STATEMENT OF WORK (SOW)

FOR

PROJECT ZNRE 12-1005

REPLACE GENERATOR – SOUTH ILS, B1301

YOKOTA AB

Date: 29 May 2015

I. PRINCIPAL FEATURES OF WORK: The Contractor shall verify site conditions and conduct all field surveys and measurements as necessary to complete this project. All performance under this project shall meet or exceed US Government and Japanese Construction Standards. The Contractor shall provide all labor, materials, equipment and any other necessary items to complete this project to ensure a usable facility.

II. GENERAL SCOPE

A. Overall objective: Design and Build project to relocate and install new generator system including transformer system, Auto Transfer Switch (ATS), and other electrical equipment. This project includes system design. The attached drawings are only for references.

B. General Description of Design and Electrical Work

1. Removal work:

a. Before starting removal work of the transformers/capacitors, the contractor shall coordinate with Yokota’s CE Electrical Shop and Environmental Element through the construction inspector.

b. When the Contractor is able to obtain the original manufacturer’s Non-PCB certificate and an appropriate written document from the appropriate maintenance function (ie., Non-Maintenance record by PCB containing dielectric fluid from CE Electrical Shop) declaring that PCB and PCB-containing material(s) were not used or added to the item at any time, the contractor is able to dispose of or recycle the transformer/capacitor to off-base in accordance with applicable Japanese laws and regulations.

c. The contractor will test the dielectric fluid by GOJ approved PCB analytical method, US EPA method outlined in Section 11, Oil Sampling Test, in this SOW.

d. When the PCB concentration is equal to or less than 0.5mg/kg, the transformer/ capacitor is considered as Non-PCB. The Contractor shall dispose of or recycle the transformer/capacitor in country in accordance with applicable Japanese laws and regulations. The Contractor shall submit analytical results with Quality Control report in English and Japanese to Environmental Office before dispose or recycle of transformer/capacitor.

e. When the PCB concentration is greater than 0.5mg/kg, the transformer/capacitor is considered PCB containing waste. The Contractor shall submit a list of PCB items turn-in such as country of manufacture, name of manufacturer, year of manufacture, type, serial number, quantity, weight, analytical results with Quality Control report in English and Japanese to Environmental Element before transporting transformer/ capacitor to the designated staging area.

f. Once a transformer has been removed from service the contractor will store the transformer in a manner that will prevent dielectric fluid releases to the environment. These storage requirements apply until the transformer is transferred to the base staging area. Transformers will be provided with secondary containment greater than 125 percent (%) of the free flowing dielectric fluid and sufficient amount of absorbent materials to prevent any spill/leak. At least 1.6 millimeter (mm)

thickness of steel shall be required to make the secondary containment. The secondary containment shall be clean with no rust, and free of standing water.

g. After placing transformer/capacitor in secondary containment, they will be crated with wood shipping crate; which shall be heat treated and officially certified. Wood packaging material and shipping crate shall meet applicable DoD and DLA requirements. At minimum of 2”x 4” (50 x 100 millimeters) thickness of wood shall be used for crating. The skid or pallet shall be banded with 1 ¼ inch (3 cm) metal strapping or equivalent metal bands, which shall be durable enough for boat shipment. CE Environmental will provide more detail information to the contractor through the Construction Inspector. After inspection by CE Environmental, the Contractor shall transport and unload the crated transformer/capacitor to a PCB staging area on Yokota AB. The Contractor shall provide their own fork-lift or applicable heavy equipment to move transformer into designated staging area on Yokota AB.

h. Oil Sampling Test: Contractor shall perform an oil sampling test to find PCB concentration level for existing transformer listed as “Need Sample Test” in the SOW 01110 Attachment B, Transformer Information. The contractor shall provide the PCB testing results of the transformer to the Government. The contractor will test the dielectric fluid by GOJ approved PCB analytical method, US EPA method 600/4-81-045, the determination of polychlorinated biphenyls in transformer fluid and waste oils, or ASTM D4059, Analysis of polychlorinated biphenyls in insulating liquids by gas chromatography. Laboratory analyses shall be conducted in a laboratory certified by a US or GOJ regulatory authority. When the PCB concentration is equal to or less than 0.5mg/kg, the transformer/ capacitor oil is considered as “Non-PCB”. When the PCB concentration is greater than 0.5mg/kg, the transformer/capacitor is considered PCB containing waste. The Contractor will coordinate with CE Electrical Shop and Environmental Element for proper disposal and/or turn-in through the Construction Inspector in accordance with Section 18, Removal and Disposal of Work, of this SOW.

h. Removal exist items below:

1) Existing Generator Unit and ATS Panel shall be removed and transported to 374 CES/CEOFP Power Production Shop.

