STATEMENT OF WORK (SOW)

FOR

PROJECT ZNRE 16-4329

REPL SMOKE ALARMS, NORTH AND EAST MFH (349 UNITS)

AT

YOKOTA AB

Date: 29 May 2015

I. PRINCIPAL FEATURES OF WORK: The Contractor shall verify site conditions and conduct all field surveys and measurements as necessary to complete this project. All performance under this project shall meet or exceed US Government and Japanese Construction Standards. The Contractor shall provide all labor, materials, equipment and anything necessary to complete this project to ensure a complete and usable facility.

II. GENERAL SCOPE:

A. Overall objective: Replace existing multiple station smoke detectors with new.

B. General Description of Electrical Work:

1. All work necessary to replace/install smoke detectors and wirings in North and East Military Family Housing (MFH) garden units. Project buildings are 1090, 1091, 1093, 1094, 1095, 1096, 1097, 4401, 4405, 4407, 4411, 4414, 4415, 4418, 4420, 4421, 4423, 4424, 4426, 4427, 4429, 4441, 4453, 4455, 4456, 4458, 4461, 4464, 4466, 4504, 4506, 4510, 4512, 4516, 4518, 4519, 4522, 4523, 4525, 4526, 4529, 4532, 4533, 4536, 4540, 4630, 4634, 4636, 4702, 4706, 4708, 4712, 4714, 4715, 4718, 4721, 4722, 4726, 4731, 4732, 4735, 4741, 4803, 4807, 4811, 4812, 4815, 4816, 4818, 4819, 4822 4823, 4825, 4829, 4830, 4831, 4901, 4904, 4905, 4906, 4909, 4910, 4913, 4914, 4917, 4918, 4921, 4923, 4927, 4929, 4930 and 4933.

2. Provide multiple station photoelectric smoke detectors, 120VAC type with 9V alkaline standby battery and 10% spare detectors.

3. Detector shall contain an 85 dBA minimum alarm sounder at 10ft, test button and visible LED indicating lamp.

4. Protect the furniture of the resident during construction.

IV. EXISTING CONDITIONS CONSIDERATION:

A. The design and analysis shall be calculated based on the attached as-built drawings.

B. The contractor is responsible for correcting any unplanned / unapproved adverse impacts to the facility due to their work at no additional costs to the Government.

V. DESIGN POLICY AND CRITERIA

A. The Government will furnish the basic design criteria to the contractor from which the project documents can be developed and produced as described below. Suggestions that will improve the operational functions, enhance appearance, and prove to be more economical and advantageous shall be made at the appropriate review conferences. The design must be accepted by the government before construction can start.

1. Design instructions: The design shall incorporate the requirements as discussed in the pre-and post-award conferences and site visits as well as the requirements of the publications listed below. Modern construction techniques may be incorporated into the project design to obtain both quality and economy and to provide a functional, complete, and useable facility.

2. Cost saving techniques: The design is to provide functionality without embellishment.

Also, the design is to achieve an optimum balance between aesthetic consideration, operations and maintenance costs, energy conservation measures, and durability of materials and systems. Design and support items will be limited to the minimum essential support construction required to provide a useable facility. Every effort will be made to reduce utility runs and site preparation requirements through proper design.

B. Government furnished information: The age of government data and as-built drawings, along with the fact that there has often been undocumented tenant construction, reduces their usefulness and reliability. The contractor shall verify the data shown on government-furnished reference documents by performing reconnaissance, surveys, and field measurements.

VI. DESIGN SERVICES

A. Site design data: The contractor shall perform field reconnaissance, visual surveys, and site investigations in accordance with requirements of this project. During the site data collection, the contractor shall record all existing conditions (inside and outside) and obtain engineering information/design data for the accomplishment of this project. All material sampling, testing, and removal shall be performed in accordance with the JEGS.

B. Field measurements: The contractor shall be required to make field investigations to verify dimensions and other information shown on Government-furnished reference data and drawings.

