Statement of Work for

Procurement and Installation of Mobile Isle Storage System

1. DESCRIPTION OF SERVICES

1.1. Scope of work. The contractor shall provide all equipment, tools, parts, transportation, personnel, labor, supervision and management to install the mobile isle storage system in the

Propulsion Flight Support Section at Misawa Air Base, Japan. In addition, the contractor shall provide training on the maintenance and use once the mobile isle storage system is installed.

1.2. Basic services. The contractor will be required to provide all system components, transportation, installation, testing, training and warranty.

1.2.1. System components. Contractor provided system components will include everything that is required for the installation and operation mobile isle storage system. Any "high fail" parts must be identified during the initial install and spares parts listing shall be included.

1.2.1.1. Technical data. Provide a commercial technical manual covering installation and removal of the mobile isle storage system. It will also cover the operation, maintenance, and inspections (if applicable) of the storage system with a step by step process for the actions.

1.2.2. Transportation. Transportation of all components, tools and installation person/persons will be provided by the contractor.

1.2.3. Tools and installation. The contractor will provide qualified person/persons for the installation of the mobile isle storage system. The storage system will be installed utilizing contractor supplied tools. Installation will take place within

Propulsion Flight Support Section located at Misawa Air Base, Japan. The shop is covered, lighted, temperature controlled and secure. Contractor will provide an estimate on how long installation will take.

1.2.4. Testing. Each i n d i v i d u a l b i n w i t h i n t h e m o b i l e i s l e s t o r a g e s ys t em will be function tested by the contractor and adjusted as needed.

1.2.5. Training. The contractor will train Maintenance personnel to maintain the mobile isle storage system and provide any technical manuals associated with the installation, maintenance, and inspections of the system. In addition, the contractor shall provide training to Propulsion Flight personnel on the specifications and use of the system.

1.2.6. Warranty. The contractor shall provide a warranty that covers all components and installation the system for the period of l year.

1.3. Concept of operation. 2. GOVERNMENT FURNISHED PROPERTY AND SERVICES

2.1. Government furnished property. N/A.

2.2. Government furnished facilities. The government will make available facilities in building 1310 (Vehicle Management). No alterations to the facilities shall be made without specific written permission from the 35 LRS Commander. The contractor shall return the facilities to the government in the same condition as received, fair wear and tear and approved modifications excepted

2.3. Government furnished services. The government shall provide the contractor with base escort, water, electricity and trash pick-up.

3. GENERAL INFORMATION

3.1. Contract manager. The contractor shall provide an on-site contract manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The name of this person and an alternate(s) who shall act for the contractor when the manager is absent shall be designated in writing to the CO. The

Contract Manager and alternate must be able to read, write, speak, and understand English.

3.1.1. Availability of contract manager. The contract manager or alternate shall be available within 60 minutes during normal duty hours (paragraph 3.1.) to meet on the installation with government personnel (designated by the CO) to discuss problem areas.

After normal duty hours the director or designated alternate shall be available within 3 hours. The contract manager or alternate shall be available for contact by telephone 24 hours a day, 7 days per week, including holidays.

3.2. Working Hours. The contractor shall perform services between the hours of 7:00 a.m.

and 3:30 p.m., Monday through Friday, excluding Saturdays, Sundays and Federal Holidays.

If the contractor desires to work after 3:30 p.m., weekends, or during Federal Holidays, the contractor must submit the request to the Contracting Officer at least 2 days in advance. Each request will be considered on a case-by-case basis.

3.3. The following Federal Holidays are observed at Misawa.

3.4. Warranty replacement. The contractor shall be responsible for honoring manufacturers' and contract warranties and shall maintain on-site records of equipment bearing warranties. The contractor shall provide support documentation for any warranty claims.

3.5. Emergency services. The government will provide fire and police services. There are emergency medical services available on base. The contractor shall dial 226-0911 for emergency medical services.

3.6. Security requirements.

3.6.1. Listing of employees. The contractor shall maintain a current listing of employees working on the contract.

3.6.2. Traffic laws. Contractor employees shall comply with all base traffic regulations.

Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action.

3.6.3. Random personnel and vehicle searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

3.6.4. Reporting requirements. Contractor personnel shall immediately report to an appropriate government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.

3.6.5. Physical security. Contractor employees shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

3.6.6. Safety. The contractor shall comply with all applicable laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor's compliance or noncompliance with such responsibilities.

3.6.7. Work area. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment and materials.

Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workman like condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site.

3.6.8. Base entry requirements. The contractor shall obtain government identification documents for all employees, by submitting an application to the CO. The contractor must provide the full name, social security account number (SSAN), driver's license number (including state), and hours of required access, for each contractor employee.

Contractor base visitor/passes will be pickup upon arrival.

3.6.9. Contractor vehicle requirements. Access for fire fighting vehicles must be maintained at all working sites. Contractor’s vehicles shall not be parked within 15 feet of any fire hydrant or Fire Department connection. Contractor’s vehicle shall give right-of-way to responding fire/emergency vehicles. Contractor’s will obtain special vehicle pass to access restricted areas, such as the flight line prior to entry. Contractor’s will follow all FO checkpoints requirements.

3.7. Employee qualifications. Installation personnel shall be trained/qualified to install hydrant servicing modules.

3.8. Warranty terms. 1 year on installation and components.
