PERFORMANCE WORK STATEMENT (PWS)
Zone 5 Cable Upgrade
Yokota AB, Japan
24 Aug 2018
1. DESCRIPTION OF SERVICES.
1.1. Background. The Contractor shall provide all personnel, equipment, tools, vehicles, materials, supervision, and other items and services necessary except those specifically identified in Section 8 of this document, Government Furnished Property and Services, to install outside plant (OSP) single mode (SM) fiber optic cable at Yokota Air Force Base, Japan. The Contractor shall perform replacement services in accordance with (IAW) this Performance Work Statement (PWS), manufacturer standards, commercial practice and local standards. All parts and materials must be compliant with the Trade Agreement Act
(TAA).
1.2. Objective. The Contractor shall install, fusion splice, terminate and test ribbon OSP Single Mode
(SM) fiber optic cable throughout the Yokota AB B5 circuit outlined within this PWS. The Contractor shall utilize drawings for cable routes, and all splice locations. The Contractor shall utilize this PWS for splicing methods, fiber counts, testing methods, labeling, and termination methods. All distances are approximates.
1.3. Scope. This PWS defines the requirements for the Contractor to install, splice, terminate and test 864 strands of SM fiber optic ribbon type cable (FOC) from Bldg. 8 to the Bldg. 1246, and create 4 splice points for Bldgs. 87, 85, 82, 80, 71, 60, and Military Messaging Handling System (MMHS). Install 24 strands of SM ribbon type FOC from each facility to the designated splice points. Splice facilities ribbon
1 (strands 1-12) to Bldg. 8, and ribbon 2 (strands 13-24) to Bldg. 1246. All other strands will be spliced straight through ribbon 1 to 1, 2 to 2, etc.
2. GENERAL INFORMATION
2.1. Quality Control. The Contractor shall develop and maintain a quality program to ensure waste management and removal are performed in accordance with commonly accepted commercial practices.
The Contractor shall develop and implement procedures to identify, prevent, and ensure safe and non-defective services. As a minimum, the Contractor shall develop quality control procedures addressing the areas identified in paragraph 5, Services Summary (SS).
2.2. Quality Assurance. The Government will evaluate the Contractor’s performance by appointing a
COR to monitor the Contractor’s performance to ensure services are received. The COR will evaluate the
Contractor’s on-base performance periodically through intermittent on-site inspections in accordance with the work schedule, SS, and 100% inspection upon project completion.
2.3. Government Remedies. The Contracting Officer (KO) shall follow FAR 52.212-4, Contract Terms and Conditions - Commercial Items, in response to the Contractor’s failure to perform satisfactory services or failure to correct nonconforming services.
2.4. Base Passes. The Contractor’s employees will be issued base passes (identification cards), which shall be carried by the individual at all times while on Yokota Air Base. The Contractor shall submit written requests for issuance of base passes to the KO, and shall be responsible for all base passes issued to his/her employees. The Contractor shall return all base passes to the Japanese Pass and ID Section at
Yokota Air Base upon termination of employment or completion of the contract.
2.5. Contractor Employees. The Contractor shall not employ persons for work on this contract if he/she is identified to the Contractor by the KO as a potential threat to the health, safety, security, general well-being or the operational mission of the installation and its population.
2.5.1. The Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee's names. The Contractor shall adhere to all base safety and traffic regulations and policies.
2.5.2. The Contractor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest. The Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies of the Department of the Air Force.
2.5.3. The Contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours or deployment. Military
Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the Contractor’s ability to perform under this contract;
however, their absence at any time shall not constitute an excuse for nonperformance under this contract.
2.6. Delivery and Storage of Materials. All materials delivered and placed in storage shall be protected from the weather, humidity and temperature variations, dirt and dust, or other contaminants by the
Contractor.
2.6.1. Disposal of Materials. All debris and unserviceable materials generated under this contract such as industrial waste, non-recyclable materials and equipment/material not listed on hand-receipts shall be disposed of off-base by the Contractor in accordance with the laws of Japan. Hand receipt items require turn-in action by the Contractor when the listed items have to be replaced.
2.7. Responsibility for Damages. Any damages caused to U.S. Government property during the course of performing this contract shall be restored, replaced and/or repaired to its original condition at no additional cost to the U.S. Government prior to final payment.
3. CONTRACTOR FULL-TIME REPORTING
3.1. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The
Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
3.1.1. Reporting inputs will be for the labor executed during the period of performance for each
Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October each calendar year. Contractors may direct questions to the CMRA help desk.
4. CONTRACT REQUIREMENTS.
4.1. Installation.
4.1.1. Bldg. 8 to Bldg. 1246. Install 864 strand SM ribbon type fiber optic cable (Fiber cable must be
American standard color code), from Bldg. 8 to Bldg. 1246 utilize routes provided. If an alternate route is available at no additional expense, approval from 374 CS/SCXP PM is required before proceeding. When utilizing an empty duct, install 3 slot (3”) Maxcell with fiber cable ( located at MH
46 through MH 6-12 // distance is appr. 3500 ft.).