C. General: The contractor will consider all design options and criteria reductions that may be necessary, in those instances, where possible cost overruns may be expected. All drawings, and specifications shall be prepared, reviewed, and approved by registered professional architects and engineers. All hardcopy and electronic submittals of the drawings, specifications, basis of design, construction schedule, and submittal register shall be submitted and delivered to the contracting officer. The contractor will submit 90% and 100% design.

One copy of the final drawings shall be submitted in specified editable electronic format for incorporation into Geobase.

D. Drawings: Drawings shall be prepared on AutoCAD release 2012 using the BCE computer aided drafting (CAD) layering standards. All contract drawings shall be well prepared, complete, and accomplished in accordance with the best professional practice.

1. Appearance: Drawings shall show clearly and concisely the type and extent of work to be performed in the project. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Notes on the drawings should be held to a minimum. Extensive explanatory notes on the drawings are not desirable and should be placed in the specifications. All units shall be shown in metric only. Standard building material indications and symbols for architectural items and for civil, architectural, mechanical, electrical and fire protection items and equipment shall be used to the greatest extent possible. The use of figures less than three (3) millimeters for building dimensions shall be limited. Drawings shall be delivered in hard copy, standard quality paper at review stages in numbers listed and in accordance with the requirements. Full size drawings shall be approximately 600 x 840 millimeters (drawing size A1).

E. Specifications: Specifications shall be delivered in hard copy, bound, and at the various stages and quantities listed and in accordance with the requirements. Specifications shall be prepared in Spec Intact (UFGS) that incorporate JASS and JISS ( MS Word version 6.0 or higher format). Each specification page shall contain a header and footer with the project number in the bottom left corner and the specification section and page number in the center separated by a dash. Specifications, including all technical and special conditions, shall be based and prepared on a format modified for Air Force construction, as directed by the CO. No references will be made to Military and Federal specifications or standards, but rather, all relevant Military and Federal specifications or standards shall be incorporated directly into the design. In general, the use of "proprietary type" specs shall be avoided. When a brand or manufacturer's name is used to describe a type of product, the words "or approved equal" shall be included. In the latter instance, more than two (2) acceptable brand or manufacture’s names shall be specified. The CO shall specify all materials, components, and equipment for approval. All items designated for submittal and test shall be sufficiently described to provide the Government a basis for review for approval or disapproval.

1. Japanese Equivalent: Where qualifications, equipment, materials, or installations are to be in accordance with American standards, the equivalent Japanese standard may be used subject to the approval of the Contracting Officer. The Contractor shall be responsible for proving the equivalency of the Japanese standard.

2. Quality Class or Grade of Materials: When materials are not specified certain quality class or grade, above the middle class or grade quality shall be applied to these materials.

F. Basis of Design: The A-E shall provide a basis of design (BOD) to insure the Civil, Architectural, Structural, Mechanical, Fire Protection, Environmental, Electrical, and other discipline parameters are adequately and appropriately explained and justified for the proposed design. The BOD shall be bound, arranged by discipline, support all design decisions made throughout the design, and be prepared using Microsoft (MS) Word format. The BOD shall include calculations, material catalog cut sheets and explanations of any Options-Additives considered. The BOD will also contain considered design alternatives and criteria reductions that were necessary where possible cost overruns were expected, previous review comments annotated with action taken, and a listing of any outstanding issues requiring resolution, each as a separate section.

G. Pre-final design submittal (90%)

1. The review of this submittal is to insure that the design is in accordance with directions provided the Contractor during the design process. The Contractor shall submit the following documents for Final Design Review:

a. Design analysis, developed to 90%

b. Drawings to 90%.

c. Specifications

d. Annotated 60% Review Comments

e. Draft submittal register

2. General Requirements:

a. A substantial number of comments generated by the Government or comments indicating that constructability and/or compliance with the Task Order is not apparent in this submittal shall constitute grounds for the requirement of another, more complete 90% design submittal.