2) Existing transformer shall be remove and dispose to off-base base (in case of PCB concentration)or on-base (in case of PCB concentration).

2. Installation work:

a. Install new items below:

1) Install new Load Break Switch (LBS) unit at the existing 6.6KV incoming panel that involves power fuse G150A. Pull box (stainless type) is required depending on the site condition.

2) Install new transformer cubicle around existing south ILS equipment shelter “T-37-60A”

(tentative called) (30KVA):

Transformer: 30KVA, 5 taps type Input: 50/60Hz, 3P 3W 6.6KV Output: 50/60Hz, 3P 4W 208/120V Type: Continuous operation for 24 hours Involves: Outdoor type steel cubicle cabinet, new concrete foundation, Load Disconnecting Switch (LDS), LBS, Air Circuit Breaker (ACB), Mold Case Circuit Breaker (MCCB), and step-down transformer. Transformer shall be Energy Star or Japanese equivalent certified type.

3) New transformer “T-37-60B” (tentative called) around Exist Bldg 1301; Contractor shall install New transformer “T-37-60B” and new four (4) circuit breakers with steel encasement cabinet for four existing 4 loads POC. The existing four (4) loads are South Overrun Traffic Light, Fence Obstruction Light, South Wind Cone and spare circuit. The “T-37-60B” shall be connected with existing transformer (“T-37-60”) all loads such as above four (4) loads by proper size and type of cable with proper conduit.

4) Install and connect associated equipment for existing load (20KVA x 2):

South ILS: Circuit breakers, step-up transformer (“TR1” Power supply side, 20KVA 2P 3W 190-200-208/560V to be replaced with new), step-down transformer (“TR2” Load side, 20KVA 3P 3W 520-540-560/ 2P 3W 240-120V 50/60Hz to be reused), including metal encasement steel cubicle outdoor type, concrete foundation, associated cable and conduit. Pull box with plate will be installed depending on site condition.

South Overrun Traffic Light (20KVA x 2): Circuit breakers, step-up transformer (“TR7” Power supply side, 20KVA 2P 3W 3150-3300-3450/560V to be replaced with new), step-down transformer (“TR8” Load side, 20KVA 3P 3W 3150-3300-3450/110V 2P 3W 240- 120V 50/60Hz to be reused), including metal encasement steel cubicle outdoor type, concrete foundation, associated cable and conduit. Pull box with pate will be installed depending on site condition.

Fence Obstruction Light: Circuit breakers with metal encasement steel cubicle outdoor type, concrete foundation, associated cable and conduit.

Pull box with plate will be installed depending on site condition.

5) Install new handhole as cable junction box for existing cables of above loads (South ILS, South Overrun Traffic Light, Fence Obstruction Light, Wind Cone, and etc.).

6) Install new Generator System Cubicle;

Generator: 13KVA, Output: 60Hz, 2P 3W 240/120V Type: Continuous operation for 24 hours.

Involves: Outdoor type steel cubicle cabinet and new concrete foundation.

Indicator: Voltage, Current, Frequency, (Power Factor).

Adjuster: Voltage, Frequency.

Test Report: Character Curve data of Frequency, Voltage, Current, Power Factor depending on Load (30%, 50%, 70%, 100%).

Operation Tests for each functions.

Calculate ability to withstand short circuit current with current flow time (Example:

18KA 2 second). Generator shall Energy Star or Japanese equivalent certified type.

7) Install new ATS Panel System

Auto Transfer Switch (ATS) system, 27, 84 relays, timer, bypass switch function, Circuit Breakers (CB), Involves: Over current and grounding fault relays, outdoor type steel cubicle cabinet, new concrete foundation.

8) Fuel Tank System

Install new double wall fuel tank system with fuel level alarm control panel, leak prevention and detecting system IAW requirement of this project.

Involves: fault indicator lamp/alarm, reset button, outdoor type steel cubicle cabinet, new concrete foundation.

9) Install lightning protection system for new equipment. Install grounding wire and grounding rods until required grounding resistance such as 10 ohms.

10) Install new cables in new flexible conduit (FEP) for above systems in accordance with industry standards.

Involves: Man/Hand holes, pull boxes, junction boxes, mounting supports.

11) Cable insulation color is required as follows; High Voltage cables have Red, White and Blue (for R, S and T phase). Low Voltage cables have Red, Black, Blue and White (for r, s, t and neutral phase).

12) Spare Conduit: Contractor shall install spare conduits. The all end hole of conduits shall be filled up caulking compound putty.

