate):

Between 150 – 200 Beds Between 150 – 200 Desks Between 150 – 200 Wooden desk chairs Between 180 – 200 End Nightstand Between 80 – 100 Entertainment Stand Between 180 – 200 Twin Mattresses

3.2 HOURS OF OPERATION. The contractor must perform the on- site services required under this contract during the following hours: 08:00 am- 16:00 pm, Monday through Friday, except on U.S. holidays specified in section 3.2.1 of this PWS and PACAF Family days. No on site work will be scheduled or performed on holidays unless previously approved or directed by the Contracting Officer. If the contractor desires to work on non-scheduled workdays, he must submit a written request to the POC for approval.

3.2.1 Holidays - The contractor is not required to provide service, except as noted on the following days:

If holiday fall on a Saturday, it will be observed on the preceding Friday. If it falls on a Sunday it will be observed on the following Monday.

3.3 SECURITY REQUIREMENTS - The contractor must be responsible for safeguarding all contractor controlled government property at the close of each work period.

3.3.1 Pass and ID - The contractor must obtain and ensure employees have the necessary identification and gate passes in accordance with U.S. military installation regulations prior to the start of this contract. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. The 718 CES/CEHU shall not under any circumstances provide escort personnel to the contractor or sub-contractors for this contract.

3.3.2 Safety and Fire - The contractor must comply with applicable government published safety requirements and regulations which will be available upon request by the contractor.

3.4 CONTRACTOR PERSONNEL REQUIREMENTS.

3.4.1 Contract Manager - The contractor must provide a contract manager who will be responsible for the performance of the work. The name of this person, and an alternate(s), who will act for the contractor when the contract manager is absent, must be designated in writing to the CO 5 working days prior to start of projects.

The contract manager or alternate will have full authority to act for the contractor on all contract matter relating to the daily operation of this contract. The contract manager or alternate will be available during normal duty hours with sixty (60) minutes to meet on the installation with the government personnel (designated by the Contracting Officer) to discuss problem areas. After normal duty hours, the manager or alternate must be available within two (2) hours. The contract manager or alternate(s) must be able to read, write, speak, and understand English.

3.4.2 Contractor Personnel - Contractor personnel must present a neat appearance and be easily recognized as contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contains the company and employee name in English. Contractor personnel must use protective clothing whenever they are performing tasks that could cause harm to the employees or damage to property.

3.4.3 The contractor must not employ persons to work on this contract if such person is identified to the contractor by the Government as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population.

GOVERNMENT SERVICES

1. Government Services. The Government will provide water and electrical power when available to the contractor.