b. Design Analysis: The design analysis submitted for the 90% design review submittal shall be in its final form. The design analysis shall include all backup material previously submitted and revised as necessary. All design calculations shall be included. The design analysis shall contain all explanatory material giving the design rationale for any design decisions that are not obvious to reviewers for the Final Drawings and Specifications.

c. Drawings: The Contractor is expected to have completed all of his coordination checks and have the drawings in a design complete condition. The drawings shall be complete at this time including the incorporation of any design review comments generated by the Preliminary design review. The drawings shall contain all the details necessary to assure a clear understanding of the work throughout construction. Shop drawings will not be considered as design drawings. All designs shall be shown on design drawings prior to submittal of shop drawings.

d. Specifications: The Draft Specifications on all items of work shall be submitted for the 90% design review submittal. Specifications shall be printed with mark-ups to show proposed changes to the UFGS. The mark-up can be done by using the SPECTSINTACT revisions feature.

e. Annotated Review Comments: Contractor shall provide all annotated review comments from the 60% submittal review.

f. Submittal Requirements: Submittal requirements below are in addition to requirements.

1) Electrical Requirements: Design Analysis, Drawing and Design Information

a) Provide the existing and new plans, one line diagrams and typical/detail drawings.

H. Final design submittal (100%)

1. This submittal is to insure that the design incorporates the government review comments. This submittal shall be submitted in digital format as well as hard copies, and include the following:

a. Design analysis

b. Drawings

c. Specifications

d. Annotated 90% Review Comments

e. Submittal register

2. Deviating from the accepted design:

a. The Contractor must obtain the approval of the contracting officer for any major deviations from the 90% design.

b. The Government reserves the right to non-concur with any revisions to the design, which may impact equipment selections or operations decisions that were made, based on the reviewed and concurred design.

c. Any revisions to the design, which deviates from the contract requirements will require a modification with concurrence of the Government. The Government reserves the right to disapprove such a revision.

d. Unless the Government initiates a change to the contract requirements, or the Government determines that the Government furnished design criteria are incorrect and must be revised, any Contractor initiated proposed change to the contract requirements, resulting in additional cost, shall strictly be at the Contractor’s expense.

e. The Contractor shall track all approved revisions to the reviewed and accepted design and shall incorporate them into the as-built design documentation, in accordance with agreed procedures. The Designer of Record shall document its professional concurrence on the as-built drawings for any revisions to the stamped and sealed drawings and specifications.

3. Other Requirements: The design will not be deemed 100% Final until the government has accepted all response(s) to comments made on previous submittals. The contractor shall make such required revisions at no additional cost to the Government.

4. Design submission copy quantities: All submittals shall occur in the number and format as prescribed below:

Note: 1, Specification Index and Specification Sections will be combined into one (1) bounded booklet.

2, Design submission electronic media quantities: Two (2) Compact Disc (CD) shall be submitted at each design phase (90% and 100% design) and include all the items submitted in hard copy.

VII. GENERAL INFORMATION:

A. Performance Period:

1. The performance period for design will be 90 days (including government review of 14 calendar days for each design stage), and construction will be 240 calendar days, total of 330 calendar days after award contract.

2. The government shall have 14 calendar days to respond to each design phase submittal.

A. Working Hours: During construction or/and tests, the facilities will be made available to the contractor from 07:30 to 16:30, Monday through Friday, 5 days a week, excluding U.S.

legal holidays but including Japanese legal holidays. Contractor must notify the Government in advance if work needs to be performed outside these working hours. Contractor shall coordinate schedule for design site visits with the government at least 21 days ahead of time.

B. Holidays: Recognized US legal holidays are listed below:

a. January 1 - New Year’s Day.

b. January, 3rd Monday - Martin Luther Kings Jr. Birthday.

c. February, 3rd Monday - President’s Day.

d. May, last Monday – Memorial Day

e. July 4 - Independence Day.

f. September, 1st Monday - Labor Day.

g. October, 2nd Monday - Columbus Day.

h. November 11 - Veteran’s Day.

i. November, 4th Thursday - Thanksgiving Day.

j. December 25 - Christmas Day.

k. Holidays falling on a Saturday will be observed on the preceding Friday.