13) Install new Tank Capacity Indicator board with danger warning sign (sign board). The sign board shall be to show same as existing contents such as Warning notice, building number, fuel kind/type, Fuel storage capacity, POC section name, POC phone number, No flammable or ignition sources within Danger notice, and etc. The material of the sign board shall be also as exist steel frame and plate with same color paint type by English and Japanese letters.

14) Install label to all new equipment and cables to conform to the labeling system in accordance with industry standards.

15) Install new Fire Extinguisher with the Case Box outdoor water proof stainless type. The

Fire Extinguisher capacity and type shall meet NFPA and UFGS regulations for the fuel fire accident.

16) Performed all installation in accordance with industry standards.

17) Provide test and test report of all new equipment and cables to conform to the test items in accordance with industry standards.

18) Provide temporary generator during construction for contractor’s work and associated necessary power.

19) Power Outage:

a) In case a power outage exceeds eight (8) hours, the contractor shall provide a temporary transformer or generator with associated parts such as panelboard, circuit breakers, cable, and wooden base.

b) The contractor shall notify the base at least 30 days before any planned outage to ensure information about power outage can be provided to affected personnel ahead of time.

c) In case an outage affects military Family Housing (MFH), the contractor shall post notices about power outage affected area with date and time on each entrance door of MFH residence that will be affected by a power outage. The notice needs to be posted one month before and again one week before the power outage.

d) In case a power outage affects steam line, contractor shall provide temporary generator to back up steam pit control units.

20) Provide temporary guard fence around construction area for safety, which should be approximately W 1,800mm x H 1,800mm, steel frame with half top wire net and half bottom steel plate, yellow and black color (B type) to secure work areas. H beam steel foundation is not required.

21) Design Calculation: Contractor shall provide design calculations such as feeder Load current analysis report and feeder fault current analysis report based on Load data, transformer %Z, FC %Z, Cable %Z of cable diameter size and cable length, CB and protective relay trip timing, tests data (II.B.2.d.) and etc.

22) Provide fuel to conduct all tank functional testing and result of piping tightness leak t

23) Contractor shall obtain Temporary Construction Waiver (TCW) approval for work within airfield’ clear and transition zone. The TCW will be required for this project and processing times are lengthy (some have taken to 5 monthes). We recommend the contractor shall submits for a TCW attached information such as construction schedule and site plan as soon as possible.

3. Details

a. Install new grounding rods (Diameter 19mm x 3000mm) with wiring to meet required grounding resistance as shown in the drawing. Grounding resistance shall be separated LA type (10 Ohms or Less), A and D type (10 Ohms or Less), and B type (10 Ohms or Less).

b. Excavation depth: New cables with new conduit and concrete encasement shall be installed at

1,200mm depth under the road area. (See UFC 3-550-01, 3-11.2, for details). Please note per this UFC, a ductbank consists of two or more conduits (or ducts) routed together in a common excavation with or without concrete encasement. New cables with new conduit shall be installed at 600mm depth under turf areas. Contractor shall restore area to original condition.

c. The new transformer shall meet all existing load equipment. The system needs a rubber vibration insulator. The new transformer system shall include VT, CT, volt meter, amp meter, LBS and LDS holder, cable, fuse and associated equipment. The voltage tap shall be 6900, 6750, 6600, 6450, and 6300 (3450, 3375, 3300, 3225, and 3150) type, an external changing tap type. Contractor shall make a working space or clearance of 6 feet between new and existing transformer cubicles. Refer to the attachments A of this SOW for details. Contractor shall submit a copy of the main and branch panelboard schedules for all new connected transformers, including all load phase and wattage capacities.

d. Transformer Specification: The new transformer shall meet JIS C 4304 6KV oil-immersed distribution transformers and JEC-2200, power transformers. The new transformer shall pass all 13 tests listed below to be acceptable. The contractor shall provide all test reports to verify meeting JIS or JEC Standard that the following tests have been passed. Do not install equipment if any of the 13 tests failed.

1) Required 13 Tests:

a) Measure Winding Coil Resistance of primary and secondary side

b) Non-Load Operation Test, Measure Non-Load Loss, X and R Data

c) Load Operation Test and Measure Load Loss Data and % Impedance

d) Test to measure Voltage Ratio and Angular Displacement,

e) Test to measure Insulation Resistances and Insulation Voltage Stress

f) Test to measure Winding Coil Temperature Rising

g) Test to measure Noise

h) Test to measure Leaking Oil

i) Test to check Fault Alarm Operation

j) Test to measure Paint Thickness

k) Calculate short circuit current at transformer secondary point and show temperature class of insulation (Example: 7KA, Class A, DGC-H).

l) Calculate ability to withstand short circuit current with current flow time (Example:

32KA 2 second).

m) Calculate worst fault current at transformer secondary side based on transformer %Z and cable %Z. (Need to check 15.l>15.k current to meet JEC-2200.)