4.2. Splice.
4.2.1. Splice Enclosure specifications. The closure (dome type) should support future growth (over
1,000 strands), be re-enterable, utilize a grommet system, and have at a minimum 7 entry ports that can be subdivided utilizing grommets, it must also support cable diameters up to 1 1/4 inch in diameter. The closure shall be sealed and suitable for enclosing a splice organizer in a protective housing. The splice organizer shall consist of splice trays and protective sleeves, and ancillary hardware necessary to house fiber optic splices. The splice tray shall house a minimum of 288 fiber optic fusion splices in a neat and orderly fashion. The splice tray shall provide individual support for each splice. The organizer shall allow for one meter of fiber to be stored without kinks, twists, or micro-bends. The organizer shall be suitable for reentry, for future maintenance or modification, without damage to the fiber or splices. The closure shall be a Pre-formed Coyote Dome or equivalent. All splice closures will be identical. Current splice cases being utilized fall under
Preformed Line Products manufacturer part number 80061056.
4.2.2. Splice method. All splicing will be fusion type only. The splicing method will be duel home type. Splice facilities ribbon 1 (strands 1-12) to Bldg. 8 and ribbon 2 (strands 13-24) to Bldg. 1247.
All other strands will be spliced straight through ribbon 1 to 1, 2 to 2 etc.

4.3. Terminate.
4.3.1. Terminate BLDG. 1246 and 8. Utilize Government furnished fiber optic distribution panels
(FODP). The FODPs are fusion splice type, preloaded with ribbonized LC pigtails.
4.3.2. Terminate Bldg. 87, 85, 71, 80, 82, 60, MMHS. Utilize Government furnished FODP. The
FODPs are fusion splice type, preloaded with ribbonized LC pigtails.
4.3.3. Termination locations. All terminations will be completed inside the facility’s communication rack in the communication room.
4.4. Test.
4.4.1. Fiber optic tests. All fiber strands shall be tested and capable of 10 Gig Transfer rates with no more than 3 DB Lose (Define DB). Both Optical Time Domain Reflectometer (OTDR) and Optical
Power Meter will be used for all end-to-end circuits. Between FODPs, bi-directional testing at
1310nm and 1550nm is required. All test results will be sent to 374 CS/SCXP. Test results will be verified prior to project completion.
4.5. Label.
4.5.1. Label cable in manhole. All cables will be labeled in the manholes. Utilize a permanent label that will not fade due to water and time.
4.5.2. Label cable at FODP. Label the cable at the FODP entrance and on the front of the FODP.