Holidays falling on a Sunday will be observed on the following Monday.

C. Weekend work: Do not perform weekend work without advance permission. Submit weekend work requests at least the Wednesday before the desired weekend workday. Only proceed with weekend work after you receive Government approval.

D. Occupancy: The facility will be occupied. Contractor shall provide construction plan and schedule, and notify and coordinate with the users and occupants a minimum of 1 week prior to all work to be performed.

E. Utility outages: Provide at least 21 calendar days advance notice of planned utility outages including fire systems. In each request, specify each utility disconnected, the time and date the service will be deactivated and the anticipated time and date service will be reactivated. Do

not disconnect or deactivate any base utility systems without advance contracting officer or construction inspector approval. Fire system disconnection and reactivation requires 374 CES/CEF (Phone 225-9112) coordination.

F. Barricades & warning signs: Prominently post bilingual (English & Japanese) “OFF LIMITS”, “WET PAINT” and “NO SMOKING” signs, as applicable, at all entrances to the job site. Stencil (paint) the black lettering in all capital letters on either 1’-0” x 2’-0” or 2’-0” x 3’-0” sheet metal or plywood signs. Protect all overnight open excavations with barricades & battery-powered flashers. Smoking is prohibited in government facilities. Smoking is allowed in the Designated Tobacco Areas (DTAs) only.

G. Utilities: The US Government will furnish water & electricity at no cost in accordance with FAR 52.236-14, Availability and Use of Utility Service (Apr 1984). Access and connection are the responsibility of the contractor.

H. Material disposal: Remove and dispose of all construction debris off base daily in accordance with Japanese laws. In case that the asbestos debris remains on base after workday, the debris needs to be properly packed or containerized in compliance with debris regulations and any of the debris shall not be scattered.

I. Final inspection: 374 CES will perform inspections. Schedule final inspections between 0800 and 1600, Monday through Friday, excluding US legal holidays. The government will perform the inspection within 3 work days after contractor initiates the final inspection request. Final inspections falling on weekends or US legal holidays will be postponed to the following workday. Postponements will not be considered part of the performance period.

J. Asbestos containing material (ACM) identified: To the best of the Government's knowledge, any and all asbestos has been identified and abated. If the contractor suspects additional asbestos containing material (ACM), the contractor shall promptly notify the construction inspector for action by the Government. Contractor shall not sample nor remove asbestos without first obtaining permission from the government.

K. Training of Government personnel: The contractor shall furnish the services of competent instructors who will give full training to the designated personnel in the adjustment, operation, and maintenance, including pertinent safety requirements, of the equipment or system installed. Each instructor shall be thoroughly familiar with all parts of the installation and shall be trained in operating theory as well as practical operation and maintenance work. Training shall be given during the first regular work week after the equipment or system has been accepted and turned over to the Government for regular operation. Training with the each equipment and other system on the job training shall be held a minimum 2 times as necessary operating and maintenance shops and number of man-days (7 hours). When more than 4 man-days of training are specified, approximately half of the time shall be used for classroom instruction. All other time shall be used for training with the equipment or system. When significant changes or modifications in the equipment or system are made under the term of the contract, additional training shall be provided to acquaint the operating personnel with the changes or modifications.

L. Request for contractor’s employee and vehicle passes:

1. 30 days prior to the start of desired date of access or on-site work, the contractor shall submit applications for Base passes (listed below) to the contracting officer, for all

employees, including subcontractor employees, and vehicles requiring access to Yokota Air Base in order to perform this contract.

a. 5AF Form 99EJ, Application for Base Pass (for employees).

b. Application for Vehicle Registration (for vehicles).

2. Upon completion of this contract, the contractor shall return all employee and vehicle passes to the visitor control section building (Building 316), and obtain a certification of Receipt. Final payment will be withheld until the contractor has submitted the certificate confirming all passes have been returned.