C. General Description of Design and Mechanical Work

1. Installation work:

b. Install new items below:

1) Install new Fuel Tank System with Fuel Level Alarm Control Panel System

Fuel Storage Tank: Double wall support AST, Capacity for 7 days continuously generator operation Including: Remote fuel fill box, inlet, valve, water drainage, fuel level controller, POL piping (galvanized steel gas pipe [SGP, black type pipe], Threaded reducers, hot-dip galvanizing steel gas type elbow [black type]), fuel supply and return pumps.

Exposed above ground system: Piping system shall be painted. Welded connection shall be accepted. Screw connection shall be accepted near valves.

Underground system: Piping system shall be galvanized. Welded connection shall be accepted. Valves shall be in the pit.

This project is Exposed above ground Tank and Fuel Piping system.

Post warning Danger and POC sign which need to see well from at least 50 feet (16m) away.

D. General Description of Design, Civil Work

1. Removal work:

a. Removal items below:

1)Remove existing Generator, fuel tank (underground type) with associated parts. Remove existing concrete pit for transformer “T-37-60” and generator system.

2. Installation work:

a. Install items below:

1) Backfill new soil to match existing ground level for above the removal pit work. Install new turf on the new soil. Coordinate Electrical new handhole installation work.

E. General Description of Design and Environmental remediation

1. POL work;

a. Temporary Fuel Storage Tanks: The use of temporary fuel storage tanks on base as part of a project must be approved by 374CES/CEIE. The approval request must contain justification

for the use of a temporary tank and design details, including a layout diagram indicating the proposed location of the tank within the project site. If a temporary fuel storage tank is approved for use, a Site-specific Spill Contingency Plan (SSCP) must be developed). After construction/installation of a temporary fuel storage tank, the Contractor shall contact project manager or 374CES/CEIE at 225-5440 to inform Fuels Management Flight (374 LRS/LGRF) to coordinate an initial inspection (AFI 23-204).

b. Aboveground Storage Tank (AST) Installation: If the Contractor is installing a permanent aboveground fuel storage tanks as part of the project, it shall be designed and installed in accordance with all applicable regulations. Each AST will be assigned a Tank ID Number by 374CES/CEIAP and this Tank ID Number shall be used on tank signage and in project documentation. After construction/installation of an AST, the Contractor shall contact project manager or 374CES/CEIE at 225-5440 and the Fuels Management Flight (374 LRS/LGRF) at 2 2 5 -4617 to coordinate an initial inspection (AFI 23-204).

c. Storage Tank Removal: When a facility with a fuel storage tank(s) is demolished, demolition of the tank(s) and associated infrastructure, including above-ground piping, underground piping to the building serviced by the tank, indoor piping to the equipment serviced by the tank, secondary containment curbing, tank saddles, tank pads, Oil Water Separators (OWS), grounding stations, signage, fencing around the AST, associated equipment and sidewalks leading to the AST shall be included in the scope of the project. Contractor shall ensure to comply with applicable regulations such as AFI, UFC and UFGS.

d. UST Removal: The following standards of UST removal include, but limited, shall be applied as required by applicable JEGS, AFI, UFC,USGF the UST Removal Procedures appendix of the PACAF UST Policy: Contractor shall ensure to comply with above regulations.

1) Site Specific Health and Safety Plan: The Contractor shall ensure that the project’s

Site Specific Health and Safety Plan addresses the particular substance or substances in the UST and addresses removal procedures.

2) Soil Removal: If exposed free product and/or obviously contaminated soil is encountered during UST removal, contact 3 7 4 C E S / C E I E immediately. When a leaking UST is removed, exposed free product and/or obviously contaminated soil in the immediate vicinity of the tank will be removed and appropriately disposed of (JEGS, C19.3.2.6). If additional contaminated soil is present beyond the immediate vicinity of the tank, 374CES/CEIE will provide additional guidance on necessary actions. Segregate clean soil from the contaminated soil. Only clean soil will be used as backfill. If the soil is contaminated with Petroleum, Oil, and Lubricants (POL), it shall be disposed of in accordance with local regulations.

e. Fuel Storage Tank Installation/Removal Report: If an AST/UST is installed or an AST/UST is removed as part of the project, the Contractor shall prepare a Fuel Storage Tank Installation/Removal Report and provide it to 374CES/CEIE within 30 days of the completion of site work. At a minimum, the report shall include the following:

A) Summary of work conducted;

B) Removal date for each removed storage tank;

C) Date placed in service, capacity, manufacturer name, model number, and serial number for each new AST;

D) As-built drawings showing the removed AST/USTs and new ASTs and associated

infrastructure;

E) Leak check report for buried piping associated with storage tank at time of installation, modification, construction, relocation or replacement.

