SECTION I
DESCRIPTION OF SERVICES
1.0. Scope of Work. The contractor shall provide all personnel, equipment, facilities, tools, materials, supervision and other items and services necessary to provide cable or satellite television service at Joint Base Elmendorf-Richardson, (JBER) AK, as defined in this Statement of Work (SOW).
1.1. Services and Programming. This SOW is for preferred cable television services, as described in paragraphs 1.1.1. This shall include all services required by the Federal Communications Commission (FCC) and any other federal laws or regulations. The contractor shall provide as a minimum, the following:
1.1.1. Cable Television Services.
1.1.1.1. Preferred Cable Services. The Contractor shall provide the following broad categories of programming:
24-hour News and Weather Programming Sports Children’s Programming Public Broadcast Services Major Network Programming – Anchorage Educational Channels – Anchorage
1.1.1.2. Program Guide. The contractor shall provide a channel guide to all new subscribers and to every subscriber upon any change in programming. The lodging facilities shall be provided a printed channel listing.
1.1.1.3 Music Channels. The contractor shall provide at a minimum the following music channels listed below:
• Hit List
• Pop Rhythmic
• Dance/EDM
• Hip-Hop and R&B
• Rap
• Hip-Hop Classics
• Throwback Jams
• R&B Classics
• R&B Soul
• Gospel
• Reggae
• Rock

1.2. Monthly Report. The contractor shall provide a monthly report of all downtime, upon request by the Contracting Office or Technical POC (listed on contract).
1.3. Repair Requirements. The Technical POC, upon notification of outages from the customer for television services will contact the contractor, who shall initiate a log for the purposes of tracking customer outages. The contractor shall submit this log to the Technical POC monthly, upon request. The contractor has one business day from the time of notification to respond to outages. If the outage is not repaired within the two (2) days then a repair date shall be established and reported to the Technical POC.
1.4. Remote Controls. The contractor shall provide one remote control per cable box/receiver.
1.5. Cable Box. The contractor shall provide one cable box/receiver per location.
1.6. HD/DVR Cable. The contractor shall provide one HD/DVR box per location upon request.
1.7 Mini Heads. The contractor shall provide mini head connections from central communication room in fitness center in order to provide service to 37 pieces of fitness equipment (additional mini heads) in the fitness center.
1.8. Contractor Provided Equipment. The contractor will be reimbursed for lost, stolen, or damaged equipment beyond fair wear and tear. The contractor must submit a claim for this reimbursement. Additionally, the contractor will replace any equipment that is found faulty or not working as needed in accordance with (IAW) with the contract.
1.9. New Installations. The contractor shall perform site surveys at no cost to the government to support new installation requirements not specified in the contract. Monthly service costs will be those costs that were previously established in the bid schedule.
1.10. Period of Performance (PoP).
1 Oct 2015 – 30 Sep 2020
1.11. Inherently Governmental Function. The Contractor shall not perform inherently governmental functions as defined in FAR Subpart 7.5 in relationship to this SOW.
1.12. Customer Satisfaction. A log will be conducted by the Technical POC quarterly to ensure all services are performed and are compliant IAW this SOW.

No government furnished property will be provided for this requirement.
SECTION IV
GENERAL INFORMATION
4.0. Technical Point of Contact. A Technical POC will be selected by the Contracting Officer to be the main focal point to the contractor from the government. The Technical POC will answer any and all questions from the contractor, and if he/she cannot, will be elevated to the Contracting Officer. The Technical POC will ensure the service is being performed IAW the SOW and to assist the installation in receiving and maintain service. The technical POC cannot obligate the government.
4.1. Construction and Installation Approval. The contractor shall not undertake any construction or installation of equipment or facilities on the base (including temporary equipment or facilities) without the prior written consent of the Contracting Officer (CO), as described below.
4.1.1. Prior to obtaining such approval, the contractor shall submit to the Contracting Officer for review any drawings, plans and other relevant supporting data. This includes, but is not limited to one set of the drawings showing cable routings, typical service entrances, antennae sites and/or towers, technical center location, and other related construction details.
4.1.2. At completion of any construction, installation or maintenance resulting in changes of cable routings, wirings or other significant changes in facilities and equipment, the contractor shall provide appropriate document changes to the Contracting Officer.
4.1.3. Notwithstanding any approval granted IAW subparagraph 4.1., the Government at any time may direct the contractor to remove, relocate, or modify any equipment or facilities, including aboveground and underground cable. The contractor shall be entitled to an equitable adjustment.
4.2. Quality Control. The contractor shall provide a Quality Control Plan (QCP) to the CO for acceptance within 5 days after contract award. The CO will notify the contractor of acceptance or required modification to the plan. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum, the contractor shall develop quality control procedures addressing the areas identified in Section II, Service Delivery Summary.
4.2.1. The Quality Control Plan shall include:
4.2.1.1. A description of the inspection system to cover all requirements listed in the SOW. The QCP shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspectors.
4.2.1.2. A description of the methods to be used for identifying and preventing defects in the quality of service performed.
4.2.1.3. A description of the records kept to document inspections and corrective or preventive actions taken.
4.2.1.4. Description of how equipment is to be maintained.
4.2.1.5. Descriptions on how to identify, prevent, and ensure non-recurrence of defective services.
4.3. Quality Assurance. The government will evaluate the contractor’s performance by appointing a representative to ensure that services are received. The government representative will evaluate the contractor’s performance through on-site inspections and the contractor’s quality control program. This representative will be the Technical POC assigned by the 673d Communication Squadron.