3. Contractor shall follow 374 SFS requirements for escorting any employees who do not have a base pass.

3. Government shall not be held responsible for any delay to the contract period of performance if contractor failed to submit required documentation in a timely manner or their employees refused entrance due to failure to follow specified procedures.

M. Documents Required: All documents required under this Contract, including but not limited to, Safety hazard analysis plans, all correspondence, and all other written communications documents, drawings, reports, etc., of every description shall be written in English or translated in English before being provided to the Government by the Contractor.

Delay encountered because a translated copy was not submitted shall be at the Contractor’s expense.

VIII. PERFORMANCE REQUIRMENTS:

A. Work clearance permit: Route work clearance request (AF Form 103) though all associated base organization and approved 42 days advance of work. Areas containing underground utilities will be designated caution areas. Carefully excavate designated caution areas. Hand excavate within 3 feet of all identified existing utility lines.

B. Traffic access: Maintain at least one open lane throughout all roadwork. Always provide at least one flag person when two-way traffic is reduced to a single lane. Obtain a work clearance request (AF Form 103) from the construction inspector 21 days in advance before road closures.

C. Communication Language: All documents required under this contract, including but not limited to, submittals, all written correspondence, written communication documents, letters, memos, emails, requests, etc., shall be written in English or translated in English before being provided to the Government by the Contractor. Delays encountered because a translation was not provided shall be the Contractor’s responsibility. Also, it is contractor’s responsibility to provide English speaking personnel fluent in both Japanese and English when dealing with the Government.

IX. INSTALLATION SAFETY & FIRE REGULATIONS:

A. The prime contractor is responsible for understanding and insuring subcontractor compliance with the following:

1. Vehicle Traffic Supervision. AFI 31-204, Motor Vehicle Traffic Supervision.

2. EM 385-1-1, Safety & Health Requirements Manual, Army Corps of Engineers.

3. NFPA 241, Safeguarding Construction, Alteration, & Demolition Operations.

B. The prime contractor shall be responsible for all workers to include subcontractor workers’ health and safety.

C. Fire safety general requirements:

1. The contractor or an individual appointed shall be placed in charge of Fire Safety of the work project. The responsibilities shall include: assuring the availability, operation, and location of fire protection equipment, general supervision of safeguards and location of portable heating equipment, and establishment and maintenance of safe cutting and welding operations.

2. Contractor's materials and equipment shall not be stored in a location or manner which will create a fire exposure to facilities adjacent to the project site. Fire hydrants shall not be fenced in on construction sites.

3. All fires, regardless of size, even if they have been extinguished, shall be reported to the Fire Emergency Services Flight immediately. Any persons discovering a fire or suspected fire shall alert the building occupants by any means available. The Fire Emergency Services Flight shall be notified in the following manner: from a base phone, dial 911; from a Japanese cell phone dial 042-552-2510, wait for the tone and dial 911.

4. Portable fire extinguishers shall be supplied by the contractor as required by the Fire Emergency Services Flight. Fire extinguishers shall be readily available and kept in a fully operational condition at all times.

5. Work that includes the fire detection or suppression systems, utility outages, road closures, and confined space entries, shall be approved by the Fire Emergency Services Flight, Fire Prevention Office prior to beginning the work. Only authorized technicians shall alter, extend, or repair installed systems.

6. Contractor shall coordinate through the construction inspector, with the Fire Alarm shop (374 CES/CEOFA) and Fire Emergency Services Flight to obtain approval before disconnecting and/or surveying existing fire alarm control system, automatic suppression system, and/or device(s).

7. Contractor is responsible for managing and securing the job site during work hours.

At the end of the working hours, the Contractor shall render the jobsite safe.

Contractor shall check and secure the job site to avoid any fire trouble/accident.

8. When building fire protection system is scheduled to be out of service for more than 4 hours, the Constructor shall coordinate through the construction inspector to notify the Facility Manager or any authorized personal of the facility.