F) Manufacturer Operations & Maintenance Manuals and product information;

G) Manufacturer Warranty Cards (Copy);

H) Photo documentation of construction activities, including AST/UST removals and/or new AST installations.

f. Spill Prevention and Response: The Contractor shall conduct all operations in a manner that prevents spills of POL and Hazardous Substances. The Contractor is required to familiarize personnel with spill prevention and response procedures, fire suppression systems, and MSDSs for all materials used and/or stored on the project site. The Contractor shall provide and maintain spill equipment, sufficient in both type and quantity, at all sites involving the storage, use, or handling of POL and/or hazardous substances.

g. Site-specific Spill Contingency Plan (SSCP): An SSCP must be developed for any project at which POL or Hazardous Substances are used and/or stored at the project site. The SSCP shall include spill response procedures, an inventory of POL and/or hazardous substances, probable spill routes, a project site layout diagram, and a spill response equipment inventory.

A template SSCP can be obtained from 374CES/CEIE. The Contractor shall train employees on the contents of the SSCP and the use of spill response equipment and shall document such training.

h. Spill Reporting: The Contractor shall immediately report all POL or Hazardous Substances spills to the Fire Department (374 CES/CEF) at 911.

i. Spill Cleanup: The Contractor is responsible for clean-up of POL and/or hazardous substance spills and disposal of clean-up material. The Contractor shall determine, as quickly as possible, the nature of the spilled substance and implement necessary safety precautions to protect both human health and the environment at no additional cost to the US Government, and shall only be performed by personnel adequately trained in spill response and cleanup techniques for the severity of the spill incident.

j. Costs incurred from any Contractor spills are the responsibility of the Contractor. If the US Government has to perform emergency spill response and cleanup, due to the non-availability of designated personnel or if the spill is beyond the capability of designated personnel, the Contractor shall be held liable for all costs associated with performing said work.

2. Transformer Disposal Work;

a. Oil Sampling Test: Contractor shall perform an oil sampling test to find PCB concentration level for existing transformer to be removed on this project. The Contractor shall provide the PCB testing results of the transformer to the Government. The Contractor will test the dielectric fluid by GOJ approved PCB analytical method, US EPA method 600/4-81-045, the determination of polychlorinated biphenyls in transformer fluid and waste oils, or ASTM D4059, Analysis of polychlorinated biphenyls in insulating liquids by gas chromatography.

Wood packaging material and shipping crate shall meet applicable DoD and DLA requirements. At minimum of 2”x 4” (50 x 100 millimeters) thickness of wood shall be used for crating. III. REFERENCES: Below references as minimum shall be incorporated to meet the USAF standard for basis of design and construction.

IV. EXISTING CONDITIONS CONSIDERATION

A. The design and analysis shall be calculated based on Contractor’s site survey. Tthe attached drawings are provided for reference only.

B. The Contractor is responsible for correcting any unplanned/unapproved adverse impacts to the facility due to their work at no additional costs.

V. DESIGN POLICY AND CRITERIA

A. The Government will furnish the basic design criteria to the Contractor from which the project documents can be developed and produced as described herein. Suggestions that will improve the operational functions, enhance appearance, and prove to be more economical and advantageous shall be made at the appropriate review conferences. Meeting minutes shall be provided by the Contractor within 3 working days from the date of conference. The design must be accepted by the Government before construction can start.

5. Design instructions: The design shall incorporate the requirements as discussed in the post-award conferences and site visits as well as the requirements of the publications listed below. Modern construction techniques may be incorporated into the project design to obtain both quality and economy and to provide a functional, complete, and useable facility.

6. Cost saving techniques: The design is to provide functionality without embellishment. Also, the design is to achieve an optimum balance between aesthetic consideration, operations and

maintenance costs, energy conservation measures, and durability of materials and systems. Design and support items will be limited to the minimum essential support construction required to provide a useable facility. Every effort will be made to reduce utility runs and site preparation requirements through proper design.

B. Government furnished information: The age of Government data and as-built drawings, along with the fact that there has often been undocumented tenant construction, reduces their usefulness and reliability.

The Contractor shall verify the data shown on Government-furnished reference documents by performing reconnaissance, surveys, and field measurements.