4.4. Freedom of Information Act Program (FOIA). All official government records affected by this contract are subject to the provisions of the FOIA (5 U.S.C. 552/DoD 5400.7-R/AF Supplement). Any request received by the contractor for access/release of information from these records to the public (including government/contractor employees acting as private citizens), whether oral or in writing, shall be immediately brought to the attention of the CO for forwarding to the Base FOIA Manager, 673 CS/SCBR, to ensure proper processing and compliance with the Act.
4.5. Safety Concerns. The Contractor is solely responsible for compliance with OSHA standards for the protection of their employees. The Air Force is not responsible for ensuring that Contractors comply with “personal” safety requirements that do not present the potential to damage Government resources.
4.6. Company/Privately Owned Vehicles. Each Contractor employee driving a company or privately owned vehicle on the installation must carry minimum liability insurance as required by the State of Alaska. Contractor employees shall adhere to all posted speed limits and shall not use hand-held cellular telephones while driving on the installation for phone calls or texting. Use of seatbelts is mandatory. Any Contractor employee cited for driving under the influence will have their driving privileges immediately revoked for a period to be determined.
4.7 Contractor Access To Air Force Installations.
4.7.1. The Contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on JBER. Contractor personnel are required to wear or prominently display installation identification badges or Contractor-furnished, Contractor identification badges while visiting or performing work on the installation.
4.7.2. The Contractor shall submit a written request on company letterhead to the Contracting Officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The Contracting Officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized Contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate, to obtain a vehicle pass.
4.7.3. During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to JBER. Upon completion or termination of the contract or expiration of the identification passes, the prime Contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. Failure to comply with these requirements may result in withholding of final payment.
4.8. Contractor Phase-In. During the Phase-In Period, the incoming contractor shall accomplish, as practicable, tasks required to ensure a smooth transition, to include, but not exclusively:
• Become thoroughly familiar with work requirements.
• Complete personnel requirements including the hiring of personnel to assure satisfactory performance beginning on the operational performance start date.
• Obtain security checks, vehicle passes, e-mail and database accounts.
• Complete contractor provided employee-training requirements.
• The contractor shall be allowed access to government facilities at JBER to familiarize supervisors, key personnel and staff with existing equipment, work scheduling and procedures. The CO will arrange the access to ensure interference does not occur.
• The incoming contractor shall cooperate with the incumbent contractor to the extent necessary to permit an orderly transition of service provider without service degradation.
4.9. Contractor Phase-Out. In the event a follow-on contract is awarded to other than the incumbent, the incumbent contractor shall cooperate with the incoming contractor to the extent necessary to permit an orderly transition of service provider without service degradation.
APPENDIX 1
Civil Engineer Requirements for Cable/Satellite Television
1. Structural. The television receiver shall not interfere with structural integrity of the walls. It shall be flush-mounted with the smallest length and width produced by industry that will work properly for the location. Installation on rooftops is not permitted.
2. Moisture Barrier. The method of installation shall not compromise the existing moisture protection barrier. If it does, a government acceptable method of patching the penetration shall be used. The contractor shall provide details for vapor barrier and finish penetrations.
3. Warranty. A warranty shall be provided on the installation in accordance with industry standards. The warranty shall assume contractor liability for any damage, present or future, to the building system as a result of any cable/satellite television installations.
4. Aesthetics. Any installation of equipment shall comply with base architectural compatibility guidelines found in the publication, “JBER Architectural Compatibility Standards.” In addition, the television equipment shall be impregnated with a permanent color that matches the facility. Cables shall not be visible on the surface of the facility and shall be buried underground if running between buildings. No roof penetrations or pavement cuts shall be allowed for installation of cable unless prior approval is received from the base civil engineer. The contractor shall comply with JBER dig permitting process. Installation of television receivers on the front of the building shall not be permitted.
5. As-Built Drawings. Drawings of the connection to the facility shall be provided for each method. In addition, the contractor shall provide accurate drawings showing the location of any cables buried underground.
6. Annual Maintenance. A plan for annual maintenance of the television receiver shall be drafted and submitted to the government prior to installation. In addition, the television equipment must not impede maintenance of the facilities.
7. Grounding. Proper grounding and lightening protection techniques shall be employed, as described in NFPA 70-1099 (NEC) and IEEE Std 142-1991.
8. Power. Contractor shall provide a proposal for tying into and using base power systems, where applicable. Details shall be provided for connection of television equipment to the power supply. Contractor shall comply with appropriate electrical code including NESC, IEEE and NEC.
9. Vandal Resistant Installation. Provisions to prevent vandalism to the television equipment shall be made wherever practical. (i.e. out of the reach of children)
10. Repeater Installation. Contractor shall provide details of any repeater, receiver and power source installation to the government prior to installation.
11. Antennae Installation Proposal. Prior to performance start date, contractor shall submit an installation proposal of all television receiving equipment for each facility or similar groups of facilities to the government for review and approval.
12. Work Clearance Requests. Prior to any work performance, contractor shall complete a 3 WG Form 3 (Base Civil Engineer Work Clearance Request), submit to 673 CES, and receive approval. This will avoid any conflict with underground utility lines. Work shall not begin until after the approval is received.
13. Contract Termination. Upon termination of this contract, the contractor shall remove any/all installed television receiving equipment unless mutually agreed to by the government and contractor. Upon removal, the facilities must be returned to their original condition, to include both structural and aesthetic fixes.
APPENDIX 2
WORKLOAD ESTIMATES/BUILDING LOCATIONS
1. The existing Cable Television locations shall be used as a workload estimate for the start of contract. The contractor shall be flexible to incidental increases or decreases in workload as requirements for service change.