D. Flammable and Combustible Liquids and Gasses:

1. Every effort shall be made to limit flammable liquid quantities to daily use.

2. Flammable liquids exceeding daily quantities shall be properly stored in approved locations only.

3. Flammable storage areas shall be marked with bilingual signs.

E. Hot Work Operations: Any hot work operations shall be approved by the Fire Emergency

Services Flight, Fire Prevention Office prior to starting. The job site shall be inspected and have an AF Form 592, Welding and Cutting permit, issued. Responsible hot work operation supervisors shall take the following actions:

1. Follow checklist on reverse side of the AF Form 592, Welding and Cutting permit.

2. Remove combustible material to a safe location.

3. Ensure that adequate fire extinguishers are provided and that one person is designated to operate the extinguisher in the event of fire.

4. Ensure fire detections and suppression systems will not be affected by the operation being preformed. Thoroughly inspect the area after operations has been completed to ensure that the area is free of fire hazards caused by the operation.

5. Return the welding permit to the Fire Emergency Services Flight, Fire Prevention

Office upon expiration of welding permit or completion of the work.

6. If the fire detection or automatic suppression is disconnected, post a fire guard in the facility during welding/cutting or open flame operations. Submit the work schedule through the construction inspector to the base fire department.

F. Driving Responsibilities:

1. Drivers shall yield the right-of-way to all emergency response vehicles.

2. Vehicles shall not be driven over fire hose and shall not proceed through areas where firefighting operations are being conducted; unless directed by a designated traffic control representative. .

G. The prime contractor is responsible for providing escort and control of all personnel and equipment being utilized on the contract.

X. GENERAL QUALITY CONTROL:

A. Standard products: All materials shall be the standard products of manufacturers regularly engaged in the manufacturer of such products and shall be essentially duplicate items that have been in satisfactory use for at least 2 years before bid opening.

B. Installer’s qualifications: All workers shall be under the supervision of a licensed or certified person in the specific work being accomplished.

C. Submit copies of license or certification with photo and renewal/retraining record of each work item listed below to 374 CONS/LGCB, contracting officer for approval. If designated license or certification does not have a photo, additional submittal may be required for personal identification.

D. Those qualified personnel with license or certification shall be at the job site, either directly supervising or performing architectural, electrical, and mechanical work. During construction, the Contracting officer and/or the Construction Inspector can verify the submitted qualification without any prior notice.

XI. PRODUCTS GENERAL:

A. The contractor is required to submit a statement affirming compliance with the following:

1. Do not use materials containing greater than 0.1% asbestos in the project.

2. Do not use materials containing lead-based paint greater than 0.009 percent lead by weight in the project.

3. Do not use materials containing polychlorinated biphenyl’s (PCB).

4. Do not use Class 1 ozone depleting substances.

5. Capture/recycle refrigerant gas from existing equipment in accordance with Japanese law.

XII. AS-BUILT DRAWINGS:

A. Submit a complete Geo-Referenced AutoCAD 2010 DWG or no earlier than AutoCAD 2007 file format as-built and PDF document. The coordinate system shall be based on the Universal Transverse Mercator (UTM) zone 54 north projection. This projection/coordinate system is based on the 1984 World Geodetic Survey (WGS 1984). As an alternative as-built drawings can be based in Japan Geodetic Datum 2000 (JGD 2000) zone 9. The AutoCAD drawing must follow the CAD standards from the latest DoD A/E/C standard. The A/E/C

standard can be provided by the Government or downloaded from USACE website at https://cadbim.usace.army.mil/default.aspx?p=a&i=7&t=1

B. Three copies of the AutoCAD as-built drawing file and PDF document will be delivered with each copy on a separate disk including a listing of each drawing file on the disk and a second listing of each layer and the contents of each layer for every drawing. Assure the accuracy of all on-site revisions and contract modifications by initialing and coordinating each alteration with the construction inspector.

C. All approved signed submittals shall be scanned pdf data which include: contractor’s certifications, letters (include signed/approved Government coordination form and etc.), catalogue cut/data, specs, drawings, photos, field test data, operation/maintenance manuals, and etc. Those data shall be itemized on the disk.