VI. DESIGN SERVICES

A. Site design data: The Contractor shall perform site investigations/surveys in accordance with requirements of this project. During the site data collection, the Contractor shall record all existing conditions (inside and outside) and obtain engineering information/design data for the accomplishment of this project. All material sampling, testing, and removal shall be performed in accordance with requirements of this project.

B. Field measurements: The Contractor is required to make field investigations to verify dimensions and other information shown on Government-furnished reference data and drawings.

C. General: The Contractor will consider all design options and criteria reductions that may be necessary, in those instances, where possible cost overruns may be expected. All drawings, and specifications shall be prepared, reviewed, and approved by registered professional architects and engineers. All hardcopy and electronic submittals of the drawings, specifications, basis of design, construction schedule, and submittal register shall be submitted and delivered to the contracting officer. One copy of the final drawings shall be submitted in specified editable electronic format for incorporation into Geobase.

7. 60% Design: 60% design submittals shall be based on the concept drawing. The preliminary design submittals will include detailing of the requirements of the project so to leave the reviewer no question as to scope of this project.

8. 90% Design, and 100% Design (including 100% corrected final): The final submittals shall be based on the previous submittals and comments furnished by the Air Force along with an anticipated design/construction time schedule (bar chart). The Final Design submittals will include all requirements shown on the Preliminary Design, a complete and thorough detailing of all the requirements of the project.

9. The design will not be deemed 100% Final until the Government has accepted all response(s) to comments made on previous submittals. The Contractor shall make such required revisions at no additional cost to the Government. It is recommended that the Contractor electronically submits to the Government a compilation of responses to comments along with pages / drawings / calculations that will change between 90% and final submittal. This will minimize the possibility of multiple versions of Final Design, (such as 100% Corrected Final Design 1, 100% Corrected Final Design 2, etc.)

11. Design submission electronic media quantities: Two Compact Disc (CD) that saved all items submitted at 60%, 90% and 100% design.

VII. GENERAL INFORMATION

A. Performance Period: The performance period for design will be 120 calendar days, and construction will be 180 calendar days, total of 300 calendar days after Notice to Proceed (NTP) is issued. NTP will be issued within 30 days after contract award.

B. Working Hours: During construction, the facilities will be made available to the Contractor from 07:30 to 16:30, Monday through Friday, 5 days a week, excluding U.S. legal holidays but including Japanese legal holidays. Contractor must notify the Government in advance if work needs to be performed outside these working hours. Contractor shall coordinate schedule for design site visits with the Government at least 21 calendar days ahead of time.

12. Holidays: Recognized US legal holidays are listed below:

a. January 1 - New Year’s Day

d. May, last Monday – Memorial Day

e. July 4 - Independence Day

k. Holidays falling on a Saturday will be observed on the preceding Friday. Holidays falling on a

Sunday will be observed on the following Monday.

C. Weekend work: Do not perform weekend work without advance permission. Submit weekend work requests at least the Wednesday before the desired weekend workday. Only proceed with weekend work after you receive Government approval.

D. Occupancy: The facility will be occupied. Contractor shall provide construction plan and schedule to be coordinated with the users for all work to be performed. Phasing will not be required.

E. Utility outages: Provide at least 30 calendar days advance notice of planned utility outages, including fire alarm systems. In each request, specify each utility outage, the time and date the service will be deactivated and the anticipated time and date service will be reactivated. Do not disconnect or deactivate any base utility systems without advance contracting officer or construction inspector approval. Fire system disconnection and reactivation requires 374 CES/CEF (Phone 225-9112) coordination.

F. Barricades and warning signs: Prominently post bilingual (English and Japanese) “OFF LIMITS”, “WET PAINT” and “NO SMOKING” signs, as applicable, at all entrances to the job site. Stencil (paint) the black lettering in all capital letters on either 1’-0” x 2’-0” or 2’-0” x 3’-0” sheet metal or plywood signs.

Protect all overnight open excavations with barricades and battery-powered flashers. Smoking is prohibited in Government facilities. Smoking is allowed in the Designated Tobacco Areas (DTAs) only.

G. Utilities: The US Government will furnish water and electricity at no cost in accordance with FAR 52.236-14, Availability and Use of Utility Service (Apr 1984). Access and connection are the responsibility of the Contractor.

H. Final inspection: 374 CES and 374 CONS will perform inspections. Schedule final inspections between 0800 and 1600, Monday through Friday, excluding US legal holidays. The Government will perform the inspection within 3 work days after Contractor initiates the final inspection request. Final inspections falling on weekends or US legal holidays will be postponed to the following workday.

Postponements will not be considered part of the performance period.