4.6. Drawings.
4.6.1. As-built drawings. The Contractor will provide as-built drawings for changes made to the affected circuits under this contract. The drawings will show only the newly installed fiber optic cables (conduit utilization). Drawings will be sent to 374 CS/SCXP (PDF format).
5. PERFORMANCE OBJECTIVES.
5.1. Services Summary (SS): The following Services Summary (SS) identifies the performance objectives and performance thresholds for critical tasks associated with providing support services for this requirement. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

**Note: Upon completion of aforementioned Task 6, the COR will schedule inspection with 374
CS/SCCP. 374 CS/SCXP will schedule inspection with back shops. An AFTO 747 acceptance will be generated by the plans work center and routed for 374 CS/CC acceptance.
6. PERFORMANCE SCHEDULE.
6.1. Place of Performance: All work shall be performed on Yokota AB. Reference Appendix A for the location.
6.2. Period of Performance: All work shall be completed within 8 months after the date of contract award. The Contractor shall be available for a post-award conference with the COR and all related customers upon notification of the conference prior to the commencement of the work. The Contractor shall be available for a post-award meeting with the customer no later than two weeks after the award date, or as directed by the COR, 374 CS/SCXP 225-9690. Additionally, the Contractor may be required to account for the location of their personnel should this information be requested.
7. INSTALLATION PLAN. The Contractor shall arrange an installation plan and schedule with the
COR, and obtain an approval from the Site POC prior to the commencement of the work.
7.1. Arrangements of the Performance Time. The Contractor will coordinate all after hour work with the COR, 374 CS/SCXP 225-9690, and shall do so at no additional cost to the Government.
7.2. Hours of Service. The Contractor’s personnel shall perform scheduled work on weekdays, Monday through Friday (exclusive of the below U.S. legal and other designated holidays), during normal duty hours, 0800 - 1630 hours unless prior arrangements have been made with the COR. Holidays falling on a
Saturday will be observed on the following Monday.

7.3. Point of Contact (POC). The authorized COR for this contract can be reached at 374 CS/SCXP
225-9690 or 225-4145. The Contractor shall coordinate all aspects of this project with the COR.
Completed work shall be subject to inspection and acceptance by the COR prior to final invoice and payment under this contract.

9. OTHER PERTINENT INFORMATION OR SPECIAL CONSIDERATIONS.
A site visit will be held on August 21, 2018 at 09:30 A.M., meet at Yokota Air Base Gate 5 (also known as Supply Gate). The Contractor shall be responsible for gathering all information needed for the work to be completed. All questions shall be submitted in writing to the Contracting Officer (CO) and
Contract Administrator (CA) by August 23, 2018.
9.1. Contractor Employees Qualifications.
The Contractor shall have confined space certification, and at least three (3) years of experience in construction, outside plant fiber optic cabling installation or maintenance and testing. At a minimum, key personnel shall have a professional knowledge of communications engineering concepts, principles and practices and extensive familiarity with other engineering disciplines; knowledge of a broad range of networking standards, methods, practices, techniques, materials and equipment to ensure work is in compliance with regulations and standards.
The Contractor shall be responsible for the transportation of all system equipment to and from the site locations. Upon request of the Contractor, the Government may provide temporary storage at a nearby warehouse. In such case, the Contractor shall be responsible for inventory and reconciliation of all shipments, which will be coordinated with and verified by a Government representative. The Contractor shall transport all equipment from the warehouse to the final destination.
The Contractor shall coordinate site visits through the COR. The COR will provide any applicable forms needed in regards to site visits. The Contractor shall provide all necessary site visit information to the
COR or designated Site POC (374 CS/SCXP) at least 10 working days prior to the requested visit date.
The Contractor shall coordinate all deliveries with the COR or designated Site POC at least 10 working days prior to delivery, and provide all necessary site visit information required for the delivery. This will ensure visit clearances are processed and obtained from the local Security/Visitor Control Office. Visits include, but are not limited to, meetings, surveys, testing, deliveries, etc.
The Contractor shall provide a standard commercial warranty to correct possible defects in materials and workmanship. The warranty shall be provided for one year commencing at the time of final system acceptance, and shall cover all equipment purchased and installed, including but not limited to, infrastructure equipment.