I. Asbestos Containing Material (ACM): To the best of the Government's knowledge, any and all asbestos has been identified and abated. If the Contractor suspects additional ACM, the Contractor shall properly notify the Construction Inspector for action by the Government. Contractor shall not sample nor remove asbestos without first obtaining permission from the Government.

J. Material disposal: Remove and dispose of all construction debris off base daily in accordance with Japanese laws. In case that the asbestos debris remains (see Paragraph I above) on base after workday, the debris needs to be properly packed or containerized in compliance with asbestos regulations and any of the debris shall not be disturb. Report amount of construction debris generated, disposed or recycled. It should be reported to the Government at the end of the project using the attached form (Attachment 4:

Construction Debris Disposal and Recycling Data Form, C-D Form).

K. Training of Government personnel: The Contractor shall furnish the services of competent instructors who will give full training to the designated personnel in the adjustment, operation, and maintenance, including pertinent safety requirements, of the equipment or system installed. Each instructor shall be thoroughly familiar with all parts of the installation and shall be trained in operating theory as well as practical operation and maintenance work. Training shall be given during the first regular work week after the equipment or system has been accepted and turned over to the Government for regular operation. All other time shall be used for training with the equipment or system. When significant changes or modifications in the equipment or system are made under the term of the contract, additional training shall be provided to acquaint the operating personnel with the changes or modifications.

L. Request for Contractor’s employee and vehicle passes:

13. 30 days prior to the start of desired date of access or on-site work, the Contractor shall submit applications for Base passes (listed below) to the contracting officer, for all employees, including subcontractor employees, and vehicles requiring access to Yokota Air Base in order to perform this contract.

a. 5AF Form 99EJ, Application for Base Pass (for employees).

b. Application for Vehicle Registration (for vehicles).

14. Upon completion of this contract, the Contractor shall return all employee and vehicle passes to the visitor control section building (Building 993), and obtain a certification of Receipt. Final payment will be withheld until the Contractor has submitted the certificate confirming all passes have been returned.

15. Contractor shall follow 374 SFS requirements for escorting any employees who do not have a base pass.

16. Government shall not be held responsible for any delay to the contract period of performance if Contractor failed to submit required documentation in a timely manner or their employees refused entrance due to failure to follow specified procedures.

VIII. PERFORMANCE REQUIREMENTS

A. Work clearance permit: Obtain a work clearance request (AF Form 103) from the construction inspector 21 calendar days in advance before any site work, especially excavation, begins. Areas containing underground utilities will be designated caution areas. Carefully excavate designated caution areas. Hand excavate within 3 feet of all identified existing utility lines.

B. Traffic access: Obtain a work clearance request (AF Form 103) from the construction inspector 21 calendar days in advance before any road closures. Maintain at least one open lane throughout all roadwork.

Always provide at least one flag person when a two-way traffic is reduced to a single lane.

IX. INSTALLATION SAFETY AND FIRE REGULATIONS

A. The prime Contractor is responsible for understanding and insuring subcontractor compliance with the following:

1. Vehicle Traffic Supervision. AFI 31-204, Motor Vehicle Traffic Supervision.

2. EM 385-1-1, Safety & Health Requirements Manual, Army Corps of Engineers.

3. NFPA 241, Safeguarding Construction, Alteration, & Demolition Operations.

B. The prime Contractor shall be responsible for all workers to include subcontractor workers’ health and safety.

C. Fire safety general requirements:

1. Even if a facility is unoccupied maintain the fire detection and automatic suppression system service throughout construction. Any outage of both systems requires a 24-hours watch.

2. The Contractor or an individual appointed shall be placed in charge of Fire Safety of the work project. The responsibilities shall include: assuring the availability, operation, and location of fire protection equipment, general supervision of safeguards and location of portable heating equipment, and establishment and maintenance of safe cutting and welding operations.

3. Contractor's materials and equipment shall not be stored in a location or manner which will create a fire exposure to facilities adjacent to the project site. Fire hydrants shall not be fenced in on construction sites.

4. All fires, regardless of size, even if they have been extinguished, shall be reported to the Fire

Emergency Services Flight immediately. Any persons discovering a fire or suspected fire shall alert the building occupants by any means available. The Fire Emergency Services Flight shall be notified in the following manner: from a base phone, dial 911; from a Japanese cell phone dial 042-552-2510, wait for the tone and dial 911.

5. Portable fire extinguishers shall be supplied by the Contractor as required by the Fire Emergency

Services Flight. Fire extinguishers shall be readily available and kept in a fully operational condition at all times.

6. Work that includes the fire detection or suppression systems, utility outages, road closures, and confined space entries, shall be approved by the Fire Emergency Services Flight, Fire Prevention Office prior to beginning the work. Only authorized technicians shall alter, extend, or repair installed systems.

D. Flammable and Combustible Liquids and Gasses

1. Every effort shall be made to limit flammable liquid quantities to daily use.

2. Flammable liquids exceeding daily quantities shall be properly stored in approved locations only.

3. Flammable storage areas shall be marked with bilingual signs.

E. Welding safety: If the fire detection or automatic suppression is disconnected, post a 24-hour fire guard in the facility during welding/cutting or open flame operations. Submit the work schedule through the construction inspector to the base fire department.

F. Hot Work Operations: Any hot work operations shall be approved by the Fire Emergency Services

Flight, Fire Prevention Office prior to starting. The job site shall be inspected and have an AF Form 592, Welding and Cutting permit, issued. Responsible hot work operation supervisors shall take the following actions:

1. Follow checklist on reverse side of the AF Form 592, Welding and Cutting permit.

2. Remove combustible material to a safe location.

3. Ensure that adequate fire extinguishers are provided and that one person is designated to operate the extinguisher in the event of fire.

4. Ensure fire detections and suppression systems will not be affected by the operation being preformed. Thoroughly inspect the area after operations has been completed to ensure that the area is free of fire hazards caused by the operation.

5. Return the welding permit to the Fire Emergency Services Flight, Fire Prevention Office upon expiration of welding permit or completion of the work.

G. Driving Responsibilities:

1. Drivers shall yield the right-of-way to all emergency response vehicles.

2. Vehicles shall not be driven over fire hose and shall not proceed through areas where firefighting operations are being conducted; unless directed by a designated traffic control representative.

H. The prime Contractor is responsible for providing escort and control of all personnel and equipment being utilized on the contract.

X. GENERAL QUALITY CONTROL

A. Standard products: All materials shall be the standard products of manufacturers regularly engaged in the manufacturer of such products and shall be essentially duplicate items that have been in satisfactory use for at least 2 years before bid opening.

B. Installer’s qualifications: Qualified installers shall have successfully completed 3 or more similar type jobs within the last 5 years. All workers shall be under the supervision of a licensed or certified person in the specific work being accomplished.

C. Submit copies of license or certification with photo and renewal/retraining record of each work item listed below to 374 CONS/LGCB contracting officer for approval. If designated license or certification does not have a photo, additional submittal may be required for personal identification.

D. Those qualified personnel with license or certification shall be at the job site, either directly supervising or performing architectural, electrical, and mechanical work. Minimum two (2) certified personnel are required to submit for each license or certification. However, all electricians and fire protection personnel shall submit their license or certification. During construction, the Contracting officer and/or the Construction Inspector can verify the submitted qualification without any prior notice.

XI. PRODUCTS GENERAL

A. The Contractor is required to submit a statement affirming compliance with the following:

1. Do not use materials containing greater than 0.1% asbestos in the project.

2. Do not use materials containing lead-based paint greater than 0.009% lead by weight in the project.

3. Do not use materials containing polychlorinated biphenyl’s (PCB).

4. Do not use Class 1 ozone depleting substances.

5. Capture/recycle refrigerant gas from existing equipment in accordance with Japanese law.

XII. AS-BUILT DRAWINGS

A. Submit a complete Geo-Referenced AutoCAD 2010 DWG or no earlier than AutoCAD 2007 file format as-built and PDF document. The coordinate system shall be based on the Universal Transverse Mercator (UTM) zone 54 north projection. This projection/coordinate system is based on the 1984 World Geodetic Survey (WGS 1984). As an alternative as-built drawings can be based in Japan Geodetic Datum 2011 (JGD 2011) zone 9. The AutoCAD drawing must follow the CAD standards from the latest DoD A/E/C standard.

B. Three copies of the AutoCAD as-built drawing file and PDF document will be delivered with each copy on a separate disk including a listing of each drawing file on the disk and a second listing of each layer and the contents of each layer for every drawing. Assure the accuracy of all on-site revisions and contract modifications by initialing and coordinating each alteration with the construction inspector.

XIII. TYPHOON AND STORM PREPARATIONS

A. In order to minimize damage to public properties and prevent personal injury, the following actions will be taken upon declaration of the typhoon conditions described below. The Contractor shall insure that the construction sites are well prepared for protection from the dangers of heavy rain and strong winds.

The Contractor will develop and establish the necessary procedures to be taken for storm preparation and provide in his/her Accident Prevention Plan, Part I, emergency telephone number and addresses where at least 3 officials of the company may be reached and notified in the event that an immediate typhoon alert is declared.
